目的 合并肺纤维化的慢性阻塞性肺疾病(COPD)是COPD的特殊亚型,患者兼具气流受限与肺组织纤维化病理特征,临床症状更严重、肺功能下降更快,且现有单一治疗方案难以同时改善气流受限与纤维化进展,预后较差。基于此,本研究旨在分析尼达尼布联合格隆溴铵治疗合并肺纤维化的COPD患者的效果及对肺功能的影响,为优化临床治疗方案提供依据。方法 选取2022年3月—2024年12月收治的96例合并肺纤维化的COPD患者,采用前瞻性随机对照研究设计,应用随机数字表法分为试验组与对照组。所有患者均采取常规治疗,对照组48例采取尼达尼布治疗,试验组采取尼达尼布联合格隆溴铵治疗。两组均治疗24周后,比较治疗前后症状评分、肺功能、纤维化指标、炎症指标,并分析两组治疗安全性。结果 治疗后,两组CAT评分、mMRC评分及VAS降低(P<0.05);且与对照组比较,试验组CAT评分、mMRC评分及咳嗽VAS评分较低(P<0.05)。治疗后,两组FVC、FEV1、DLCO及FEV1/FVC比值均较治疗前改善(P<0.05);且与对照组比较,试验组FVC、FEV1、DLCO及FEV1/FVC比值较优(P<0.05)。治疗后,两组血清KL-6、SP-D水平及CT纤维化评分均降低(P<0.05);且与对照组比较,试验组血清KL-6、SP-D水平及CT纤维化评分较低(P<0.05)。治疗后,两组血清IL-6、TNF-α及TGF-β1水平降低(P<0.05);且与对照组比较,试验组血清IL-6、TNF-α及TGF-β1水平较低(P<0.05)。试验组总不良反应发生率为8.33%(4/48),对照组为10.42%(5/48),两组比较差异无统计学意义(P>0.05)。结论 尼达尼布联合格隆溴铵治疗合并肺纤维化的COPD效果良好,可减轻患者临床症状,改善肺功能与肺纤维化,降低机体炎症反应,安全性较高。
Objective To analyze the effects of the combination of nintedanib and glycopyrrolate in treating chronic obstructive pulmonary disease(COPD)patients with associated pulmonary fibrosis and its impact on lung function,providing a basis for optimizing clinical treatment strategies.Methods Ninety-six COPD patients with pulmonary fibrosis admitted from March 2022 to December 2024 were selected,and divided into experimental group and control group using a random number table method.Using a prospective randomized controlled study design,all patients received conventional treatment,with 48 cases in the control group receiving treatment with nintedanib and the experimental group receiving treatment with nintedanib combined with glycopyrrolate bromide.After 24 weeks of treatment in both groups,the symptom scores,lung function,fibrosis indicators,and inflammation indicators were compared before and post-treatment,and the drug safety of the two groups was analyzed.Results Post-treatment,CAT score,mMRC score and VAS decreased in both groups(P<0.05).Compared with the control group,CAT score,mMRC score and cough VAS score were lower in the experimental group(P<0.05).Post-treatment,FVC,FEV1,DLCO and FEV1/FVC ratio of both groups improved compared with that before treatment(P<0.05).Compared with the control group,FVC,FEV1,DLCO and FEV1/FVC ratio of the experimental group were better(P<0.05).Post-treatment,serum KL-6,SP-D levels and CT fibrosis scores of both groups decreased(P<0.05).Compared with the control group,serum KL-6,SP-D levels and CT fibrosis scores of the experimental group were lower(P<0.05).Post-treatment,serum IL-6,TNF-α and TGF-β1 levels in both groups decreased(P<0.05).Compared with the control group,serum IL-6,TNF-α and TGF-β1 levels in the experimental group were lower(P<0.05).The incidence of total adverse reactions in the experimental group was 8.33%(4/48),and that in the control group was 10.42%(5/48).There was no difference between the two groups(P>0.05).Conclusions The combination of nintedanib and glycopyrrolate has a significant effect on the treatment of COPD complicated with pulmonary fibrosis,which can alleviate its clinical symptoms,improve lung function and pulmonary fibrosis,reduce the body’s inflammatory response,which is relatively safe.
