目的:描述发病48 h内急性缺血性卒中(acute ischemic stroke,AIS)患者依达拉奉右莰醇真实世界用药特征,探讨48 h内不同启动时间与住院期间神经功能改善及短期预后的关系。方法:回顾性连续筛选2023年12月1日至2026年4月30日本院诊断为AIS并使用依达拉奉右莰醇的住院患者,药学部基于药学信息系统、住院医嘱及病历记录提取资料。初筛147例,排除35例,纳入112例发病至首次用药时间(onset-to-treatment time,OTT)≤48 h者;按预设24 h界值分为24 h内用药组(n=67)和24~48 h用药组(n=45)。主要结局为住院期间美国国立卫生研究院卒中量表(National Institutes of Health Stroke Scale,NIHSS)变化值(ΔNIHSS=入院NIHSS评分-出院前NIHSS评分);次要结局包括出院前NIHSS评分、出院改良Rankin量表(modified Rankin Scale,mRS)评分、显著神经功能改善、住院时间、出院去向及安全性事件。采用多因素线性回归分析24 h内用药与ΔNIHSS的相关性,并行简化模型、排除再灌注治疗患者、完整疗程人群、进一步调整大血管闭塞及OTT连续变量模型等敏感性分析。结果:总体OTT为19.65(14.97,34.68)h,疗程12.00(11.00,13.00)d,完成相对完整疗程比例89.3%。两组入院NIHSS评分差异无统计学意义[8.00(6.00,9.00)分 vs 8.00(6.00,10.00)分,P=0.447];24 h内用药组出院前NIHSS评分更低[5.00(4.00,6.50)分 vs 6.00(5.00,8.00)分,P=0.025],ΔNIHSS更高[3.00(2.00,3.00)分 vs 2.00(1.00,2.00)分,P<0.001],显著神经功能改善率更高(77.6% vs 60.0%,P=0.045)。多因素校正后,24 h内用药仍与更大的ΔNIHSS相关(β=0.768,95%CI:0.377~1.159,P<0.001);Logistic探索性分析显示其与显著神经功能改善发生可能性较高相关(OR=2.475,95%CI:1.047~5.853,P=0.039)。两组出血转化、症状性颅内出血、药物相关不良反应及院内死亡差异均无统计学意义。结论:本单中心真实世界队列中,发病48 h内依达拉奉右莰醇治疗疗程完成比例较高、短期安全性事件发生率低;24 h内启动治疗与住院期间NIHSS改善幅度较大及显著神经功能改善率较高相关。由于为回顾性观察性研究,结果应解释为关联性证据,需前瞻性、多中心研究及长期功能结局验证。
Objective: To describe real-world treatment characteristics of edaravone dexborneol in acute ischemic stroke (AIS) patients treated within 48 hours of onset and to explore the association between initiation time and short-term in-hospital outcomes. Methods: This single-center retrospective real-world study screened hospitalized AIS patients with edaravone dexborneol records from December 1, 2023 to April 30, 2026. Medication data were extracted from the pharmacy information system, inpatient orders, and medical records. After 35 exclusions, 112 of 147 patients with onset-to-treatment time (OTT) ≤48 h were included and classified by a prespecified 24-hour cutoff into a within-24-hour group (n=67) and a 24-48-hour group (n=45). The primary outcome was in-hospital National Institutes of Health Stroke Scale (NIHSS) change (ΔNIHSS=admission NIHSS minus pre-discharge NIHSS); secondary outcomes included pre-discharge NIHSS, discharge modified Rankin Scale (mRS), marked neurological improvement, length of stay, discharge destination, and safety events. Multivariable linear regression assessed the association between within-24-hour treatment and ΔNIHSS, with sensitivity analyses using simplified adjustment, exclusion of reperfusion-treated patients, the complete-course population, additional adjustment for large-vessel occlusion, and continuous OTT modeling. Results: Median OTT was 19.65 (14.97, 34.68) h, treatment duration was 12.00 (11.00, 13.00) days, and 89.3% completed a relatively complete course. Baseline NIHSS was comparable [8.00 (6.00, 9.00) vs 8.00 (6.00, 10.00), P=0.447]. The within-24-hour group had lower pre-discharge NIHSS [5.00 (4.00, 6.50) vs 6.00 (5.00, 8.00), P=0.025], greater ΔNIHSS [3.00 (2.00, 3.00) vs 2.00 (1.00, 2.00), P<0.001], and a higher marked improvement rate (77.6% vs 60.0%, P=0.045). After adjustment for age, admission NIHSS, reperfusion therapy, and atrial fibrillation, within-24-hour treatment remained associated with greater ΔNIHSS (β=0.768, 95% CI: 0.377-1.159, P<0.001). Exploratory logistic regression showed a higher likelihood of marked improvement (OR=2.475, 95% CI: 1.047-5.853, P=0.039). Hemorrhagic transformation, symptomatic intracranial hemorrhage, drug-related adverse reactions, and in-hospital death did not differ significantly. Conclusion: In this real-world cohort, edaravone dexborneol within 48 h of AIS onset showed a high complete-course proportion and low short-term safety event incidence. Initiation within 24 h was associated with greater in-hospital NIHSS improvement and a higher marked improvement rate than initiation at 24-48 h. These findings are associative and need prospective multicenter validation with long-term functional outcomes.

