目的 探讨福莫特罗联合格隆溴铵对慢性阻塞性肺疾病急性加重期(AECOPD)患者的疗效。方法 选择2021年1月—2023年6月廉江市人民医院收治的97例AECOPD患者进行研究,在双方知情基础上,通过单双号抽签方式分组,抽取单号纳入对照组(n=48,予以福莫特罗治疗),抽取双号纳入观察组(n=48,予以福莫特罗联合格隆溴铵治疗),对比两组肺功能指标、血气指标、症状改善情况及出现的不良反应。结果 治疗后两组的肺功能指标均高于治疗前,而观察组各指标高于对照组(P<0.05);治疗后两组PaO2高于治疗前,PaCO2低于治疗前,且观察组上述血气指标与对照组比较差异有统计学意义(P<0.05);治疗后两组的CAT及SGRQ评分均低于治疗前,其中观察组评分低于对照组(P<0.05);观察组(8.33%)与对照组(12.24%)不良反应率比较,差异不显著(P>0.05)。结论 采用福莫特罗联合格隆溴铵治疗AECOPD患者,有助于改善患者肺功能,使其血气指标得到改善,还可有效缓解其呼吸道症状,且安全性得到保障。
Objective To explore the curative effect of formoterol combined with glycopyrronium bromide on patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD). Methods The research cut-off period was from January 2021 to June 2023.A total of 97 patients with AECOPD were selected from Lianjiang People's Hospital for research.On the basis of mutual knowledge,they were divided into groups by odd and even numbers.The odd numbers were drawn into the control group(n=48,treated with formoterol)and the odd numbers were drawn into the observation group(n=48,treated with formoterol+glycopyrronium bromide),the pulmonary function index,blood gas index,symptom improvement and adverse reactions were compared between the two groups. Results After treatment,the lung function indexes of the two groups were higher than those before treatment,while the indexes of the observation group were higher than those of the control group(P<0.05).PaO2 was higher and PaCO2 was lower in the two groups after treatment,and the blood gas indexes in the observation group were different from those in the control group(P<0.05).The scores of COPD assessment test and St. George's Respiratory Questionaire in the two groups after treatment were lower than those before treatment,while the scores in the observation group were lower than those in the control group(P<0.05);There was no significant difference between the observation group(8.33%)and the control group(12.24%)(P>0.05). Conclusions The choice of formoterol and qualified ammonium bromide for AECOPD is helpful to improve the lung function,blood gas index and respiratory symptoms,And the security is guaranteed.

目的 探究新斯的明拮抗顺阿曲库铵的时机对患者腹腔镜结直肠癌根治术后肌松残留和早期肺功能的影响。方法 选取于2021年2月—2022年9月在我院行结肠癌根治手术的194例患者为研究对象,以随机数字表法将患者分为空白对照组和试验组,然后结合给予新斯的明治疗时所体现的4个成串刺激(TOF)比值将试验组患者分为试验组A(TOF≤0.1)、试验组B(0.10.8)。对比组间肌松残余情况以及肺功能情况。结果 插管前,试验组A患者TOF比值低于其他组(P<0.01),空白对照组患者TOF比值高于其他组(P<0.01);空白对照组分别与试验组A~E在复苏室接受观察的时间比较,差异无统计学意义(P>0.01);试验组A患者肌松恢复指数低于其他组(P<0.01),空白对照组患者肌松恢复指数高于其他组(P<0.01)。插管前、拔管0.5 h以及拔管24 h时,组间1秒用力呼气容积(FEV1)、用力肺活量(FVC)和FEV1/FVC比较差异无统计学意义(P>0.05)。结论 结直肠癌根治术后给予新斯的明,有助于加快肌松恢复,但不会影响复苏室停留时间与肺功能水平。
Objective To investigate the effect of timing of neostigmine antagonizing cisatracurium on residual muscle relaxation and early lung function in patients underwent laparoscopic radical resection of colorectal cancer.Methods February 2021 to September 2022,194 patients who underwent radical colon cancer surgery in our hospital were selected as the research subjects.The patients were randomly divided into a blank control group and an experimental group using a random number table method.Then,combined with the train-of-four(TOF)ratios observed during neostigmine treatment,the experimental group patients were divided into experimental group A(TOF≤0.1),experimental group B(0.10.8).The residual muscle relaxation and lung function between groups were compared.Results Before intubation,the TOF ratio of patients in experimental group A was lower than that of other groups(P<0.01),while the TOF ratio of patients in the blank control group was higher than that of other groups(P<0.01).There was no significant difference in the observation time between the blank control group and the experimental group A~E in the postanesthesia care unit(P>0.01).The muscle relaxation recovery index of patients in experimental group A was lower than that of other groups(P<0.01),while the muscle relaxation recovery index of patients in the blank control group was higher than that of other groups(P<0.01).There were no significant differences in force expiratory volume in one second(FEV1),forced Vital capacity(FVC)and FEV1/FVC between groups before intubation,0.5 h after extubation and 24 h after extubation(P>0.05).Conclusions Administration of neostigmine after radical surgery for colorectal cancer can accelerate muscle relaxation recovery,but it will not affect the residence time of the postanesthesia care unit and lung function levels.