目的 通过年龄、性别等多方面研究各亚型注意缺陷多动障碍(ADHD)儿童的智力结构分布特征的临床研究,为ADHD的个体化干预提供科学依据。方法 实验组纳入ADHD儿童754例,其中注意缺陷型(ADHD-I)280例(37.13%)、多动-冲动型(ADHD-HI)212例(28.12%)、混合型(ADHD-C)262例(34.75%),按年龄分为三组:低龄组(6~8岁)、中龄组(9~11岁)及高龄组(12~14岁);对照组为正常儿童412例,为同期选择的性别、年龄相匹配的儿童。选择采用韦氏智力评估等方法分别对两组儿童进行评估等,对评估结果进行比较和综合分析。结果 ADHD儿童的智力评估结果低于对照组(P<0.01),且在言语智商方面表现更为明显,在各分测验中以A因子(言语理解)和C因子(记忆/注意集中)为主,而B因子(知觉组织能力)方面则无差异。ADHD-HI型及ADHD-C型的总智商及操作智商均高于ADHD-I型,ADHD-C型言语智商和A因子均高于ADHD-I型(P<0.01),ADHD-HI型的智力水平最高,而ADHD-I型与ADHD-C型的智力水平差异无统计学意义(P>0.05)。各年龄段ADHD儿童的智力发育均受到不同程度的影响,尤其是中龄组儿童及高龄组女童所受影响更明显;低、中龄组儿童的智力发育水平均高于高龄组儿童,提示ADHD儿童的智力损害随年龄增长而加重;同一年龄段ADHD女童的智力损害较男童更为严重。结论 本地区6~14岁ADHD儿童的智力水平较低,以言语智商低下更为明显,ADHD-HI型在3个亚型中智力损害最小,ADHD儿童的智力损害随年龄增长而加重,同一年龄段ADHD女童的智力损害较男童更为严重。
Objective To investigate the distribution characteristics of intellectual structure in children with different subtypes of attention deficit hyperactivity disorder(ADHD)by age and gender,providing scientific basis for individualized treatment and intervention of ADHD.Methods The experimental group consisted of 754 children with ADHD,including 280 cases(37.13%)of inattentive type(ADHD-I),212 cases(28.12%)of hyperactivity impulsive type(ADHD-HI),and 262 cases(34.75%)of combined type(ADHD-C).They were divided into three groups by age:young group(6-8 years old),middle group(9-11 years old),and older group(12-14 years old).The control group consisted of 412 normal children with matched sex and age recruited during the same period and had similar sex ratio.The specific methods of Wechsler Intelligence Scale were mainly applied to evaluate normal children and children with ADHD,and the evaluation results were subjected to multidimensional analysis and comparison.Results The intelligence assessment results of ADHD children were significantly lower than those of the control group(P<0.01),with a more pronounced performance in verbal intelligence.In various sub tests,factors A(verbal comprehension)and C(memory and attention concentration)were the main factors,while there was no difference in factor B(perceptual organization ability).The ADHD-HI and ADHD-C types had higher total intelligence and operational intelligence than the ADHD-I type,and the ADHD-C type had higher verbal intelligence and factor A than the ADHD-I type(P<0.01).The intelligence level of the ADHD-HI type is the highest,while there is no statistically significant difference in intelligence level between the ADHD-I type and the ADHD-C type.The intellectual development of children in different age groups was affected to varying degrees,especially in the middle and older groups of girls.This study found that the intellectual development of children in the young and middle group was higher than that of children in the older group,indicating that the intellectual development of ADHD children will be increasingly damaged with age.At the same time,it was also found that ADHD girls in the same age group had more severe intellectual development impairment than boys.Conclusions The intelligence level of children with ADHD aged 6-14 in this region is relatively low,with verbal intelligence quotient being more pronounced.ADHD-HI has the smallest intellectual impairment among the three subtypes,and the intellectual development of ADHD children becomes more severe with age.ADHD girls in the same age group have more severe intellectual development impairment than boys.
