医院药房代煎代送服务的质量控制管理探讨

       目的   探讨医院药房外包代煎代送服务的质量管理。方法   成立中药代煎质量控制管理小组,研究给出完善质量控制体系的策略,涉及建立标准化操作流程、加强人员培训、优化信息化管理系统以及健全监督评估机制。   通过对中药库房的采购、入库验收、在库管理、调剂环节的审方、调剂、煎药质量管理以及物流、发药人员全流程管理,提高了患者的满意度,更多的患者选择了中药代煎服务。结论   实施有效的质量控制措施可提高代煎代送服务的质量,为医院药房提升服务质量给予理论依据与实践指导。
       Objective  To explore quality management of outsourced decoction preparation and delivery services in hospital pharmacies.Methods  A quality control management team for traditional Chinese medicine(TCM)decoction preparation was established to develop strategies for improving the quality control system.These strategies involved establishing standardized operating procedures,strengthening personnel training,optimizing the information management system,and improving the supervision and evaluation mechanism.Results  Through comprehensive process management,covering procurement,warehouse acceptance,inventory management,prescription review in the dispensing stage,dispensing itself,quality control of decoction preparation,logistics,and management of dispensing personnel,patient satisfaction was enhanced,leading more patients to opt for the TCM decoction service.Conclusions  Implementing effective quality control measures can improve the quality of decoction preparation and delivery services,providing a theoretical basis and practical guidance for hospital pharmacies to enhance their service quality.

医院门诊西药房“分-全带教”模式的探索与实践

目的 分析医院门诊西药房“分-全带教”模式的应用效果,为提高药房实习生的教学质量提供参考价值。方法 选取广州市番禺区中心医院药学部门诊西药房2020年9月—2022年7月的48例实习生(包含进修生、研究生)作为研究对象,对所有实习生实施“分阶段、分层级、分诊断单元窗口学习、全面能力培养”的“分-全带教”模式。观察所有实习生入科实习前后的考核成绩。结果 通过“分-全带教”实施后,实习生的理论知识(94.15±3.38)分、药学服务技能评分(90.56±3.99)分均比实习前有所提升,且实习生的学习兴趣(94.21±3.70)分、巩固知识点(91.98±3.56)分、提高医患沟通(92.73±3.81)分、认识自身技能不足(92.06±3.50)分同时提升(P<0.05)。结论 采用“分-全带教”模式明显提升对医院门诊西药房的实习生中的教学效果,实习生的理论和实践技能均有所提高,其药学服务能力同时得到提高。
Objective To analyze the application effectiveness of the “divided-comprehensive precepting” model in the hospital outpatient pharmacy and provide reference value for improving the teaching quality of pharmacy interns.Methods Forty-eight interns(including visiting scholars and graduate students)from the Pharmacy Department of Panyu Central Hospital in Guangzhou,China,from September 2020 to July 2022 were selected as the research subjects.The “divided-comprehensive precepting” model,characterized by “staged,tiered,diagnosis unit-specific window learning,and comprehensive competency development” was implemented for all interns.The scores of all interns before and after entering the internship were observed.Results After the implementation of the“divided-comprehensive precepting”model,the scores of interns in theoretical knowledge(94.15±3.38)and pharmaceutical service skills(90.56±3.99)were improved compared to those before the internship.Moreover,interns’scores in learning interest(94.21±3.70),consolidating knowledge(91.98±3.56),enhancing doctor-patient communication(92.73±3.81)and recognizing their own skill deficiencies(92.06±3.50)were also improved.Conclusions The “divided-comprehensive precepting” precepting model significantly enhances the teaching effectiveness in the internship of hospital outpatient pharmacy.Interns’ theoretical and practical skills are improved,enhancing their pharmaceutical service capabilities.

