目的 分析我院2011—2015年我院儿科住院患者下呼吸道病原菌分布及其耐药性。方法 采用全自动生化鉴定仪对痰标本分离株进行鉴定,用全自动微生物药敏系统和纸片扩散法对病原菌的耐药性进行检测,并用头孢硝噻吩纸片法对β-内酰胺酶进行检测。结果 2011—2015年共分离得到下呼吸道病原菌518株,包括肺炎链球菌(21.62%)、金黄色葡萄球菌(16.99%)、流感嗜血杆菌(14.48%)、肺炎克雷伯菌(11.97%)、大肠埃希菌(8.11%)、卡他莫拉菌(5.41%)、鲍曼不动杆菌(3.86%)和铜绿假单胞菌(3.86%)等。药敏结果显示,肺炎链球菌对克林霉素(90.18%)、红霉素(92.86%)和复方新诺明(87.50%)的耐药率较高,金黄色葡萄球菌则对青霉素G(90.91%)和红霉素(68.18%)有较强耐药性,未发现对万古霉素或利奈唑胺耐药的革兰阳性球菌。流感嗜血杆菌对氨苄西林耐药率为32%,与其β-内酰胺酶阳性率较一致,肺炎克雷伯菌和大肠埃希菌对头孢类药物(17.33%~45.33%)和喹诺酮类药物(34.67%~50.67%)耐药性较高,并发现1株碳青霉烯耐药的肺炎克雷伯菌。结论 本院下呼吸道感染病原菌谱较广,主要包括多种革兰阳性球菌和革兰阴性杆菌,并对多种抗菌药物表现出较强耐药性,临床应注重合理应用相关抗生素,严格防控病原菌的医院感染及传播。
Objective To analyze the antimicrobial resistance and the profile of pathogens from lower respiratory tract infections in pediatric in patients. Methods Sputum bacterial isolates were identified by an automated biochemical identification system. Antimicrobial resistance was detected by an automated drug susceptibility detection system and the disc diffusion method. The β-lactamases was tested by the nitrocefin disc detection method. Results Five hundred and eighteen bacterial pathogens were isolated from sputum samples during 2011-2015, including streptococcus pneumoniae(21.62%), staphylococcus aureus(16.99%), haemophillus influenzae(14.48%), klebsiella pneumoniae(11.97%), escherichia coli(8.11%), moraxelle catarrhalis(3.8%), acinetobacter baumanii(3.86%) and pseudomonas aeruginosa(3.86%). High resistant rates were detected for S. pneumoniae to clindamycin(90.18%), erythromycin(92.86%) and sulfamethoxazole (85.50%), while S. aureus was highly resistant to penicillin G(90.91%) and erythromycin(68.18%). Resistance to vancomycin and linezolid was not detected for gram positive cocci. The resistant rate to ampicillin was 32% for H. influenzae, which was in concordance with the production of β-lactamases. Relatively high resistance was detected for K. pneumoniae and E. coli to cephalosporins and quinolones. A carbapenem-resistant K. pneumoniae isolate was also detected. Conclusion Multiple bacterial species were isolated from lower respiratory tract infections in our hospital, including different species of gram positive cocci and gram negative bacilli, and these isolates exhibited high resistance to antibiotics tested. The clinical use of antibiotics and hospital infection and transmission of these pathogens should be controlled.
目的 探讨信息预警系统在住院患者压疮风险管理中的运用。方法 应用描述性分析,χ2检验和t检验方法,对实施信息预警系统前后的院内压疮发生率、压疮护理质量管理指标以及使用纸质版和电子版Waterlow量表评估患者压疮危险的平均时间进行比较。结果 试行信息预警系统后,院内压疮发生率下降至0.11‰;护士对压疮危险患者评估率、动态评估率和预防压疮措施落实率都明显升高;护士使用电子版Waterlow量表评估患者压疮危险耗时显著减少。结论 信息预警系统在住院患者压疮风险管理中起到重要作用,为临床护理和护理管理提供了良好的工具,保障患者安全。
Objective To explore the application of risk management information system in the inpatients' risk management of pressure ulcers (PUrs). Methods A descriptive analysis, Chi-square test and T-test were used to analyze the incidence of inpatients' PUrs and indicators of the nursing quality control before and after applying the risk management information system. Results After using this information system, incidence of PUrs decreased to 0.11‰ . Rate of registered nurses' assessment on inpatients' with PUrs risk and dynamically assessment rose obviously. The rate of prevention of PUrs went up as well. The mean time to assess inpatients' PUrs risk with electronic Waterlow scale was significantly less than the mean time of paper Waterlow scale. Conclusion The risk management information system played a vital role in the inpatients' PUrs risk management. It could provide an effective tool for the clinical nursing's quality control, improve registered nurses' efficiency and protect inpatients'safety.
