中西医结合型公共卫生人才培养是应对复杂公共卫生挑战的重要方向,但国内外普遍存在整合医学与群体健康脱节、实践薄弱及标准缺失等问题。大连医科大学依托其国家重点学科及国家中西医协同“旗舰”医院,探索构建了以问题导向模块化课程、“临床—社区”双轨实践体系及量化评价追踪机制为核心的系统化培养模式。数据显示,新模式培养后学生“治未病”应用认知优秀率从28%提升至65%;社区老年高血压管理项目中,患者血压达标率提高25%;毕业1-3年的学生中93%认为该模式对处理复杂公共卫生问题“至关重要”。该模式直面国内外双重困境,实现了从知识拼接向能力融合、从理论讲授向实践闭环、从主观评价向数据追踪的三大创新,在系统性、实操性和可评估性上形成独特优势,为我国新医科背景下中西医结合型公共卫生人才培养提供了可复制、可量化的范式参考。

基于智慧教室的PBL模式在肾内科住院医师临床技能带教中的应用与启示

【摘要】 目的 评价基于智慧教室的PBL教学法在肾内科临床教学中的应用效果,并探讨该模式对提升临床科室带教同质化水平的潜在价值。方法 选取2023年9月—2024年10月在江苏大学附属医院肾内科轮转的临床医学专业学生及住院医师规范化培训学员共80名为研究对象,采用区组随机化方法分为实验组和对照组,每组40名。实验组在智慧教室环境中实施PBL教学,对照组采用传统讲授法。两组课堂学时完全一致(每周2次,每次2学时,共16学时),但实验组课前预习与课后拓展为自愿完成,未对课外学习时间进行量化记录。通过理论考核、临床技能评估、模拟临床演练评估及问卷调查评价教学效果。结果 Mini-CEX评估显示,实验组在临床判断(7.65±0.78 vs 6.82±0.85,P<0.001)和沟通技能(7.80±0.72 vs 6.95±0.88,P<0.001)维度改善最为明显,均从达标水平(4~6分)跨越至优秀水平(7~9分);临床技能考核中腹膜透析置管术成绩实验组(45.82±1.45分)高于对照组(42.15±2.03分)(t=9.24,P<0.001,Cohen's d=2.08)。实验组理论考核成绩(89.45±3.12)分高于对照组(84.23±4.56)分(t=5.89,P<0.001, Cohen's d=1.32),临床技能考核成绩(45.82±1.45)分高于对照组(42.15±2.03)分(t=9.24, P<0.001, Cohen's d=2.08)。实验组在学习动机、课堂互动性和学习效率方面自评优于对照组(P<0.05),但团队合作能力培养两组差异无统计学意义(62.5% vs 52.5%, P=0.340)。结论 该联合模式能有效促进规培生临床核心胜任力的转化,可为临床科室高仿真技能带教提供实操路径。

       脑卒中后认知障碍(PSCI)是脑卒中患者常见的并发症,严重影响患者的生活质量和社会功能。近年来,随着认知储备理论的发展和早期干预理念的普及,PSCI的早期干预模式逐渐成为研究热点。文章综述了PSCI早期干预模式的研究现状与进展,包括干预的理论基础、具体措施及未来发展方向,以期为临床实践和科研提供参考。
       Post-stroke cognitive impairment(PSCI)is a common complication among stroke patients.It seriously affects the patients’ quality of life and social functions.In recent years,with the development of the cognitive  reserve theory and the popularization of the concept of early intervention,the early intervention model for PSCI has gradually become a research hotspot.This article reviews the current research status and progress of the early intervention model for PSCI,including the theoretical basis of the intervention,specific measures,and future development directions,aiming to provide  references for clinical practice and scientific research.

