目的 探讨BOPPPS[(导引(B),目标(O),课前测试(P),参与式学习(P),课后测试(P),总结(S)]模式下模块化教学结合模拟演练在创伤急救培训中的应用效果。方法 选取福建省泉州市正骨医院急诊科2022年8月—2024年2月90名护士,随机分为对照组和干预组,每组45名护士。对照组采用传统授课模式进行教学培训;干预组采用BOPPPS模式下模块化教学结合模拟演练进行教学培训。培训结束对两组学员的创伤急救综合理论及单项技能、创伤急救救护综合能力、培训效果满意度进行统计分析。结果 干预组的综合理论成绩为(80.74±6.87)分、单项技能成绩为(92.13±2.26)分,相较于对照组均提高(P<0.05)。干预组对创伤急救课程的总体满意度:非常同意23例、同意20例,比例均高于对照组(P<0.05)。干预组的创伤急救救护综合能力如病情评估预判能力,优秀26例、良好15例;护理计划分析实施能力,优秀25例、良好13例;团队协作与资源管理能力,优秀27例、良好15例;情景感知与应变能力,优秀30例、良好11例;综合救护能力,优秀36例、良好5例。显著优于对照组(P<0.05)。结论 BOPPPS模式下模块化教学结合模拟演练能够提高急诊科护士的综合创伤救护能力,且取得了较高的满意度,值得进一步推广应用。
Objective To explore the application effect of modular teaching combined with simulation exercise in trauma first aid training under BOPPPS model.Methods A total of 90 nurses in the emergency department from August 2022 to February 2024 were randomly divided into control group(n=45) and intervention group(n=45).The control group was trained by traditional teaching mode.The intervention group was trained by BOPPPS modular teaching combined with simulation exercise.After the training,the comprehensive theory of trauma first aid,individual skills,comprehensive ability of trauma first aid and satisfaction degree of training effect of the two groups of nurses were statistically analyzed.Results The comprehensive theoretical scores and single skill scores of the intervention group were significantly improved compared with the control group(P<0.05).The satisfaction degree of trauma first aid training in the intervention group was significantly higher than that in the control group(P<0.05).The comprehensive ability of trauma first aid in the intervention group was significantly better than that in the control group(P<0.05).Conclusions The combination of modular teaching and simulation exercise in BOPPPS model can improve the comprehensive trauma rescue ability of nurses in emergency department,and has obtained a high degree of satisfaction,which is worthy of further popularization and application.

目的 探讨基于迷你临床演练评估(Mini-CEX)的可视化思维导图联合案例教学在全科住院医师规范化培训中的应用效果。方法 选取参加全科住院医师规范化培训的52例学员并分为试验组(n=26)和对照组(n=26)。对照组采取以案例讲授为主的传统教学模式,试验组采取基于Mini-CEX的可视化思维导图联合案例教学模式。对比两组Mini-CEX评分、考核成绩及教学满意度。结果 试验组Mini-CEX测评的问诊技巧、体格检查、临床判断、诊治能力、技能操作及整体表现得分均高于对照组(P<0.05)。试验组理论知识[(86.89±4.75)分 vs (82.96±4.87分)]、专业技能[(84.20±3.46)分 vs (70.18±4.93)分]及病历书写成绩[(80.64±5.26)分 vs (75.58±5.94)分]均高于对照组(t分别为2.678、13.685、2.764,P分别为0.009、<0.001、0.007)。试验组住院医师教学满意度高于对照组(P<0.05)。结论 基于Mini-CEX的可视化思维导图联合案例教学模式用于全科住院医师规范化培训,有助于提高教学效果及教学满意度。
Objective To evaluate the effect of visual mind mapping combined with case teaching based on mini-clinical evaluation exercise(Mini-CEX)for standardized training of general practitioners.Methods A total of 52 students who participated the standardized training of general practitioners were selected and divided into an experimental group(n=26)and a control group(n=26).The control group adopted the traditional teaching mode based on case teaching and the experimental group adopted the visual mind mapping combined with case teaching based on Mini-CEX.The Mini-CEX score,assessment results and teaching satisfaction of two groups were compared.Results The interrogation skills,physical examination,clinical judgment,diagnosis and treatment ability,skill operation and overall performance of Mini-CEX in experimental group were higher than control group(P<0.05).Theoretical knowledge score([86.89±4.75] vs [82.96±4.87]),professional skills score([84.20±3.46] vs [70.18±4.93])and medical record writing score([80.64±5.26] vs [75.58±5.94])of experimental groups were higher than control group(t=2.678,13.685,2.764,P=0.009,<0.001,0.007).The teaching satisfaction of residents in experimental group was higher than control group(P<0.05).Conclusions The visual mind mapping combined with case teaching based on Mini-CEX for standardized training of general practitioners is helpful to improve teaching effect and teaching satisfaction.

