目的 优化纸质版脊髓损伤神经功能评估系统, 探讨其在脊柱脊髓损伤患者临床管理中的实际应用效果。方法 广泛检索国内外关于脊柱脊髓损伤患者神经运动功能评估的文献, 结合课题组成员的临床经验, 初步筛选出神经运动功能评估的关键项目。通过两轮德尔菲专家函询, 确定评估项目内容, 对纸质版脊髓损伤神经功能评估系统进行优化, 并进行临床应用验证。选取2021年8月1日—2023年5月31日在广州市第一人民医院脊柱外科住院的120例脊柱脊髓疾病患者作为研究对象, 随机分为观察组和对照组。对比两组在应用优化前后评估系统的神经功能评估准确性、评估所需时间以及临床应用便捷性等方面的差异。结果 观察组的评估准确性显著高于对照组,错评率由20.3%降至4.7%,漏评率由32.0%降至5.0%; 此外,优化后的评估系统显著缩短了评估时间,平均减少6.0 min, 有效提升了评估效率。结论 优化后的脊髓损伤神经功能评估系统显著提高了评估的准确性和效率, 有效减少了主观误差。
Objective To refine and optimize the paper-based assessment tool for evaluating neurological function in spinal cord injury patients, and to explore its impact on clinical practice.Methods A comprehensive review of existing literature on neuromotor function assessment in spinal cord injury patients was conducted, complemented by discussions among members of this study.This led to the integration of a preliminary evaluation framework.Subsequently, a two-round Delphi consensus process involving experts was undertaken to finalize the content of the assessment project.Based on this, a paper-based evaluation system was developed and converted into an electronic format by computer professionals.This system was then tested in a clinical setting, where it was administered to 120 spinal cord injury patients at the Spine Surgery Department of Guangzhou First People’s Hospital between 1 August 2021 and 31 May 2023.Patients were randomly assigned to either an experimental group or a control group, allowing for a comparison of accuracy, efficiency, and convenience in clinical application.Results The experimental group demonstrated a notable improvement in assessment accuracy over the control group, with a significant reduction in error rate from 20.3% to 4.7% and a decrease in omission rate from 32.0% to 5.0%.Furthermore, the average assessment time was shortened by 6.0 minutes, markedly enhancing the efficiency of the evaluation process.Conclusions The optimized neurological function assessment system for spinal cord injury patients has proven effective in enhancing both the accuracy and efficiency of assessments, while significantly minimizing subjective errors.This system holds considerable potential for widespread clinical adoption and application.
脊髓损伤是一种高致残性中枢神经系统疾病,目前缺乏有效的治疗措施。干细胞组织工程兴起和神经调控技术的发展,给脊髓损伤的治疗带来新的希望。目前,多项针对脊髓损伤的干细胞相关治疗项目的临床研究已在全球注册,干细胞疗法是脊髓损伤领域的研究热点,具有良好的应用前景。而神经调控技术一直在临床上脊髓损伤后的康复治疗中发挥着重要作用,特别是靶向神经调控技术近年在脊髓损伤治疗方面取得突破性进展。有研究尝试联合干细胞疗法和神经调控技术应用治疗脊髓损伤,试图取得更好的效果。本文综述了干细胞疗法和神经调控技术在脊髓损伤治疗中的研究进展,旨在探讨其作用效果、修复机制、应用前景以及面临的问题,进一步为脊髓损伤的基础研究和临床转化提供参考。
Spinal cord injury is a highly disabling disease of the central nervous system without effective treatment to date.With the rise of stem cell and tissue engineering,and the breakthrough of neuromodulation technology,it brings new hope to the treatment of spinal cord injury.At present,a number of clinical studies on stem cell-related treatment projects for spinal cord injury have been registered worldwide,which has become a research hotspot.Neuromodulation technology has been playing an important role in the clinical rehabilitation of spinal cord injury.Especially,breakthroughs have been made in the treatment of spinal cord injury by targeted neuromodulation technology in recent years,which is encouraging.Some studies have attempted to combine stem cell therapy and neuromodulation technology to treat spinal cord injuries in an attempt to achieve better effect.This review summarizes the research progress of stem cell therapy and neuromodulation technology in the treatment of spinal cord injury,with the aim of discussing their effect,repair mechanisms,application prospect and various problems to face,and providing further reference for the basic research and clinical transformation of spinal cord injury.