目的 调查住院患儿的营养风险及营养状况,探讨儿科营养风险筛查新方法的应用。方法 使用新的儿科营养风险筛查工具,对住院患儿进行营养风险筛查,根据体格测量评价儿童营养状况;与国外三种儿科营养风险筛查工具比较一致性。结果 402例住院患儿中高营养风险患儿占24.1%,营养不良的检出率为18.4%。有36.8%的患者接受营养支持,其中肠外营养支持率为23.1%,肠内营养支持率为16.7%,PN:EN为1.39:1。结论 对住院患儿采用新方法做营养风险筛查,能客观地反映住院患儿的营养风险,为临床营养支持提供依据。
Objective To investigate nutritional risk and the nutritional status in hospitalized children,and to validate the new screening tool of nutritional risk in hospitalized pediatric patients. Methods The nutritional risk of hospitalized pediatric patients was investigated using the new screening tool,and compared with other three pediatric nutritional risk screening tools;the nutritional status was assessed according to children physical measurement. Results Among 402 hospitalized children,children with high nutritional risk accounted for 24.1%.The overall prevalence of malnutrition was 18.4%.The proportion of patients receiving nutritional support was 36.8%.The rate of parenteral and enteral nutrition support was 23.1% and 16.7%,respectively.The ratio of parenteral nutrition to enteral nutrition was 1.39:1. Conclusion The new screening tool can reflect the possible nutritional risk in hospitalized pediatric patients objectively and provide the basis for clinical nutritional support.
目的 通过构建结构方程模型,分析某三级甲等综合医院腹股沟疝患者住院费用的影响因素,旨在为合理控制腹股沟疝单病种费用提供依据。方法 收集4 328份高州市人民医院2016—2022年主要诊断疾病名称为腹股沟疝且行腹股沟疝手术的患者的病历资料,预分析单个影响因素,采用AMOS28.0拟合构建模型。结果 结构方程模型拟合达到标准。性别、年龄、费别、住院次数、入院途径、伴随病、单双侧疝对总费用所产生的总效应数值分别为0.008、-0.044、0.062、0.014、-0.119、0.106、0.236;性别、年龄、费别、住院次数、入院途径、伴随病、单双侧疝通过住院时间间接对住院费用产生影响。结论 对住院费用产生的影响因素有性别、年龄、费别、住院次数、入院途径、伴随病、单双侧疝、住院时间,建议推行患者预住院模式及日间手术,在正式住院前完成相关检查,优化医疗服务流程,从而合理有效控制单病种住院费用。
Objective By utilizing a structural equation model, to analyze determinants that affect the hospitalization costs for individuals with inguinal hernia at a tertiary-level comprehensive medical center, offering insights for the potential management of costs associated with this specific ailment. Methods This study entailed the compilation of 4 328 patient files from individuals who received surgical treatment for inguinal hernia at a third-level general hospital over the period spanning 2016 to 2022. Preliminary analysis was conducted on isolated variables, followed by the development of a model using AMOS 28. 0 for fit assessment. Results The fitting of structural equation model reached the standard. The total effect values of gender, age, cost, number of hospitalizations, admission route, concomitant disease, unilateral and bilateral hernia on the total cost were 0. 008, -0. 044, 0. 062, 0. 014, -0. 119, 0. 106, 0. 236, respectively. Gender, age, cost, number of hospitalizations, admission route, concomitant disease, unilateral and bilateral hernia indirectly affected hospitalization expenses through hospitalization days. Conclusions Gender, age, cost, number of hospitalizations, of admission, concomitant diseases, unilateral and bilateral hernia, and length of hospital stay have an impact on hospitalization costs. It is suggested to implement the pre-hospitalization mode and day surgery, complete relevant examinations before formal hospitalization, and optimize the medical service process, so as to reasonably and effectively control the hospitalization cost of single disease.