目的 探讨对分课堂对《老年人健康照护技术》实训教学效果的影响,为进一步完善智慧健康养老服务与管理专业实训教学方案提供参考依据。方法 以广州卫生职业技术学院2022级智慧健康养老服务与管理专业学生为研究对象,设定其中一班为观察组,另一班为对照组,在《老年人健康照护技术》实训教学过程中,观察组采用对分课堂的实训教学模式,对照组采用传统实训教学模式;实训结束后对两组均进行理论和技能考核,并在教学前后均采用《一般自我效能感量表》对两组学生进行测评。结果 试验开始之前,对两组学生一般自我效能感测评得分进行比较,差异无统计学意义(P>0.05);试验结束后,将观察组与对照组的理论和技能考核成绩、一般自我效能感得分进行比较,观察组均明显高于对照组,差异有统计学意义(P<0.01);且观察组一般自我效能感得分优于本组试验前,差异有统计学意义(P<0.01)。结论 对分课堂有利于提高学生的一般自我效能感,有助于改善《老年人健康照护技术》的实训教学效果,能够为进一步完善智慧健康养老服务与管理专业实训教学方案提供参考依据。
Objective To explore the impact of split courses on the practical training and teaching effectiveness of “Elderly Health Care Technology”,and to provide reference for further improving the practical training and teaching plan of smart health and elderly care services and management.Methods A study was conducted on students majoring in Intelligent Health and Elderly Care Services and Management from Guangzhou Health Vocational and Technical College in 2022.One class was set as the observation group and the other as the control group.During the training and teaching process of “Elderly Health Care Technology”,the observation group adopted a split courses training teaching mode,while the control group adopted a traditional training teaching mode.After the practical training,theoretical and skill assessments were conducted on both groups,and the General Self Efficacy Scale was used to evaluate both groups of students before and after teaching.Results Before the start of the experiment,there was no statistically significant difference in the general self-efficacy evaluation scores between the two groups of students(P>0.05).After the experiment,the theoretical and skill assessment scores, general self-efficacy scores of the observation group, and the control group were compared.The observation group was significantly higher than the control group,with a significant statistical difference(P<0.01),and the general self-efficacy score of the observation group was better than before the experiment,with a statistically significant difference as well(P<0.01).Conclusions split courses were beneficial for improving students’ general self-efficacy,enhancing teaching effectiveness,and providing reference basis for further improving the practical training teaching plan for smart health and elderly care services and management.

