目的 探讨多参数MRI在鉴别乳腺影像报告和数据系统(BI-RADS)4类乳腺病变良恶性中的应用。方法 回顾性分析2022年1月—2024年12月于同济大学附属东方医院庐江分院行乳腺MRI检查为BI-RADS 4类且病理结果明确肿瘤的病例268例,其中良性组166例(包括乳腺纤维腺瘤93例,导管内乳头状瘤39例,乳腺囊性增生24例,乳腺分叶状肿瘤2例,浆细胞性乳腺炎8例),恶性组102例(包括浸润性导管癌89例,浸润性小叶癌6例,黏液癌2例,炎性乳腺癌5例)。对比分析单一序列和多序列联合诊断BI-RADS 4类乳腺病变良、恶性的效能;绘制基于多参数MRI受试者工作特征(ROC)曲线,并计算曲线下面积(AUC)、特异度和灵敏度,量化多序列对于BI-RADS 4类乳腺病变良恶性的诊断效能。结果 基于多参数MRI联合诊断效能最高(AUC:0.912,95%CI:0.871~0.953),特异度为0.922,灵敏度为0.902;基于ADC值评估BI-RADS 4类乳腺病变最佳截断值为0.84×10-3 mm2 /s,且均具有统计学意义(P<0.05)。结论 基于多参数MRI能够有效诊断BI-RADS 4类乳腺病变的良、恶性,为乳腺外科疾病进一步诊疗提供影像学诊断依据。
Objective To explore the application of multiparametric MRI in differentiating benign and malignant lesions classified as BI-RADS category 4 in breast imaging.Methods A total of 268 cases of breast MRI classified as BI-RADS category 4 with confirmed tumor pathology were retrospectively analyzed from January 2022 to December 2024 at the Tongji University Oriental Hospital Lujiang Branch. Among them,166 cases were categorized as benign lesions(including 93 cases of breast fibroadenoma,39 cases of intraductal papilloma,24 cases of cystic hyperplasia,2 cases of lobular tumor,and 8 cases of plasma cell mastitis),while 102 cases were categorized as malignant lesions(including 89 cases of invasive ductal carcinoma,6 cases of invasive lobular carcinoma,2 cases of mucinous carcinoma,and 5 cases of inflammatory breast cancer).The efficacy of single-sequence versus combined multiple-sequence imaging in diagnosing benign and malignant BI-RADS category 4 breast lesions was compared and analyzed.The diagnostic efficacy of the multiparametric MRI sequences was quantified by plotting a receiver operating characteristic curve and calculating the area under the curve(AUC),as well as the specificity and sensitivity.Results The combined diagnostic efficacy of multiparametric MRI yielded the highest performance,with an AUC of 0.912(95%CI:0.871-0.953),demonstrating a specificity of 0.922 and a sensitivity of 0.902.The optimal cutoff value for differentiating BI-RADS category 4 breast lesions based on apparent diffusion coefficient values was determined to be 0.84×10-3 mm2 /s,with all results being statistically significant(P<0.05).Conclusions Multiparametric MRI can effectively diagnose benign and malignant lesions classified as BI-RADS category 4,providing a reliable imaging basis for further diagnosis and treatment in breast surgery.
