子宫瘢痕的超声弹性成像结合厚度分析对剖宫产后再妊娠产妇子宫破裂的预测应用

目的 探讨子宫瘢痕的超声弹性成像结合厚度分析对剖宫产后再妊娠产妇子宫破裂的预测应用。方法 选择2020年1月—2021年12月在中山市中医院分娩的剖宫产术后再次妊娠经阴道分娩(VBAC)产妇作为研究对象。根据纳入和排除标准,共纳入子宫破裂的VBAC产妇32例、非子宫破裂的VBAC产妇90例。通过住院病历信息系统查询研究对象的基本信息及其在妊娠晚期(≥37周)用B超对研究对象行子宫瘢痕厚度和弹性的测量结果,采用受试者工作特征曲线(ROC)曲线分析子宫瘢痕厚度和弹性SI值对子宫破裂的预测作用。结果 子宫破裂组中年龄>35岁、妊娠>2次、与上次剖宫产间隔<2年、新生儿体质量≥3 kg、单层缝合者的比例高于非子宫破裂组(P<0.05)。122例产妇子宫瘢痕厚度的均值为(3.42±0.49)mm,SI的均值为(2.57±0.45)。ROC曲线分析结果显示:子宫瘢痕厚度单独预测子宫破裂的曲线下面积(AUC)为0.805(95%CI:0.730~0.880,P<0.05),cut off值为3.05 mm,灵敏度为0.726,特异度为0.910,约登指数为0.636;子宫瘢痕SI单独预测子宫破裂的AUC为0.730(95%CI:0.635~0.824,P<0.05),cut off值为2.11,灵敏度为0.767,特异度为0.781,约登指数为0.548;子宫瘢痕厚度联合预测子宫破裂的AUC为0.874(95%CI:0.812~0.937,P<0.01),灵敏度为0.875,特异度为0.811,约登指数为0.686。子宫瘢痕厚度结合子宫瘢痕SI值预测子宫破裂的AUC高于单独使用子宫瘢痕厚度(Z=7.611,P=0.041)和子宫瘢痕SI值(Z=25.864,P=0.025)。结论 子宫瘢痕的超声弹性成像SI值联合子宫厚度可有效提高超声对于VBAC产妇子宫破裂的预测效能,具有一定的应用意义。
Objective To study the application of ultrasound elasticity imaging combined thickness analysis of uterine scar in predicting uterine rupture in women pregnant after cesarean section.Methods Pregnant women with vaginal birth after cesarean(VBAC)from January 2020 to December 2021 in Zhongshan Hospital of Traditional Chinese Medicine were selected as the research subjects.A total of 32 VBAC parturients with uterine rupture and 90 VBAC parturients without uterine rupture were included according to the inclusion and exclusion criteria.The basic information of the subjects was queried through the medical record information system of the hospital.In the third trimester(≥37 weeks),the thickness and elasticity of uterine scar were measured by ultrasound,and the predictive effect of uterine scar thickness and elastic SI value on uterine rupture was analyzed by ROC curve.Results Chi-square test showed that the incidence of uterine rupture was higher in patients with age>35 years,pregnancy>2 times,interval from last cesarean section<2 years,newborn weight≥3kg,and the proportion of uterine rupture in single suture was higher than that in double suture(P<0.05).The mean uterine scar thickness of 122 subjects was(3.42±0.49)mm,and the mean SI was(2.57±0.45).The area under curve(AUC)of uterine scar thickness alone for predicting uterine rupture was 0.805(95%CI:0.730-0.880,P<0.05),the cut off value was 3.05 mm,the sensitivity was 0.726,the specificity was 0.910,and the Youden coefficient was 0.636 by ROC curve analysis.The AUC of uterine scar SI alone for predicting uterine rupture was 0.730(95%CI:0.635-0.824,P<0.05),the cut off value was 2.11,the sensitivity was 0.767,the specificity was 0.781,and the Youden coefficient was 0.548 by ROC curve analysis.The AUC of uterine scar thickness combination for predicting uterine rupture was 0.874(95%CI:0.812-0.937,P<0.01),the sensitivity was 0.875,the specificity was 0.811,and the Youden coefficient was 0.686 by ROC curve analysis.The AUC predicted by uterine scar thickness combined with uterine scar SI value was higher than that predicted by uterine scar thickness alone(Z=7.611,P=0.041)and uterine scar SI value(Z=25.864,P=0.025).Conclusions Elastic SI value of ultrasound imaging of uterine scar combined with uterine thickness can effectively improve the prediction efficiency of ultrasound for VBAC maternal uterine rupture,which has certain application significance,but further demonstration is still needed.