目的 了解深圳市助产士培训的现况,为今后进一步加强助产士培训工作提供参考依据。方法 利用方便整群抽样法选取2022年在深圳市助产医疗机构工作的助产士,通过问卷调查及数据分析,了解他们在2009—2020年间参加助产士培训的情况以及对临床实操技能掌握的自评现况。结果 本研究共调查481名助产士,大部分来自三级医院,平均拥有9(5,15)年的助产工作经验,根据其工作年限和经验分别参加初级、中级和(或)高级培训,以参加一次初级或中级培训居多。助产士未参加培训的主要原因为医院人员不足和新入职。临床实操技能掌握自评现况分析显示,助产士初、中级母儿急救实操技能亟待加强,中级培训的助产士带教能力需提升。不同职称助产士的实操技能自评分数比较差异有统计学意义(Z=9.23,P=0.026)。结论 深圳市助产士对于初级培训和中级培训的参与度较高,但对部分临床实操技能的掌握仍存在不足,有必要进一步强化助产士临床实操技能培养,以全面满足孕产妇的健康需求。
Objective To understand the current state of midwifery training in Shenzhen,providing insights for the enhancement of future midwifery training programs.Methods Using a convenience cluster sampling method,midwives working in obstetric medical institutions in Shenzhen in 2022 were selected.Through questionnaire surveys and data analysis,the participation in midwifery training from 2009 to 2020 was examined along with their self-assessment of mastery over clinical practical skills.Results A total of 481 midwives,mostly from tertiary hospitals,with an average of 9(5,15)years of midwifery experience,were surveyed in this study and participated in primary,intermediate and/or advanced training according to their years of experience and years of work,with the majority participating in primary or intermediate training once.The main reasons for midwives not attending training were insufficient hospital staff and new recruits.The analysis of the self-assessment of the mastery of clinical practical skills showed that the practical skills of midwives in maternal and pediatric emergencies at the primary and intermediate levels needed to be strengthened,and the teaching ability of midwives with intermediate training needed to be improved.There was a difference in the self-rating of practical skills among midwives of different titles(Z=9.23,P=0.026).Conclusions Though participation in primary and intermediate midwifery training in Shenzhen City is high,there remains a deficiency in mastering specific clinical practical skills.This research suggests that there’s a pressing need to further enhance the practical clinical skill training for midwives to comprehensively meet the health needs of pregnant and postnatal women.

目的 探讨间歇经口至食管管饲法与自创吞咽功能训练联合应用于脑卒中吞咽障碍患者的效果。方法 选取河南大学淮河医院2022年1月—2023年12月收治的50例脑卒中吞咽障碍患者,应用随机数字表法将其分为两组,每组各25例。对照组采取持续留置鼻饲的同时应用自创吞咽功能训练进行训练,观察组采取间歇经口至食管管饲的同时应用自创吞咽功能训练进行训练。对比两组吞咽障碍改善情况、干预前后营养状况指标及生活质量变化,比较两组并发症发生率。结果 观察组总有效率高于对照组(P<0.05)。干预后两组血清总蛋白、白蛋白和血红蛋白水平和体质量指数均升高,观察组高于对照组(P<0.05)。干预后两组生活质量综合评定量表评分升高,观察组高于对照组(P<0.05)。观察组并发症发生率低于对照组(P<0.05)。结论 对脑卒中吞咽功能障碍患者采取间歇经口至食管管饲法与自创吞咽功能训练,能够更有效改善患者吞咽功能,提升其生活质量,且可降低留置饲管营养支持过程中的并发症发生率。
Objective To explore the application effect of intermittent oral to esophageal feeding combined with self created swallowing function training exercises in stroke patients with swallowing disorders.Methods Fifty stroke patients with swallowing disorders admitted to our hospital from January 2022 to December 2023 were selected and divided into two groups by random number table method,both with 25 cases.Control group patients received continuous nasogastric feeding while using solf created swallowing function training exercises for training,while observation group patients received intermittent oral to oesophageal tube feeding and using solf created swallowing function training exercises for training.The improvement of swallowing disorders,changes in nutritional status indicators and quality of life before and after the intervention were compared,and also the frequency of complications.Results The observation group was generally more effective than the control group(P<0.05).After intervention,the serum total protein,albumin hemoglobin,and BMI levels of both groups of patients increased,with the observation group being higher than the control group(P<0.05).After the intervention,GQOLI-74 score increased in both groups,and the observation group was higher than the control group(P<0.05).The complication rate was lower in the observation group(P<0.05).Conclusions The intervention method of intermittent oral to esophageal tube feeding combined with self created swallowing function training exercises can further improve the swallowing function of stroke patients with swallowing dysfunction,improve their quality of life,and reduce the occurrence of complications during the nutritional support process of feeding tubes.