目的 分析“侧俯卧位”MRI引导腰脊神经后根神经节脉冲射频治疗腰椎间盘突出症的手术体位护理效果。方法 对2018年9月—2020年12月在广州市荔湾中心医院住院接受经MRI引导腰脊神经后根神经节脉冲射频的腰椎间盘突出症患者62例随机分为两组:侧俯卧位组和俯卧位组,每组各31例。侧俯卧位组采用患侧抬高约30 °的侧俯卧位手术,俯卧位组采用标准俯卧位手术。记录两组手术时间、手术并发症、手术体位相关并发症及术者对术野显露的满意度评价。结果 所有患者均顺利完成手术,未见出血、感染、下肢麻痹加重神经损伤、脏器损伤等手术并发症,无患者发生眼压增高、臂丛损伤、压疮等体位相关并发症。侧俯卧位组手术时间(60.65±12.45)min,俯卧位组手术时间(70.58±10.25) min,组间比较差异有统计学意义(t=3.429,P=0.001)。术者对侧俯卧位组和俯卧位组术野侧显露总满意度分别为93.55%和77.42%,组间比较差异有统计学意义(χ2=4.292,P=0.038)。结论 侧俯卧位MRI引导腰脊神经后根神经节脉冲射频治疗腰椎间盘突出症安全、有效,做好手术体位护理可有效防止并发症。
Objective To analyze the effect of surgical position nursing for radiofrequency ablation in posterior root ganglion in lumbar spinal nerve guided by MRI in lateral prone position.Methods Sixty-two patients with lumbar disc herniation hospitalized in Liwan Central Hospital of Guangzhou from September 2018 to December 2020 were randomly divided into two groups(the lateral prone position group and the prone position group,31 patients in each group).The lateral prone position group used the lateral prone position with about 30 degrees elevation of the affected side,and the prone position group used the standard prone position for operation.Operating time,surgical complications,surgical position related complications,and operator satisfaction evaluation of surgical field exposure were recorded in both groups.Results All patients completed the operation successfully.No surgical complications such as bleeding,infection,lower limb paralysis,aggravated nerve injury or organ injury were found in the two groups.No surgical position related complications such as intraocular pressure raising,brachial plexus injury or pressure ulcers in both the groups either.The operation time was(60.65±12.45)min in the lateral prone position group and(70.58±10.25)min in the standard prone position group,respectively,and the difference was statistically significant(t=3.429,P=0.001).The surgeon’s satisfaction evaluation of the surgical field exposure was much higher in the lateral prone position group(93.55%)compared with the standard prone position group(77.42%),and the difference was statistically significant(χ2=4.29,P=0.038).Conclusions Radiofrequency ablation in posterior root ganglion of lumbar spinal nerve guided by MRI in lateral prone position is safe and effective.Good surgical position nursing can effectively prevent complications.
目的 探讨振幅整合脑电图(aEEG)联合头颅磁共振成像(cMRI)对早产儿矫正12月龄时神经发育的预测价值。方法 选取110例早产儿为研究对象,并在矫正12月龄时采用Gesell 发育量表评估发育商(DQ),依据DQ分为Gesell 正常组(DQ≥85,n=83)、Gesell 异常组(DQ<85,n=27)。采集早产儿及母亲临床资料,对比两组出生后72 h内aEEG、矫正胎龄37周时cMRI检查指标差异。结果 两组早产儿及母亲基线资料比较差异无统计学意义(P>0.05)。相较于Gesell 正常组,Gesell 异常组双顶径(BPW)降低[(70.68±5.32)mm vs(66.54±3.69)mm],睡眠-觉醒周期(SWC)不成熟率(20.48% vs 85.19%)、aEEG异常率(30.12% vs 70.37%)、两半球间距(IHD)增高[(2.95±0.83) mm vs(3.56±0.72)mm](P<0.05)。Pearson相关分析结果显示,DQ值与IHD呈负相关,DQ值与BPW呈正相关(r=-0.361、0.598,P<0.05)。二元Logistic回归分析结果显示,BPW增高是Gesell 异常的独立保护因素(P<0.05),IHD增高、SWC不成熟及aEEG异常是Gesell 异常的独立危险因素(P<0.05)。结论 早产儿出生后72 h内aEEG异常、矫正胎龄37周时cMRI异常可能提示矫正12月龄时不良神经发育结局。
Objective To evaluate the predictive value of amplitude-integrated electroencephalogram combined with cranial magnetic resonance on neurodevelopment for preterm infants at corrected age of 12 months.Methods A total of 110 preterm infants were selected as study subjects,and Gesell developmental scale was used to evaluate developmental quotient(DQ)at corrected age of 12 months.According to DQ,they were divided into normal Gesell group(DQ≥85,n=83)and abnormal Gesell group(DQ<85,n=27).Clinical data of preterm infants and their mothers were collected,and the differences of amplitude-integrated electroencephalogram and cranial MRI(cMRI)were compared between two groups.Results There was no significant difference in baseline data between two groups(P>0.05).Compared with the normal Gesell group,the biparirtal width(BPW)in the abnormal Gesell group was decreased(70.68±5.32mm vs 66.54±3.69mm),the immaturity rate of sleep-wake cycle(SWC)(20.48% vs 85.19%),the abnormal rate of aEEG(30.12% vs 70.37%),and(IHD)(2.95±0.83mm vs 3.56±0.72mm)were increased(P<0.05).The results of Pearson correlation analysis showed that DQ was negatively correlated with IHD,and DQ was positively correlated with BPW(r=-0.361、0.598,P<0.05).Binary Logistic regression analysis showed that increased BPW was an independent protective factor for abnormal Gesell(P<0.05),and increased IHD,immature SWC and abnormal aEEG were independent risk factors for abnormal Gesell(P<0.05).Conclusions Abnormal aEEG within 72h after birth and abnormal cMRI at corrected age of 37 weeks may lead to adverse neurodevelopmental outcomes at corrected age of 12 months.