玻璃体腔内注射康柏西普治疗湿性老年性黄斑变性对视网膜神经纤维厚度的影响观察

目的 观察康柏西普玻璃体腔注射治疗湿性老年性黄斑变性病变对于视网膜神经纤维厚度的影响。方法 对2016年10月—2017年10月在汕头大学附属韶关市粤北人民医院采用康柏西普玻璃体腔注射治疗湿性老年性黄斑变性35例38眼中符合标准的患者18例(18眼)的临床资料进行回顾性分析,患者行最佳矫正视力、眼压(NCT)检查、OCT、荧光眼底血管造影(FFA)检查后,均接受0.05 mL康柏西普玻璃体腔注射,分别注射后1和2月观察患者最佳矫正视力 (BCVA)、视网膜神经纤维厚度(RNFL)变化。结果 18眼共接受康柏西普玻璃体腔注射54次,所有患眼均注射3次。注射3个月后,OCT检查结果显示有18眼视力有提高,CRT厚度有下降。第1次注射时和注射后1个月、2个月的BCVA分别为0.1(0.04,0.12),0.20(0.10,0.30)和0.25(0.12,0.30),总体比较差异有统计学意义(χ2=13.880,P<0.001);第1次注射时和注射后1个月、2个月的CRT(μm) 319.50(269.50,390.50), 271.00(219.00,296.25) 和234.50(182.75,273.25)总体比较差异有统计学意义(χ2=11.978,P<0.05),第1次注射时和注射后1个月、2个月后的 ARNFL(μm)86.00(76.25,98.00) 83.00(76.00,95.50)和 83.00(76.25,94.75) 总体比较差异无统计学意义(χ2=11.978,P>0.05),第1次注射时和注射后1个月、2个月眼压(kPa)2.27(1.97,2.44),16.0(13.7,17.0),和 2.00(1.84,2.31) 总体比较差异无统计学意义(χ2=1.604,P>0.05)。结论 玻璃体腔注射康柏西普治疗湿性老年性黄斑变性对视网膜神经纤维厚度无明显的损害,安全且有效,但仍需要大样本量及长期随访观察。
Objective To observe the effect on the thickness of retinal nerve fibers in the patients with wet age-related macular degeneration by adopted intravitreal injection with Conbercept. Methods We analyzed 18 cases (18 eyes) that conform to the standard from 35 cases(38 eyes) retrospectively,who were treated with wet age-related macular degeneration by adopted intravitreal injection with Conbercept in Shaoguan Yuebei People's Hospital Affiliated to Shantou University from October in 2016 to October in 2017.After undergoing best corrected visual acuity (BCVA),intraocular pressure (IOP) and fluorescein angiography (FFA),all of them were adopted intravitreal injection with 0.05mL Conbercept. We observed the changes of best corrected visual acuity (BCVA) and retinal nerve fiber thickness (RNFL) after 1 and 2 months of adopted intravitreal injection with 0.05 mL Conbercept. Results 18 eyes were adopted intravitreal injection Conbercept 54 times totally. All the eyes were injected three times. After 3 months of injection,OCT showed that the visual acuity of 18 eyes improved and the thickness of CRT decreased. The BCVA values at the first injection,after the first injection and after the second injection were 0.1(0.04,0.12),0.20(0.10,0.30)和0.25(0.12,0.30)respectively. The overall difference was statistically significant (χ2=13.880,P< 0.001). CRT(μm)values at the first injection,after the first injection and after the second injection were 319.50(269.50,390.50),271.00(219.00,296.25) and 234.50(182.75,273.25)respectively. The overall difference was statistically significant (χ2=11.978,P< 0.05). ARNFL(μm)values at the first injection,after the first injection and after the second injection were 86.00(76.25,98.00),83.00(76.00,95.50) and 83.00(76.25,94.75)respectively. There was no significant difference (χ2=11.978,P> 0.05). The IOP(mmHg)values at the first injection,after the first injection and after the second injection were 17.0(14.8,18.3),16.0(13.7,17.0),和 15.0(13.8,17.3)respectively. There was no significant difference(χ2=1.604,P>0.05). Conclusion There was no obvious damage to the retinal nerve fiber by adopted intravitreal injection with Conbercept to treat the patients with wet age-related macular degeneration. That’s safe and effective,but need a large sample to follow-up for a longtime.

