云南省某三级甲等医院2018—2023年工作人员职业暴露状况分析

目的 分析医院职业暴露工作人员特征、发生职业暴露环节、类型及处理方式,为医院制订干预措施提供理论基础。方法 回顾性分析玉溪市人民医院2018—2023年共301例发生职业暴露工作人员资料, 包括职业暴露时间、性别、年龄、科室、岗位类型、在院工作时间、职位、职业暴露发生环节、职业暴露类型、暴露源、职业暴露后是否需要用药等处置信息。结果 301例职业暴露工作人员平均年龄为(28.81±10.92)岁, 女性占比84.7%(255例); 57.8%(174例)职业暴露发生在检查/穿刺/注射/采血/置管/治疗/手术等操作中, 38.9%(117例)发生在医疗废物处置环节;87.7%(264例)的暴露类型为针刺伤;45.5%(137例)接触暴露源为有血源性传播疾病的患者。发生职业暴露工作人员中, 不同岗位职业暴露者的暴露类型、职位、科室、是否需要药物干预及接触暴露源情况比较差异有统计学意义(P<0.05)。结论 应加强医务工作者职业暴露相关知识及应急处置培训, 对不同工龄、岗位医务工作者制定针对性培训方案, 加强医疗废物处置流程及临床操作技能规范等知识培训,预防医务工作者职业暴露的发生。
Objective To analyze the characteristics,occupational exposure links, types, and treatment methods of occupational exposure among healthcare staff hospital, providing a theoretical basis for the development of related intervention strategies for hospitals.Methods A retrospective study was conducted to explore 301 cases of occupational exposure among healthcare staff in The People’s Hospital of Yuxi City from 2018 to 2023.Data of 301 cases were collected, including information on gender, age,department,personnel category, length of service in the hospital, job title, and the circumstances of occupational exposures, which covered exposure links, type, source, and post-exposure medication treatment measures.Results Among the 301 healthcare staff experiencing occupational exposure, the average age was(28.81±10.92)years, with 84.7%(255 cases)being female.Occupational exposure most frequently occurred during procedures such as examination, puncture, injection, blood collection, catheterization, treatment, and surgery(57.8%,174 cases), followed by the disposal of medical waste(38.9%, 117 cases).The primary type of exposure was needle-stick injury(87.7%).Contact with patients suffering from blood-borne infectious diseases accounted for 45.5%(137 cases)of the occupational exposure incidents.There were significant differences in exposure types, job positions, departments, sources of exposure, and post-exposure medication treatment measures among healthcare staff of different categories(P<0.05).Conclusions To prevent the occurrence of occupational exposure among healthcare staff, it is necessary to enhance training on occupational exposure knowledge and emergency management, particularly improving the disposal of medical waste and clinical operational skills.Additionally, it is crucial to have personalized training programs tailored to healthcare staff based on their varying lengths of service and positions.
       医务人员职业暴露是指医务人员在从事诊疗、护理、医学检查、医学检验、预防接种等相关操作过程中接触有毒、有害物质或传染病病原体,导致健康损害或危及生命的情况,包括生物性职业暴露、物理性职业暴露、化学性职业暴露及其他职业暴露[1]。据世界卫生组织统计,医疗工作者职业暴露的发生例次率约为8.57%[2],严重影响医务人员健康[3-4]。研究发现,我国职业暴露的发生率为16.10%[5],中国助产士的血源性职业暴露发生率高达46.77%,工龄≤5年、未采取标准预防措施、不了解人类免疫缺陷病毒(Human Immunodeficiency Virus,HIV)治疗程序以及不戴手套与患者体液接触是其独立危险因素[6]。男性、未使用洗手设施、未佩戴防护用品与医务人员接触患者血液和体液率(76.2%)呈正相关[7]喀麦隆一项2019年6—10月分层横断面调查显示,30~39岁、工作地点为三级医院和暴露后预防认知不足是当地医务工作者面临HIV职业暴露的风险因[8]。此外,职业暴露可能使医务工作者出现一定程度的职业倦怠、创伤后应激障碍、抑郁和焦虑等方面的心理困扰,这也是间接降低医疗服务质量的因素之一[9-11]。本研究旨在分析云南省玉溪市人民医院职业暴露工作人员特征、发生职业暴露环节、类型及处理方式为提高医务工作者的职业安全性提供理论依据,避免职业暴露带来的意外伤害和感染,为医院制订干预措施提供理论基础。

1  资料与方法

1.1  对象

       本研究采用回顾性研究方法,选取云南省玉溪市人民医院2018—2023年发生职业暴露的工作人员为研究对象。选择标准:(1)经医院感染管理专职人员确定为发生职业暴露者;(2)既往无传染性疾病感染/精神疾病史。

