       1型糖尿病(T1DM)是一种免疫介导的胰岛β细胞特异性破坏的自身免疫性疾病,全球发病率逐年上升。胰岛自身抗体(IAbs)是T1DM最可靠的生物标志物,用于早期预测和诊断。然而,传统的放射配体法(RBA)虽然具有高实验特异度,但在疾病特异度方面存在局限性,尤其是单抗体阳性的预测价值较低。近年来,电化学发光法(ECL)作为一种无放射性污染的新方法,能够区分高亲和力和低亲和力的IAbs,显著提高了疾病特异度。多项研究表明,ECL法在预测T1DM风险方面优于RBA法,特别是在单抗体阳性的情况下。本文综述了IAbs检测方法的进展及其在T1DM预测和诊断中的应用,强调了ECL法在提高疾病特异度方面的优势。
       Type 1 diabetes mellitus(T1DM)is  an  autoimmune  disease  characterized  by the immune-mediated destruction of pancreatic β-cells,with a rising global incidence.Islet autoantibodies(IAbs)are the most  reliable biomarkers for early prediction and diagnosis of T1DM.However,the traditional radio-binding assay(RBA),despite its high experimental specificity,has limitations in disease specificity,particularly in the predictive value of single autoantibody positivity.Recently,the electrochemiluminescence(ECL)method,a non-radioactive approach,has been developed to distinguish high-affinity from low-affinity IAbs,significantly improving disease specificity.Multiple studies have shown that the ECL method outperforms RBA in predicting T1DM risk,especially in cases of single autoantibody positivity.This review discusses the advancements in IAbs detection methods and their applications in T1DM prediction and diagnosis,highlighting the advantages of the ECL method in enhancing disease specificity.

Th17/Treg与2型糖尿病患者胰岛素抵抗及胰岛β细胞功能的关系

目的 观察辅助性T17细胞(Th17)与调节性T细胞(Treg)比值与2型糖尿病(T2DM)患者胰岛素抵抗及胰岛β细胞功能的关系。方法 纳入2022年4月—2023年4月在贵州医科大学第二附属医院内分泌科住院及健康体检人群各100例, 分为糖耐量正常组(NGT组, n=100)和T2DM组(n=100), 分别测定糖化血红蛋白(HbA1c)、空腹血糖(FPG)、甘油三酯(TG)等生化指标, 电化学发光法测定空腹胰岛素(FINS), 稳态模型计算胰岛β细胞功能指数(HOMA-β)、胰岛素抵抗指数(HOMA-IR)及胰岛素敏感指数(HOMA-ISI)。流式细胞术检测Th17、Treg水平。HOMA-IR、HOMA-β和HOMA-ISI的影响因素采用多元线性回归分析。结果 与NGT组相比, T2DM组BMI、FPG、HbA1c、LDL-C 、TG、TC、FINS、HOMA-IR、Th17及Th17/Treg水平均升高(P<0.01), HDL-C、HOMA-β、HOMA-ISI、Treg水平均降低, 且差异有统计学意义(P<0.01)。Th17与BMI(r=0.251, P<0.001)及HOMA-IR(r=0.305, P<0.001)呈正相关; 与HOMA-β(r=-0.204, P<0.001)及HOMA-ISI(r=-0.359, P<0.001)呈负相关。Treg与HOMA-ISI之间呈正相关(r=0.170, P=0.008), 而与HOMA-IR呈负相关(r=-0.153, P=0.017); Th17/Treg与BMI(r=0.332, P<0.001)及HOMA-IR(r=0.374, P<0.001);与HOMA-β(r=-0.249, P<0.001)及HOMA-ISI(r=-0.427, P<0.001)呈负相关。多元线性回归分析显示, Th17/Treg是HOMA-IR(β=5.915)升高及HOMA-ISI(β=-2.557)下降的影响因素(P<0.01)。结论 Th17/Treg可能通过影响胰岛素抵抗、降低胰岛素敏感性参与T2DM的发生。
Objective To explore the relationship among the proportion of helper T17 cells(Th17)to regulatory T cells(Treg), insulin resistance, and the function of islet beta cells.Methods One hundred cases of hospitalized patients and 100 cases of health check-ups people in the Department of Endocrinology of the Second Affiliated Hospital of Guizhou Medical University from April 2022 to April 2023 were included.Patients were divided into normal glucose tolerance group(NGT group, n=100)and type 2 diabetes mellitus group(T2DM group, n=100).The biochemical indexes of HbA1c, fasting blood glucose(FPG), triglyceride(TG)and fasting insulin(FINS)were determined by electrochemiluminescence.Islet beta cell function index(HOMA-β), insulin resistance index(HOMA-IR)and insulin sensitivity index(HOMA-ISI)were calculated in homeostasis model.The levels of Th17 and Treg were detected by flow cytometry.Spearman was used to analyze the correlation between indicators, and multiple linear regression analysis was used to analyze the influencing factors of HOMA-IR, HOMA-β and HOMA-ISI.Results In contrast to the NGT group, the T2DM group exhibited elevated levels of BMI, FPG, HbA1c, LDL-C, TG, TC, FINS, HOMA-IR, Th17 and Th17/Treg, with these variances being signifincantly different(P<0.01).There was a notable reduction in the levels of HDL-C,HOMA-β,HOMA-ISI,Treg,with those changes being significantly different(P<0.01).Th17 was positively correlated with BMI(r=0.251, P<0.001)and HOMA-IR(r=0.305, P<0.001), it was negatively correlated with HOMA-β(r=-0.204, P<0.001)and HOMA-ISI(r=-0.359, P<0.001).Treg was positively correlated with HOMA-ISI(r=0.170, P=0.008), while it was negatively correlated with HOMA-IR(r=-0.153, P=0.017).The ratio of Th17/Treg was positively correlated with BMI(r=0.332, P<0.001)and HOMA-IR(r=0.374, P<0.001), it was negatively correlated with HOMA-β(r=-0.249, P<0.001)and HOMA-ISI(r=-0.427, P<0.001).Multiple linear regression analysis showed that Th17/Treg was an influential factor in the increase of HOMA-IR(β=5.915)and the decrease of HOMA-ISI(β=-2.557)(P<0.01).Conclusions Th17/Treg may be involved in the development of T2DM by affecting insulin resistance and reducing insulin sensitivity.