急性缺血性脑卒中介入治疗优化护理路径的构建及应用

Development and application of an optimized nursing pathway for endovascular therapy in acute ischemic stroke

       目的   探讨优化护理路径在急性缺血性脑卒中介入手术中的应用及效果。方法   采用随机对照研究,纳入2024年1月—2025年1月收治的112例急性缺血性脑卒中取栓患者,观察组(56例)实施优化介入护理路径,对照组(56例)采用常规护理。结果   观察组患者的满意度评分、医护满意度评分均优于对照组(P<0.05);观察组的术前等待时间、患者自进入介入室至股动脉穿刺时间、手术时间、住院费用、并发症发生率、差错事故发生率也优于对照组(P<0.05)。结论   通过标准化术前准备、术中多学科协作及术后康复衔接,优化护理路径可显著提升急性缺血性脑卒中治疗时效性(缩短急性脑卒中患者从进入医院到开始静脉溶栓给药的时间至黄金时间窗内),改善患者神经功能预后。
       Objective  To explore the application and efficacy of an optimized nursing pathway in endovascular therapy for acute ischemic stroke(AIS).Methods  A randomized controlled trial was conducted,enrolling 112 AIS patients undergoing mechanical thrombectomy from January 2024 to January 2025.Patients were randomly assigned to either the observation groupn=56),which received the optimized interventional nursing pathway,or the control group(n=56),which  received  routine nursing care.Results  Patient satisfaction score,medical staff satisfaction score,and postoperative favorable  recovery  rate in observation group were better than those of control group(P<0.05).Additionally,the observation group  showed  significant improvements in preoperative waiting time,time from entering the operation room to femoral artery puncture,surgical duration,hospitalization costs,complication incidence rate,and medical error incidence rate(P<0.05).Conclusions  By standardizing preoperative preparations,implementing intraoperative multidisciplinary collaboration,and enhancing postoperative  rehabilitation coordination,the optimized nursing pathway significantly improved the timeliness of thrombectomy treatment(shortening door-to-needle time to reach the golden time window)and enhanced neurological functional prognosis in AIS patients,demonstrating substantial clinical applicability.