目的:探讨沙库巴曲缬沙坦钠对慢性心力衰竭(CHF)患者心电稳定性及心脏负荷的改善作用。方法:病例纳入2023年10月~2025年5月收治的102例CHF患者为研究对象,依据就诊时间段及治疗方案不同,将2023年3月~2024年6月沿用缬沙坦+螺内酯+比索洛尔治疗的51例患者列为常规组,将2024年7月~2025年9月采用沙库巴曲缬沙坦+螺内酯+比索洛尔治疗的51例患者列为试验组,比较两组患者的心肌纤维化,心电稳定性,心脏负荷及治疗安全性。所有患者均接受为期半年随访,比较两组患者的预后情况。结果:治疗后,试验组的基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)、半乳糖凝集素-3(Gal-3)、可溶性生长刺激表达基因2蛋白(sST2)均低于常规组(t=5.045,2.889,4.115,4.582;P<0.05)。试验组的校正QT间期(QTc)、QT离散度(QTd)、T波峰-末间期(Tp-e)、室性早搏次数分别为(405.39±40.26)ms、(45.25±5.33)ms、(90.33±5.28)ms、(80.36±5.39)次/24h,均低于常规组[(450.22±42.19)ms、(50.37±6.15)ms、(95.29±6.44)ms、(85.27±6.18)次/24h](t=5.490,4.493,4.253,4.276;P<0.05)。试验组的收缩期肺动脉压(sPAP)、三尖瓣反流峰值速度(TRVmax)、左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)分别为(20.48±5.26)mmHg、(2.13±0.25)m/s、(45.29±5.62)mm、(30.61±5.33)mm,均低于常规组[(25.35±6.29)mmHg、(3.22±0.47)m/s、(50.45±6.15)mm、(35.49±6.27)mm](t=4.242,14.622,4.423,4.235;P<0.05)。试验组的药物相关副反应发生率与常规组比较,差异无统计学意义(P>0.05)。截至随访结束时,试验组的不良预后发生率9.80%(5/51)低于常规组27.45%(14/51)(x2=5.239;P<0.05)。结论:沙库巴曲缬沙坦钠能延缓CHF患者的心肌纤维化进程并改善心电稳定性,在有效改善心电稳定性并降低不良预后发生风险同时未显著增加治疗风险。
Objective:To explore the improvement effect of sacubitril valsartan sodium on electrocardiogram stability and cardiac load in patients with CHF.Methods:A total of 102 CHF patients admitted from October 2023 to May 2025 were included in the study. Based on the treatment period and regimen, 51 patients treated with valsartan, spironolactone, and bisoprolol from March 2023 to June 2024 were classified as the conventional group, while 51 patients treated with sacubitril/valsartan, spironolactone, and bisoprolol from July 2024 to September 2025 were designated as the experimental group. The myocardial fibrosis, electrocardiographic stability, cardiac workload, and treatment safety were compared between the two groups. All patients underwent a six-month follow-up to assess their prognosis.Results:After treatment, the levels of MMP-2, MMP-9, Gal-3, and sST2 in the experimental group were lower than the control group (t=5.045,2.889,4.115,4.582; P<0.05). The QTc, QTd, Tp-e, and number of ventricular premature beats in the experimental group were (405.39 ± 40.26) ms, (45.25 ± 5.33) ms, (90.33 ± 5.28) ms, and (80.36 ± 5.39) beats per 24 hours, lower than the control group [(450.22 ± 42.19) ms, (50.37 ± 6.15) ms, (95.29 ± 6.44) ms, and (85.27 ± 6.18) beats per 24 hours] (t=5.490,4.493,4.253,4.276; P<0.05). The sPAP, TRVmax, LVEDD, and LVESD of the experimental group were (20.48 ± 5.26) mmHg, (2.13 ± 0.25) m/s, (45.29 ± 5.62) mm, and (30.61 ± 5.33) mm, lower than the conventional group [(25.35 ± 6.29) mmHg, (3.22 ± 0.47) m/s, (50.45 ± 6.15) mm, and (35.49 ± 6.27) mm] (t=4.242,14.622,4.423,4.235; P<0.05). The incidence of drug-related side effects in the experimental group was not significantly different from that in the control group (P>0.05). As of the end of follow-up, the incidence of poor prognosis in the experimental group was 9.80% (5/51) lower than that in the conventional group 27.45% (14/51) (x2=5.239; P<0.05).Conclusion:Sacubitril valsartan sodium can delay the progression of myocardial fibrosis and improve electrocardiogram stability in CHF patients, effectively improving electrocardiogram stability and reducing the risk of adverse prognosis without significantly increasing treatment risk.