长病程2型糖尿病患者胰岛β细胞功能与BMI的相关性研究

目的 探讨长病程2型糖尿病(T2DM)患者体质指数(BMI)与胰岛β细胞功能间的相关关系。方法 选取2023年12月—2024年3月于承德市中心医院内分泌风湿免疫科住院的260例长病程(病程≥10年)T2DM患者作为研究对象,依据BMI将其分成正常组、超重组和肥胖组,比较三组间一般资料、检验学指标及检查的差异,分析胰岛β细胞功能与各指标间的相关性。结果 三组研究对象在空腹静脉血糖(FPG)、空腹C肽(FCP)、胰岛素抵抗指数(HOMA-IR)、尿酸(UA)、甘油三酯(TG)等上差异有统计学意义(P<0.05),在胰岛β细胞功能指数(HOMA-β)比较差异无统计学意义(P>0.05);肥胖组的FPG、FCP、HOMA-IR、UA、TG均高于正常组(P<0.05),超重组的UA、TG均高于正常组(P<0.05),肥胖组的FPG、HOMA-IR、UA高于超重组(P<0.05),Spearman相关分析结果显示HOMA-β与体质量指数(BMI)无相关性(r=0.046,P=0.461),HOMA-β与UA(r=0.226,P<0.001)、TG(r=0.148,P=0.017)呈正相关,HOMA-IR与BMI(r=0.279,P<0.001)与、UA相关(r=0.284,P<0.001)及TG(r=0.349,P<0.001)呈正相关,多元线性回归分析显示UA是HOMA-β的影响因素(P<0.05),BMI、UA、TG是HOMA-IR的影响因素(P<0.05)。结论 长病程的T2DM患者,其胰岛素抵抗水平随着BMI的增加逐渐升高,而胰岛β细胞功能指数与BMI的相关性不显著。同时,UA和TG也是长病程T2DM患者胰岛β细胞功能的影响因素。
Objective To explore the correlation between body mass index(BMI)and islet β cell function in patients with long course type 2 diabetes mellitus(T2DM).Methods A total of 260 patients with T2DM with a long course of disease(course≥10 years)admitted to the Department of Endocrinology and Rheumatology of Chengde Central Hospital from December 2023 to March 2024 were selected as the study objects,and were divided into normal group,overweight group and obese group according to BMI.Comparison among the three groups in general data,inspection index and and the difference of the islet β cell function were performed,and the correlation among the indexes was analyzed.Results There were statistically significant differences in fasting plasma glucose(FPG),fasting C peptide(FCP),homeostasis model assessment-insulin resistance(HOMA-IR),uric acid(UA)and triglycerides(TG)among the three groups(P<0.05),but no statistically significant differences in homeostatic model assessment of β-cell function(HOMA-β)(P>0.05).The levels of FPG,FCP,HOMA-IR,UA and TG in the obese group were higher than those in the normal group(P<0.05);the levels of UA and TG in the overweight group were higher than those in the normal group(P<0.05);the levels of FPG,HOMA-IR and UA in the obese group were higher than those in the overweight group(P<0.05).Spearman correlation analysis showed that HOMA-β was not correlated with BMI(r=0.046,P=0.461),but was positively correlated with UA and TG(r=0.226,P<0.001;r=0.148,P=0.017),HOMA-IR was positively correlated with BMI,UA and TG(r=0.279,P<0.001;r=0.284,P<0.001;r=0.349,P<0.001).Multiple linear regression analysis showed that UA was the influencing factor of HOMA-β(P<0.05),BMI,UA and TG were the influencing factors of HOMA-IR(P<0.05). Conclusions In T2DM patients with long disease course,the level of insulin resistance increased gradually with the increase of BMI,but the correlation between β-cell function index and BMI was not significant.At the same time,UA and TG are also factors affecting the function of islet β cells in patients with long-course T2DM.