目的 探讨复方脑肽节苷脂注射液联合阿替普酶静脉溶栓治疗急性缺血性脑卒中的疗效。方法 选择2019年6月—2023年1月在河南省许昌中医院诊治的79例缺血性脑卒中患者,按照入院就诊顺序采用奇偶法把患者分为联合组40例与传统组39例。传统组给予阿替普酶静脉溶栓治疗,联合组给予复方脑肽节苷脂注射液联合阿替普酶静脉溶栓治疗,评价与记录联合组与传统组的疗效与安全性及治疗前、治疗14 d后Fugl-Meyer评定量表(FMA)与改良版Banhel评价指数(MBI)、血清β-内啡肽、前列腺素E2(PGE2)水平变化。结果 治疗14 d后联合组的总有效率更高(97.5% vs 82.1%,P<0.05),并发症发生率更低(5.0% vs 20.5%,P<0.05)。联合组与传统组治疗14 d后的FMA与MBI评分高于治疗前(P<0.05),联合组治疗14 d后的FMA评分、MBI评分比传统组提高(P<0.05)。联合组与传统组治疗14 d后的血清β-内啡肽、PGE2水平低于治疗前(P<0.05),联合组治疗14 d后的血清β-内啡肽、PGE2水平低于传统组(P<0.05)。结论 复方脑肽节苷脂注射液联合阿替普酶静脉溶栓治疗急性缺血性脑卒中能提高效果,降低患者的肺部感染、尿路感染、静脉血栓、褥疮等并发症发生率,改善患者的运动与日常生活功能,还可降低患者血清β-内啡肽、PGE2水平。
Objective To explore and analysis the efficacy and safety of compound brain peptide ganglioside injection combined with alteplase intravenous thrombolysis in the treatment of acute ischemic stroke. Methods From June 2019 to January 2023,79 patients with ischemic stroke who were diagnosed and treated at Henan Xuchang Traditional Chinese Medicine Hospital were selected as the research subjects.According to the order of admission,the patients were divided into the combination group of 40 cases and the traditional group of 39 cases.The traditional group received intravenous thrombolytic therapy with alteplase,while the combination group received treatment with compound brain peptide ganglioside injection on the basis of the traditional group.The efficacy and safety and changes of the Fugl-Meyer rating Scale(FMA)and modified Banhel Evaluation Index(MBI),serum β-endorphin,and PGE 2 levels in the combined and traditional groups were evaluated and recorded. Results After 14 days of treatment,the combined group had a higher overall response rate(97.5% vs 82.1%,P<0.05)and a lower complication rate(5.0% vs 20.5%,P<0.05).FMA scores and MBI scores of the two groups after 14 days of treatment were higher than those before treatment(P<0.05),and FMA scores and MBI scores of the combined group after 14 days of treatment were higher than those of the traditional group(P<0.05).The contents of β-endorphin and prostaglandin E2 in two groups after 14 days of treatment were lower than those before treatment(P<0.05),and the contents of β-endorphin and prostaglandin E2 in combination group after 14 days of treatment were lower than those in traditional group(P<0.05). Conclusions The combination of compound brain peptide ganglioside injection and alteplase intravenous thrombolysis in the treatment of acute ischemic stroke can improve the treatment effect,reduce the incidence of complications such as pulmonary infections,urinary tract infections,venous thrombosis and bedsores in patients,improve patients' motor and daily life functions,and also reduce patients' serum levels of β-endorphins and prostaglandin E2.

目的 评价银杏二萜内酯葡胺注射液(DGMI)联合依达拉奉右崁对急性缺血性脑卒中(AIS)的治疗效果。方法 回顾性分析我院2021年3月—2022年6月收治的86例AIS患者,根据治疗方法不同,分为单药组和联合组,每组43例,2组均予以DGMI治疗,联合组加用依达拉奉右崁醇。对比2组的治疗效果。结果 治疗后,联合组美国国立卫生研究院卒中量表(NIHSS)评分低于单药组(P<0.05),简易智能精神状态检查量表(MMSE)和Barthel指数(BI)得分高于单药组(P<0.05);联合组治疗后脑血流动力学指标(Qmin和Vmin)高于单药组(P<0.05),而全血还原黏度、血浆黏度和血小板聚集指数低于单药组(P<0.05);联合组治疗后血清丙二醛(MDA)、超敏C-反应蛋白(hsCRP)和白细胞介素-6(IL-6)低于单药组(P<0.05),而超氧化物歧化酶(SOD)高于单药组(P<0.05);联合组治疗总有效率88.37%,高于单药组的67.44%(P<0.05),且2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论 DGMI联合依达拉奉右崁醇可有效提高AIS患者的神经功能、认知功能及日常生活能力,降低血液高凝状态,改善脑部血流,缓解机体氧化应激及炎症反应,且不良反应发生率较低,安全性良好。
Objective To evaluate the clinical efficacy of diterpene ginkgolides meglumine injection(DGMI)combined with edaravone dextracanthol in the treatment of acute ischemic stroke(AIS).Methods A total of 86 patients with AIS admitted to our hospital from March 2021 to June 2022 were divided into single drug group and combination group according to different treatment methods,with 43 patients in each group.Both groups were treated with DGMI,and the combination group was added with edaravone dextracanthol.The therapeutic effect,neurological function and adverse reactions of the two groups were compared.Results After treatment,compared with the single drug group,the NIH Stroke Scale(NIHSS)score of the combination group was significantly lower(P<0.05),and the Mini-Mental State Examination(MMSE)and Barthel index(BI)scores were significantly higher(P<0.05).After treatment,the cerebral hemodynamic indexes(Qmin and Vmin)in the combination group were significantly higher(P<0.05),while the whole blood reducing viscosity,plasma viscosity and platelet aggregation index were significantly lower(P<0.05).After treatment,compared with the single drug group,the serum malondialdehyde(MDA),high sensitive C-reactive protein(hsCRP)and interleukin- 6(IL-6)levels in the combination group were significantly lower(P<0.05),while the level of superoxide dismutase(SOD)was significantly higher(P<0.05).The total effective rate of the combination group was 88.37%,which was significantly higher than 67.44% of the single drug group(P<0.05).The incidence of adverse reaction between two groups was not significant(P>0.05).Conclusions DGMI combined with edaravone dextracanthol can effectively improve the neurological function,cognitive function and daily living ability of patients with AIS,reduce blood hypercoagulability,improve cerebral blood flow,alleviate oxidative stress and inflammatory reaction,improve the therapeutic effect,without increasing the incidence of adverse reactions,which has good safety.