目的 探讨综合呼吸康复训练对老年慢性阻塞性肺疾病急性期(AECOPD)患者的临床症状、生活质量、活动能力、肺功能、肺康复依从性的影响。方法 纳入70 例AECOPD 患者,随机数字分为对照组(n=35)和干预组(n=35),对照组在常规治疗基础上予缩唇腹式呼吸功能锻炼,干预组在常规治疗基础上进行综合呼吸康复训练,干预前后采用慢性阻塞性肺疾病评估测试(CAT)调查问卷、改良版英国医学研究委员会呼吸问卷(mMRC)、肺功能第一秒用力呼气容积(FEV1)预计值进行评估。结果 对照组呼吸康复训练前后CAT评分、mMRC量表呼吸困难评级比较差异均有统计学意义(t=16.781,t=8.103,均P<0.001);干预组呼吸康复训练前后CAT评分、mMRC呼吸困难评级比较差异也均有统计学意义(t=24.035,t=15.938,均P<0.001);干预组患者的CAT评分、mMRC量表呼吸困难评级较对照组下降明显。结论 综合呼吸康复训练能有效改善 AECOPD 患者临床症状、增强患者活动能力、提高生活质量、提高肺康复依从性。
Objective To investigate the effects of comprehensive respiratory rehabilitation training on clinical symptoms,quality of life,activity ability,lung function and compliance of elderly patients with acute exacerbation of obstructive pulmonary disease(AECOPD).Methods Seventy patients with AECOPD were included and randomly divided into control group(n=35)and intervention group(n=35).The control group was given routine treatment and respiratory muscle function exercise.The intervention group was given routine treatment and comprehensive respiratory rehabilitation training.COPD Assessment Test(CAT),modified Medical Research Council Scale(mMRC)and FEV1 predicted value of lung function were used to evaluate before and after intervention.Results In the control group,differences of CAT score and mMRC score were significant(t=16.781,t=8.103,P<0.001)before and after respiratory rehabilitation training.In the intervention group,the CAT score before and after respiratory rehabilitation training showed a 0.01 level of significance(t=24.035,P<0.001),and the mMRC score before and after training showed a 0.01 level of significance(t=15.938,P<0.001).There were significant differences between control group and intervention group(P<0.01).Compared with the control group,CAT score and mMRC score were significantly lower in the intervention group.Conclusions Comprehensive respiratory rehabilitation training can effectively improve the clinical symptoms of AECOPD patients,enhance the activity ability of patients,improve the quality of life,and improve the compliance of patients to perform pulmonary rehabilitation.

目的 探讨本体感觉训练联合等速肌力训练对早期膝骨关节炎(KOA)患者的应用效果。方法 选择郑州市骨科医院2022年5月—2023年5月收治的120例早期膝骨关节炎患者为研究对象,将所有对象进行编号,采用简单随机方法分成研究组和对照组,每组各60例,两组均接受常规治疗,对照组在此基础上接受等速肌力训练,研究组在对照组基础上联合应用本体感觉训练,比较两组治疗前和治疗6周后疼痛视觉模拟量表(VAS)评分、膝关节伸屈肌群峰力矩(PT)、关节功能以及本体感觉。结果 治疗后研究组VAS评分为(1.33±0.31)分,对照组为(2.05±0.34)分,研究组低于对照组(t=12.121,P<0.05);治疗后两组膝关节伸、屈肌群PT均较治疗前明显增加(P<0.05),研究组大于对照组(t1=2.696,t2=3.360,P<0.05);治疗后研究组WOMAC评分为(24.58±5.14)分,对照组为(26.85±5.11)分,研究组低于对照组(t=2.426,P<0.001),研究组Lequesne指数为(6.55±1.21)分,对照组为(7.02±1.25)分,研究组低于对照组(t=2.093,P<0.001);治疗后研究组平均轨迹误差为(21.35±5.96)%,对照组为(24.06±5.88)%,研究组改善程度优于对照组(t=2.507,P<0.001),研究组平均负重力量差为(0.77±0.22)kg,对照组为(1.01±0.24)kg,研究组改善程度明显优于对照组(t=5.710,P<0.001)。结论 本体感觉训练联合等速肌力训练可以显著提升早期膝骨关节炎患者肌力,改善关节功能和本体感觉作用明显,适合推广。
