戊型肝炎病毒感染诊疗的研究进展

       戊型肝炎病毒(HEV)是导致急性肝炎的重要病原体,部分HEV感染者可进展为肝衰竭,此外,慢性感染和肝外表现可也在HEV感染者中发生。全球每年感染HEV的患者数达2000万,其中330万例患者有肝炎相关的临床症状,年死亡例数约为4.4万(2015年数据)。在我国,HEV以散发流行为主。近年来由于对其研究的重视,HEV病原学、流行病学、临床诊疗和预防取得较大的进展,文章拟对目前HEV防治热点以及新进展进行总结和分析。
       Hepatitis E virus(HEV)is an important pathogen that causes acute hepatitis.Some HEV-infected individuals 
may progress to liver failure.In addition,chronic infection(including liver fibrosis and cirrhosis)and extrahepatic manifestations can also occur in HEV infection.Worldwide,there are 20 million cases of HEV infection each year,with  3.3 million cases presenting clinical symptoms related to hepatitis,and an annual death toll of approximately 44,000(data from 2015).In China,HEV mainly present as sporadic outbreaks.In recent years,there has been significant progress in the pathogenesis,epidemiology,clinical diagnosis and treatment,and prevention of HEV.This review aims to summarize and analyze the current hotspots and new developments in the prevention and treatment of HEV.

       戊型肝炎病毒(hepatitis E virus,HEV)是导致急性肝炎的常见病原体,在20世纪80—90年代首次被发现,主要经粪口传播,目前在我国以散发流行为主。近年来,由于对HEV研究的重视,该领域的研究取得较大的进展,本综述结合2018年欧洲肝病学会(European Association for the Study of the Liver,EASL)发布的戊型肝炎临床实践指[1]、国内2022年中华医学会肝病学分会发布的戊型肝炎防治共识[2]以及近年发表的文献,围绕HEV病原学、流行病学、常规和特殊人群的临床诊疗、HEV预防等主题,对目前HEV的防治热点和新进展进行总结和分析。

1  HEV 病原学

       HEV在多种物种中可被检出,感染人类的HEV病毒属为二十面体、无包膜的球形颗粒,直径约为27~34 nm。HEV的外壳由衣壳蛋白同源二聚体构成,每个衣壳蛋白包含三个线性结构域:S、P1和P2,中和抗原表位位于P2结构域。每个结构域均含有一个多糖结合位点,与细胞受体相互作用有关[3-4]。HEV为单股正链RNA病毒,全长约为7.2 kb,其基因组的构成包括5’-端非编码区(5'-端的起始部位为m7G帽)、3’-端非编码区(3'-端末尾为多聚腺苷酸结构)和3~4个开放读码框(open reading frame,ORF),见图1。各个ORF的功能如下:(1)ORF1基因编码在病毒复制中起关键作用的复制酶;(2)ORF2基因编码病毒的衣壳蛋白;(3)ORF3基因与ORF2部分基因重叠,编码分子量约为13kDd的磷蛋白(VP13)参与病毒从受感染细胞中释放的过程[5];(4)ORF4是在ORF1编码区发现的一个新的ORF,仅存在于HEV基因1型,其表达的蛋白可增加RNA依赖的RNA聚合酶活性,促进病毒复制,并可能与增加妊娠期感染严重程度有关[6]。感染人类的HEV在血液中通常以完整的病毒颗粒在血液中存在,但粪便中常以无包膜的形式存在[7]