重复经颅磁刺激应用于帕金森病失眠患者中的效果及对睡眠质量的影响研究

目的 探讨重复经颅磁刺激应用于帕金森病失眠患者中的效果及对睡眠质量的影响。方法 选取2021年1月—2022年3月南阳南石医院神经内科收治的帕金森病失眠患者100例,按照随机数字表法均分为参照组(n=50)和研究组(n=50),参照组采用常规西药治疗,研究组在参照组基础上采用重复经颅磁刺激治疗,对比两组治疗效果、神经递质指标水平、帕金森症状表现及睡眠质量评分的差异。结果 研究组治疗总有效率98.00%(49/50)高于参照组82.00%(41/50),对比差异有统计学意义(P<0.05)。治疗后,研究组神经递质多巴胺及5-羟色胺水平分别为(6.84±0.62)、(22.75±0.59),均高于参照组,对比差异均有统计学意义(P<0.05)。治疗后,研究组帕金森病症状表现及睡眠质量总评分均低于参照组,对比差异均有统计学意义(P<0.05)。结论 重复经颅磁刺激应用到帕金森病失眠患者的治疗中,能提升疗效,明显改善患者睡眠质量和帕金森病症状。
Objective To investigate the effect of repetitive transcranial magnetic stimulation(rTMS)on Parkinson's insomnia and its influence on sleep quality.Methods A total of 100 Parkinson's patients with insomnia admitted to the Department of Neurology,Nanyang Nanshi Hospital from January 2021 to March 2022 were selected and divided into the reference group(n=50)and the study group(n=50)according to random number table method.The reference group was treated with conventional western medicine,and the study group was treated with rTMS on the basis of the reference group.The levels of neurotransmitters,symptoms of Parkinson's disease and sleep quality were compared.Results The effective rate of the study group was 98.00%(49/50),which was higher than 82.00%(41/50)of the reference group,with statistical significance(P<0.05).After treatment,the levels of dopamine and 5-hydroxytryptamine in the study group,(6.84±0.62)and(22.75±0.59),were higher than those in the reference group,and the comparison was statistically significant(P<0.05).After treatment,the total score of Parkinson's symptom and sleep quality in the study group were lower than those in the reference group,with statistical significance(P<0.05).Conclusions The application of rTMS in the treatment of Parkinson's patients with insomnia can improve the curative effect,significantly improve the sleep quality and symptoms of Parkinson's disease,and is worthy of further application and promotion.

