目的 分析肺肝样腺癌(HAL)的临床病理特征、诊断、免疫表型、基因检测及治疗预后等。方法 对1例HAL临床及影像学、组织学形态、免疫组化及基因检测结果等进行观察,并结合相关文献综合分析。结果 患者为48岁吸烟男性,镜下肿瘤具有肝细胞样和腺样分化特征,血清AFP升高。免疫组化: Hepatocyte,AFP, Arginase-1均阳性,ARMS-PCR法均未检测到EGFR,ALK/ROS1,KRAS及BRAF突变。结合相关文献分析: HAL常见于有吸烟史的男性,血清AFP值升高也是该肿瘤的一个特点。肿物多见于肺上叶,体积较大,易发生淋巴结和远处转移,预后相对较差。结论 HAL非常少见,易误诊,其诊断需结合形态学特点、临床病理特征及免疫组化结果等。
Objective To explore the clinicopathologic characteristics, diagnosis, immunophenotype, gene detection and prognosis of primary hepatoid adenocarcinoma of the lung (HAL). Methods A case of hepatoid adenocarcinoma of the lung was analyzed with clinical manifestations, histology, immunohistochemical staining and gene detection, and relevant literatures were reviewed. Results The patient was a 48-years-old man with smoking history. Microscopically, the tumor has the characteristics of hepatocellular carcinoma and adenoid differentiation, also serum AFP was elevated. The immunohistochemical results showed that Hepatocyte, AFP and Arginase-1were positive. No mutations were detected for EGFR, ALK/ROS1, KRAS and BRAF by ARMS-PCR. Combining with literature analysis, HAL is common in males and most patients with this tumor are smokers. Serum AFP in very high levels has been a distinguishing feature of this tumor. HAL usually presents as a large bulky solitary mass in the upper lobe. Lymph nodes and distant metastases are prone to occur. Therefore, the prognosis is very poor. Conclusion HAL is a rare malignant tumor and easy to be misdiagnosed. The diagnosis of primary hepatoid adenocarcinoma of the lung should be combined with morphological features, clinicopathological features and immunohistochemical findings.
临床诊疗
目的 探讨无创呼吸机辅助常规药物治疗重症哮喘的疗效及对肺功能的影响。方法 选取我院于2014年2月—2017年6月间收治的60例重症哮喘患者作为研究对象,按照随机数字法分为对照组和研究组,对照组30例予以常规药物治疗,研究组30例予以无创呼吸机辅助常规药物治疗。比较两组患者临床疗效、肺功能指标、气血指标、心率、哮喘症状评分以及药物使用情况。结果 研究组临床有效率(96.66%)高于对照组(76.66%)(P<0.05)。2组患者干预前肺功能指标、气血指标及心率比较无差异(P>0.05),干预后两组心率、PaCO2均较干预前降低,PaO2、FEV1、FEV1/FVC均较干预前升高(P<0.05),研究组较对照组改善明显(P<0.05)。两组患者干预前哮喘症状评分比较无差异(P>0.05),干预后两组哮喘症状评分均较干预前降低,且研究组较对照组降低明显(P<0.05)。研究组使用茶碱类药物、糖皮质类激素、受体激动剂药物比例均低于对照组(P<0.05)。结论 无创呼吸机辅助常规药物治疗重症哮喘可显著改善患者血气指标及肺功能,疗效确切。
临床诊疗
目的 了解足月小样儿生后早期的潮气呼吸肺功能及1岁内发生喘息的情况。方法 选择我院2015年3月—2016年3月阴道分娩的足月小样儿及同期胎龄相近、阴道分娩的健康足月儿,比较2组生后5~7天的潮气呼吸肺功能各项指标及1岁内发生喘息的次数。结果 2组出生胎龄、性别、Apgar评分、母亲吸烟史、一级亲属中哮喘史比较均无差异;研究组中分钟通气量(MV)、潮气量(VT/kg)、达峰时间比(TPEF/TE)、达峰容积比(VPEF/VE)均低于对照组,差异有统计学意义(P<0.05);呼吸频率(RR)、吸气时间(Ti)、呼气时间(Te)、吸呼比(Ti/Te)、潮气呼吸呼气峰流(PTEF)比较无差异(P>0.05);研究组1岁内发生喘息的次数多于对照组,差异有统计学差异。结论 足月小样儿生后早期肺容量小于健康足月儿,小气道阻塞更明显,可能影响1岁内发生喘息的几率。

目的 探讨CD117在肺基底样细胞鳞状细胞癌(BSCC)与小细胞肺癌(SCLC)中的鉴别诊断作用。方法 免疫组织化学检测CD117在肺BSCC与SCLC中的表达情况,并进行统计学分析,同时文献复习CD117在这两种肿瘤中的表达情况。结果 CD117在SCLC中特异性高表达(阳性率为78%),并且具有高的敏感度、特异度、阳性预测值及阴性预测值(分别为100%、78.2%、68.7%、100%),与文献报道一致(阳性率37%~100%);而在BSCC中未见有CD117的表达,文献未见有CD117在BSCC中表达情况报道。结论 CD117有可能成为鉴别诊断BSCC与SCLC的重要指标之一。但由于本报道例数有限,还需要更大型的研究进一步证明CD117在鉴别诊断二者中的敏感度及特异度。