2  HEV 基因分型

       国际病毒分类学委员会(I n t e r n a t i o n a l Committee on Taxonomy of Viruses,ICTV)在2022年将感染人类和其他哺乳动物的HEV归类为戊型肝炎病毒科(Hepeviridae),正戊型肝炎病毒亚科(Subfamily Orthohepevirinae),帕斯拉戊型肝炎病毒属(Genus Paslahepevirus),巴拉扬尼种(Species Balayani)。巴拉扬尼种可分为8个基因型,其中基因1~4型可再分为多种亚型,如3型可至少分为14种亚型(3a~3m,3ra),4型可至少分为9种亚型(4a~4i)[8-12]。目前对于不同基因亚型差异的研究数据较少,尚不能明确不同亚型的流行病学和临床表现是否存在差异。HEV在不同地区基因型分布并不一致,基因1和2型主要分布在印度等发展中国家或卫生条件相对较差的国家或地区;基因3和4型主要流行于高收入国家或卫生条件相对较好的国家,如基因3型在欧洲地区(意大利和法国等国家)的流行率显著高于其他基因型[13]。此外,相同的国家或地区,由于不同的社会状态,HEV流行的基因型可出现变迁,如国内既往主要的HEV基因型为1型[14],但近年来,特别是在中国沿海经济较发达的地区,HEV流行的基因型已从基因1型转变为基因4型[15]。对于相同的国家或地区HEV基因型变迁的原因尚不明确,可能与该地区卫生条件提高或特定的基因型(如基因4型)基本传染指数(R0)显著高于其他基因型有关。与人类感染相关的HEV基因型主要为1~4型以及7型,基因1和2型仅感染人类,3型和4型可在人和动物之间跨物种传播,7型可从单峰骆驼传染给人类[16]。根据目前文献报告,不同的HEV基因分型的临床表现和预后并不一致。

3  HEV 流行病学特点

       根据世界卫生组织的数据,全世界每年感染HEV的患者数达2 000万,2015年导致约4.4万人死[2]。HEV在卫生条件较差的发展中国家可出现聚集和暴发流行,但工业化国家(包括我国)以散发流行为主[17]。HEV的传播途径包括粪口传播、密切接触、血液、器官移植和母婴垂直传播等方式[2],其中主要的传播方式为粪口传播,受污染的水源是其感染的重要载体,人类通常因直接接触受感染的动物、食用未煮熟的受污染动物肉类或内脏、水产品而感染[18]。在80年代,我国新疆地区曾有HEV的暴发流行,其传染途径主要与受污染的水源有关[19]。但目前我国HEV主要以散发流行为主,与食源性感染相关,且冬春季发病率较高[20]。不同HEV基因型感染的物种(宿主)并不一致,我国流行的HEV基因4型主要感染的物种为猪、兔、鹿、牛、山羊等,可引起跨物种传播,动物传染至人类主要通过食用受感染动物的肉类和内脏。由于猪在全球范围被广泛养殖,故成为HEV传播的重要宿主和感染源,目前关于HEV传染至人类的证据有以下几种:(1)流行病学的研究提示,与猪肉或内脏接触较多的职业(如猪养殖户、屠户以及兽医等),HEV感染的风险明显增加[21-22];(2)多项研究结果提示,猪体内可检测出抗-HEV IgM和(或)IgG,且部分研究结果提示猪体内HEV抗体的阳性率较[23-24];(3)较多的研究结果表明HEV RNA可在猪肉或猪肝相关制品中被检测出[25-28];(4)基因序列分析结果提示猪源性HEV和患者来源的HEV序列具有极高的同源性,研究结论主要基于HEV部分核酸序列(部分HEV ORF2基因),表明人和猪的HEV序列高度同源[27,29]。作者本人也曾对1例感染HEV从事猪养殖的患者粪便样本以及该养殖场的猪粪便样本进行HEV近全长序列分析(均为基因4型),研究结果也表明患者来源的HEV与猪源性HEV在核酸水平高度同源[30]

4  HEV 检测方法与诊断标准

       EASL指南以及国内防治共识对肝功能异常的患者,均建议进行HEV检测,HEV的实验室检查主要包括HEV抗体、HEV RNA和HEV抗原。

4.1  HEV抗体检测

       HEV抗体检测是临床最常用的检测方法,HEV感染的潜伏期约为15~60 d,大部分患者在出现临床症状时可测到抗-HEV IgM和IgG,在我国,HEV RNA检测和HEV抗原检测未完全普及,且部分患者发病时HEV RNA已阴转,因此HEV抗体检测是HEV临床诊断的重要指标之一。但单纯依靠HEV抗体检测可能出现误诊或漏诊,原因如下:(1)抗-HEV IgM通常持续3~4个月,少数患者可持续阳性1年,而抗-HEV IgG可在患者体内长期存在,单纯依靠HEV抗体检测可能出现误诊;(2)抗-HEV IgM特异性欠佳,可能与其他病毒有交叉反应[31];(3)部分老年人、免疫抑制人群出现临床症状时抗-HEV IgM尚未阳转,仅依靠抗体检测可能出现漏诊,对于初始抗-HEV IgM和IgG阴性的患者,临床上不能排除HEV感染者,可间隔1周左右再次进行抗体检测,以避免漏诊[32]