氟哌噻吨美利曲辛联合帕罗西汀对重度抑郁障碍患者躯体化症状、睡眠和认知功能的影响分析

目的 探讨氟哌噻吨美利曲辛联合帕罗西汀对重度抑郁障碍(MDD)患者躯体化症状、睡眠和认知功能的影响分析以及临床应用效果。方法 回顾性分析2020年8月—2023年2月在南昌市某医院接受治疗的120例MDD患者相关资料,按照其治疗方案不同分为帕罗西汀治疗组(常规组,n=55)和氟哌噻吨美利曲辛联合帕罗西汀治疗组(联合组,n=65)。两组患者治疗周期均为4周,比较两组患者治疗前和治疗第2、4周的汉密尔顿抑郁量表(HAMD-17)评分、躯体化症状自评量表(SSS)评分、睡眠质量评分(PSQI)、神经心理状态评定量表(RBANS);且治疗后对患者进行1个月的随访比较两组患者治疗后总体疗效及不良反应发生情况。结果 经治疗第2、4周联合组RBANS评分高于常规组(P<0.05),而PSQI评分、SSS评分、HAMD-17评分均低于常规组(P<0.05)。治疗后1个月随访资料显示,两组患者不良反应总发生率比较,差异无统计学意义(P>0.05),且总有效率高于常规组(P<0.05)。结论 氟哌噻吨美利曲辛联合帕罗西汀对MDD患者临床应用疗效确切,还可以帮助患者减轻躯体化症状,改善患者睡眠质量,并且提高患者认知功能。
Objective To investigate the effect of haloperitoxine melitraxine combined with paroxetine on somatic symptoms,sleep and cognitive function in patients with major depressive disorder(MDD)and its clinical application effects.Methods A retrospective analysis was performed on the relevant data of 120 patients with MDD who received treatment in our hospital from August 2020 to February 2023,and divided into conventional group(treated with paroxetine,55 cases)and combined group(haloperitoxetex melitraxine combined with paroxetine,65 cases)according to their different treatment regimens.The treatment duration of the two groups was 4 weeks,and the Hamilton Rating Scale for Depression(HAMD-17)score,Somatized Symptom Self-rating Scale(SSS)score,Sleep Quality Score(PSQI) and Neuropsychological State Rating Scale(RBANS)scores were compared before treatment and at the 2nd and 4th week of treatment.After treatment,the patients were followed up for 1 month,and the total efficacy and adverse reactions of the two groups of patients after treatment were compared.Results After 2 and 4 weeks of treatment,the combined group showed significantly higher RBANS scores compared to the control group(P<0.05),while PSQI scores,SSS scores and HAMD-17 scores were significantly lower in the combined group compared to the control group(P<0.05).One month after treatment,follow-up data showed that there was no statistically significant difference in the overall incidence of adverse reactions between the two groups(P>0.05).Additionally,the total effective rate was significantly higher in the combined group compared to the control group(P<0.05).Conclusions Haloperitoxine melitrexine combined with paroxetine has a definite clinical effect in patients with MDD,and can also help patients reduce somatization symptoms,improve patients' sleep quality,and improve patients' cognitive function.