目的 探讨哮喘患者的气道炎症表型分布及肺功能指标情况。方法 选择226 例哮喘患者为研究对象,其中50 例为重症哮喘,76 例为普通哮喘,对比哮喘患者的气道炎症表型分布情况及患者肺功能指标情况。结果 226 例哮喘患者中,嗜酸性粒细胞型最为常见,占36.73%,之后为中性粒细胞型(31.86%)、混合细胞型(22.12%)、寡细胞型(9.29%);重度哮喘患者中,中性粒细胞型患者肺功能相关指标均低于其它类型的重症患者(P<0.05)。结论 在哮喘气道炎症表型中,最常见的表型为嗜酸性粒细胞型,其中中性粒细胞型的哮喘患者的肺功能最差。
Objective To explore the phenotype distribution and lung function indicators of airway inflammation in asthmatic patients. Methods 226 cases of asthma patients were chosen as the research objects,in which 50 cases of severe asthma,76 cases of asthma,to compare asthma airway inflammation phenotype distribution and lung function index. Results Among 226 asthma patients,eosinophilic granulocytes were the most common,accounting for 36.73%,followed by neutrophilic granulocytes (31.86%),mixed cell types (22.12%) and oligocytes (9.29%). Among patients with severe asthma, the lung function of neutrophil patients was lower than that of other severe patients(P < 0.05). Conclusion Among asthmatic airway inflammatory phenotypes, the most common phenotype is eosinophilic granulocyte type, among which neutrophil asthmatic patients have the worst lung function.

目的 探讨无创呼吸机辅助常规药物治疗重症哮喘的疗效及对肺功能的影响。方法 选取我院于2014年2月—2017年6月间收治的60例重症哮喘患者作为研究对象,按照随机数字法分为对照组和研究组,对照组30例予以常规药物治疗,研究组30例予以无创呼吸机辅助常规药物治疗。比较两组患者临床疗效、肺功能指标、气血指标、心率、哮喘症状评分以及药物使用情况。结果 研究组临床有效率(96.66%)高于对照组(76.66%)(P<0.05)。2组患者干预前肺功能指标、气血指标及心率比较无差异(P>0.05),干预后两组心率、PaCO2均较干预前降低,PaO2、FEV1、FEV1/FVC均较干预前升高(P<0.05),研究组较对照组改善明显(P<0.05)。两组患者干预前哮喘症状评分比较无差异(P>0.05),干预后两组哮喘症状评分均较干预前降低,且研究组较对照组降低明显(P<0.05)。研究组使用茶碱类药物、糖皮质类激素、受体激动剂药物比例均低于对照组(P<0.05)。结论 无创呼吸机辅助常规药物治疗重症哮喘可显著改善患者血气指标及肺功能,疗效确切。
临床诊疗
目的 了解足月小样儿生后早期的潮气呼吸肺功能及1岁内发生喘息的情况。方法 选择我院2015年3月—2016年3月阴道分娩的足月小样儿及同期胎龄相近、阴道分娩的健康足月儿,比较2组生后5~7天的潮气呼吸肺功能各项指标及1岁内发生喘息的次数。结果 2组出生胎龄、性别、Apgar评分、母亲吸烟史、一级亲属中哮喘史比较均无差异;研究组中分钟通气量(MV)、潮气量(VT/kg)、达峰时间比(TPEF/TE)、达峰容积比(VPEF/VE)均低于对照组,差异有统计学意义(P<0.05);呼吸频率(RR)、吸气时间(Ti)、呼气时间(Te)、吸呼比(Ti/Te)、潮气呼吸呼气峰流(PTEF)比较无差异(P>0.05);研究组1岁内发生喘息的次数多于对照组,差异有统计学差异。结论 足月小样儿生后早期肺容量小于健康足月儿,小气道阻塞更明显,可能影响1岁内发生喘息的几率。
临床诊疗
目的 探讨吸烟对稳定期COPD患者炎症反应和肺功能的影响。方法 选取2013年8月—2016年9月我院门诊收治的稳定期COPD患者70例为研究对象,其中吸烟35例(X1组)、不吸烟35例(X2组),另选取同期入院的不吸烟健康志愿者35例纳入健康组,采用酶联免疫吸附试验(ELISA)测定血清白介素-6(IL-6)、白细胞介素-8(IL-8)及肿瘤坏死因子-α(TNF-α)水平,以肺功能检测仪测定三组一秒用力呼气容积(FEV1)、一秒用力呼气容积/用力肺活量比值(FEV1/FVC)、FEV1占预计值百分比(FEV1%),并采用自拟症状评分表及简明健康调查简表(SF-36)评价呼吸困难程度及生活质量。结果 X1组IL-6、IL-8及TNF-α依次为(135.27±1.24)pg/mL、(189.45±1.14)pg/mL、(39.39±1.14)pg/mL,明显高于X2组、健康组(P均<0.05);X1组FEV1(0.75±0.14)L、FEV1/FVC(3.65±1.87)%、FEV1%(3.45±0.12)%低于X2组、健康组(P均<0.05);X1组症状积分(10.17±1.02)分较X2组、健康对照组高(P<0.05),而其SF-36评分(54.27±1.46)分明显低于X2及健康组(P<0.05);X2组上述指标与健康组比较亦有统计学意义(P均<0.05)。结论 吸烟可明显增加稳定期COPD患者IL-6、IL-8、TNF-α等炎症因子水平,同时降低肺功能,临床应采取措施进行有效干预,防止患者病情恶化。