4.2  HEV RNA检测

       HEV RNA检测是诊断HEV感染的重要方法,血清或粪便中检测出HEV RNA均可作为HEV的诊断标准,在HEV感染大约3周后,HEV RNA可在血液和粪便中被检测出,其于粪便中的存在时间更长。在出现临床症状的1~2周内,70%~80%患者的血清或粪便样本可检测出HEV RNA,而HEV在粪便存在的时间常比血清中更长。HEV RNA检测是HEV感染的最直接的证据,但20%~30%的患者在发病时HEV RNA已不可检测,因此不能以单一HEV RNA阴性排除急性HEV感染[33-34]。目前基于荧光定量PCR的方法对HEV常见的基因1~4型检测均有效[35]

4.3  HEV抗原检测

       除HEV核酸检测以外,HEV抗原检测也有较高的灵敏度[36-37],血清、尿液和粪便的检测也是HEV感染的直接证据。基于双抗体夹心法建立的HEV抗原检测是常用的检测方法。根据文献报道,HEV抗原的水平还可区分急性和慢性的HEV感染[38]。除了血清抗原检测,尿液HEV抗原水平可能比血清更高,因此尿液样本的抗原检测也可考虑用于HEV的诊断[39]。但目前在我国HEV抗原试剂未普及,因此大部分地区暂未开展HEV抗原检测。

4.4  HEV实验室诊断标准

       对于存在肝功能异常或肝炎临床症状的患者,EASL指南以及国内防治共识均建议对免疫健全和免疫抑制为不同的人群进行分类以便进行正确的诊断。但EASL指南和国内专家共识对HEV的实验室诊断标准稍有不一致,见表1。

5  HEV 感染的临床表现

        HEV感染主要表现为急性感染,包括急性无黄疸型肝炎和急性黄疸型肝炎,部分患者可进展为重症肝炎(肝衰竭)。大部分HEV感染者无症状,表现为隐匿性感染,且常为自限性,仅检测结果提示肝功能轻度异常。急性黄疸型肝炎占HEV感染的5%~30%,胆汁淤积型肝炎约占8%[2]HEV感染在妊娠、老年人或存在基础肝病等人群可致病情加重。除此以外,HEV感染可出现肝外表现[1-2]

6  HEV 感染与肝衰竭

       HEV感染可导致肝衰竭的发生,其病情重,死亡率高,临床上应予足够重视。HEV感染导致肝衰竭与以下因素有关:(1)HEV基因组变异,Borkakoti等[40]报道在基因1型的HEV感染者中,HEV ORF1编码的甲基转移酶中的D29N、V27A和H105R变异与急性肝衰竭的发生相关,其中D29N和V27A变异与患者的不良结局密切相关。Wang[41]研究结果提示ORF1 Y结构域和螺旋酶结构域A317T和1120I变异与肝衰竭的发生有关,其机制在于上述变异可增强病毒的复制能力。(2)HEV感染特殊人群,如老人和孕妇感染HEV,可增加HEV相关肝衰竭发生的风险[42-43]。(3)HEV感染者存在慢性基础肝病,如HEV感染合并乙型肝炎病毒、肝硬化或药物性肝炎等患者,常加重病情的进展,增加肝衰竭发生的风险[44-45]

7  HEV 感染与妊娠

       HEV在一般人群的死亡率低,但孕妇感染HEV死亡率相对较高,也可导致不良妊娠结局的发生。丹麦Bergløv等[43]的Meta分析结果提示,感染HEV的孕妇中位数死亡率为26%,胎儿和新生儿死亡率分别为33%和8%。孕妇感染HEV可发生暴发性肝衰竭(发生率中位数为45.3%),并可出现早产、产后出血、低出生体质量和HEV垂直传播等不良后果。孕妇感染HEV导致的不良结果与HEV基因型密切相关,感染基因1型和2型的孕妇死亡率及胎儿不良事件发生率高于其他基因型[46]。我国流行的基因型以4型为主,虽然孕妇感染基因4型HEV死亡率低于基因1型和2型,但是研究提示基因4型HEV感染妊娠不良结局和胎儿不良结局的发生率也高于非HEV感染的孕妇,且不良结局的发生与年龄、孕次和产次有关[47]。妊娠合并HEV容易增加妊娠和胎儿不良结局的原因复杂,可能与妊娠期激素变化、免疫失衡、炎症反应和宿主等因素有关。在妊娠的女性中目前尚无有效且安全的抗病毒药物,治疗以支持对症治疗为主,如发生重症肝炎,应根据孕妇和胎儿情况决定是否提前终止妊娠。对于育龄期且有感染HEV高风险的人群,可在计划妊娠前接种HEV疫苗,减少孕期感染HEV的风险[48]