家庭睡眠习惯调查在孤独症患儿睡眠评估中的信效度研究

目的 检验家庭睡眠习惯调查(FISH)在评估孤独症谱系障碍(ASD)患儿睡眠评估中的信度和效度。方法 随机抽取在清远市妇幼保健院儿童语言行为科干预的199例2~6岁ASD共患睡眠障碍患儿,其照顾者同时完成FISH和儿童睡眠习惯问卷(CSHQ),2周后再次同时完成上述两个问卷,以检验FISH的信度和效度。结果 验证性因素分析的结果表明,五因素模型拟合良好(χ2=79.05,df=44,χ2/df=1.80, P<0.001,SRMR=0.06,GFI=0.94,IFI=0.89,RMSEA=0.06),总量表及各分量表有较高的内部一致性信度(0.71~0.77)和重测信度(0.79~0.88)。FISH的白天习惯分别与CSHQ的入睡延迟、夜醒呈负相关,入睡前习惯分别与CSHQ的睡眠抵触、睡眠焦虑、白天睡眠呈负相关,睡眠常规分别与CSHQ的睡眠抵触、入睡延迟、睡眠焦虑呈负相关,睡前父母行为分别与CSHQ的睡眠抵触、睡眠焦虑呈负相关,总分与CSHQ总分呈负相关,差异均有统计学意义(P<0.05)。结论 FISH具有较好的信度和效度,可作为ASD患儿睡眠习惯的评估工具。
Objective To test the reliability and validity of the famity inveritory of sleep habits(FISH) in the sleep assessment among children with autism spectrum disorder (ASD). Methods A random sample of 199 children with ASD and sleep disorders intervened in the Children's Language and Behavior Department of Maternal and Child Health Hospital of Qingyuan City were selected, their caregivers completed the FISH and the children's sleep habit questionnaire(CSHQ) at the same time, and completed the above two questionnaires again 2 weeks later to test the reliability and validity of the FISH. Results The results of confirmatory factor analysis showed that the five-factor model fits well (χ2=79.05, df=44, χ2/df=1.80, P<0.001, SRMR=0.06, GFI=0.94, IFI =0.89, RMSEA=0.06), the total scale and each subscale had high internal consistent reliability (0.71~0.77) and test-retest reliability (0.79~0.88). The daytime habits of FISH were negatively correlated with sleep delay and waking up at night of CSHQ respectively, the habits before sleep of FISH were negatively correlated with sleep conflict, sleep anxiety and daytime sleep of CSHQ respectively, the sleep routine of FISH was negatively correlated with sleep conflict, sleep delay and sleep anxiety of CSHQ respectively, the parents' behavior before sleep of FISH was negatively correlated with sleep conflict and sleep anxiety of CSHQ respectively, and the total score of FISH was negatively correlated with CSHQ, with statistically significant differences (P<0.05). Conclusions The FISH had good reliability and validity, and can be used as an assessment tool for children with ASD.

超低频经颅磁刺激联合艾司唑仑对失眠症患者睡眠脑电图参数及血清神经营养因子表达的作用

目的 探讨超低频经颅磁刺激(ILF-TMS)联合艾司唑仑对失眠症患者睡眠脑电图参数及血清神经营养因子表达的影响。方法 选取2018年8月—2020年4月我院失眠症患者114例,随机数字表法分为研究组(n=57)、对照组(n=57)。对照组予以艾司唑仑联合ILF-TMS假性刺激,研究组予以艾司唑仑联合ILF-TMS真性刺激,均治疗1个月。对比2组疗效与治疗前、治疗1个月后睡眠进程参数(总睡眠时间、入睡时间、睡眠效率、觉醒时间)、睡眠结构(非快速眼动睡眠期、快速动眼睡眠期)、匹兹堡睡眠质量量表(PSQI)评分、失眠严重程度指数量表(ISI)评分、焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分、血清神经营养因子[胶质细胞源性神经营养因子(GDNF)、脑源性神经营养因子(BDNF)]水平。结果 (1)疗效:研究组治疗1个月后总有效率高于对照组(P<0.05);(2)睡眠进程参数:治疗1个月后研究组总睡眠时间、睡眠效率高于对照组,入睡时间、觉醒时间短于对照组(P<0.05);(3)睡眠结构:治疗1个月后研究组Ⅲ期、Ⅱ期、非快速眼动睡眠期高于对照组,Ⅰ期睡眠期低于对照组(P<0.05);(4)PSQI、ISI评分:治疗1个月后研究组PSQI、ISI评分低于对照组(P<0.05);(5)SAS、SDS评分:治疗1个月后研究组SAS、SDS评分低于对照组(P<0.05);(6)血清神经营养因子:治疗1个月后研究组血清GDNF、BDNF水平高于对照组(P<0.05)。结论 ILF-TMS联合艾司唑仑治疗失眠症效果确切,可上调血清神经营养因子表达,改善睡眠脑电图参数,提高睡眠质量,控制焦虑、抑郁症状。
Objective To investigate the effects of infra-low frequency transcranial magnetic stimulation (ILF-TMS) combined with estazolam on sleep EEG parameters and serum neurotrophic factors expression in patients with insomnia. Methods One hundred and fourteen patients with insomnia in our hospital from August 2018 to April 2020 were divided into study group (n=57) and control group (n=57) by random number table method. The control group was treated with estazolam combined with pseudo ILF-TMS stimulation, and the study group was treated with estazolam combined with real ILF-TMS stimulation, all of which were treated for 1 month. Comparing curative effect of two groups before and 1 month after treatment, and sleep process parameters (total sleep time, falling asleep time, sleep efficiency, waking time), the structure of the sleep stages (stage Ⅲ, Ⅱ, Ⅰ, rapid eye movement sleep), Pittsburgh sleep quality index (PSQI), insomnia severity index (ISI), self-rating anxiety scale (SAS) score, self-rating depression scale (SDS) score, serum neurotrophic factors (GDNF, BDNF) levels. Results (1) Efficacy: the total effective rate of the study group was higher than that of the control group after 1 month of treatment (P<0.05). (2) Sleep process parameters: after 1 month of treatment, the total sleep time and sleep efficiency in the study group were higher than those in the control group, and the falling asleep time and waking time were shorter than those in the control group (P<0.05). (3) Sleep structure: after 1 month of treatment, compared to the control group, the study group had more in stageⅢ, Ⅱ, rapid eye movement sleep, and less in stage Ⅰsleep(P<0.05). (4) PSQI and ISI scores: after 1 month of treatment, PSQI and ISI scores of the study group were lower than those of the control group (P<0.05). (5) SAS and SDS scores: after 1 month of treatment, SAS and SDS scores in the study group were lower than those in the control group (P<0.05). (6) Serum neurotrophic factors: after 1 month of treatment, serum GDNF and BDNF levels in the study group were higher than those in the control group (P<0.05). Conclusions ILF-TMS combined with estazolam is an effective treatment of insomnia. It can up-regulate the expression of serum neurotrophic factors, improve the parameters of sleep EEG, improve sleep quality, and control the symptoms of anxiety and depression.