1.2  方法

       采用回顾性调查方法对医院感染管理部门收集的《工作人员职业暴露登记表》进行分析,资料收集的主要内容:发生职业暴露时间、性别、年龄、所在科室、岗位类型、在院工作时间、职位、暴露环节、暴露类型、暴露源、有无防护措施,以及发生职业暴露后是否需要药物干预、接种疫苗等信息。

1.3  统计学处理

       采用Office EXCEL 365对数据进行收集整理,采用SPSS 19.0 软件进行统计学分析,计数资料的分布采用构成比及频数描述,采用χ 2 检验比较医院在职工作人员和各类学员职业暴露分布差异,多分类的计数资料的两两比较采用调整检验水准(α1=α/比较次数)的χ 2 检验,(近似)正态分布的计量资料的描述采用均数±标准差α=0.05。

2  结 果

2.1  医院职业暴露现状分析

       医院2018—2023年共发生职业暴露301人次,其中2018年66人次(21.93%)、2019年36人次(11.76%)、2020年49人次(16.28%)、2021年58人次(19.27%)、2022年45人次(14.95%)、2023年47人次(15.61%)。描述性分析显示,该三甲医院从2020年开始职业暴露发生例数有上升趋势,2021—2023年有下降趋势,2018年发生职业暴露人数最多(>60例)(见图1)。301名发生职业暴露各类型人员年龄范围为16~64岁,平均(28.81±10.92)岁。女性职业暴露占比84.70%(255例);在科室分布方面,外科职业暴露率(45.80%)最高,其中占比前列的科室分别为胃肠外科18例(占外科13.04%);在岗位方面,在职人员占50.50%(152例),各类学员(包括实习学员、进修学员、规培学员)占49.50%(149例);在医院工作时间方面,工龄<5年者职业暴露发生率较高(66.80%,201例);在职位方面,护士职业暴露占比最高(65.10%,196例);暴露类型主要以针刺伤(87.70%,264例)为主;暴露环节主要以检查/穿刺/注射/采血/置管/治疗/手术等操作(57.80%,174例)中为主,其次为医疗废物处置环节(38.90%,117例),详见表1。

2.2  不同岗位工作者暴露环节、类型、职位、科室、暴露后需处置情况及接触的暴露源情况比较

       不同岗位职业暴露者暴露环节、补打疫苗、职业防护情况差异无统计学意义(P>0.05),在职与实习/进修/规培人员均在检查/注射/置管/治疗/手术等操作中发生职业暴露的比例较高,发生职业暴露时有防护措施、发生职业暴露后无需补打疫苗占比较高;不同岗位职业暴露者暴露类型、职位、科室、职业暴露后药物/免疫球蛋白注射率、接触暴露源情况比较差异有统计学意义P<0.05);针刺伤在实习/进修/规培人员中占比更高,其他暴露类型(非针刺锐器伤/喷溅等)在在职工作人员中占比高(P<0.05);与护士相比,医生、其他职务中在职人员占比更高,护士中实习/进修/规培占比更高(P<0.01);与其他科室相比,内科、外科中实习/进修/规培占比人员占比更高,其他科室中在职人员占比更高(P<0.01);在职人员发生职业暴露后药物/免疫球蛋白注射率更高(P<0.01),见表2。

