不同类型职业病危害因素影响暴露企业工人的生命质量调查研究

目的 探究不同类型职业病危害因素对暴露工人生命质量的影响。方法 在某市电子制造业、灯饰制造业、金属冶炼业等能够接触到粉尘、铅、苯以及噪声的6家企业中,采用整群随机抽样法,共计抽取742人为研究对象,对上述企业2015—2017年的职业病危害因素检测报告进行回顾性分析,主要对各企业的职业病危害因素以及暴露水平实施分析,并对企业员工的生命质量情况实施调查。结果 三种不同行业之间的有机溶剂检测合格率比较差异均有统计学意义(P<0.05),其中有机溶剂检测合格率最低的是金属冶炼业;三种不同行业的粉尘检测合格率差异以及噪声检测合格率比较差异无统计学意义(P>0.05),其中灯饰制造业的噪声及粉尘检测合格率最低;各行业工作人员的生理、社会功能,心理、情感职能,精神健康,身体疼痛、活力以及总计健康评分差异有统计学意义(P<0.05),电子制造企业的各项评分均高于金属冶炼业以及灯饰制造业。多重线性回归分析结果发现,电子制造企业的生理与心理健康维度得分均高于灯饰制造业、金属冶炼业,差异有统计学意义(P<0.05),且主要与运动锻炼以及所从事的行业有较大关联。结论 不同类型的职业病危害因素暴露水平存在一定差异,企业职工的生命质量也会因此受到不同程度的影响,应对存在噪声、粉尘以及大量使用有机溶剂等职业病危害因素企业作为健康监护的重点。

120例COPD稳定期患者用药情况调查分析

目的 了解我区慢性阻塞性肺疾病(COPD)稳定期患者药物使用现状,为临床规范治疗COPD指明方向。方法 对在我院治疗的COPD患者采取问卷调查的方式对其稳定期自我用药情况进行调查分析,调查内容包括吸烟史、病程、稳定期用药的种类、用药方式等。结果 我区COPD患者稳定期治疗药物种类繁多,应用LABA/ICS、吸入LAMA比例较低,存在滥用口服抗生素及无指征使用口服激素等不规范用药情况。结论 我区COPD患者稳定期药物部分符合COPD指南推荐,但存在不规范用药。

临床技能学课程的满意度调查研究

目的 分析本科医学生对临床技能学培训课程的满意度,探索可提高临床技能学课程的改进措施。方法 抽取2015级不同专业医学生参加临床技能学课程的学生,通过问卷调查学生对培训课程的满意度。结果 临床技能学体系改革后,学生对临床技能学课程设置和老师的满意度都很高,对课程设置非常满意有18.31%,满意有58.03%,基本满意有21.69%,不满意的只有1.41%;对老师非常满意有36.9%,满意有49.86%,基本满意有12.68%,不太满意只有0.56%。结论 临床技能学可提高学生的临床综合能力和思维能力,可以持续的开展。
Objective To Through the analysis of the satisfaction surveys of clinical skills course, we aimed to make improvements on the teaching of clinical skills course. Methods With the method of stratified sampling, we chose medical students of different majors in the grade 2015 who attended the clinical skills course, and carried out the surveys of their satisfaction on the course. Results Students were satisfied with the design and the teaching of clinical skills course after the reform of clinical skills teaching. With the design of clinical skills course, 18.31% students were very satisfied, 58.03% were satisfied, 21.69% were basically satisfied, and just 1.41% were unsatisfied; With the teachers of clinical skills, 36.9% students were very satisfied, 49.83% were satisfied, 12.68% were basically satisfied, and just 0.56% were unsatisfied. Conclusion Clinical skills course may improve students' clinical comprehensive abilities and clinical thinking. It should be carried out continuously.

中医妇科门诊患者焦虑及抑郁状态的现状调查

目的 探讨中医妇科门诊患者的焦虑及抑郁的发生情况,优化诊治工作,更好地服务患者。方法 采用焦虑自评量表(SAS)和 抑郁自评量表(SDS)评定190名中医妇科门诊患者的心理状态。结果 中医妇科患者焦虑均分高于正常常模组(P<0.05),抑郁评分与全国常模相比则无统计学差异(P>0.05),有焦虑及抑郁症状的患者分别为6.3%及10%。结论 中医妇科门诊患者的焦虑症状较正常人群高,医生在临床诊治过程应重视对患者心理状态的评价,并进行相应的心理干预及人文关怀, 以提高患者对治疗的依从性和满意度,为其身体及心理健康打下良好基础。
Objective To explore the occurrences of anxiety and depression in female TCM gynecology outpatients, and thus to make improvements in diagnosis, treatment, and service for the patients. Methods We adopted Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) to evaluate the mental status of 190 TCM gynecology outpatients. Results Anxiety scores in TCM gynecology group were higher than those in the normal group (P<0.05), while there was no statistical difference in depression scores between these two groups (P>0.05). Among the outpatients, 6.3% had symptoms of anxiety, and 10% had symptoms of depression. Conclusion TCM gynecology outpatients show more anxiety symptoms than normal people. We should pay attention to the assessment of those patients' mental status in the process of diagnosis and treatment, and offer psychological intervention and humane care to those patients accordingly. In this way, we could enhance patients' compliance and satisfaction, and help them build a solid foundation in physical health as well as in mental health.