斑点追踪显像对阻塞性睡眠呼吸暂停综合征患者左心房功能的研究

目的 应用斑点追踪显像定量评价阻塞性睡眠呼吸暂停综合征患者的左心房功能,并进行对比研究。方法 OSAS患者60例,根据睡眠呼吸暂停低通气指数(AHI)分为3组,轻度组,中度组,重度组,每组20人;另选择20名健康正常人作为正常对照组。采用二维斑点追踪成像技术测量各组左心房相关应变参数:收缩期应变(Ss),舒张早期应变(Se),舒张晚期应变(Sa),左心房收缩期应变率(SRs),左心房舒张早期应变率(SRe),左心房舒张晚期应变率(SRa),分析并比较各组间参数的差异性。结果 随着疾病严重程度的增加Ss、SRs、Se、SRe呈逐渐减低的趋势,而Sa、SRa先升高后减低,差异有统计学意义(P<0.05)。结论 应用斑点追踪成像技术可在OSAS患者发生左心房重构前检测出心房功能的变化。
Objective The left atrial function of patients with obstructive sleep apnea syndrome was quantitatively evaluated by speckle tracing imaging and compared. Methods Sixty patients with OSAS were divided into three groups according to the sleep apnea hypopnea index(AHI),including the mild group, the moderate group and the severe group, with 20 patients in each group. Another 20 healthy people were selected as the normal control group. Left atrial correlation strain parameters:systolic strain(Ss), early diastolic strain(Se), late diastolic strain(Sa), left atrial systolic strain rate(SRs), early diastolic strain rate(SRe), left atrium left atrium late diastolic strain rate(SRa) were measured by two dimensional speckle tracking imaging technology, and the differences of parameters between groups were compared. Results With the increase of disease severity, Ss, SRs, Se and SRe showed a trend of gradual decrease, while Sa and SRa increased first and then decreased, and the difference was statistically significant(P<0.05). Conclusion Changes in atrial function may be detected in OSAS patients prior to left atrial remodeling using speckle tracking imaging.