PDCA管理法在医院药事管理中的应用分析

目的 分析PDCA管理法在医院药事管理中的应用效果。方法 将我院2 000批药品设置为研究对象,样本纳入的起始、截止时间分别为2022年1月、2022年12月,依据时间对纳入样本开展组别划分,将2022年1月—6月1 000批药品设置为PDCA管理前研究对象,期间接受常规药品管理;将2022年7月—12月1 000批药品设置为PDCA循环法管理后研究对象。对比PDCA管理前后药品不良事件发生率及药事管理质量评分。结果 PDCA循环法管理后药学差错事件发生率低于PDCA循环法管理前,分别为0.30%、1.90%,对比差异有统计学意义(P<0.05);PDCA循环法管理后建立药品质量监控体系、药品调剂制度、抗肿瘤药物分级管理方案和处方管理、临床药师参与四个维度评分较PDCA循环法管理前均有明显提升,分别为18.15±1.83、18.15±1.64、18.22±1.67、18.07±1.84,管理前后评分对比差异具有统计学意义(P<0.05)。结论 医院药事管理采用PDCA管理法可提升药事管理质量,降低药品不良事件发生率,值得应用。
Objective To analyze the application effect of PDCA cycle method in hospital pharmaceutical management.Methods A total of 2 000 batches of drugs in our hospital were included as research objects,the inclusion start and end of samples were January 2022 and December 2022.The included samples were divided into groups according to the time,1 000 batches of drugs from January 2022 to June 2022 were included as research objects before PDCA management,during which they were subjected to routine drug management.1 000 batches of drugs from July 2022 to December 2022 were included as the research objects under management by PDCA cycle method.The incidence of adverse drug events and the quality score of drug administration before and after PDCA administration were compared.Results The incidence of pharmaceutical error events after PDCA cycle management was lower than that before PDCA cycle management,which were 0.30% and 1.90%,respectively,and the differences were statistically significant(P<0.05).After PDCA cycle management,the scores of establishment of drug quality control system(18.15±1.83),drug dispensing system(18.15±1.64),anti-tumor drug grading management scheme and prescription management(18.22±1.67),clinical pharmacist participation(18.07±1.84)were significantly improved compared with those before PDCA cycle management,and the difference between scores before and after PDCA cycle management was statistically significant(P<0.05).Conclusions PDCA cycle can improve the quality of pharmaceutical management and reduce the incidence of adverse drug events,which is worthy of application.

目的 对我院静脉用药调配中心2017年1月—2019年12月期间的不合理医嘱情况进行汇总及分析,以提高合理用药水平。方法 收集2017年1月—2019年12月广州市第一人民医院静脉用药调配中心的不合理医嘱资料进行回顾性分析,归纳不合理医嘱类型、汇总分析不合理医嘱表现以及改正不合理情况。结果 主要不合理医嘱类型包括溶媒选择错误、溶媒量过多或过少、药物剂量不合理、药物配伍禁忌、其他(用药频次错误、药物选择错误、重复用药等)。结论 根据不合理医嘱情况的汇总分析,及时与临床科室沟通改正,保证合理用药。
Objective To summarize and analyze their rational prescriptions in pharmacy intravenous admixture services(PIVAS)of our hospital from January 2017 to December 2019,so as to improve the level of rational drug use. Methods The data of irrational prescriptions from January 2017 to December 2019 in the pharmacy intravenous admixture services of Guangzhou First People's Hospital were collected for retrospective analysis, and the types of irrational prescriptions were summarized, the performance of irrational prescriptions was analyzed and the irrational situation was corrected. Results The main types of irrational prescriptions include incorrect selection of solvent, improper consumption of solvent, improper drug dosage, incompatibility and others(improper frequency of administration, incorrect selection of drug, repeated administration, etc.). Conclusion According to the summary and analysis of irrational prescriptions, we timely communicate with clinical departments and ensure rational drug use.

药学本科生在毕业实习前的观念及心态调查

Investigation of overall concept and attitude before graduation practice for pharmacy undergraduates

目的 调查药学本科大学三年级学生在毕业实习前对实习的整体观念及心态。方法 采用问卷表方式调查,将回收问卷表进行数据统计并分析结果。结果 占总比63.2%学生对未来实习方向较模糊,并期望获得相关指导;72.8%学生认为药学专业知识对实习有帮助;90.0%学生认为实习是为即将就业打基础;对于寻找实习单位的方式,三分之一以上学生比较倾向于药学专业老师介绍推荐,大约三分之一的学生有意自己联系实习单位。结论 该调查结果较为真实反映某高校药学本科生实习前的心态及其对实习整体观念,为高校实习管理及学生工作部门了解学生的实习想法提供可靠数据依据,也为教务部门制定“以人为本”实习计划提供了科学依据。
Objective To investigate the overall concept and state of mind of the undergraduates in the grade three undergraduate students before internship. Methods The questionnaire was used to investigate the form of the questionnaire, and the data were collected and analyzed. Results 63.2% of the students for the future practice direction was fuzzy, and hope to get the guidance;72.8% of the students thought pharmacy specialized knowledge is helpful to practice; 90.0% students thought practice was foundation for employment; To find the internship units, more than one third of the students tend to get recommend by pharmacy professional teachers. About one third of students wished to contact their internships by themselves. Conclusion The survey results showed mentality and the overall concept in practice of pharmacy students in our school. It can provided reliable data basis for the practice of management to understand the students practice idea, and also for the educational administration department to develop the “people-oriented” internship program.