实体化形势下社区医护人员心理健康状况调查及干预效果评估

目的 了解社区医护人员的心理健康状况,评估心理干预措施效果,找到提高社区医护人员心理健康状况的有效措施。方法 采用《症状自评量表 SCL-90》对社区医护人员的心理健康状况进行基线调查,在干预3、6、9个月时分别进行SCL-90复测,以评估心理干预措施效果;根据《医护人员工作压力测试量表》的调查结果制定有针对性的心理干预措施并适时调整。结果 社区医护人员SCL-90测评总分和躯体化、强迫、人际敏感、焦虑、敌对、恐怖、偏执7个因子得分均高于全国常模水平(P<0.05);经过综合干预,随访期间SCL-90测评总分呈逐步下降趋势,干预6个月时,除人际敏感因子外,SCL-90测评总分及各因子分已趋于正常水平,与全国常模比较差异没有统计学意义(P<0.05)。至9个月时,人际敏感因子得分也恢复至正常水平。结论 社区医护人员心理健康状况低于全国常模水平,进行针对性的综合干预可提高其心理健康水平,实验显示干预时长应不低于6个月。
Objective To understand the mental health status of community health care workers, assess the effect of psychological intervention measures, and find effective measures to improve the mental health status of community health care workers.Methods With the help of Symptom Checklist SCL-90, we firstly carried out baseline survey on community medical staff mental health; then retested that at 3, 6 and 9 months later. Thus, we could assess the effect of psychological intervention. According to the results of the medical staff working pressure test questionnaire, we could develop targeted psychological intervention measures and adopt prompt adjustment.Results Community health care personnel SCL-90 evaluation score and 7 factors scores(physical,forced,interpersonal sensitivity,anxiety,hostility,terror,and paranoia) were higher than the national norm(P<0.05);after comprehensive intervention in the 9 months follow-up,the SCL-90 evaluation score was decreasing gradually.After intervention for 6 months,except for the interpersonal sensitivity factor,SCL-90 total scores and the other factor scores tend to normal levels, to compare the national norm, there was no statistical significance in differences (P<0.05). 9 months later, the scores of interpersonal sensitivity also return to normal levels.Conclusion The mental health status of community health care workers was lower than the national norm,the comprehensive intervention may improve their mental health level, and the experiment showed that the intervention should be no less than 6 months.

第三方满意度调查结合学生需求在全科医学教学中的实践分析

目的 分析全科医学教学中应用第三方满意度调查,结合学生需求的实践效果。方法 2016年3—12月接收全科医学学生者60例,按照常规方法进行,2017年3—12月全科医学学生62例,教学中应用第三方满意度调查结合学生需求方法,观察教学效果。结果 第二年时,学生期末考试总成绩、病例题成绩、实习报告成绩,课外自学时间、利用网络自学时间、与他人讨论学习时间,教学效果总体评分、课间实习评分均高于第一年,差异有统计学意义(P<0.05)。结论 全科医学教学中应用第三方满意度调查结合学生需求方法后,可有效的提升教学效果,增强学生的自学能力及自学意识。
Objective To analyze the practical effect of applying the third party satisfaction survey to the needs of the students in the teaching of general medicine. Methods From March 2016 to December 2016, 60 cases of general practice medical students were enrolled. According to the routine method, 62 cases of general medical students from March 2017 to December 2017 were selected. Third party satisfaction survey and student demand method were applied in teaching, and the teaching effect was observed. Results In th second year, students' final exam scores, the total score, case questions internship report scores, extracurricular self-study time, self-study time, learning time, using the network to discuss with othersthe teaching effect of practice during the total score, all above were higher than the first year, there was significant difference (P<0.05). Conclusion The application of third party satisfaction survey combined with students' needs method in general practice teaching may effectively improve teaching effect; enhance students' self-learning ability and self-learning consciousness.