目的 探究益气活血汤剂联合热敏灸在脊髓损伤(SCI)后尿潴留患者中的应用效果。方法 选取2019年7月—2021年7月我院SCI后尿潴留患者80例,根据治疗方案不同分为观察组、对照组,各40例。对照组给予益气活血汤剂,观察组在此基础上进行热敏灸治疗。观察2组疗效、治疗前后排尿情况、膀胱功能、尿动力学指标[膀胱容量(VH2O)、膀胱顺应(BC)、逼尿肌压力(Pdet)、膀胱压力(Pves)、平均尿流速(Qave)]及不良事件发生率。结果 观察组总有效率92.50%高于对照组70.73%(P<0.05);治疗后观察组膀胱功能积分、平均排尿次数、平均漏尿次数、平均尿量、残余尿量均低于对照组(P<0.05);治疗后与对照组相比,观察组Pves较低,VH2O、Pdet、Qave、BC较高(P<0.05);观察组不良事件率2.50%与对照组5.00%相比,无差异(P>0.05)。结论 热敏灸联合益气活血汤剂治疗SCI尿潴留患者效果确切,可改善患者排尿异常症状,促进膀胱功能恢复,且安全可靠。
Objective To explore the effect of Yiqi Huoxue Decoction combined with heat sensitive moxibustion in patients with urinary retention after spinal cord injury (SCI). Methods From July 2019 to July 2021, 80 patients with urinary retention after SCI in our hospital were selected and divided into observation group and control group according to different treatment plans, 40 cases in each group. The control group was given Yiqi Huoxue decoction, and the observation group was treated with heat sensitive moxibustion additionally. Efficacy, urination before and after treatment, bladder function, urodynamic indexes [bladder volume (VH2O), bladder compliance (BC), pressure of detrusor (Pdet), pressure of vesical (Pves), average flow rate (Qave)] and incidence of adverse events in 2 groups were observed. Results The total effective rate of observation group was 92.50%, higher than that of control group (70.73%, P<0.05). After treatment, bladder function score, average times of urination, average times of urine leakage, average urine volume and residual urine volume in the observation group were lower than those in the control group (P<0.05). After treatment, compared with the control group, the observation group had lower Pves, higher VH2O, Pdet, Qave and BC (P<0.05). The adverse event rate of the observation group (2.50%) was not significantly different from control group (5.00%, P>0.05). Conclusions Heat sensitive moxibustion combined with Yiqi Huoxue decoction is effective in the treatment of SCI patients with urinary retention, which can improve the symptoms of abnormal urination and promote the recovery of bladder function, and is safe and reliable.
目的 研究磁共振成像(magnetic resonace imaging, MRI)检查颈椎病患者椎间盘及颈髓病变的检出率。方法 选取2017年3月—2019年3月我院颈椎病患者216例,均行MRI检查、CT检查。比较MRI检查、CT检查颈椎病的检出率、颈椎病影像征象的检出率及神经根型、脊髓型颈椎病的检出率。结果 MRI检查颈椎病患者椎间盘膨出及突出、椎间盘变性、椎间盘纤维环病变、颈髓缺血性损伤、后纵韧带病变、颈椎病椎间隙狭窄、钩突增生、椎体后缘骨赘、小关节突增生、骨性椎管狭窄、颈椎曲度异常检出率均高于CT检查(P<0.05);MRI检查、CT检查颈椎病患者椎间孔狭窄、椎管狭窄检出率比较,差异无统计学意义(P>0.05);MRI检查椎间盘钙化、椎小关节骨质增生、椎间盘积气检出率低于CT检查,椎间盘突出、黄韧带增厚、脊髓变性、硬膜囊受压、神经根受压检出率高于CT检查(P<0.05);MRI检查神经根型颈椎病检出率94.44%、脊髓型颈椎病检出率88.89%高于CT检查检出率75.00%、25.25%(P<0.05)。结论 MRI检查颈椎病患者能提高椎间盘及颈髓病变的检出率,对多种颈椎病变征象显著,有助于颈椎病分型的鉴别诊断,从而为临床治疗方案的制定提供科学依据。
Objective To study the detection rate of cervical disc and spinal cord lesions in patients with cervical spondylosis by magnetic resonance imaging (MRI). Methods A total of 216 patients with cervical spondylosis in our hospital from March 2017 to March 2019 were selected, all of them underwent MRI and computed tomography (CT) examination. The detection rate of cervical spondylosis, imaging signs of cervical spondylosis, and cervical spondylosis of nerve root type and spinal cord type by MRI and CT were compared. Results The detection rates of cervical spondylosis patients with disc herniation and protrusion, disc degeneration, disc annulus fibrosus disease, cervical spinal cord ischemic injury, posterior longitudinal ligament disease, cervical intervertebral space stenosis, uncinate process hyperplasia, vertebral posterior