       文章围绕康复治疗学专业创新创业人才培养模式展开研究,讨论了国内外高校创新创业人才培养现状。从教育理念、课程体系、师资力量、资源配套等方面探讨了人才培养所面临的问题。结合广东药科大学康复治疗学专业培养现状,从管理、教学、平台、服务四大体系明确了具体要求,有望为其他高校康复治疗学专业培养模式改革提供思路。
  The article focuses on innovative and entrepreneurial talent-cultivation models in the Rehabilitation Therapy specialty,discussing the current status of such cultivation in domestic and international universities.It explores challenges in talent development from perspectives including educational philosophy,curriculum system,faculty resources,and resource allocation.Based on the current training status of Guangdong Pharmaceutical University’s Rehabilitation Therapy Program,the study specifies detailed requirements through four major systems:management,teaching,platform,and service.This  research is expected to provide valuable insights for the  reform of talent cultivation models in  rehabilitation therapy programs at other higher education 
institutions.

炎症性肠病与 IgA 肾病共病特征基因及信号通路激活模式的整合分析

      目的   免疫球蛋白A肾病(IgAN)与炎症性肠病(IBD)的相互作用机制尚未阐明。本研究旨在解析IBD与IgAN共病的关键特征基因及核心信号通路,以揭示肠-肾轴的分子调控网络。方法   于GEO数据库获取IBD(GSE75214)和IgAN(GSE93798)基因表达谱,筛选差异表达基因(DEGs)。通过蛋白互作网络(PPI)和拓扑算法(MCC、MNC、Degree、EPC等)识别核心特征基因,并结合公共数据库(CTD、DISEASES和GeneCards)和单细胞转录组测序(GSE171314)进行验证。通过Nephroseq数据库验证基因表达与临床表型的相关性。结果   共筛选出17个IBD-IgAN共病DEGs,PPI网络分析等确定以FOS、EGR1、CXCL2JUNB为核心特征基因。功能富集分析显示白细胞介素-17(IL-17)信号通路显著激活。单细胞测序验证FOS、EGR1、CXCL2JUNB基因在IgAN特异性高表达,并通过Nephroseq数据库验证其与尿蛋白和估算的肾小球滤过率下降(eGFR)显著相关。结论  本研究揭示IBD与IgAN共享IL-17通路异常激活及FOS、EGR1、CXCL2JUNB的基因网络,为开发基于肠-肾轴调控的靶向治疗策略提供理论依据。
       Objective  The complex interplay between immunoglobulin A nephropathy(IgAN)and inflammatory bowel disease(IBD)remains poorly understood.This  study  aimed to identify  key  cross-talk  genes  and  pivotal  signaling pathways shared between IBD and IgAN,thereby elucidating the molecular regulatory network underlying the gut-kidney axis.Methods  Transcriptomic datasets for IBD(GSE75214)and IgAN(GSE93798)were retrieved from the GEO database.Differentially expressed genes(DEGs)were screened,and shared DEGs were intersected.Protein-protein interaction(PPI)networks were constructed using STRING and Cytoscape,with topological algorithms applied to identify hub genes.Gene expression profiles were validated through(CTD,DISEASES and GeneCards)and single-cell RNA sequencing(GSE171314)and the Nephroseq database,focusing on clinical correlations with proteinuria and estimated glomerular filtration rate(eGFR).Results  Seventeen shared DEGs were identified between IBD and IgAN.PPI network analysis revealed FOS,EGR1,CXCL2 and JUNB as core hub genes.Functional enrichment analysis demonstrated significant activation of the interleukin-17(IL-17)signaling pathway.Single-cell sequencing confirmed the specific upregulation of these genes in renal tubular epithelial cells of IgAN patients,which was further validated to correlate with proteinuria and eGFR decline.Conclusions  IBD and  IgAN share aberrant activation of the IL-17 pathway and a co-regulatory gene network involving FOS,EGR1,CXCL2 and JUNB,providing a theoretical foundation for developing therapeutic strategies centered on the gut-kidney axis.