3  讨 论

       医务工作者职业暴露可能给医疗机构造成巨大的经济负担[12-13],对医务工作者生理和心理造成负面影响,包括恐惧、情绪困扰、焦虑甚至抑郁,出现创伤后应激障碍(PTSD)及疾病的传染[14]本研究显示,目标医院2018年发生职业暴露人数最多,2019—2023年职业暴露人数均少于2018年,这可能与近年来医院关注职业暴露防护,及时采取相应的管理措施,组织临床操作规范、职业暴露等相关的培训有关。
       发生职业暴露中女性、外科、工龄<5年、护士占比较高,发生职业暴露环节主要以检查/穿刺/注射/采血/置管/治疗/手术等操作为主,其次为医疗废物处置环节,暴露源为接触有血源性传播疾病者占比相对较高[15]。低年资医生暴露于血液、体液的风险更高,暴露地点主要在病房,这可能与低年资医生主要承担着临床常规诊疗工作有关,而高年资医生发生职业暴露的地点主要在手术室和康复室[16]。与高年资工作者相比,低年资工作者自我防护意识缺乏,预防措施落实不到位,临床操作不熟练、不规范等问题更为突出[17]因此,医疗机构应关注该类人员的职业防护及日常操作规范培训及考核,从而防止职业暴露的发生。
       外科医生职业暴露风险高于其他专科的医务人员[18]。与其他专科相比,外科工作者暴露于患者血液、体液风险更高,应强化外科工作人员薄弱环节的培训,操作前充分评估可能存在的暴露风险,做好个人防护。本研究中,各类学员职业暴露环节主要发生在检查/注射/置管/治疗/手术等有创操作中,特别是在处置针头环节,暴露类型以针刺伤为主,回顾性研究显示,手术科室锐器伤年发生率可高达5%,锐器伤的发生环节主要为处理和交接锐器的过程中,通过有效的培训及干预,锐器伤的发生是可以预防的[19],应加强该类医务工作者职业暴露相关预防知识的培训。在发生血源性职业暴露工作人员中,护士占比最[20-21],这可能与护士工作接触锐器频率较高有[22]。护士是医务工作者中发生锐器伤最主要的群体,锐器伤的发生主要是在锐器使用后处理[23]。护士是医疗废物收集环节的重要参与者,医疗废物规范化处置的培训对象不仅是后勤人员,应增强临床、护理工作人员对医疗废物规范化收集、处置的意识。规范收集、处置医疗废物,是预防自身及他人发生职业暴露的重要措施。
       针刺伤是医务人员最主要的职业暴露类型,各类肝炎病毒(尤其是乙型肝炎病毒、丙型肝炎病毒)和HIV是主要的暴露风险,从2018—2023年该三甲医院职业暴露的类型主要为针刺伤,也进一步证实这一观点。据统计全球每年37 00万医务工作者约有200万~300万发生针刺伤,每年因职业暴露发生HBV感染约66 000例、HCV感染16 000例和HIV感染1 000例[24-25]。除此之外,锐器伤还可导致其他细菌、真菌或病毒的感染,如白喉、疟疾等[26]。美国每年发生385 000例锐器伤,每天约发生1 000例锐器伤[27],在中国,73.4%的针刺伤有明确的暴露源,医务工作者针刺伤发生率为8.47%,针刺伤是医务工作者最主要的职业暴露类型,且容易被忽视,应提高安全和保护意识[28]
       本研究职业暴露工作人员中进修/规培/实习人员占比47.7%,在实际工作中,学员与在职工作者同样面临着较大的职业暴露风险,特别是锐器伤后接触患者的血液和体液,从而感染血源性传染病,学生对职业暴露的了解有限及临床工作实践经验的不足,职业暴露的发生可能对以后的工作及就业选择有重要影响,应加强对学生的职业暴露防控、处置的培训[29]。一项对实习护士职业暴露研究显示,实习护士针刺伤的发生率为60.8%,且实习初期高于实习后期,针刺伤主要发生在护理操作后,取下注射器或输液针时刺伤的发生率最高,受教育水平、护理知识和相关的培训是影响实习护士是否发生职业暴露的因素[30],因此,医院应该加强岗前培训及职业防护教育,减少实习/进修/规培同学职业暴露的发生,规范操作行为,为以后的临床工作奠定坚实的基础。
       本研究中,不同岗位工作者暴露类型、职位、科室、暴露后需处置情况及接触的暴露源有差异,在职人员需要注射免疫球蛋白/预防用药者及接触有血源性传染性疾病传染源占比较高。医疗机构安装安全的设备来减少医务工作者暴露的风险,通过培训改变医务工作者的行为、精细化管理等方式均能降低医务工作者发生锐器伤等职业暴露的风险[31-32]。对于不同类型的人员应开展不同的强化培训及考核,此外,医疗机构应关注医务人员的疫苗接种情况,医务人员的工作具有较高的职业暴露风险,医务人员自身应定期监测自身抗体情况,主动进行疫苗接种,保障职业安全,预防职业暴露后血源性传染病的感染。由于本研究采用回顾性资料进行数据分析,不能全面了解职业暴露人员的社会人口学特征、职业特征,无法全面、详细分析职业暴露的高危因素。另外,由于本研究为单中心研究,可能存在样本代表性不足、选择偏倚、地域限制等问题,影响研究成果的可靠性与普遍性。
       总之,医务人员职业暴露不容忽视,诊疗操作不规范、防护措施不到位均可导致医务人员发生职业暴露,发生职业暴露后未进行及时处置,导致医务人员发生血源性传染性疾病的感染,严重危害医务工作的身心健康,甚至对医务工作者的家庭产生负面影响,因此,医疗机构应重视职业安全及临床规范操作培训,对于不同岗位、工龄、职位工作人员有针对性地培训及定期考核,选择不容易发生职业暴露的医疗设备,从而降低医务工作人员发生职业暴露的风险,保护医务工作者职业安全,提升医疗质量。