margin osteophyte, facet hyperplasia, bony spinal stenosis, abnormal cervical curvature by MRI examination were higher than that by CT examination (P<0.05). There were no significant differences in the detection rates of foramen stenosis and spinal canal stenosis between MRI and CT examination (P>0.05); the detection rates of intervertebral disc calcification, vertebral facet joint hyperosteogeny and intervertebral disc pneumatosis by MRI examination were lower than that by CT examination; the detections rates of intervertebral disc herniation, ligamentum flavum thickening, spinal degeneration, dural sac compression and nerve root compression by MRI examination were higher than that by CT examination (P<0.05); the detection rates of cervical spondylotic radiculopathy and cervical spondylotic myelopathy by MRI examination were 94.44% and 88.89%, which were higher than 75.00% and 25.25% of the detection rates by CT examination (P<0.05). Conclusion MRI examination of patients with cervical spondylosis can improve the detection rates of intervertebral disc and cervical spinal cord lesions, and has significant signs for a variety of cervical lesions, which is helpful for the differential diagnosis of cervical spondylosis classification, so as to provide scientific basis for the formulation of clinical treatment plan.
目的 系统评价手术与保守治疗无骨折脱位型颈脊髓损伤的疗效。方法 应用计算机检索 PubMed、Cochrane Library、EMbase、OVID、CNKI、维普及万方数据库,纳入关于手术及保守治疗无骨折脱位型颈脊髓损伤疗效比较的随机或非随机对照试验。由2名研究人员独立对文献进行筛选、提取和纳入文献评价,采用 Rev-Man 5.3软件对两种治疗的JOA评分、ASIA运动评分、神经功能恢复率、ASIA分级改善进行Meta分析。结果 ①共纳入13 篇研究,共598例患者,手术和保守治疗组分别371例和227例;②Meta分析显示,与保守治疗相比,手术治疗后的JOA评分更高[MD=3.08,95%CI(2.71,3.45),Z=16.29,P<0.000 01];神经功能恢复率更高[MD=15.87,95%CI(9.28,22.46),Z=4.72,P<0.000 01];ASIA分级改善更明显[OR=2.3,95%CI(1.25,4.15),Z=2.76,P=0.006];在ASIA运动评分方面,两者无差异[MD=5.45,95%CI(-7.56,18.47),Z=0.82,P=0.41]。结论 对于无骨折脱位型颈脊髓损伤的患者,手术治疗更有利于患者神经功能的改善。
Objective To systematically assess the clinical efficacy of surgical treatment versus conservative treatment for CSCIWFD from previously clinical investigations. Methods A systematic search of all the studies published was conducted on the PubMed, Cochrane Library, EMbase, OVID, CNKI, VIP and Wanfang databases. Randomized and non-randomized controlled trials that compared between surgical treatment and conservative treatment for CSCIWFD were identified. Two reviewers independently screened the literature according to the inclusive and exclusive criteria, extracted the data, and assessed the quality of included studies. Meta-analyses were performed to assess variables including Japanese Orthopaedic Association scores(JOA), Neurological function recovery rate, ASIA impairment scale grade, ASIA motor scores. Results ①A total of 13 articles were included, involving 598 patients, of which 371 and 227 patients received sugical or conservative treatment. ②The results of the meta-analysis indicated that, compared with conservative group, surgical group has higher JOA scores[MD=3.08,95%CI(2.71,3.45),Z=16.29,P<0.000 01], better Neurological function recovery rate[MD=15.87,95%CI(9.28,22.46),Z=4.72,P<0.000 01], better ASIA impairment scale grade improvement[OR=2.3,95%CI(1.25,4.15),Z=2.76,P=0.006], and there were no significant differences in the ASIA motor scores[MD=5.45,95%CI(-7.56,18.47),Z=0.82,P=0.41]. Conclusion These result suggests that surgical treatment can improve the neurological function more effectively for patients with cervical spinal cord injury without fracture and dislocation.
