胃镜下幽门肌切开术对先天性肥厚性幽门狭窄患儿幽门部黏膜影响的研究

目的 了解CHPS患儿经胃镜下幽门肌切开术治疗后幽门部黏膜近期各种并发症的发生情况及对预后的影响。方法 选取2006—2012年我院新生儿科确诊的34例CHPS患儿。以超细胃镜进入十二指肠乳头处,使用电切刀按上行性方向,沿幽门管纵轴切割肥厚狭窄的幽门管黏膜及环形肌以解除幽门部梗阻。术后第1、3、6月复查胃镜。结果 34例CHPS患儿共行胃镜下幽门肌切开术48次,均未发生胃及幽门穿孔、撕裂等严重并发症。24例患儿术后通过胃镜进行了复查,胃镜见幽门部及黏膜改变主要有黏膜充血肿胀、切口未完全愈合、切口疤痕形成、幽门部息肉形成等。结论 经胃镜下幽门肌切开术是一种安全性较高的治疗CHPS的新技术。术后黏膜充血肿胀、切口未完全愈合等幽门部黏膜改变的发生率随时间下降。切口疤痕形成并未影响幽门功能。

超声定位在重度水肿患儿颈内静脉置管的应用

目的 探讨超声定位技术在重度水肿患儿颈内静脉置管中的应用效果。方法 选择重度水肿患儿23例,年龄1岁4个月~6岁,在超声引导下行颈内静脉置管。结果 本组23例患者均置管成功,置管成功率为100%,一次性穿刺成功率为91.3%,无1例发生出血、血气胸、血肿、空气栓塞等并发症。结论 超声引导定位重度水肿患儿颈内静脉置管,能有效提高穿刺成功率,减少并发症的发生。

目的 探讨对注意缺陷多动障碍(ADHD)患儿联合应用维生素D与盐酸托莫西汀的效果及对其行为功能障碍和神经功能损伤的影响。方法 选择于我院接受治疗的105例ADHD患儿,纳入时间为2023年7月—2024年3月,按照计算机分组法分为对照组52例给予盐酸托莫西汀治疗,观察组53例给予维生素D联合盐酸托莫西汀治疗,比较两组临床疗效、神经与行为功能、不良反应。结果 观察组总有效率(96.23%)高于对照组(78.85%)(P<0.05)。治疗后,与对照组比较,观察组神经元特异性烯醇化酶水平与行为、学习、躯体、冲动多动、焦虑及多动指数评分更低(P<0.05)。两组不良反应发生率对比差异无统计学意义(P>0.05)。结论 对于ADHD患儿,使用维生素D联合盐酸托莫西汀显示出更为显著的疗效,能有效缓解行为功能障碍,减少神经功能损伤,且具有良好的安全性。
Objective To explore the effect of combined use of vitamin D and atomoxetine hydrochloride on children with attention deficit hyperactivity disorder(ADHD)and its impact on their behavioral dysfunction and neurological damage. Methods A total of 105 children with ADHD treated in the hospital were included from July 2023 to March 2024. They were divided into control group with 52 cases treated with atomoxetine hydrochloride, and observation group with 53 cases treated with vitamin D combined with atomoxetine hydrochloride using a computerized grouping method. The clinical efficacy, neurological function, behavioral function, and adverse reactions were observed in both groups. Results The total effective rate of the observation group(96. 23%)was higher than that of the control group(78. 85%)(P<0. 05). After treatment, compared with the control group, the levels of neuron-specific enolase in the observation group were significantly lower in terms of behavior, learning, physical fitness, impulsivity hyperactivity, anxiety and hyperactivity index scores(P<0. 05). The comparison of the incidence of adverse effects between the two groups was not significant(P>0. 05). Conclusions The combined use of vitamin D and atomoxetine hydrochloride has a more significant therapeutic effect on children with ADHD, which can effectively alleviate behavioral dysfunction, reduce neurological damage, and has good safety.

激素治疗对原发性肾病综合征患儿淋巴细胞亚群及免疫因子的影响

       目的   分析原发性肾病综合征(PNS)患儿在糖皮质激素(激素)治疗后淋巴细胞亚群及免疫因子的水平变化,以探讨PNS耐药机制。方法   选取PNS患儿共71例,正常对照组108例,收集PNS患者在激素治疗前、后及正常对照组儿童的淋巴细胞亚群[CD4+ 、CD8+ 、CD4+ /CD8+ 、CD19+ 和自然杀伤(NK)细胞]及免疫因子水平,并分析激素治疗后激素敏感患儿和激素耐药患儿相关指标的差异。结果  PNS患儿淋巴细胞亚群及免疫因子水平异常,激素治疗后PNS患儿总免疫球蛋白E(IgE)水平[767.50(270.25,1 937.50)IU/mL vs 311.00(62.70,757.00)IU/mL](P=0.008)下降,而CD4+ T细胞比例[(33.88±7.42)% vs(38.25±7.16)%](P=0.004)升高,激素治疗敏感患儿NK细胞比例高于激素治疗耐药患儿[(8.39±4.60)% vs(4.72±1.99)%](P=0.034),IgE水平低于耐药患儿[311.00(62.70,633.00)IU/mL vs783.00(88.05,1 290.00)IU/mL](P<0.001)。结论  PNS患儿淋巴细胞亚群分布及免疫球蛋白水平异常,激素治疗可影响患儿CD4+ T细胞比例及IgE水平,并且NK细胞比例和IgE水平与患儿激素耐药相关。
       Objective  To evaluate the changes of lymphocyte subsets and immune factors levels in children with primary nephrotic syndrome(PNS),and explore the pathogenesis of PNS.Methods  A total of 71 patients with PNS and 108 normal control cases were selected.Flow cytometry was used to detect the concentration of lymphocyte subsets(CD4+ ,CD8+ ,CD4+ /CD8+ ,CD19+  and natural killer[NK] cells)and immune factors before and after treatment.The difference of  related factors between steroid-sensitive and steroid-resistant children after therapy were analyzed.Results  Lymphocyte subsets and immune molecule levels were abnormal in children with NPS.The level of IgE(767.50[270.25,1 937.50]IU/mL vs 311.00[62.70,757.00]IU/mL,P=0.008)was significantly decreased after therapy(P<0.05),while CD4+  T cells([33.88±7.42]% vs[38.25±7.16]%,P=0.004)were significantly increased.The level of NK cells in steroid-sensitive children was significantly higher than that in steroid-resistant children([8.39±4.60]% vs[4.72±1.99]%,P=0.034),while the level of  IgE was significantly lower than that of steroid -resistant children(311.00[62.70,633.00]IU/mL vs 783.00[88.05,1 290.00]IU/mL,P<0.001).Conclusions  The distribution of lymphocyte subsets and the level of immune factors in PNS children were abnormal.Steroid therapy could affect the levels of CD4+  T cells and IgE,and the levels of NK cells and IgE were related to steroid-resistance in PNS children.