目的:分析冠状动脉CT血管成像(CTA)联合动态心电图(DCG)与冠心病患者冠脉狭窄程度及预后情况的关联。方法:研究对象选择我院2024年1月~2025年3月收治的210例冠心病患者及同期接受检查的210例非冠心病患者,分别列为病例组和对照组,比较两组CTA参数、DCG参数间差异。依据入院测得(Gensini)评分不同,将入组患者分别列为轻度组(60例,Gensini评分≤30分)、中度组(75例,Gensini评分>30分、≤60分)和重度组(75例,Gensini评分>60分),比较三组CTA参数、DCG参数间差异,分析CTA参数、DCG参数与Gensini评分的相关性。统计入组患者不良预后发生情况,比较不同预后患者CTA参数、DCG参数间差异,归纳冠心病患者预后影响因素,检验CTA参数、DCG参数对患者不良预后的预测效能。结果:病例组的最小管腔直径(MLD)、最小管腔面积(MLA)、血流储备分数(FFR)、正常窦性间期的标准差(SDNN)、每5min平均RR间期的标准差(SDANN)、相邻RR间期差值的均方根(RMSSD)均低于对照组,斑块总体积(TPV)、低频/高频比值(LF/HF)均高于对照组(t=24.128,25.811,15.613,37.636,26.858,9.195,59.862,29.389;P<0.05)。重度组的MLD、MLA、FFR、SDNN、SDANN、RMSSD均低于中度组,轻度组,TPV、LF-HF均高于中度组,轻度组(F=190.291,51.562,186.482,42.084,44.413,22.541,56.503,109.983;P<0.05)。MLD、MLA、FFR、SDNN、SDANN、RMSSD均与Gensini评分负相关,TPV、LF-HF均与Gensini评分正相关(r=-0.352,-0.377,-0.445,-0.472,-0.332,-0.356,0.401,0.355;P<0.05)。经统计,210例冠心病患者的不良预后发生率为38.10%(80/210)。预后不良组的MLD、MLA、FFR、SDNN、SDANN、RMSSD均低于预后良好组,TPV、LF-HF均高于预后良好组(t=6.827,12.219,19.313,6.097,7.097,5.027,7.088,12.465;P<0.05)。MLA、FFR、SDNN升高为冠心病不良预后的保护因素,LF/HF升高为冠心病不良预后的危险因素。FFR、SDNN联合检测预测不良预后的 AUC 值优于两项指标单独检测(Delong检验,P<0.05)。结论:CTA、DCG能客观评估冠心病患者冠脉狭窄程度,联合检测FFR、SDNN可作为预测冠心病不良预后的重要辅助手段。
Objective:To analyze the correlation between CTA combined with DCG and the degree of coronary stenosis and prognosis in patients with coronary heart disease.Methods:The research subjects selected were 210 patients with coronary heart disease admitted to our hospital from January 2024 to March 2025, as well as 210 non coronary heart disease patients who underwent examinations during the same period. They were divided into a case group and a control group. The differences in CTA parameters and DCG parameters between the two groups were compared. According to the different Gensini scores obtained upon admission, the enrolled patients were divided into mild group (60 cases, Gensini score ≤ 30 points), moderate group (75 cases, Gensini score>30 points, ≤ 60 points), and severe group (75 cases, Gensini score>60 points). The differences in CTA parameters and DCG parameters among the three groups were compared, and the correlation between CTA parameters, DCG parameters, and Gensini score was analyzed. Statistically analyze the occurrence of poor prognosis in enrolled patients, compare the differences in CTA and DCG parameters among patients with different prognoses, summarize the factors affecting the prognosis of coronary heart disease patients, and test the predictive power of CTA and DCG parameters for poor prognosis in patients.Results:The MLD, MLA, FFR, SDNN, SDANN, and RMSSD in the case group were all lower than the control group, while the TPV and LF/HF were higher than the control group (t=24.128,25.811,15.613,37.636,26.858,9.195,59.862,29.389; P<0.05). The MLD, MLA, FFR, SDNN, SDANN, and RMSSD of the severe group were lower than the moderate group, mild group, while the TPV and LF-HF of the mild group were higher than the moderate group, mild group (F=190.291,51.562,186.482,42.084,44.413,22.541,56.503,109.983; P<0.05). MLD, MLA, FFR, SDNN, SDANN, and RMSSD are all negatively correlated with Gensini score, while TPV and LF-HF are positively correlated with Gensini score (r=-0.352,-0.377,-0.445,-0.472,-0.332,-0.356,0.401,0.355; P<0.05). According to statistics, the incidence of poor prognosis in 210 patients with coronary heart disease was 38.10% (80/210). The MLD, MLA, FFR, SDNN, SDANN, and RMSSD of the poor prognosis group were lower than the good prognosis group, while TPV and LF-HF were higher than the good prognosis group (t=6.827,12.219,19.313,6.097,7.097,5.027,7.088,12.465; P<0.05). High MLA, FFR, and SDNN are protective factors for poor prognosis of coronary heart disease, while higher values than LF/HF are risk factors for poor prognosis of coronary heart disease. The combined detection of FFR and SDNN has a better AUC value for predicting poor prognosis of coronary heart disease than the detection of FFR and SDNN alone (Delong test, P<0.05).Conclusion:CTA and DCG can objectively evaluate the degree of coronary stenosis in patients with coronary heart disease, and combined detection of FFR and SDNN can be an important auxiliary tool for predicting poor prognosis of coronary heart disease.