目的 评价鞘内注射雷帕霉素对CCI神经病理性痛大鼠的痛阈及脊髓背角胶质细胞表达的影响。方法 健康雄性SD大鼠30只随机分为6组:①CCI组:CCI术后14天处死;②正常对照组:不做任何处理; ③前对照剂组:鞘内置管3天后行CCI术,术后4小时后鞘内给同体积生理盐水,连给3天; ④前给药组:鞘内置管3天后行CCI术,术后4小时鞘内给雷帕霉素溶液,连给3天; ⑤后对照剂组:鞘内置管3天后行CCI术,术后7天鞘内给同体积生理盐水,连给3天;⑥后给药组:鞘内置管3天后行CCI术,术后7天鞘内给雷帕霉素溶液,连给3天。各组于CCI术前1天和术后第2、4、6、8、10、12、14天测机械痛阈和热痛阈。术后14天测痛后用多聚甲醛灌注大鼠,取L4~5脊髓,免疫组化染色,星形胶质细胞标记蛋白(GFAP)检测星形胶质细胞表达变化,并定量分析。结果 与对照组相比,CCI手术组热痛阈和机械痛阈从CCI手术后第4天开始下降(P<0.05);前后给药对照剂组与CCI组相比,差别无统计学意义(P>0.05)。前给药组痛阈从CCI手术后第4天开始上升并持续至手术后第14天,与CCI组相比,差别有统计学意义 (P<0.05)。与CCI组相比,后给药组痛阈从CCI第8天开始上升并持续至手术后第14天,差别有统计学意义(P<0.05)。 与正常对照组比较,CCI组、前、后对照剂组手术侧脊髓背角GFAP染色阳性区平均光密度与阳性面积均有增加,差别有统计学意义(P<0.05)。前、后给药组手术侧GFAP染色阳性区平均光密度与阳性面积与CCI组比较,均有明显降低,差别有统计学意义(P<0.05)。结论 鞘内注射雷帕霉素可缓解大鼠神经病理性痛,并抑制脊髓背角胶质细胞的激活。
Objective To evaluate the effects of intrathecal injection of rapamycin on pain threshold and spinal cord gliacyte activation in rats of neuropathic pain. Methods Healthy 30 male SD rats were randomly divided into 6 groups(n=5 in each group): ① control group without operation or intrathecal injection. ②CCI group without intrathecal injection. ③ intrathecal injection of rapamycin 10 μg(10 μL) 4 hours after CCI operation and the next 2 days once a day. ④ intrathecal injection of NS10 μL 4 hours after CCI operation and the next 2 days once a day. ⑤ intrathecal injection of rapamycin 10 μg(10 μL) 7 days after CCI operation and the next 2 days once a day.⑥ intrathecal injection of NS10 μL 7 days after CCI operation and the next 2 days once a day. Mechanical and thermal threshold were tested 1 day before the CCI operation and 2th、4th、6th、8th、10th、12th、14th days after the CCI operation for all the rats. Lumbar segment of spinal cords was removed for determination of glial fibrillary acidic protein(GEAP) in spinal cord by immuohistochemistry dyeing and assay in the 14th day after CCI operation for all the rats. Results Mechanical and thermal hyperalgesia emerged on 4th day and maintained till 14th day after CCI operation(P<0.05). After intrathecal injection of rapamycin 4 hours or 7days after CCI, mechanical and thermal threshold significantly increased compared to intrathecal injection of NS(P<0.05). And the sum area of GFAP positive and the mean density of GFAP positive area in the dorsal horn of operation side greatly increased in rapamycin treated groups compared NS treated groups(P<0.05). Conclusion Intrathecal injection of rapamycin may attenuate CCI induced hyperalgesia and inhibit the activation of astrocyte.