8  老年 HEV 感染者的特点

       老年人是HEV感染的高发人群,根据文献的数据,在我国50~69岁的老年人和男性感染HEV的比例分别为37.47/万人和29.95/万人[49]。老年HEV感染者,尤其是老年男性感染HEV常有别于一般的HEV感染者。不同HEV基因型易感人群有显著的年龄差异,基因1型和2型在儿童和年轻成年人常见,而基因3型和4型常见于年龄更大的患者。我国流行的基因型主要为4型,因此,老年HEV感染者在我国常见[50]。老年HEV感染者感染后胆汁淤积、胆红素峰值异常的情况比非老年HEV感染者更明显,病情更重,住院时间更长,肝衰竭的发生率高于非老年HEV感染者[51]。除此以外,国内学者对108例老年男性和40例老年女性HEV感染者进行比较,结果提示老年男性HEV感染者肝功能异常的程度高于老年女性[52]。老年HEV感染者疾病严重程度更重可能与下列因素有关:(1)老年人免疫力低于年轻人;(2)老年人常合并较多基础病,导致身体机能下降;(3)老年人肝脏体积缩小、肝脏代偿能力差,肝脏结构老化和正常肝功能生理性下降有关。但目前对于老年男性感染HEV后肝功能异常程度高于老年女性的具体机制未被阐明,有待进一步研究。

9  HEV 感染的肝外表现

       HEV在肝脏的临床表现和其他肝炎病毒类似,但除肝内表现,HEV感染可引起肝外的表现,主要包括神经系统、肾脏、消化系统和血液系统的病变。HEV感染所致的肝外表现与HEV基因型关系密切。

9.1  HEV感染与神经系统病变

       HEV感染和神经系统疾病的发生关系密切,包括神经纤维肌肉萎缩症(neurofibromuscular atrophy,NA)、吉兰-巴雷综合征(Guillain-Barre Syndrome,GBS)、脑炎/脊髓炎、多发性单神经炎、贝尔氏面瘫、前庭神经炎、肌炎和周围神经病变等,主要发生在基因3型的HEV感染者,1型和4型的报道较少。van Eijk等[53]对来自7个欧洲国家的11个中心NA患者进行研究,其中包括57例HEV感染者(38例可检测出HEV-RNA,均为基因3型)和61例非HEV感染者,结果提示HEV感染的NA患者常无出现黄疸,相对于非HEV感染者具有独特的临床表型,主要表现为双侧非对称臂丛受累,以及更广泛的损伤,臂丛以外的受累更为普遍。研究结果提示,无论NA患者肝功能检查结果如何,应对出现NA症状的患者进行HEV检测。近期一项来自瑞典的研究纳入51例HEV感染者,大部分患者有转氨酶升高,研究结果也提示HEV感染与NA的发生显著相关[54]。Stevens等[55]对73例GBS患者进行研究,发现8%的患者抗-HEV IgM阳性,提示HEV感染与GBS的发生有密切的相关性。Moret等[56]对82例慢性GBS患者进行研究,发现39%的患者抗-HEV IgG阳性,明显高于非慢性GBS的对照组(17.3%),提示HEV感染可能与慢性GBS有关。此外,国外有零星的HEV感染导致脑炎/脊髓炎、多发性单神经炎、贝尔氏面瘫、前庭神经炎、肌炎和周围神经病变的报道[1]。HEV感染导致神经系统病变的具体机制未明,可能与HEV感染与机体内的分子模拟有关,具体的机制仍有待进一步研究。