SII、NLR和PLR 水平对急性缺血性卒中伴发卵圆孔未闭的诊断价值

目的 评估全身免疫炎症指数(SII)、中性粒细胞/淋巴细胞比值(NLR)和血小板/淋巴细胞比值(PLR)在区分急性缺血性卒中(AIS)伴发卵圆孔未闭(PFO)与非伴发PFO患者的价值。方法 回顾性分析100例AIS患者的血液和血清指标,计算SII、NLR和PLR,使用Logistic回归及受试者操作特征(ROC)曲线分析3项指标在鉴别AIS伴发PFO与非伴发PFO中的价值。结果 伴发PFO的AIS患者SII、NLR、PLR高于非伴发PFO的AIS患者,其中以SII最为明显(P均<0.05)。单因素Logistic回归显示,中性粒细胞计数、淋巴细胞计数、PLR、NLR、SII与AIS伴发PFO有关(P<0.05)。ROC曲线分析结果,SII、NLR、PLR鉴别AIS伴PFO与非伴PFO患者,最佳阈值分别为476.4、1.99、115.3,曲线下面积分别为0.777、0.767、0.708。结论 SII、NLR和PLR可作为鉴别AIS患者是否伴发PFO的生物标志物,具有潜在临床应用价值。
Objective To evaluate the value of systemic immune-inflammatory index(SII),neutrophil/lymphocyte ratio(NLR)and platelet/lymphocyte ratio(PLR)in distinguishing acute ischemic stroke(AIS)patients with patent foramen ovale(PFO)and without PFO.Methods A retrospective analysis of blood and serum indicators in 100 AIS patients was conducted,and SII,NLR and PLR indices were calculated.Logistic regression and ROC curve analyses were performed.Results SII,NLR and PLR were significantly higher in PFO patients than in non-PFO patients,with SII being the most significant.Univariate logistic regression showed that Neu,Lym,PLR,NLR,and SII variables were significantly associated with AIS combined with PFO(P<0.05).ROC curve analysis revealed that the optimal cut-off values for SII,NLR and PLR in distinguishing AIS patients with PFO from those without PFO were 476.4,1.99 and 115.3,respectively,with area under the curve of 0.777,0.767 and 0.708.Conclusions SII,NLR and PLR can serve as biomarkers for identifying AIS patients with PFO,offering potential clinical application value.

老年急性缺血性脑卒中并发肺部感染的危险因素

The risk factors of pulmonary infection in elderly patients with acute ischemic stroke

目的 探讨老年急性缺血性脑卒中并发肺部感染患者的临床特征和相关危险因素。方法 采用回顾性研究方法,选择2017年7月— 2019年10月深圳市第二人民医院神经内科收治的1 113例老年急性缺血性脑卒中患者,其中卒中并发肺部感染患者(108 例)纳入感染组,未并发肺部感染患者(1 005例)纳入对照组。以单因素对比分析两组患者的临床资料,采用 Logistic多因素回归分析方法分析合并肺部感染的高危因素。结果 单因素分析提示两组年龄(尤其是高龄患者)、住院天数、房颤、脑梗死史、慢性肺疾病、吞咽困难、言语不清、意识障碍差异有统计学意义(P <0.05)。多因素 Logistics 回归分析显示,高龄(≥80岁)、住院天数、脑梗死史、吞咽困难、言语不清、意识障碍与老年急性缺血性脑卒中并发肺部感染密切相关。结论 老年急性缺血性脑卒中并发肺部感染的独立危险因素主要是高龄(≥80 岁)、住院天数、脑梗死史、吞咽困难、言语不清、意识障碍,临床应高度重视。
Objective Objective To investigate the clinical characteristics and risk factors of elderly patients with acute ischemic stroke complicated with pulmonary infection. Methods A retrospective study was conducted on 1 113 elderly patients with acute ischemic stroke admitted to the department of neurology, Shenzhen Second People's Hospital from July 2017 to October 2019. Among them, 108 patients with stroke complicated with pulmonary infection were included in the infection group and 1 005 patients without concurrent pulmonary infection were included in the control group. The clinical data of the two groups were analyzed by single factor comparison, and the risk factors for pulmonary infection were analyzed by logistic multiple factor regression analysis. Results Single factor analysis showed there were statistically significant differences between the two groups in age (especially elderly patients), length of stay in hospital, atrial fibrillation, history of cerebral infarction, chronic pulmonary disease, dysphagia, slurred speech, and disturbance of consciousness (P <0.05). Multi-factor logistic regression analysis showed that old age (≥80 years old), length of stay in hospital, history of cerebral infarction, dysphagia, slurred speech, and disturbance of consciousness were closely related to pulmonary infection in elderly patients with acute ischemic stroke. Conclusion The independent risk factors of acute ischemic stroke complicated with pulmonary infection in the elderly mainly include old age (≥80 years old), length of stay in hospital, history of cerebral infarction, dysphagia, slurred speech and disturbance of consciousness, which should be paid more attention to clinically.