中医诊断学是一门中医学专业的主干课程, 也是连接基础理论与临床实践的桥梁课程,其知识点繁杂、实践性强导致学生感觉枯燥无味, 教师教学质量欠佳。随着“互联网+教育”的高速发展, 混合式教学作为一种创新型教学模式,有诸多优势, 但也存在一些不足。文章从多元化教学方法、思政教育、多元化评价三个方面出发, 研究创新混合教学模式措施, 以期提升教学效果,提高学生的中医思维能力和临床水平, 培养学生的医德医风,实现知识传授与价值引领, 为社会输送德才兼备的中医药人才。文章研究了中医诊断学课程混合教学模式的教学现状和实践探索, 以进一步提高教学质量, 可为各大中医院校混合教学模式的开展提供借鉴。
Traditional Chinese medicine(TCM)diagnostics is a main course of TCM specialty, and it is also a bridge course connecting basic theory and clinical practice.Its complex knowledge and strong practicability lead to students feeling bored and poor teaching quality.With the rapid development of “Internet + Education”, blended teaching, as an innovative teaching mode,has many advantages, but it also has some shortcomings.This paper starts from three aspects of diversified teaching methods, civic education and diversified evaluation to study the measures of innovative mixed teaching mode, with a view to enhancing the teaching effect, improving the students’ thinking ability and clinical level of Chinese medicine, cultivating the students’ medical ethics and medical style,realizing the knowledge pass on and value leadership, and delivering both moral and talented talents in traditional Chinese medicine for the society.This paper studies the teaching status and practical exploration of the mixed teaching mode of TCM diagnostics curriculum in order to further improve the teaching quality and provide reference for the development of the mixed teaching mode in major TCM colleges.

基于儿童早期预警评分的分级干预模式促进重症肺炎患儿康复进程及对呼吸功能的影响

目的 研究基于儿童早期预警评分(PEWS)的分级干预模式促进重症肺炎患儿康复进程及对呼吸功能的影响。方法 回顾性分析2021年4月—2023年4月我院收治的100例重症肺炎患儿临床资料。将其按照干预方式的差异分为研究组(n=50)及对照组(n=50)。对照组选用常规干预,研究组则于对照组基础上增加基于PEWS的分级干预。对比两组康复进程(相关指标涵盖症状持续时长及住院天数)、呼吸功能(涵盖通气流速、每分钟最大通气量、肺活量、用力肺活量及深吸气量)、并发症发生情况(涵盖呼吸机相关性肺炎、肺大疱及胸膜炎)、患儿家属满意度。结果 研究组各项症状持续时长及住院天数均短于对照组(均P<0.05)。研究组各项呼吸功能指标水平均高于对照组(均P<0.05)。两组各项并发症发生率对比差异无统计学意义(P>0.05)。研究组患儿家属满意度高于对照组(96.00% vs 82.00%,P<0.05)。结论 基于PEWS的分级干预模式促进重症肺炎患儿康复进程的效果较佳,且能改善呼吸功能,提高患儿家属满意度。
Objective To study the effect of graded intervention mode based on Pediatric Early Warning Score(PEWS)on the recovery process and respiratory function of children with severe pneumonia.Methods The clinical data of 100 children with severe pneumonia treated in our hospital from April 2021 to April 2023 were retrospectively analyzed.Those children were divided into study group(n=50)and control group(n=50)according to the difference of intervention methods.Conventional intervention was used in the two groups,and PEWS-based graded intervention was added to the study group.The two groups were compared with each other in terms of recovery process(including duration of symptoms and length of stay),respiratory function(including ventilation velocity,maximum volume per minute,vital capacity,forced vital capacity and deep inspiratory capacity),complications(including ventilators associated pneumonia,bullosa and pleurisy),and family member satisfaction.Results The duration of symptoms and hospitalization days in the study group were shorter than those in the control group(all P<0.05).The levels of respiratory function indexes in study group were higher than those in control group(all P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).The satisfaction of family member in the study group was higher than that in the control group(96.00% vs 82.00%)(P<0.05).Conclusions The PEWS based graded intervention model has a good effect on promoting the rehabilitation process of children with severe pneumonia,and can improve respiratory function,and increase the satisfaction of family member of children with severe pneumonia.