9.2  HEV感染与肾损伤

       根据目前的文献报道,HEV感染可引起肾损伤。HEV感染所致肾损伤主要在免疫抑制的患者中发生,常表现为膜增生性肾小球肾炎、膜性肾小球肾炎和IgA肾病。Kamar等[57]对51例已进行实体器官移植并感染HEV基因3型的患者进行研究,其中56.8%为慢性感染。结果提示在HEV感染期间,患者的肾功能明显受损,部分患者发生肾小球疾病,包括膜增生性肾小球肾炎和IgA肾病,在HEV清除后肾功能和蛋白尿均有改善,提示其发生与HEV感染密切相关。目前关于HEV感染和肾损伤关系主要为零散的病例报道,且主要发生在HEV基因1型和3型,其相关性和具体的机制有待进一步研究阐明[58]

9.3  HEV感染与血液系统疾病

       HEV感染所致的血液系统病变包括血小板减少症、单克隆丙种球蛋白血症、冷球蛋白血症、噬血细胞综合征、再生障碍性贫血以及溶血性贫血。HEV感染导致血液系统病变可能与HEV在外周血单个核细胞中复制、感染骨髓源性的巨噬细胞和免疫介导的炎症有关。HEV感染所致的血小板减少症主要在基因1型和3型的患者中发生,目前大部分的报道为零散的病例报道[59-60],部分HEV感染的病例可导致血小板减少,值得引起临床医师的重视。单克隆丙种球蛋白血症的发生率在HEV基因3型的感染者(25%)高于正常人群(3%)[61-62],可能与HEV感染激活免疫系统和免疫细胞有关,其临床意义未明,有待进一步研究。HEV感染与再生障碍性贫血也有一定相关性,Shah等[63]报道1例来自巴基斯坦的32岁男性,感染HEV后发生再生障碍性贫血,并排除其他常见的和再生障碍性贫血相关的病毒感染,且患者使用常规的免疫抑制治疗效果欠佳。目前HEV感染与再生障碍性贫血关系的报道较少,仍有待进一步研究。除此以外,HEV感染与溶血相关,尤其容易发生在葡萄糖-6-磷酸脱氢酶(glucose-6-phosphate dehydrogenase,G6PD)缺乏症的患者。国内最早的报道是1995年1例青海的58岁男性患者[64],该患者诊断为G6PD缺乏症,在HEV感染后出现溶血性贫血,经治疗后患者好转。随后国内外有零散的关于HEV感染与溶血性贫血相关的报道。作者本人曾对零散的病例进行总结,结果提示HEV相关溶血性贫血常合并G6PD缺乏症[65]

9.4  HEV感染与其他肝外疾病

       除上述提及的HEV所致的肝外表现,文献报道HEV感染与急性胰腺炎、心肌炎、过敏性紫癜、甲状腺疾病、肌肉骨骼疾病、淋巴增生性疾病、不孕不育等有关,但目前大部分为病例报道,其与HEV感染的关系仍有待进一步确定和研[66]

10  急性 HEV 感染的治疗

       急性HEV感染通常会被自发清除,在一般人群的病死率较低,约为0.2%~1.0%,治疗上主要以一般治疗和对症治疗为主,不需要抗病毒治疗。曾有个案研究表明急性HEV感染使用利巴韦林对降低病毒量和改善肝功能作用[67]。虽然部分HEV感染者可进展至肝衰竭,但使用利巴韦林需要考虑贫血、胃肠道症状和干咳等不良反应,且HEV存在体内的时间较短,是否能通过抗病毒治疗获益有待进一步研究。HEV相关肝衰竭死亡率相对较高,根据文献报道,曾有学者对HEV相关肝衰竭的患者使用糖皮质激素(激素)治疗[68],使用后患者的肝功能指标有明显的恢复。但根据临床经验,肝衰竭的患者容易合并感染,常规使用激素的风险较大,目前尚无足够的证据支持在HEV感染相关急性肝衰竭患者中使用激素治疗,另外,EASL指南以及国内防治共识对HEV所致肝衰竭患者均无推荐激素的治疗。