超声造影评价不同厚度颈动脉斑块内新生血管的临床研究

目的 通过超声造影成像技术( CEUS)观察颈动脉斑块内新生血管的情况,探讨颈动脉斑块的厚度与新生血管的关系。方法 选择颈动脉粥样硬化患者40例,经常规超声确诊存在斑块,共64个,应用CEUS成像技术检测斑块内新生血管情况。结果 不同厚度的颈动脉斑块的增强强度与斑块厚度成正相关(r=0.897 6,P=0.000 0)。结论 超声造影成像技术可以在常规超声的基础上通过检测斑块内的新生血管,进而判断斑块的性质,提供更多有用的信息进一步评估颈动脉斑块内新生血管增强强度与斑块厚度存在线性正相关性。
Objective To evaluate the neovascularization in carotid plaques by gray-scale harmonic contrast echocardiography (CEUS), and to investigate the relationship between the thickness of carotid artery plaque and the neovascularization.Methods 40 patients with carotid atherosclerosis were selected, and 64 plaques were diagnosed by regular ultrasound. The neovascularization in plaques was detected by CEUS imaging technique.Results The enhanced strength of carotid plaques with different thickness was positively correlated with plaque thickness (r=0.8976,P=0.0000).Conclusion Ultrasound contrast imaging can detect angiogenesis within the plaque based on conventional ultrasound, and then determine the nature of the plaque. More useful information is provided to further evaluate the linear positive correlation between the intensity of neovascularization and the thickness of plaque in carotid plaque.

Sirius与A超测量近视眼中央角膜厚度的比较

Comparison of central corneal thickness measurements with Sirius and ultrasound pachymetry in myopia

目的 探讨Sirius与A超对不同程度近视患者中央角膜厚度(CCT)的测量差异。方法 对225例(450眼)拟行角膜屈光手术的近视患者,按低、中、高度近视分三组,依次用Sirius和A超测量CCT。 测量结果行配对t检验、 Pearson相关及 Bland-Altman分析。结果 低、中、高度近视组中,Sirius所测CCT均数分别为(542.61±29.19)μm、(543.96±27.02)μm、(547.25±22.53)μm;A超CCT均数分别为(538.51±29.12)μm、(540.98±26.56)μm、(542.19±21.64)μm。不同近视组中,Sirius测量值均略大于A超值,且两者高度正相关,差异有统计学意义(P<0.05)。Bland-Altman分析,不同近视组中,两种仪器测量CCT的一致性较好。结论 Sirius在绝大多数情况下能够替代传统的A超角膜测厚仪,但对拟行激光手术的高度近视患者应该综合考虑两种仪器测量结果。
Objective To study the measurement differences of central corneal thickness(CCT) in different degree of myopia by Sirius and ultrasound pachymetry(UP). Methods 225 myopic subjects(450 eyes)were recruited and divided into three groups according to the degree of refractive errors. The CCT were obtained from the eyes for each subjects using the Sirius and UP. Data were compared by using paired t-tests and the Pearson correlations. Bland- Altman analysis of all pairs were determined. Results The mean CCT in low myopic group obtained from Sirius and UP were (542.61±29.19)μm and(538.51±29.12)μm,respectively. The mean CCT in medium myopic group by Sirius and UP were(543.96±27.02)μm and(540.98±26.56)μm.The measured mean CCT in high myopic group were(547.25±22.53)μm and(542.19±21.64)μm by Sirius and UP. The mean CCT measurements between the two instruments were high correlated and statistically different(P<0.05) in the three groups respectively. In different myopia groups Sirius has good coincidence with UP in measuring CCT according to the Bland-Altman plot. Conclusion In vast majority of cases,Sirius can replace traditional UP in measuring CCT. But for the preopreative examination of the high myopia patients, we need to think twice about the difference between the two measurements.