急性脑梗死静脉溶栓决策延迟影响因素

Factors influencing decision delays in intravenous thrombolysis for acute ischemic stroke

       目的   探讨影响急性脑梗死患者接受重组组织型纤溶酶原激活剂(rt-PA)静脉溶栓治疗决策延迟的因素,并提出减少溶栓决策时间的建议。方法   采用回顾性分析方法,对泉州市第一医院2023年通过急诊收治的120例急性脑梗死患者及其家属的资料进行分析。根据溶栓决策时间,以5 min为界限,将患者分为非延迟组(62例)和延迟组(58例),并对两组资料进行比较分析。采用SPSS 23.0统计软件分析数据,运用t检验(针对连续变量)和χ 2 检验(针对分类变量)比较组间的差异。此外,通过Logistic回归分析,确定影响急性脑梗死患者静脉溶栓治疗决策的危险因素。结果   对比两组患者数据后发现,籍贯地区、冠心病史、外院转入、发病到入院时间以及决策家属数量比较差异具有统计学意义(P<0.05)。多因素Logistic回归分析进一步揭示,非泉州籍贯地区[OR(95%CI):9.29(2.21~38.97),P=0.002]、决策家属人数≥2人[OR(95%CI):18.73(5.96~58.80),P<0.001]、从外院转入[OR(95%CI):10.26(2.09~50.42),P=0.004]以及发病到入院时间3.0~4.5 h[OR(95%CI):4.09(1.45~11.48),P=0.008]是导致治疗决策延迟的独立危险因素。结论   患者非泉州籍贯地区、外院转入、溶栓决策家属人员≥2个、发病到入院时间3~4.5 h是溶栓决策延误的影响因素,提出优化卒中急诊抢救流程以及通过卒中宣教以缩短溶栓决策时间的建议。
       Objective  To investigate the factors influencing delays in decision-making for intravenous thrombolysis with recombinant tissue plasminogen activator(rt-PA)in patients with acute cerebral infarction and to propose  recommendations for reducing thrombolysis decision-making time.Methods  A  retrospective analysis was conducted on data from 120 patients with acute cerebral infarction admitted to the emergency department at Quanzhou First Hospital in 2023,including information from their families.Patients were divided into non-delay group (62 cases) and delay group (58 cases) based on a 5-minute threshold for thrombolysis decision-making time.Comparative analysis between the two groups was performed using SPSS 23.0 statistical software,with t-tests for continuous variables and χ 2  tests for categorical variables.Logistic  regression analysis was employed to identify  risk factors affecting decision-making for intravenous thrombolysis in these patients.Results  Statistical analysis  revealed significant differences between the two groups in terms of native region,history of coronary heart disease,transfer from other hospitals,time from onset to hospital admission,and number of family members involved in decision-making(P<0.05).Multivariate Logistic regression identified independent risk factors for delayed treatment decisions:non-Quanzhou native region (OR[95%CI]:9.29[2.21-38.97],P=0.002),having two or more decision-making family members (OR[95%CI]:18.73[5.96-58.80],P<0.001),transfer from other hospitals (OR[95%CI]:10.26[2.09-50.42],P=0.004),and a time from onset to hospital admission of 3-4.5 hours (OR[95%CI]:4.09[1.45-11.48],P=0.008).Conclusions  Factors  such as non-Quanzhou native region,transfer from other hospitals,having two or more family members involved in decision-making,and a time from onset to hospital admission of 3-4.5 hours are associated with delays in thrombolysis decision-making.Optimizing the emergency  rescue process for stroke and shortening the thrombolytic decision-making time through stroke education are suggested.