目的 探讨CT、MRI影像学表现对原发性肝细胞癌(HCC)微血管侵犯(MVI)的诊断价值。方法 选取2018年1月—2024年7月江门市第二人民医院(江门市中心医院蓬江分院)和江门市中心医院120例(共158个病灶)HCC患者,均行上腹部CT、MRI平扫+增强及弥散加权成像(DWI)检查;以术后病理结果为金标准。比较CT、MRI平扫+增强及DWI对HCC MVI诊断效能;分析HCC MVI诊断中CT、MRI平扫+增强及DWI检查与术后病理确诊结果之间的一致性;比较HCC MVI与无HCC MVI患者影像学表现及表观扩散系数(ADC)值。结果 DWI检查对HCC MVI的诊断效能(灵敏度、特异度、准确度、阳性预测值、阴性预测值)均显著性高于CT、MRI平扫+增强(P<0.05);CT、MRI、DWI对原发性肝细胞癌患者微血管侵犯的诊断效能比较,差异均无统计学意义(P>0.05)。在HCC MVI诊断效能中,CT、MRI影像学表现与术后病理确诊结果之间为中度一致性;DWI与术后病理确诊结果之间为高度一致性。HCC MVI患者的强化方式在非边缘动脉期强化、强化包膜、晕状强化、结中结、门脉分支癌栓占比均显著性高于无HCC MVI患者(P<0.05)。在不同b值(400、800、1 000、1 500 s/mm2)下,HCC MVI患者的ADC值均显著性高于无HCC MVI患者(P<0.05)。结论 CT、MRI平扫+增强及DWI对HCC MVI均具有较好的诊断效能,而MRI诊断结果与病理诊断一致性更佳,尤其DWI图中ADC值可更加精准地判断HCC的患者是否发生微血管侵犯,有助于指导临床医生建立“个体化”精准诊疗策略。
Objective To explore the diagnostic value of CT and MRI imaging manifestations for microvascular invasion(MVI)in primary hepatocellular carcinoma(HCC).Methods A total of 120 patients(158 lesions in total)with HCC in the Second People’s Hospital of Jiangmen(Pengjiang Branch of Jiangmen Central Hospital)and Jiangmen Central Hospital were selected from January 2018 to July 2024,all underwent CT and MRI plain + enhanced and diffusion-weighted imaging(DWI)of the upper abdomen;postoperative pathology Results was used as the diagnostic gold standard.The diagnostic efficacy of CT,MRI plain + enhanced and DWI for HCC MVI was compared.The concordance among CT,MRI plain + enhanced and DWI examinations with postoperative pathological diagnostic findings in the diagnosis of HCC MVI.Imaging manifestations and apparent diffusion coefficient(ADC)values in patients with and without HCC MVI were compared.Results Diagnostic effectiveness of DWI examination for HCC MVI(sensitivity,specificity,accuracy,positive predictive value,negative predictive value)were all significantly higher than those of CT and MRI plain + enhanced(P<0.05);none of the differences were statistically significant(P>0.05)in the comparison of diagnostic effectiveness of CT,MRI,and DWI for the diagnosis of MVI in patients with primary HCC.In HCC MVI diagnostic effectiveness,moderate concordance was found among CT,MRI imaging phenotypes and postoperative pathology Results;high concordance was found between DWI and postoperative pathology Results.In HCC MVI patients,the proportion of non-marginal arterial reinforcement,enhanced envelope,halo reinforcement,nodal in nodal and portal branch cancer thrombi was significantly higher than that in patients without HCC MVI(P<0.05).At different b-values(400,800,1 000,1 500 s/mm2),ADC values were all significantly higher in patients with HCC MVI than in patients without HCC MVI(P<0.05).Conclusions CT,MRI plain + enhanced and DWI have good diagnostic effectiveness for HCC MVI,while MRI diagnostic Results are in better concordance with pathologic diagnosis.In particular,ADC values in DWI maps can more accurately determine whether MVI occurs in patients with HCC,which helps to guide clinicians to establish“individualized”and precise diagnosis and treatment strategies.