Objective To explore the application effect of proprioceptive training combined with isokinetic muscle strength training on early knee osteoarthritis(KOA)patients.Methods A total of 120 patients with early knee osteoarthritis admitted to a hospital from May 2022 to May 2023 were selected as the study subjects.All subjects were numbered and randomly divided into an experimental group and a control group,with 60 cases in each group.Both groups received routine treatment,while the control group received isokinetic muscle strength training additionally.The experimental group received proprioceptive training in addition to the control group treatment.After 6 weeks of treatment,compare the pain situation,peak torque(PT)of knee extensor and flexor muscles,joint function,and proprioception before and after treatment between two groups were compared.Results After treatment,the VAS scores of both groups were lower than those before treatment(P<0.05),and the experimental group was lower than the control group(t=12.121,P<0.05).After treatment,the PT of the knee joint extensor and flexor muscle groups in both groups significantly increased compared to before treatment(P<0.05),and the experimental group was significantly larger than the control group(P<0.05).After treatment,the WOMAC score and Lequesne index of the two groups significantly decreased compared to those before treatment(P<0.05),and the experimental group was smaller than the control group(P<0.05).After treatment,the average trajectory error and average weight difference between the two groups were significantly smaller than those before treatment(P<0.05),while the experimental group was smaller than the control group(P<0.05).Conclusion sThe combination of proprioceptive training and isokinetic muscle strength training can significantly improve muscle strength,joint function,and proprioceptive sensation in patients with early knee osteoarthritis,making it suitable for promotion.

目的 探讨低强度体外冲击波联合本体感觉训练治疗双侧颞下颌关节紊乱病的临床疗效。方法 给予1例双侧颞下颌关节紊乱病半年余患者低强度体外冲击波联合本体感觉训练治疗2周,比较治疗前、治疗2周后的最大张口度(MIO)、疼痛压力阈值(PPT)和Fricton颞下颌关节紊乱指数量表 (CMI),并于2个月后随访。结果 经过2周治疗,MIO、PPT和CMI均出现明显改善,2个月后随访未出现反弹,持续改善。结论 低强度体外冲击波联合本体感觉训练治疗颞下颌关节关节紊乱病临床疗效显著。
Objective To investigate the clinical efficacy of low intensity extracorporeal shock wave combined with proprioceptive training in the treatment of bilateral temporomandibular disorders (TMD). Methods A patient with bilateral TMD was treated with low intensity extracorporeal shock wave combined with proprioceptive training for 2 weeks.The maximal interincisal opening (MIO)、pain pressure thresholds (PPT), and Fricton's craniomandibular index (CMI) were compared before and after treatment,and followed up for 2 months. Results After 2 weeks of treatment, MIO, PPT and CMI were significantly improved, and there was no relapse after 2 months of follow-up. Conclusions Low intensity extracorporeal shock wave combined with proprioceptive training was effective in treating TMD.