支气管镜联合雾化吸入药物对肺炎支原体肺炎患儿症状持续时间及肺部影像学好转情况分析

Analysis of the duration of symptoms and pulmonary imaging conversion in children with Mycoplasma pneumoniae pneumonia treated with bronchoscopy combined with nebulized inhalation of medication

        目的   探究支气管镜联合雾化吸入药物对肺炎支原体肺炎患儿症状持续时间及肺部影像学好转情况。方法   选取94例肺炎支原体肺炎患儿为研究对象,以随机数字表法分为A组、B组,各47例,分别实施布地奈德雾化吸入治疗、支气管镜肺泡灌洗联合布地奈德雾化吸入治疗,比较两组症状持续时间、治疗后肺部影像改善情况、炎症指标水平及不良反应发生率。结果  B组体温恢复时间(2.73±0.51)d、咳嗽消失时间(5.98±1.24)d、住院时间(10.96±3.36)d,A组分别为(3.14±0.83)(7.06±2.33)(13.27±3.18)d,B组较A组短(t=2.885、2.809、3.423,均P<0.05);治疗后B组40.43%阴影完全消失、34.04%阴影显著缩小、23.40%阴影有所缩小、2.13%阴影改善不明显,A组分别为21.28%、36.17%、25.53%、17.02%,B组肺部阴影改善情况优于A组(Z=8.311,P<0.05);治疗前B组白细胞计数(WBC)、超敏C-反应蛋白(hs-CRP)、降钙素原(PCT)水平与A组相近(P>0.05);治疗后B组WBC(7.71±1.94)×109 /L、hs-CRP(4.96±1.44)mg/L、PCT(84.32±21.40)pg/mL,A组分别为(9.05±2.48)×109 /L、(6.17±1.85)mg/L、(105.46±34.02)pg/mL,B组水平较A组低(t=2.918、3.538、3.606,均P<0.05);B组不良反应发生率为8.52%,A组为4.26%,B与A组相近(χ 2 =0.178,P>0.05)。结论   对肺炎支原体肺炎患儿实施支气管镜联合雾化吸入药物治疗,可缩短康复时间,促进肺部阴影消退,降低其炎症指标水平,且未增加不良反应发生率。
     Objective  To investigate the effect of bronchoscopy combined with nebulized inhalation of medication on the duration of symptoms and pulmonary imaging conversion in children with Mycoplasma pneumoniae pneumonia(MPP).Methods  A total of 94 children with MPP were selected as the research subjects and randomly divided into Group A and Group B using a random number table,with 47 cases in each group.They were treated with budesonide nebulization inhalation therapy and bronchoalveolar lavage combined with budesonide nebulization inhalation therapy,respectively.The duration of symptoms,improvement of lung imaging before and after treatment,levels of inflammatory indicators,and incidence of adverse  reactions were compared between the two groups.The results showed that the temperature recovery time was(2.73±0.51)days,cough disappearance time was(5.98±1.24)days,and hospitalization time was(10.96±3.36)days in Group B,(3.14±0.83)days,(7.06±2.33)days,and(13.27±3.18)days in Group A,respectively.Durpation in Group B was shorter than Group A(t=2.885,2.809,3.423,all P<0.05). After treatment,40.43% of the shadows in Group B completely disappeared,34.04% of the shadows significantly reduced,23.40% of the shadows reduced,and 2.13% of the shadows showed no significant improvement,better than 21.28%,36.17%,25.53%,and 17.02% in Group A(Z=8.311,P<0.05). Before treatment,the white blood cell count(WBC),high-sensitivity C-reactive protein(hs CRP),and procalcitonin(PCT)levels in group B were similar to those in Group A(P>0.05).After treatment,the WBC(7.71±1.94)×109 /L,hs CRP(4.96±1.44)mg/L,and PCT(84.32±21.40)pg/mL in Group B were lower than those in Group A(9.05±2.48)×109 /L,(6.17±1.85)mg/L,and(105.46±34.02)pg/mL,respectivelyt=2.918,3.538,3.606,all P<0.05).The incidence of adverse reactions in Group B was 8.52%,while in Group A it was 4.26%.The incidence of adverse reactions in group B was similar to that in group A(χ 2 =0.178,P>0.05).Conclusions  Bronchoscopy combined with nebulized inhalation therapy for children with MPP can shorten the recovery time,promote the disappearance of lung shadows,reduce their inflammatory index levels,and without increasing the incidence of adverse reactions.