11  慢性 HEV 感染的诊疗

       慢性HEV感染的定义为HEV RNA或抗原存在于患者血液或粪便超过3个月。大部分慢性HEV感染者发生在免疫抑制人群,如HIV感染者、使用免疫抑制治疗的血液系统肿瘤、实体器官移植者或因基础疾病需长期使用免疫抑制剂的患者,部分慢性HEV感染者可引起肝纤维化甚至进展为肝硬化。根据目前文献报道,仅基因3型和4型可发生慢性HEV感染[69-70]。由于慢性HEV感染者的免疫状态,抗-HEV IgM或IgG可能为阴性,因此,其诊断需基于HEV-RNA或HEV抗原的检测[1]
       目前对于慢性HEV的治疗,主要包括以下要点:(1)EASL指南以及国内防治共识,对于合并慢性HEV感染的实体器官移植患者,均建议首先考虑适当减少免疫抑制剂剂量。Kamar等[71]来自美国和欧洲17个中心85例接受实体器官抑制的HEV的患者进行研究,其中56例患者进展为慢性HEV感染,18例(32.1%)慢性HEV患者在对免疫抑制剂减量后可出现病毒阴转,且无患者复发,提示调整免疫抑制剂剂量可增加病毒清除的机会。(2)慢性HEV感染者的抗病毒治疗主要包括聚乙二醇干扰素和利巴韦林。如Dalton等[72]道1例48岁合并慢性HEV感染(HEV 3a型)的HIV患者进行聚乙二醇干扰素单药并序贯加用利巴韦林抗病毒治疗,成功治愈HEV慢性感染。由于实体器官移植(心脏、肾脏、胰腺和肺等器官)并使用免疫抑制剂的患者使用聚乙二醇干扰素治疗可能出现排异反应,故常用利巴韦林单药进行抗病毒治疗,疗程为单药治疗3个月,用药期间应注意监测利巴韦林相关不良反应。肝移植的患者使用利巴韦林无应答时,可考虑使用聚乙二醇干扰素抗病毒治疗。(3)经利巴韦林单药治疗3个月的慢性HEV患者,如治疗后病毒未清除,可再次使用利巴韦林抗病毒治疗6个月。Kamar等[73]的另一项研究纳入59例已进行实体器官移植的并诊断为慢性HEV感染的患者,利巴韦林中位数治疗剂量为600 mg/d,中位治疗时间为3个月。经治疗后95%的患者在治疗结束时病毒阴转,10例患者在治疗结束后复发,在停药半年后46例患者(78%)仍维持病毒学应答。其中4例治疗失败的患者在延长利巴韦林的疗程后可获得病毒学应答,提示对利巴韦林治疗后复发的患者可通过延长利巴韦林给药时间再次治疗。

12  HEV 预防

       HEV的预防首先需对HEV感染者进行管理,HEV感染者粪便中病毒存在的时间较长,对其排泄物和粪便应进行严格消毒。HEV的传播主要为粪口途径,需要加强水源卫生和改善供水条件,另外,应避免食用未煮熟的猪或其他易感动物肉类和内脏、未煮熟的牡蛎和贝壳等海产品。HEV在室温28 d仍有感染性,但对热敏感,大部分细胞培养和动物实验的结果提示加热至70 ℃ 2 min以上可显著减少HEV的活性[74],因此,彻底煮熟动物肉类、肝脏和海鲜等食品可对HEV进行灭活。接触HEV易感动物应注意手卫生,及时进行消毒。2011年12月1日,我国自行研发的HEV疫苗已批准上市,其对基因1~4型均有良好的保护作用[75]对于HEV感染的高风险人群(国外疫区旅行者、从事畜牧养殖业的工作人员和餐饮业人员等)以及感染HEV后可能进展至病情严重的人群(育龄期妇女、老年人和慢性肝病等患者)可进行HEV疫苗接种,预防感染的发生。

13  总结和展望

       HEV的传播途径主要为粪口传播,虽然目前在我国以散发流行为主,但既往在国内外都曾有HEV暴发流行的报道,不排除未来出现HEV暴发流行的可能,因此对HEV感染的防治仍是重要的公共卫生问题。近年来,由于对HEV研究的重视,目前该领域取得较大的进展,但HEV总体呈散发流行的特点,研究难度较大,仍有较多待研究和未解决的问题,例如我国HEV真实的流行情况、基因型/亚型的分布情况,妊娠/老年患者HEV感染重症化的机制、治疗和预防,HEV抗病毒药的研发等问题。世界卫生组织已提出2030年消除病毒性肝炎作为公共卫生危害的目标,HEV是病毒性肝炎常见的病原体之一,为实现该目标,仍需加强对HEV防治的研究。