目的 探讨加味六君子汤联合经口营养补充对COPD稳定期伴营养不良患者营养状况和肺功能的影响。方法 收集呼吸内科COPD稳定期并伴营养不良的患者40例,分为试验组和对照组各20例。试验组在对照组基础上再给予加味六君子汤和经口营养补充,观察治疗前、治疗后3月的营养相关指标和肺功能指标,然后进行统计分析。结果 ①治疗后3月,试验组的体质量、白蛋白、前白蛋白和肺功能指标较治疗前均明显改善(P<0.05)。②与对照组比较,试验组的白蛋白、前白蛋白、血红蛋白均明显升高(P<0.05),肺功能亦有显著改善(P<0.05)。③上臂肌围(MAMC)、三头肌皮褶厚度(TSF)在治疗后的组内及组间比较差异均无统计学意义(P>0.05)。结论 加味六君子汤联合经口营养补充有助于COPD稳定期伴营养不良患者体质量增加、蛋白升高,营养状况好转,从而增加呼吸肌储备,改善患者肺功能。
Objective To observe the influence of modified-liujunzi-decoction combined with Oral nutritional supplements(ONS) on the nutrition status and lung function in patients with chronic obstructive pulmonary disease(COPD). Methods 40 patients of COPD stable phase with malnutrition were divided into two groups and each group with 20 cases. On the basis of the control group, the experimental group was given again modified-liujunzi-decoction and ONS. All cases were tested before and 3 months after treatment, for detecting the nutrition related index and lung function index; T-test was used for data statistical analysis. Results ①3 months after treatment, the body weight, serum albumin, prealbumin and lung function index of the experimental group were significantly improved compared with those before treatment (P<0.05). ②Compared with the control group, the serum albumin, prealbumin and hemoglobin were significantly increased in the experimental group (P<0.05), and lung function was also significantly improved (P<0.05). ③After treatment, the mid-arm muscle circumference (MAMC), triceps skinfold thickness (TSF) comparison of intra group and intra group differences were not statistically significant(P>0.05). Conclusion Modified-liujunzi -decoction combined with oral nutritional supplements help to increase body weight, protein, and improve nutritional status in patients of COPD stable phase with malnutrition, thereby increasing their respiratory muscle reserve and improving lung function.

目的 本研究通过检测吸入布地奈德治疗支气管哮喘患儿前、后气道重塑、肺功能及外周血TGF-β浓度的变化,探讨布地奈德在哮喘患儿气道重塑早期干预的作用。方法 将2013年1月—2014年10月在我院确诊为支气管哮喘患儿18例,给予规律吸入布地奈德治疗6个月,分别于治疗前、后检测肺功能、外周血TGF-β浓度以及通过高分辨CT(HRCT)测量支气管管壁厚度(T)并计算管壁面积(WA)、支气管管壁厚度与气管外径之比(WT%)、气道壁横截面面积占气道总截面积的的百分比(WA%)。结果 吸入布地奈德治疗治疗6个月后,哮喘患儿WT%、WA%、肺功能以及TGF-β浓度均明显改善,与治疗前比较差异有统计学意义(P<0.05)。结论 规律吸入布地奈德治疗能减轻支气管哮喘患儿早期气道重塑,改善肺功能,可能通过下调细胞因子TGF-β的表达有关。
Objective We detected the changes of Airway remodeling,lung function and peripheral blood concentrations of TGF-β in asthmatic children before and after inhaled budesonide, and explored the role of budesonide in airway remodeling before early intervention in asthmatic children. Methods 32 cases of children were diagnosed asthma in our hospital,and given treatment of inhaled budesonide for 6 months, and to detect lung function,pheral blood TGF-β concentration as well as measuring the bronchial wall thickness (T) and calculate the wall area (WA), the outer diameter of the trachea and bronchial wall thickness ratio (WT%), the cross-sectional area of the airway wall accounted for airway percentage (WA%) of the total cross-sectional area by high-resolution CT (HRCT) before and after treatment. Results After treatment with inhaled budesonide in six months, the asthma WT%, WA%, lung function, and TGF-β concentrations were significantly improved, the differences compared with that before treatment were statistically significant(P<0.05). Conclusion The treatment of inhaled budesonide can alleviate asthmatic airway remodeling in early, improve lung function,possibly related to down the expression TGF-β.