共同照护模式联合回授法健康教育对2型糖尿病患者病情控制的影响

目的 分析共同照护模式联合回授法健康教育在2型糖尿病患者中的应用效果。方法 将2023年1月—2023年12月广州市第一人民医院收治 的114例2型糖尿病患者作为此次研究对象,分为研究组(n=57)和对照组(n=57),对照组患者给予常规护理结合健康教育,研究组患者给予共同照护模式联合回授法健康教育,评估两组在培训前及培训后1、3个月的血糖控制情况及糖尿病患者自我管理行为量表(SDSCA)等指标。结果 培训1、3个月后,两组患者的空腹血糖、餐后2 h血糖、糖化血红蛋白均低于培训前,且研究组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均低于对照组(P<0.05)。培训1、3个月后,两组患者的糖尿病患者SDSCA评分高于培训前,且研究组各项评分高于对照组(P<0.05)。重复测量方差分析表明随着时间的推移,患者的血糖控制状况随着不同护理培训策略而得到改善。结论 使用共同照护模式联合回授法健康教育对2型糖尿病患者可以更显著改善血糖控制水平、提高自我管理效能。
Objective To analyze the effect of co-care mode and feedback health education on patients with type 2 diabetes.Methods From January 2023 to December 2023,114 patients with type 2 diabetes admitted to Guangzhou First People's Hospital were divided into two groups:experimental group(n=57)and control group(n=57).Patients in control group were given routine nursing combined with health education.Patients in study group were given co-care and feedback health education. Before intervention and 1,3 months after intervention,patients' blood glucose control and diabetic self-management behavior scale(SDSCA)were evaluated.Results After 1 and 3 months of intervention,the fasting blood glucose,2-hour blood glucose and HbA1c of the two groups were significantly lower than those before intervention,and the fasting blood glucose,2-hour blood glucose and HbA1c of the experimental group were significantly lower than those of the control group(P<0.05).After 1 month and 3 months of intervention,the SDSCA scores of diabetic patients in both groups were higher than those before intervention,and the scores of study group were higher than those of control group(P<0.05).Repeated measures analysis of variance indicating that glycemic control improved significantly over time with different nursing training strategies.Conclusions Co-care mode and feedback health education can significantly improve blood glucose control and self-management efficiency in patients with type 2 diabetes.

BOPPPS模式下模块化教学结合模拟演练在创伤急救培训中的应用

目的 探讨BOPPPS[(导引(B),目标(O),课前测试(P),参与式学习(P),课后测试(P),总结(S)]模式下模块化教学结合模拟演练在创伤急救培训中的应用效果。方法 选取福建省泉州市正骨医院急诊科2022年8月—2024年2月90名护士,随机分为对照组和干预组,每组45名护士。对照组采用传统授课模式进行教学培训;干预组采用BOPPPS模式下模块化教学结合模拟演练进行教学培训。培训结束对两组学员的创伤急救综合理论及单项技能、创伤急救救护综合能力、培训效果满意度进行统计分析。结果 干预组的综合理论成绩为(80.74±6.87)分、单项技能成绩为(92.13±2.26)分,相较于对照组均提高(P<0.05)。干预组对创伤急救课程的总体满意度:非常同意23例、同意20例,比例均高于对照组(P<0.05)。干预组的创伤急救救护综合能力如病情评估预判能力,优秀26例、良好15例;护理计划分析实施能力,优秀25例、良好13例;团队协作与资源管理能力,优秀27例、良好15例;情景感知与应变能力,优秀30例、良好11例;综合救护能力,优秀36例、良好5例。显著优于对照组(P<0.05)。结论 BOPPPS模式下模块化教学结合模拟演练能够提高急诊科护士的综合创伤救护能力,且取得了较高的满意度,值得进一步推广应用。
Objective To explore the application effect of modular teaching combined with simulation exercise in trauma first aid training under BOPPPS model.Methods A total of 90 nurses in the emergency department from August 2022 to February 2024 were randomly divided into control group(n=45) and intervention group(n=45).The control group was trained by traditional teaching mode.The intervention group was trained by BOPPPS modular teaching combined with simulation exercise.After the training,the comprehensive theory of trauma first aid,individual skills,comprehensive ability of trauma first aid and satisfaction degree of training effect of the two groups of nurses were statistically analyzed.Results The comprehensive theoretical scores and single skill scores of the intervention group were significantly improved compared with the control group(P<0.05).The satisfaction degree of trauma first aid training in the intervention group was significantly higher than that in the control group(P<0.05).The comprehensive ability of trauma first aid in the intervention group was significantly better than that in the control group(P<0.05).Conclusions The combination of modular teaching and simulation exercise in BOPPPS model can improve the comprehensive trauma rescue ability of nurses in emergency department,and has obtained a high degree of satisfaction,which is worthy of further popularization and application.