Objective To investigate the role of CD117 in differentiating basaloid squamous cell carcinoma (BSCC) from small cell carcinoma (SCLC) in lung. Methods Immunohistochemistry staining of CD117 and statistic data were analyzed in BSCC and SCLC, and relevant literature were reviewed. Results CD117 was specifically expressed in high level ( positive rate 78%) in SCLC with high sensitivity, specificity, positive predictive value and negative predictive value (respectively 100%、78.2%、68.7%、100%), which was highly consistent with literature reports (positive rate 37%-100%). CD117 was not detected in BSCC in our report and it was the same as in literatures. Conclusion Although our results showed that it was possible that the expression of CD117 would play an important role in differentiating BSCC from SCLC, due to limited cases in number, more studies are needed to elucidated the sensitivity and specificity.

目的 探讨肺结核合并肺癌患者的CT影像特征,为临床疾病诊断提供参考依据。方法 选取2015年—2018年我院收治的肺结核合并肺癌患者50例作为观察组,另选取同期于我院治疗的单纯肺结核患者50例为对照组。观察两组患者临床症状及CT影像表现并作出对比分析。结果 两组患者临床症状及体征相比,差异无统计学意义(P>0.05);观察组患者分叶征、毛刺状结节比例高于对照组患者,差异有统计学意义(P<0.01),观察组患者实质内空洞比例低于对照组患者,差异有统计学意义(P<0.01),两组患者肿物及条索影发生比例相比差异无统计学意义(P>0.05)。结论 肺结核合并肺癌患者的CT影像学特征主要在分叶征、毛刺状结节和空洞症,临床应根据这些特征并结合其他检查进行定性诊断。
Objective To investigate the CT images of patients with pulmonary tuberculosis complicated with lung cancer and provide reference for the diagnose. Methods 50 cases of pulmonary tuberculosis and lung cancer treated in our hospital in 2015~2018 in January were selected as observation group, and 50 cases of simple pulmonary tuberculosis treated in our hospital for the same period were selected as control group. The clinical symptoms and CT imaging findings of the two groups were observed and compared. Results There was no significant difference in clinical symptoms and signs between the two groups (P>0.05), and the proportion of lobular sign and burr nodular nodules in the observation group was higher than that of the control group (P<0.01), and the proportion of parenchymal cavities in the observation group was lower than that of the control group (P<0.01). There was no significant difference between the two groups in the incidence of tumor and streak shadow (P>0.05). Conclusion The CT imaging features of patients with pulmonary tuberculosis and lung cancer are mainly lobular sign, burr like nodules and cavities, which should be qualitatively diagnosed according to these features combined with other tests.

目的 在原来研究的基础上进一步研究Wnt-1信号通路蛋白-3(WISP-3)在高氧诱导肺上皮细胞凋亡中的作用。方法 通过Western blot检测和免疫组化检测不同肺上皮细胞中WISP-3的蛋白表达量。利用质粒转染和siRNA的方法在Beas-2B细胞中高表达和基因沉默WISP-3,通过细胞活性检测和流式细胞学技术检测高氧刺激后细胞的凋亡情况。结果 与空气对照相比,高氧刺激使肺上皮细胞的WISP-3蛋白表达量下降;WISP-3基因沉默或高表达使高氧诱导的肺上皮细胞凋亡增加或减少。结论 高氧刺激下,肺上皮细胞中WISP-3表达下降,WISP-3对高氧诱导的肺上皮细胞凋亡具有保护作用。
Objective To explore how Wnt-1 inducible signaling pathway protein-3 (WISP-3) participate in and play a regulatory role in the process of hyperoxia induced apoptosis in lung epithelial cells. Methods The expression of WISP-3 was detected via Western blot and immunohistochemistry. High expression and low expression of WISP-3 were performed by plasmid transfection and siRNA. Cell viability and flow cytometry were executed to detect the hyperoxia-induced apoptosis in Beas-2B. Results Compared to the group of air control,the expression of WISP-3 protein in lung epithelial cells decreased obviously after hyperoxia. Cell survival decrease and apoptosis increased after hyperoxia in Beas-2B cells with low expression of WISP-3. Vice versa. Conclusion The expression of WISP-3 decreased after hyperoxia in lung epithelial cells. The role of WISP-3 in this process may be protective.