【摘要】目的:探讨器械护士术中分区精细化器械管理对心脏瓣膜置换术(HVR)手术效率及安全性的影响。方法:研究对象包括2024年6月~2026年5月至我院行HVR治疗的110例心脏瓣膜病(VHD)患者,以及同期参与手术的22名器械护士,采用随机数字表法将入组患者及护士分别列为常规组(55例患者、11名护士)、试验组(55例患者、11名护士),常规组内护士术中实施常规器械管理,试验组内护士术中实施分区精细化器械管理。比较两组患者的手术效率、围手术期安全事件,以及两组护士所参与手术的工作效率、术中器械管理情况。结果:在不同器械管理模式下,试验组内患者的主动脉阻断时间、体外循环时间、术中非必要等待时间、手术总耗时均低于常规组(t=8.817,5.730,20.609,9.632;P<0.05)。试验组内患者的围手术期安全事件发生率9.09%(5/55)低于常规组25.45%(14/55)(x2=5.153;P<0.05)。试验组内护士的器械摆台时间、器械传递反应时间、手术中断次数、器械整理时间、器械移交时间均低于常规组(t=3.347,6.505,5.971,3.795,5.838;P<0.05)。试验组内护士的器械管理差错率低于常规组(x2=5.238;P<0.05)。结论:器械护士术中分区精细化器械管理能提高HVR患者的手术效率并降低围手术期安全事件发生风险,对提高器械护士的器械管理质量、预防器械管理差错有积极影响
[Abstract]Objective:To explore the impact of refined intraoperative instrument management by operating room nurses on the efficiency and safety of HVR surgery.Methods:The study subjects included 110 patients with VHD who underwent HVR treatment at our hospital from June 2024 to May 2026, along with 22 instrument nurses who participated in the surgeries synchronously. Using the random number table method, the enrolled patients and nurses were divided into a control group (55 patients, 11 nurses) and an experimental group (55 patients, 11 nurses). Nurses in the conventional group implemented routine instrument management during surgery, while nurses in the experimental group implemented zoned and refined instrument management during surgery. The surgical efficiency, perioperative safety events of the two groups of patients, as well as the work efficiency and intraoperative instrument management of the surgeries participated by the nurses in the two groups were compared.Results:Under different instrument management modes, the aortic occlusion time, extracorporeal circulation time, intraoperative non-essential waiting time, and total surgical time of patients in the experimental group were all lower than the control group (t=8.817,5.730,20.609,9.632; P<0.05). The incidence of perioperative safety events in the experimental group was 9.09% (5/55) lower than that in the control group 25.45% (14/55) (x2=5.153; P<0.05). The instrument placement time, instrument delivery response time, number of surgical interruptions, instrument sorting time, and instrument handover time of nurses in the experimental group were lower than those in the control group (t=3.347,6.505,5.971,3.795,5.838; P<0.05). The error rate of nurses in the experimental group was lower than the control group (x2=5.238; P < 0.05).Conclusion:Refined intraoperative zone-specific instrument management by instrument nurses enhances surgical efficiency for HVR patients and reduces the risk of perioperative safety incidents, thereby positively impacting the quality of instrument care provided by nurses and preventing instrument-related adverse events.
目的:基于文献计量学梳理惯性测量技术在步态分析领域的研究演进与热点结构,并从护理评估与干预转化角度分析其应用空白。方法:检索 Web of Science 核心合集2005年1月1日至2025年4月5日相关英文文献,纳入1,079篇记录;采用 CiteSpace 6.3.R1、VOSviewer 1.6.20 分析年度发文、国家/地区合作、期刊分布、关键词共现与突现,并在 Python 3.10 中以 PPMI/TF-IDF 表征、SVD降维、UMAP-HDBSCAN聚类开展关键词和摘要语义分析。结果:2006—2024年发文量由1篇增至140篇,年复合增长率为31.6%,2024年达到峰值;最高频关键词为 gait(404次)、gait analysis(268次)、walking(252次)、balance(183次)和 inertial sensors(156次)。关键词与摘要语义聚类的二维轮廓系数分别为0.579和0.642,热点集中于帕金森病/冻结步态、跌倒风险、平衡稳定性、可穿戴传感器、机器学习和康复干预。含 nursing/care 等护理相关词项的记录为142篇,但“护理”尚未形成独立主题簇。结论:惯性测量步态分析已形成医工交叉的成熟热点,但护理主导的连续评估、风险预警和干预闭环仍不足。未来应将步速、步态变异性、稳定性、对称性等参数转化为可执行的护理评估指标,推动精准护理场景中的临床验证与流程整合。
Objective: To map the research evolution and hotspot structure of inertial-measurement-based gait analysis and to examine its translational gap in nursing assessment and intervention. Methods: A total of 1,079 English records published from January 1, 2005 to April 5, 2025 were retrieved from the Web of Science Core Collection. CiteSpace 6.3.R1 and VOSviewer 1.6.20 were used for annual output, collaboration, journal distribution, keyword co-occurrence and burst analyses. Keyword and abstract semantic clusters were further examined in Python 3.10 using PPMI/TF-IDF representation, SVD, UMAP and HDBSCAN. Results: Publications increased from 1 in 2006 to 140 in 2024, with a compound annual growth rate of 31.6%. The most frequent terms were gait, gait analysis, walking, balance and inertial sensors. The two-dimensional silhouette coefficients of keyword and abstract semantic clusters were 0.579 and 0.642, respectively. Major hotspots involved Parkinson disease/freezing of gait, fall risk, balance and stability, wearable sensors, machine learning and rehabilitation. Records containing nursing/care-related terms accounted for 142 publications, but nursing did not form an independent topic cluster. Conclusion: Inertial-measurement-based gait analysis has become a mature medical-engineering research field, while nurse-led continuous assessment, risk warning and intervention feedback loops remain underdeveloped. Translating gait speed, variability, stability and symmetry into actionable nursing indicators should be prioritized in future clinical validation.