胶质瘤是颅内最常见的原发性恶性肿瘤,其分级对患者治疗方式的选择和预后至关重要。尽管目前组织病理学仍是其最为可靠的分级手段,但需通过有创性手术以获取组织样本,存在一定的风险。相较之下,磁共振成像(MRI)作为一种非侵入性影像诊断工具,在胶质瘤分级中发挥着不可或缺的作用。然而,传统MRI评估受限于医师个体主观性强和可重复性差的问题,一定程度上影响了准确的分级结果。近年来,影像组学技术的崭露头角为解决上述难题开辟了新视角,通过高通量提取影像数据特征捕捉并量化肿瘤的影像学表现,避免因主观因素而导致的不确定性,协助医师更准确地评估肿瘤的恶性程度。本文对近五年来MRI影像组学在胶质瘤术前分级预测方面的相关研究进行了简要综述,旨在为相关领域研究者提供有益的参考和借鉴,以推动MRI影像组学在临床实践中的应用。
Glioma is the most common primary malignant brain tumor,and its grading is crucial for treatment decisions and prognosis.Currently,histopathology remains the gold standard for grading,but it requires invasive procedures and carries inherent risks.In contrast,magnetic resonance imaging(MRI),a non-invasive diagnostic tool,plays an indispensable role in glioma grading.However,traditional MRI assessment is hampered by interobserver subjectivity and limited repeatability,which compromise grading accuracy.In recent years,radiomics,a burgeoning field,has offered a promising solution to address these challenges.By extracting high-dimensional imaging data features,radiomics enables the quantification of tumor radiological characteristics and elimination of subjectivity-related discrepancies.This technology assists clinicians in more precisely assessing the malignancy of gliomas.This article summarizes relevant studies in the past five years on the application of MRI radiomics in preoperative glioma grading,aiming to provide valuable insights and guidance to researchers in the field and promote the clinician implementation of MRI radiomics.
目的 研究磁共振成像(MRI)、经直肠超声(TRUS)结合血清前列腺特异性抗原(PSA)诊断前列腺癌诊断的应用价值。方法 收集2020年1月—2023年1月前在武穴市第一人民医院检查的疑似前列腺癌患者140例,均给予MRI、TRUS检查,并进行血清PSA水平检测,以患者手术病理结果为金标准,观察单一MRI、TRUS、血清PSA及联合诊断时漏诊、误诊情况,进行一致性分析,计算各项单一诊断及联合诊断的诊断灵敏度、特异度、准确率。结果 经手术病理结果证实为前列腺癌81例,非前列腺癌59例,前列腺癌患者血清PSA水平高于非前列腺癌患者(P<0.05);单一MRI、TRUS或PSA诊断前列腺癌与手术病理结果的一致性一般(Kappa=0.641、0.624、0.536,均P<0.001),联合诊断与手术病理结果的一致性较好(Kappa=0.906,P<0.001);联合诊断的灵敏度、特异度、准确率为高于单一MRI、TRUS、血清PSA及各诊断方式两两联合(P<α,α=0.007)。结论 前列腺癌诊断中单一MRI、TRUS、血清PSA诊断均存在漏诊、误诊风险,联合诊断可弥补单一诊断的不足,提高前列腺癌患者的诊断准确率。
Objective To study the application value of magnetic resonance imaging(MRI),transrectal ultrasound(TRUS)combined with serum prostate-specific antigen(PSA)in the diagnosis of prostate cancer.Methods A total of 140 patients with suspected prostate cancer who were examined at Wuxue First People's Hospital from January 2020 to January 2023 were enrolled.MRI and TRUS examinations were performed,and serum PSA levels were tested.The surgical pathology results of the patients were used as the gold standard.The missed diagnosis and misdiagnosis in single MRI,TRUS,serum PSA and combined diagnosis were observed,consistency was analyzed,and the diagnostic sensitivity,specificity,and accuracy of each single diagnosis and combined diagnosis were calculated.Results There were 