目的 探讨基于迷你临床演练评估(Mini-CEX)的可视化思维导图联合案例教学在全科住院医师规范化培训中的应用效果。方法 选取参加全科住院医师规范化培训的52例学员并分为试验组(n=26)和对照组(n=26)。对照组采取以案例讲授为主的传统教学模式,试验组采取基于Mini-CEX的可视化思维导图联合案例教学模式。对比两组Mini-CEX评分、考核成绩及教学满意度。结果 试验组Mini-CEX测评的问诊技巧、体格检查、临床判断、诊治能力、技能操作及整体表现得分均高于对照组(P<0.05)。试验组理论知识[(86.89±4.75)分 vs (82.96±4.87分)]、专业技能[(84.20±3.46)分 vs (70.18±4.93)分]及病历书写成绩[(80.64±5.26)分 vs (75.58±5.94)分]均高于对照组(t分别为2.678、13.685、2.764,P分别为0.009、<0.001、0.007)。试验组住院医师教学满意度高于对照组(P<0.05)。结论 基于Mini-CEX的可视化思维导图联合案例教学模式用于全科住院医师规范化培训,有助于提高教学效果及教学满意度。
Objective To evaluate the effect of visual mind mapping combined with case teaching based on mini-clinical evaluation exercise(Mini-CEX)for standardized training of general practitioners.Methods A total of 52 students who participated the standardized training of general practitioners were selected and divided into an experimental group(n=26)and a control group(n=26).The control group adopted the traditional teaching mode based on case teaching and the experimental group adopted the visual mind mapping combined with case teaching based on Mini-CEX.The Mini-CEX score,assessment results and teaching satisfaction of two groups were compared.Results The interrogation skills,physical examination,clinical judgment,diagnosis and treatment ability,skill operation and overall performance of Mini-CEX in experimental group were higher than control group(P<0.05).Theoretical knowledge score([86.89±4.75] vs [82.96±4.87]),professional skills score([84.20±3.46] vs[70.18±4.93])and medical record writing score([80.64±5.26] vs [75.58±5.94])of experimental groups were higher than control group(t=2.678,13.685,2.764,P=0.009,<0.001,0.007).The teaching satisfaction of residents in experimental group was higher than control group(P<0.05).Conclusions The visual mind mapping combined with case teaching based on Mini-CEX for standardized training of general practitioners is helpful to improve teaching effect and teaching satisfaction.
目的 对该培训基地师资2021—2023年教学能力进行评价,了解和分析“1+3+n”的院级督导推行前的2021年、“1+3+n”的院级督导推行后的 2022年、2023年师资的教学能力变化。方法 采用问卷调查法收集评价数据,采用SPSS.27.0对教学评价资料进行统计分析,采用净推荐值计算教学口碑,对多年度结果进行纵向比较分析。结果 2021—2023年教学评价次数共5 483次;2021—2023年对师资教学设计能力评价由实施前的8.8提高到9.6、教学实施能力由实施前的9.0提高到9.5、教学热情由实施前的9.0提高到10.0、时间投入由实施前的9.3提高到10.0,不同年份组间比较的差异均有统计学意义(P<0.001);课程净推荐值由实施前的60.5%提高到81.3%,师资课程推荐者占比由实施前的65.5%提高到83.2%,贬损者占比由实施前的5.0%降低到1.9%,差异有统计学意义(χ 2 =175.4,P<0.001)。结论 “1+3+n”院级督导模式的相关举措,对师资教学能力的提高具有促进作用,且该促进作用持续存在,师资教学能力逐年递增。
Objective To evaluate the teaching ability change of teachers from 2021 to 2023,including of teachers’ teaching ability in 2021 before the implementation of "1+3+n" college-level supervision,and in 2022 and 2023 after the implementation of "1+3+n" college-level supervision.Methods Questionnaire survey was used to collect evaluation data,SPSS.27.0 was used to make statistical analysis of teaching evaluation data,and NPS(Net Promoter Score)was used to evaluate teaching quality.Results From 2021 to 2023,there were 5483 teaching evaluations.From 2021 to 2023,teaching design ability evaluation score increased from 8.8 before the implementation to 9.6,teaching implementation ability evaluation score increased from 9.0 before the implementation to 9.5,teaching enthusiasm evaluation score increased from 9.0 before the implementation to 10.0,time investment evaluation score increased from 9.3 before the implementation to 10.0,and the differences between different years were statistically significant(P<0.001).The courses quality increased from 60.5% before the implementation to 81.3%,and the proportion of teachers' course recommenders has increased from 65.5% before the implementation to 83.2%,while the proportion of detractors has decreased from 5.0% before the implementation to 1.9%,with statistical significance(χ 2 =175.4,P<0.001).Conclusions The measures of "1+3+n" college-level supervision mode promote the improvement of teachers’ teaching ability,the promotion effect persists,and the teaching ability increases year by year.