目的:基于文献计量学梳理惯性测量技术在步态分析领域的研究演进与热点结构,并从护理评估与干预转化角度分析其应用空白。方法:检索 Web of Science 核心合集2005年1月1日至2025年4月5日相关英文文献,纳入1,079篇记录;采用 CiteSpace 6.3.R1、VOSviewer 1.6.20 分析年度发文、国家/地区合作、期刊分布、关键词共现与突现,并在 Python 3.10 中以 PPMI/TF-IDF 表征、SVD降维、UMAP-HDBSCAN聚类开展关键词和摘要语义分析。结果:2006—2024年发文量由1篇增至140篇,年复合增长率为31.6%,2024年达到峰值;最高频关键词为 gait(404次)、gait analysis(268次)、walking(252次)、balance(183次)和 inertial sensors(156次)。关键词与摘要语义聚类的二维轮廓系数分别为0.579和0.642,热点集中于帕金森病/冻结步态、跌倒风险、平衡稳定性、可穿戴传感器、机器学习和康复干预。含 nursing/care 等护理相关词项的记录为142篇,但“护理”尚未形成独立主题簇。结论:惯性测量步态分析已形成医工交叉的成熟热点,但护理主导的连续评估、风险预警和干预闭环仍不足。未来应将步速、步态变异性、稳定性、对称性等参数转化为可执行的护理评估指标,推动精准护理场景中的临床验证与流程整合。
Objective: To map the research evolution and hotspot structure of inertial-measurement-based gait analysis and to examine its translational gap in nursing assessment and intervention. Methods: A total of 1,079 English records published from January 1, 2005 to April 5, 2025 were retrieved from the Web of Science Core Collection. CiteSpace 6.3.R1 and VOSviewer 1.6.20 were used for annual output, collaboration, journal distribution, keyword co-occurrence and burst analyses. Keyword and abstract semantic clusters were further examined in Python 3.10 using PPMI/TF-IDF representation, SVD, UMAP and HDBSCAN. Results: Publications increased from 1 in 2006 to 140 in 2024, with a compound annual growth rate of 31.6%. The most frequent terms were gait, gait analysis, walking, balance and inertial sensors. The two-dimensional silhouette coefficients of keyword and abstract semantic clusters were 0.579 and 0.642, respectively. Major hotspots involved Parkinson disease/freezing of gait, fall risk, balance and stability, wearable sensors, machine learning and rehabilitation. Records containing nursing/care-related terms accounted for 142 publications, but nursing did not form an independent topic cluster. Conclusion: Inertial-measurement-based gait analysis has become a mature medical-engineering research field, while nurse-led continuous assessment, risk warning and intervention feedback loops remain underdeveloped. Translating gait speed, variability, stability and symmetry into actionable nursing indicators should be prioritized in future clinical validation.
【摘要】目的:探讨超声骨刀与高速涡轮手机在颌骨囊肿患者外科手术中的应用效果及安全性。方法:研究选择2024年1月~2025年6月至我院行囊肿刮治术治疗的100例颌骨囊肿患者,通过隐藏信封法将入组患者随机列为常规组、试验组,每组50例。常规组术中应用高速涡轮手机去骨,试验组术中应用超声骨刀去骨,比较两组患者的手术情况,术后疼痛肿胀情况及住院期间并发症发生情况。术毕随访半年,比较两组患者神经损伤情况及骨愈合情况。结果:试验组的术中出血量、术后24h引流量、切骨精度偏差分别为(30.59±5.24)mL、(20.55±5.28)mL、(0.18±0.05)mm,均低于常规组[(40.19±7.33)mL、(30.46±6.45)mL、(0.59±0.12)mm](t=7.534,8.407,22.301;P<0.05);手术耗时与常规组比较,差异无统计学意义(P>0.05)。试验组术后24h、48h、72h的视觉模拟疼痛量表(VAS)评分,肿胀评分均低于常规组(t=4.403,3.354,12.986,4.610,2.911,14.888;P<0.05)。试验组住院期间的并发症发生率4.00%(2/50)低于常规组18.00%(9/50)(x2=5.005;P<0.05)。试验组随访第1个月、第3个月、第6个月的神经传导速度(NCV)均高于常规组,两点辨别觉(TPD)均低于常规组(t=4.598,5.784,6.322,3.194,3.595,3.501;P<0.05)。截至随访结束时,试验组的成骨率分别为(75.27±8.14)%高于常规组(68.18±5.27)%],骨缺损面积、创面愈合时间、骨吸收量分别为(55.29±5.42)%、(3.22±0.47)月、(1.25±0.36)mm,均低于常规组[(62.44±7.51)%、(5.08±1.33)月、(2.49±0.32)mm](t=5.170,5.459,9.324,18.204;P<0.05)。结论:与高速涡轮手机相比,超声骨刀可降低颌骨囊肿患者术中出血风险并实现精准去骨,在减轻术后疼痛、肿胀程度同时能一定程度减轻术后神经损伤,对促进患者骨愈合也有积极影响。
[Abstract]Objective:To explore the application effect and safety of ultrasound bone scalpel and high-speed turbine mobile phone in surgical procedures for patients with jaw cysts.Methods:A total of 100 patients with maxillary cysts who underwent curettage surgery in our hospital from January 2024 to June 2025 were selected for the study. The enrolled patients were randomly divided into a control group and an experimental group using the hidden envelope method, with 50 patients in each group. The conventional group used high-speed turbine mobile phones for bone removal during surgery, while the experimental group used ultrasonic bone knives for bone removal during surgery. The surgical conditions, postoperative pain and swelling, and incidence of complications during hospitalization were compared between the two groups of patients. Follow up for six months after surgery to compare the nerve damage and bone healing between the two groups of patients.Results:The intraoperative bleeding volume, postoperative 24-hour drainage volume, and bone cutting accuracy deviation of the experimental group