目的 本研究拟成功复制犬热蒸汽吸入性肺损伤模型,并探讨急性吸入性肺损伤早期重要生命体征的变化情况,以及与心肺功能密切相关的连续心输出量(CCO)、心排指数(CI)、每搏量变异度(SVV)等核心血流动力学指标的变化规律。方法 建立犬热蒸汽吸入性肺损伤模型,动物随机分为4组,对照组(未予以致伤)。肺水肿组:根据肺损伤肺水肿后观察时间的不同分为B1组:1 h;B2组:2 h及B3组:4 h。试验中监测ABP、SVV、CO等血流动力学指标。结果 犬蒸汽吸入性肺损伤致伤后,早期即发生循环动力学的变化,以致伤后30min至1 h最为明显,ABP、HR、CO持续下降,SVV较前升高。结论 犬蒸汽吸入性肺损伤致伤后,早期即发生循环动力学的变化,造成动物氧合及代谢障碍,并可以迅速导致肺水肿,以致伤后30min至1 h最为明显。
Objective To establish a model of acute inhalation injury in dogs, and to investigate the changes of vital signs in the early stage of acute inhalation injury and the correlation between cardiac output (CCO), cardiac output index CI), stroke volume variability (SVV) and other core hemodynamic variables. Methods The animal models of inhaled lung injury were established. The animals were randomly divided into four groups: control group (no injury); Lung edema group: according to the observation time of lung injury after lung injury, they were divided into B1 group: 1 h; B2 group: 2 h and B3 group: 4 h. The hemodynamic indexes such as ABP, SVV and CO were monitored. Results The changes of circulatory kinetics occurred in the early stage of the injury after aspiration of canine vapor inhalation, which was most obvious in 30 minutes to 1 hour after injury. The levels of ABP, HR, CO were decreased and SVV was increased. Conclusion Inhalation of lung injury in dogs after inhalation, early changes in the occurrence of circulatory dynamics occured, resulting in animal oxygenation and metabolic disorders, and these can quickly lead to pulmonary edema, so that 30 minutes to 1 hour after injury, hemodynamic chcnge were the most obvious.
临床诊疗
目的 探讨吸烟对稳定期COPD患者炎症反应和肺功能的影响。方法 选取2013年8月—2016年9月我院门诊收治的稳定期COPD患者70例为研究对象,其中吸烟35例(X1组)、不吸烟35例(X2组),另选取同期入院的不吸烟健康志愿者35例纳入健康组,采用酶联免疫吸附试验(ELISA)测定血清白介素-6(IL-6)、白细胞介素-8(IL-8)及肿瘤坏死因子-α(TNF-α)水平,以肺功能检测仪测定三组一秒用力呼气容积(FEV1)、一秒用力呼气容积/用力肺活量比值(FEV1/FVC)、FEV1占预计值百分比(FEV1%),并采用自拟症状评分表及简明健康调查简表(SF-36)评价呼吸困难程度及生活质量。结果 X1组IL-6、IL-8及TNF-α依次为(135.27±1.24)pg/mL、(189.45±1.14)pg/mL、(39.39±1.14)pg/mL,明显高于X2组、健康组(P均<0.05);X1组FEV1(0.75±0.14)L、FEV1/FVC(3.65±1.87)%、FEV1%(3.45±0.12)%低于X2组、健康组(P均<0.05);X1组症状积分(10.17±1.02)分较X2组、健康对照组高(P<0.05),而其SF-36评分(54.27±1.46)分明显低于X2及健康组(P<0.05);X2组上述指标与健康组比较亦有统计学意义(P均<0.05)。结论 吸烟可明显增加稳定期COPD患者IL-6、IL-8、TNF-α等炎症因子水平,同时降低肺功能,临床应采取措施进行有效干预,防止患者病情恶化。

目的 探讨超声引导下经皮肺穿刺活检术在艾滋病周围型肺部病变中的应用价值。方法 我院52例经皮肺穿刺活检的病例,术前均采用高效抗逆转录病毒治疗,依据CD4+T淋巴细胞确定是否手术,术中规范手术方式,做好防护措施,27例行超声引导穿刺,25例行CT引导穿刺,术后应用抗生素预防机会性感染等。结果 所有患者穿刺成功率100%,超声组肺结核13例,真菌感染67例,细菌感染4例,肺腺样囊性癌2例、卡波西肉瘤1例。CT组肺结核11例,真菌感染7例,细菌感染5例,肺腺样囊性癌1例。术后并发症:超声组5例病灶少量气胸,2例周围少量出血,咯血4例;CT组8例病灶少量气胸,4例周围少量出血,咯血7例。结论 经过术前控制患者计数及预防感染治疗,超声引导下经皮肺穿刺活检术在艾滋病周围型肺部病变中应用更为安全、有效、可行的,值得临床应用及推广。