综合医院护士的日间过度嗜睡及其影响因素的研究

目的 本研究通过调查中国华南地区一间大型医疗机构(综合医院)中护士日间过度嗜睡的发生率及相关影响因素。方法 纳入了1 102名在职护士要求他们在一段规定的时间内完成一份自我报告问卷,其中包括人口学资料采集、生活习惯(BMI、小睡习惯、打鼾等)、失眠、抑郁、焦虑、MEQ分型、倒班、工作兴趣等与工作及睡眠相关的特征。了解EDS在被试人群中的发生率及其影响因素。结果 共有1 048名护士有效的完成了问卷(应答率为95.1%)。其中169名(16.1%)护士存在日间过度嗜睡(EDS)。抑郁症状(校正后的OR值= 2.24,95%的可信区间 1.51~3.31)、焦虑症状(1.65; 1.02~2.67)、失眠(2.29; 1.56~3.36)、倒班工作(1.98; 1.03~3.83)和对工作的低兴趣(1.74; 1.01~2.99)是EDS发生的独立危险因素。结论 日间过度嗜睡(EDS)在华南地区的综合医疗机构中的青年护士群体中普遍存在。EDS的发生可能与抑郁症状、焦虑症状、失眠、倒班工作和对工作的低兴趣存在相关。
Objective To investigate the prevalence and correlates of excessive daytime sleepiness EDS in a population of hospital nurses in South China. Methods A total of 1 102 nurses working in a large medical center were invited to participate in this cross-sectional study (females 96.9%,mean age 29.6 years). They all completed a self-reported questionnaire consisting of items on demographic variables,lifestyle factors,insomnia,anxiety,depression,and both work-related and sleep related characteristics. Results A total of 1 048 nurses gave a valid response (response rate 95.1%). Among them,169 (16.1%) reported EDS as defined as an Epworth Sleepiness Scale≥14. Depression (adjusted odds ratio=2.24,95% confidence interval 1.51~3.31),anxiety (1.65; 1.02 ~2.67),insomnia (2.29; 1.56~3.36),rotating shift work (1.98; 1.03~3.83) and low interest in work (1.74; 1.01~2.99) were all independent risk factors of the occurrence of EDS. Conclusion EDS were common among this relatively young and healthy nurse population in south China. There were clear associations with EDS and depression,anxiety,insomnia,rotating shift work and low work-related interest.

三维斑点追踪成像评估睡眠呼吸暂停综合征患者心室整体心肌应变

目的 应用三维斑点追踪成像(3D-STI)技术评价睡眠呼吸暂停综合征患者左、右心室整体收缩功能。方法 OSAS患者60例,根据睡眠呼吸暂停低通气指数(AHI)分为3组,轻度组,中度组,重度组,每组20人;另选择20名健康正常人作为正常对照组。用三维斑点追踪成像技术测量各组左心室整体纵向应变(LVGLS)、圆周应变(LVGCS)、径向应变(LVGRS)、面积应变(LVGAS)以及右心室整体纵向应变(RVGLS)、圆周应变(RVGCS)、径向应变(RVGRS),并比较各组间参数的差异。结果 与轻度OSAS组比较,中、重度OSAS组LVGLS、LVGAS降低,差异有统计学意义(P<0.05);与中度OSAS组相比较,重度组LVGLS、LVGCS、LVGRS以及LVGAS均降低,差异有统计学意义(P<0.05);右心室部分应变参数变化早于左心室。结论 应用3D-STI技术能够在患者左心室射血分数(LVEF)发生改变之前发现早期左、右心室功能收缩障碍,为临床早期诊断及干预提供可靠的依据。
Objective To evaluate left and right ventricular systolic function of patients with obstructive sleep apnea syndrome (OSAS) by using three-dimensional speckle tracking imaging(3D-STI). Methods Sixty patients with OSAS were divided into three groups according to the sleep apnea hypopnea index (AHI), the mild group, the moderate group and the severe group, with 20 patients in each group. Another 20 healthy people were selected as the normal control group. The left ventricle global longitudinal strain (LVGLS), left ventricle global circumferential strain (LVGCS), left ventricle global radial strain (LVGRS), left ventricle global area strain (LVGAS) and right ventricle global longitudinal strain (RVGLS), right ventricle global circumferential strain (RVGCS) and right ventricle global radial strain (RVGRS) were measured by three-dimensional speckle tracking imaging technology, and the differences of parameters between groups were compared. Results Compared with mild OSAS group, LVGLS and LVGAS were decreased in moderate and severe OSAS groups, and the difference was statistically significant (P<0.05). Compared with the moderate OSAS group, LVGLS, LVGCS, LVGRS and LVGAS in the severe group all decreased, and the difference was statistically significant (P<0.05). Some of right ventricle strain parameters were earlier than that of left ventricle. Conclusion The application of 3D-STI technology can detect early left and right ventricular dysfunction before left ventricular ejection fraction(LVEF)changes, provide a reliable basis for early clinical diagnosis and intervention.