were (30.59 ± 5.24) mL, (20.55 ± 5.28) mL, and (0.18 ± 0.05) mm, respectively, all lower than those of the control group [(40.19 ± 7.33) mL, (30.46 ± 6.45) mL, and (0.59 ± 0.12) mm] (t=7.534,8.407,22.301; P<0.05); The surgical time of the experimental group was similar to the control group (P>0.05). The VAS scores and swelling scores of the experimental group at 24, 48, and 72 hours after surgery were lower than the control group (t=4.403,3.354,12.986,4.610,2.911,14.888;; P<0.05). The incidence of complications during hospitalization in the experimental group was 4.00% (2/50) lower than the control group 18.00% (9/50) (x2=5.005; P<0.05).The NCV of the experimental group was higher than the control group at the1,3,6 months of follow-up, and TPD was lower than the control group (t=4.598,5.784,6.322,3.194,3.595,3.501; P<0.05). As of the end of follow-up, the osteogenic rate of the experimental group was (75.27 ± 8.14)% higher than the control group (68.18 ± 5.27)%. The bone defect area, wound healing time, and bone resorption were (55.29 ± 5.42)%, (3.22 ± 0.47) months, and (1.25 ± 0.36) mm, lower than the control group [(62.44 ± 7.51)%, (5.08 ± 1.33) months, and (2.49 ± 0.32) mm] (t=5.170,5.459,9.324,18.204; P<0.05).Conclusion:Compared with high-speed turbo phones, ultrasonic bone scalpel can reduce the risk of intraoperative bleeding in patients with jaw cysts and achieve precise bone removal. It can alleviate postoperative pain and swelling while greatly avoiding postoperative nerve damage, and has a positive impact on promoting bone healing in patients.
摘要目的 探讨基于E-Coach健康管理模式的营养管理联合主动循环呼吸训练(ACBT)在非小细胞肺癌(NSCLC)化疗患者中的应用效果。方法 选取2023年6月至2025年8月我院收治的98例NSCLC化疗患者,采用随机数字表法将所有研究对象分为联合组和常规组,每组49例。常规组给予常规干预,联合组在常规组基础上予以E-Coach健康管理模式的营养管理联合ACBT干预。比较两组干预前后营养状况、肺功能、运动耐力、生活质量以及营养不良发生率。结果 干预12周后,两组BMI、ALB、PA、Hb均较干预前上升且联合组高于常规组(P<0.05);联合组FVC、FEV1、MVV及6MWT均显著高于常规组(P<0.05);干预前两组6MWT组间对比差异无统计学意义(P>0.05),干预4周、6周、8周、12周后,两组6MWT均较干预前增加,且联合组远于常规组(P<0.05);干预12周后,两组身体功能、社会或家庭功能、情感功能、功能性状况得分均较干预前上升,且联合组高于常规组(P<0.05)。结论基于E-Coach健康管理模式的营养管理联合ACBT能够有效改善NSCLC化疗患者的营养状况和肺功能,提高生活质量和运动耐力。
Abstract Objective To investigate the application effect of nutrition management based on the E-Coach health management model combined with active cycle of breathing technique (ACBT) in patients undergoing chemotherapy for non-small cell lung cancer (NSCLC). Methods A total of 98 NSCLC patients receiving chemotherapy in our hospital from June 2023 to August 2025 were selected and randomly divided into a combination group and a conventional group using a random number table method, with 49 cases in each group. The conventional group received routine intervention, while the combination group received nutrition management based on the E-Coach health management model combined with ACBT in addition to the routine intervention. The nutritional status, lung function, exercise endurance, quality of life, and incidence of malnutrition were compared between the two groups before and after the intervention. Results After 12 weeks of intervention, BMI, ALB, PA, and Hb in both groups increased compared with baseline, and the levels in the combination group were higher than those in the conventional group (P<0.05). The FVC, FEV1, MVV, and 6MWT in the combination group were significantly higher than those in the conventional group (P<0.05). There was no statistically significant difference in 6MWT between the two groups before intervention (P>0.05); after 4, 6, 8, and 12 weeks of intervention, the 6MWT in both groups increased compared with baseline, and the walking distance in the combination group was significantly longer than that in the conventional group (P<0.05). After 12 weeks of intervention, the scores of physical function, social/family function, emotional function, and functional well-being in both groups increased compared with baseline, and the scores in the combination group were higher than those in the conventional group (P<0.05). Conclusion Nutrition management based on the E-Coach health management model combined with ACBT can effectively improve the nutritional status and lung function of NSCLC patients undergoing chemotherapy, and enhance their quality of life and exercise endurance.