目的 探讨良肢位训练联合风险预控急救应用于重症脑卒中的效果。方法 采用回顾性分析,纳入我院2020年3月—2021年3月期间收治的41例重症脑卒中患者为对照组,救治方式为常规救治联合良肢位训练。同时纳入我院2021年4月—2022年4月期间收治的41例重症脑卒中患者为观察组,救治方式为良肢位训练联合风险预控急救。对比2组抢救情况、抢救效果、运动功能。结果 观察组院外吸氧率(46.34%)、院外插管率(34.15%)均高于对照组(17.07%、9.76%),转入专科治疗时间、发病至入院时间均短于对照组(P<0.05);干预3 d后,观察组格拉斯哥昏迷量表分值高于对照组,APACHEⅡ评分分值低于对照组(P<0.05);观察组Fugl-Meyer运动评估、Berg平衡量表分值均高于对照组,NIHSS量表分值低于对照组(P<0.05)。结论 良肢位训练联合风险预控急救应用于重症脑卒中可有效改善抢救情况、提升抢救效果,从而增强运动功能。
Objective To explore the effect of normal extremity position training combined with risk pre-control emergency treatment in severe stroke.Methods Forty-one severe stroke patients from March 2020 to March 2021 included in the control group were retrospective studied,and the treatment method was conventional treatment combined with normal extremity position training.At the same time,41 severe stroke patients from April 2021 to April 2022 were included in the observation group,which the treatment method was the risk pre-control and emergency treatment.The rescue situation,rescue effect and sports function of the two groups were compared.Results Incidences of oxygen inhalation(46.34%)and intubation(34.15%)outside the hospital in observation group were higher than that in control group(17.07%,9.76%);specialist treatment,onset to admission time were longer in control group(P<0.05).Glasgow coma scale score in observation group was higher than that in control group,while APACHE Ⅱ score was lower in observation group(P<0.05),Fugl-Meyer assessment and Berg Balance Scale score were higher,and NIHSS score was lower than control group(P<0.05).Conclusions Combination of normal extremity position training and risk pre-control emergency treatment in severe stroke treatment can effectively improve the rescue effect,thus enhance the motor function.

目的 观察中等强度有氧运动联合力量训练对老年高尿酸血症(HUA)尿酸、身体机能及生活质量影响分析。方法 2019年1月—2019年12月在我社区收治的老年HUA患者60例纳入研究,随机数字表法进行分组,选取其中的30例患者实施常规饮食、运动指导为对照组,另30例患者配合中等强度有氧运动联合力量训练为研究组。比较2组患者对干预的依从性,干预前、后的血尿酸水平,身体机能及生活质量评分(WHOQOL-100评分)。结果 2组患者均在干预期内完成训练,依从性优良率比较,差异无统计学意义(P>0.05)。干预前2组患者的血尿酸水平、身体机能比较,差异无统计学意义(P>0.05)。干预后2组患者的血尿酸水平、安静心率较干预前降低,研究组低于对照组,而肺活量则较干预前提升,研究组高于对照组,差异有统计学意义(P<0.05)。干预前2组患者的WHOQOL-100评分比较,差异无统计学意义(P>0.05),干预后2组WHOQOL-100各项评分较干预前提升,研究组高于对照组,差异有统计学意义(P<0.05)。结论 在老年HUA患者的相关指导中,运用中等强度有氧运动联合力量训练的运动干预,可较为明显的降低血清血尿酸水平,降低并平稳安静心率,提升肺活量,提升生活质量,效果理想。
Objective To observe the effect of moderate intensity aerobic exercise combined with strength training on uric acid level, physical function and quality of life in elderly patients with hyperuricemia (HUA). Methods Sixty elderly patients with HUA were treated in our center from January 2019 to December 2019. They were randomly divided into two groups, 30 cases in each group. Among them, 30 cases given routine diet and exercise guidance were included in control group, and the other 30 cases given moderate intensity aerobic exercise combined with strength training on the basis of the control group were included in study group. The compliance, blood uric acid level, physical function and quality of life (WHOQOL-100 scores) before and after the intervention were compared between the two groups. Results Two groups of patients completed the training within the expected time, and there was no significant difference in compliance (P>0.05). Before the intervention, there were no significant differences in blood uric acid level and physical function between the two groups (P>0.05). After the intervention, the serum uric acid level and resting heart rate of the two groups were lower than those before the intervention, and those of the research group were even lower than the control group, while the vital capacity was higher than that before the intervention, and the research group was higher than the control group, those differences were significant (P<0.05). Before the intervention, there was no significant difference in WHOQOL-100 score between the two groups (P>0.05). After the intervention, the scores of WHOQOL-100 in the two groups were higher than those before the intervention, and that in the study group was higher than the control group (P<0.05). Conclusions In the relevant guidance of elderly patients with hyperuricemia (HUA), the application of moderate intensity aerobic exercise combined with strength training can significantly lower the serum uric acid level, reduce and stabilize the resting heart rate, improve the vital capacity and quality of life, with ideal effect.