河源地区医疗机构临床用血合理性调查分析

目的 分析河源地区医疗机构临床用血合理性现状,提高临床合理输血水平。方法 选择本地区临床用血量前三位的3家综合性医院,随机抽取其2017年住院患者1 012例输血病例,判断每次输血合理性并对不合理输血原因进行分析。结果 1 012份输血病例中合理输血病例718例,合理性输血比例为70.94%。不同血制品和临床科室间用血合理性比例差异具有统计学意义(χ2=6.734、6.119,P=0.000、0.000)。红细胞输注不合理原因主要为指征过宽和无指征输血,血浆、血小板、冷沉淀输注不合理原因主要为无指征输血。结论 加强医疗机构临床用血管理,真正做到科学用血、合理用血,节约用血。

中山市某三甲医院2013-2015年抗菌药物不良反应发生状况调查分析

目的 调查分析中山市某三甲医院抗菌药物不良反应的发生和分布情况,为临床合理应用抗菌药物提供相关信息。方法 以中山市某三甲医院2013—2015年上报国家药品不良反应监测系统的528例抗菌药物的不良反应为考察对象,按照给药途径、不良反应严重程度、年龄、性别、药物种类、不良反应累及的系统-器官、不良反应发生时间进行分类、整理、归纳和总结。结果 累及系统-器官涉及皮肤及其附件、血液系统、神经系统、消化系统、泌尿系统等。其中皮肤及其附件损害、神经系统、胃肠道反应容易发现,消化系统、血液系统、泌尿系统、肝胆系统的反应具有隐匿性。结论 临床在使用抗菌药物时,既要关注其抗菌效应,也要高度警惕其不良反应的危害性。避免无指征用药,合理选用抗菌药物,科学地开展ADR 监测工作,确保临床安全、有效、合理用药。
Objective To investigate occurrence and distribution of adverse drug reaction ADR of antibiotic drugs in a hospital of Zhongshan, in order to provide relevant information for clinical rational use of antibacterial drugs. Methods We classified and analyzed 528 cases of adverse drug reaction of antibiotic drugs which was reported to National center for ADR Monitoring during 2013~2015, according to the administration route、the severity of ADRs、age、sex、types of drugs、the organs systems involved by ADRs, the time of ADRs occurrence. Results Adverse drug reaction of antibiotic drugs involved in skin and its appendix、hematological system、nervous system、digestion system、urinary system and so on. Among them skin and its appendix、nervous system and gastrointestinal reactions were easy to acquire, others were obscure and difficult to find. Conclusion When the clinical use of antibacterial drugs, should not only focus on its antibacterial effect, also need to keep high vigilance against the dangers of its adverse reactions. To ensure the clinical safety, effective and rational drug use, we need to avoid no indication of medicine, take rational use of antibiotic drugs,scientifically to carry out the ADR monitoring.

广东省罗定市中小学教师职业倦怠的调查

A survey of job burnout in primary and secondary school teachers in Luoding city of Guangdong province

目的 了解广东省罗定市中小学教师的职业倦怠情况,为制定政策提供初步依据。方法 采用中文版Maslach职业倦怠量表(C-MBI)对罗定市42所中小学共590名毕业班教师进行评估。结果 受访者的情感衰竭(emotional exhaustion, EE)和知识枯竭(intellectual burnout, IB)因子分显著高于全国常模(P<0.05),个人成就感降低(reduced personal accomplishment, PA)因子分显著低于全国常模(P<0.01)。女性受访者的去个性化(depersonalization, DP)、EE和IB因子分均显著高于男性受访者,PA因子分显著低于男性受访者(P均<0.01)。结论 广东省罗定市中小学毕业班教师的职业倦怠高于全国平均水平,女性教师的职业倦怠感高于男性教师。
Objective To investigate job burnout among primary and secondary teachers in Luoding city and to provide a preliminary basis for policy formulation. Methods We randomly selected 590 graduating class teachers from 42 primary and secondary schools in Luoding. The outcome variable of burnout was measured by the Chinese Maslach Burnout Inventory for primary and secondary school teachers (C-MBI). Results Emotional exhaustion (EE) score and intellectual burnout (IB) score in respondents were both significantly higher than the national norm used as reference (P<0.05), reduced personal accomplishment (PA) score was significantly lower than the national norm (P<0.01). The factor scores of DP, EE and IB were obviously higher in female respondents than that those in the male, while the score of PA was obviously lower (all P<0.01). Conclusion The level of burnout in these teachers is significantly higher than the national average, and the burnout in female teachers is more serious.

目的 了解南沙新区三级医院门诊患者健康教育需求情况,为制定医院健康教育策略提供科学依据。方法 采用随机抽样的方法,利用自制调查问卷对287名门诊患者进行健康教育需求调查。结果 青年期患者、大专以上文化程度患者健康教育需求率高,需求的健康教育内容最高的为传染病防治健康教育,需求的健康教育方式依次为医患交流、护患交流、微信公众号或网站宣传。结论 南沙新区三级医院门诊患者健康教育需求较高,应根据门诊患者的不同特点,制定有针对性措施进行健康教育,以提高其健康素养水平。