目的 探讨清咳平喘颗粒联合乙酰半胱氨酸对痰热闭肺型大叶性肺炎(LP)患儿的治疗效果。方法 回顾性选取我院收治的118例痰热闭肺型LP患儿(选例时间:2023年1月~2025年11月)为研究对象,根据治疗方案分为参照组(59例,采用乙酰半胱氨酸治疗)、联合组(59例,采用清咳平喘颗粒联合乙酰半胱氨酸治疗)。对比两组临床疗效、不良反应及治疗前、后肺功能[潮气量(VT)、达峰时间比(TPTEF/TE)、吸呼比(Ti/Te)、达峰容积比(VPEF/VE)]、中医证候、免疫功能、炎症因子[白细胞介素-8(IL-8)、趋化因子配体3(CCL3)、肿瘤坏死因子-α(TNF-α)、高迁移率族蛋白B1(HMGB1)]水平。结果 联合组总有效率为96.61%,高于参照组的83.05%(P<0.05);与参照组相比,治疗后联合组各中医证候积分均较低(P<0.05);联合组治疗后VT、TPTEF/TE、Ti/Te、VPEF/VE水平均较参照组高(P<0.05);联合组治疗后CD8+水平较参照组低,CD3+、CD4+、CD4+/CD8+水平较参照组高(P<0.05);治疗后联合组血清IL-8、CCL3、TNF-α、HMGB1水平均较参照组低(P<0.05);不良反应发生情况组间比较,无明显差异(P>0.05)。结论 清咳平喘颗粒联合乙酰半胱氨酸可提高痰热闭肺型LP患儿治疗效果,减轻临床症状,改善肺功能、免疫功能,降低机体炎症反应程度。
Objective To explore the therapeutic efficacy of Qingke Pingchuan granules combined with acetylcysteine on children with lobar pneumonia (LP) of phlegm-heat obstructing lung type. Methods A total of 118 children with LP of phlegm-heat obstructing lung type admitted to our hospital from January 2023 to November 2025 were retrospectively selected as the research subjects. According to different treatment regimens, they were divided into the control group (59 cases, treated with acetylcysteine) and the combined group (59 cases, treated with Qingke Pingchuan granules combined with acetylcysteine). The clinical efficacy, adverse reactions, as well as the levels of pulmonary function indicators [tidal volume (VT), time to peak tidal expiratory flow ratio (TPTEF/TE), inspiration-expiration ratio (Ti/Te), volume to peak tidal expiratory flow ratio (VPEF/VE)], traditional Chinese medicine (TCM) syndrome scores, immune function and inflammatory factors [interleukin-8 (IL-8), C-C motif chemokine ligand 3 (CCL3), tumor necrosis factor-α (TNF-α), high mobility group box 1 protein (HMGB1)] before and after treatment were compared between the two groups. Results The total effective rate of the combined group was 96.61%, which was higher than 83.05% of the control group (P<0.05). After treatment, the TCM syndrome scores of the combined group were lower than those of the control group (P<0.05). The levels of VT, TPTEF/TE, Ti/Te and VPEF/VE in the combined group were higher than those in the control group after treatment (P<0.05). After treatment, the level of CD8? in the combined group was lower, while the levels of CD3?, CD4? and CD4?/CD8? were higher than those in the control group (P<0.05). The serum levels of IL-8, CCL3, TNF-α and HMGB1 in the combined group were significantly lower than those in the control group after treatment (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion The combination of Qingke Pingchuan granules and acetylcysteine can enhance the therapeutic efficacy on children with LP of phlegm-heat obstructing lung type, relieve clinical symptoms, improve pulmonary function and immune function, and reduce systemic inflammatory response.