Objective Objective to investigate the value of ultrasound-guided percutaneous lung biopsy in the diagnosis of peripheral lung lesions in AIDS patients. Methods 52 cases in our hospital of percutaneous pulmonary biopsy cases, preoperative highly active antiretroviral therapy was used, basis of CD4+T lymphocytes count determined whether the operation were performed, standard operation mode in the operation. It must to take good protective measures. 27 cases underwent ultrasound guided puncture, 25 cases underwent CT guided puncture and had postoperative application of antibiotics to prevent opportunistic infections. Results The successful rate of puncture was 100% in all patients. Tuberculosis in the ultrasound group was 13 cases, fungal infection was 67 cases, bacterial infection was 4 cases, pulmonary adenoid cystic carcinoma was 2 cases, and Posey's sarcoma was 1 cases. There were 11 cases of pulmonary tuberculosis in CT group, 7 cases in fungal infection, 5 cases in bacterial infection, and 1 cases in adenoid cystic carcinoma of the lung. Postoperative complications included: 5 cases of small pneumothorax in the ultrasound group, a small amount of bleeding around the surrounding, hemoptysis in 4 cases, CT group of patients with 8 cases of a small amount of pneumothorax in the lesions, 4 cases of a small amount of bleeding around the case, 7 cases of hemoptysis. Conclusion After preoperative control count and prevention of infection in patients with treatment, ultrasound guided percutaneous lung biopsy in peripheral pulmonary lesions of AIDS is more safe and effective, worthy of clinical application and promotion.

目的 探讨加味六君子汤联合经口营养补充对COPD稳定期伴营养不良患者营养状况和肺功能的影响。方法 收集呼吸内科COPD稳定期并伴营养不良的患者40例,分为试验组和对照组各20例。试验组在对照组基础上再给予加味六君子汤和经口营养补充,观察治疗前、治疗后3月的营养相关指标和肺功能指标,然后进行统计分析。结果 ①治疗后3月,试验组的体质量、白蛋白、前白蛋白和肺功能指标较治疗前均明显改善(P<0.05)。②与对照组比较,试验组的白蛋白、前白蛋白、血红蛋白均明显升高(P<0.05),肺功能亦有显著改善(P<0.05)。③上臂肌围(MAMC)、三头肌皮褶厚度(TSF)在治疗后的组内及组间比较差异均无统计学意义(P>0.05)。结论 加味六君子汤联合经口营养补充有助于COPD稳定期伴营养不良患者体质量增加、蛋白升高,营养状况好转,从而增加呼吸肌储备,改善患者肺功能。
Objective To observe the influence of modified-liujunzi-decoction combined with Oral nutritional supplements(ONS) on the nutrition status and lung function in patients with chronic obstructive pulmonary disease(COPD). Methods 40 patients of COPD stable phase with malnutrition were divided into two groups and each group with 20 cases. On the basis of the control group, the experimental group was given again modified-liujunzi-decoction and ONS. All cases were tested before and 3 months after treatment, for detecting the nutrition related index and lung function index; T-test was used for data statistical analysis. Results ①3 months after treatment, the body weight, serum albumin, prealbumin and lung function index of the experimental group were significantly improved compared with those before treatment (P<0.05). ②Compared with the control group, the serum albumin, prealbumin and hemoglobin were significantly increased in the experimental group (P<0.05), and lung function was also significantly improved (P<0.05). ③After treatment, the mid-arm muscle circumference (MAMC), triceps skinfold thickness (TSF) comparison of intra group and intra group differences were not statistically significant(P>0.05). Conclusion Modified-liujunzi -decoction combined with oral nutritional supplements help to increase body weight, protein, and improve nutritional status in patients of COPD stable phase with malnutrition, thereby increasing their respiratory muscle reserve and improving lung function.