医院-社区-家庭一体化康复护理模式干预对慢性创面患者生活质量及创面愈合的影响研究

目的 评估医院-社区-家庭一体化康复护理模式对慢性创面患者生活质量及创面愈合的潜在影响。方法 选择2023年1月—2024年6月在广州市第一人民医院接受治疗的慢性创面患者80例作为研究对象,所有患者在住院期间均接受基于溃疡面积、深度及是否合并感染等因素的综合治疗,包括彻底清创、创缘处理、负压治疗、感染控制等治疗,并接受常规护理。出院前,通过随机数字表法将患者分为两组,每组40例。两组患者在住院期间均接受常规护理,出院后,对照组接受延续护理并定期复查。干预组40例患者在出院后接受医院-社区-家庭一体化康复护理模式。入组时及护理3个月后,使用生活质量调查表(SF-36)对患者生活质量和创面愈合率进行评估。结果 干预组创面愈合率为(32.61±4.26)%,高于对照组(11.48±1.04)%,差异具有统计学意义(t=19.473,P<0.05)。两组患者在护理3个月后的数字评定量表评分和创面疼痛频率评分均较入组时降低(P<0.05)。其中干预组护理3个月后的创面VAS评分为(1.82±0.17)分,创面疼痛频率评分为(1.28±0.25)分;而对照组分别为(3.91±0.22)分和(2.63±0.37)分,干预组低于对照组(P<0.05)。此外,干预组在护理后3个月的总体健康、生理功能、生理职能、精神健康、情感功能、社会功能评分均高于对照组(P<0.05)。在居家3个月期间,干预组的感染发生率为5.00%,而对照组为17.50%,干预组感染发生率低于对照组(χ2=3.781,P<0.05)。结论 本研究表明,医院-社区-家庭一体化康复护理模式干预能够促进慢性创面患者的创面愈合,降低居家期间感染的风险,并提升患者的生活质量。
Objective To assess the potential impact of the integrated hospital-community-home rehabilitation nursing model on the quality of life and wound healing in patients with chronic wounds.Methods A total of 80 patients with chronic wounds treated at the Guangzhou First People’s Hospital from January 2023 to June 2024 were selected as the study subjects.All patients received comprehensive treatment during their hospital stay,including thorough debridement,edge treatment,vacuum therapy,infection control and routine nursing care.Prior to discharge,the patients were randomly divided into two groups using a random number table,with 40 patients in each group.Both groups received routine nursing care during their hospital stay,and the control group received continuing nursing care and regular follow-up after discharge.Forty patients in the intervention group received the integrated hospital-community-home rehabilitation nursing model after discharge.Quality of life(QoL)and wound healing rates were assessed using the Short Form 36(SF-36)questionnaire at the time of enrollment and 3 months after nursing.Results The wound healing rate in the intervention group was significantly higher than that in the control group ([32.61±4.26]% vs [11.48±1.04]%),with a statistical difference(t=19.473,P<0.05).The numerical rating scale(NRS)scores and frequency of wound pain scores decreased in both groups 3 months after nursing compared to the enrollment period(P<0.05).Specifically,the VAS score for wound pain in the intervention group 3 months after nursing was(1.82±0.17),and the frequency of wound pain was(1.28±0.25),in the control group,these scores were(3.91±0.22)and(2.63±0.37),respectively,with the intervention group scoring significantly lower than the control group(P<0.05).Furthermore,the scores for overall health,physical function,role physical,mental health,emotional function,and social function in the intervention group were higher than those in the control group 3 months after nursing(P<0.05).During the 3-month home recovery period,the incidence of infection in the intervention group was 5.00%,whereas it was 17.50% in the control group,with the intervention group showing a lower incidence of infection(χ2=3.781,P<0.05).Conclusions This study demonstrates that the hospital-community-home integrated rehabilitation care model intervention can promote wound healing in chronic wound patients,reduce the risk of infection during home care,and significantly improve patients’ quality of life.