81 cases of prostate cancer and 59 cases of non-prostate cancer confirmed by surgical pathology results.The serum PSA level of prostate cancer patients was higher than that of non-prostate cancer patients(P<0.05).The difference between the diagnosis of prostate cancer by single MRI,TRUS or PSA and the results of surgical pathology was with general consistency(Kappa=0.641,0.624,0.536,all P<0.001),and the consistency of combined diagnosis and surgical pathology results was good(Kappa=0.906,P<0.001).The sensitivity,specificity,and accuracy of combined diagnosis were higher than single MRI,TRUS,serum PSA and the combination of two diagnostic methods(P<α,α=0.007).Conclusions In the diagnosis of prostate cancer,single MRI,TRUS and serum PSA diagnosis all have risks of missed diagnosis and misdiagnosis.Combined diagnosis can make up for the shortcomings of single diagnosis and improve the diagnostic accuracy of prostate cancer.
目的 探讨动态对比增强磁共振成像(DCE-MRI)多参数定量特征对乳腺癌腋窝淋巴结转移(ALNM)风险的预测价值。方法 回顾性收集2020年3月—2022年11月在佛山市高明区人民医院经手术病理确诊的155例乳腺癌患者临床资料,根据患者是否发生ALNM分为ALNM 组(n=39)和无ALNM 组(n=116)。采用单因素分析和多因素Logistic回归分析乳腺癌发生ALNM的影响因素。结果 ALNM组和无ALNM 组患者的肿块质地、肿块直径、肿块部位、肿块形状、肿块内部强化特征等指标比较差异无统计学意义(t/χ2=2.249、0.977、1.369、0.524、2.158,P>0.05)。两组患者肿块表观扩散系数(ADC)值、腋窝淋巴结(ALN)短径、肿块边缘、动态增强时间-信号强度曲线(TIC)曲线等指标比较,差异有统计学意义(t/χ2=6.573、9.873、29.441、2.031,P<0.05)。二元Logistic回归模型结果显示,肿块ADC值、ALN 短径(≥5 mm)、TIC曲线(流出型)为乳腺癌ALNM发生的危险因素(OR=0.251、0.106、0.002,P<0.05)。结论 DCE-MRI多参数定量特征中,乳腺癌患者的肿块ADC值低、ALN 短径(≥5 mm)、TIC曲线(流出型)为乳腺癌ALNM发生的危险因素。
Objective To investigate the predictive value of multi-parameter quantitative features of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in the risk of axillary lymph node metastasis(ALNM)in breast cancer.Methods Clinical data of 155 patients with breast cancer diagnosed by surgery and pathology in Foshan Gaoming District People's hospital from March 2020 to November 2022 were retrospectively collected.According to whether the patients had ALNM,they were divided into ALNM group(n=39)and non-ALNM group(n=116).Univariate analysis and multiple Logistic regression models were used to explore the influencing factors of ALNM in breast cancer.Results There was no significant difference in mass texture,mass diameter,mass location,mass shape and internal enhancement between the ALNM group and the non-ALNM group(t/χ2=2.249,0.977,1.369,0.524,2.158,P>0.05).There were significant differences in ADC value,ALN short diameter,tumor margin and TIC curve between the two groups(t/χ2=6.573,9.873,29.441,2.031,P<0.05).Binary Logistic regression model showed that ADC value,ALN short diameter(≥5 mm)and tumor margin(blur)were risk factors for the occurrence of breast cancer ALNM(OR=0.251,0.106,0.002,P<0.05).Conclusions Among the multi-parameter quantitative features of DCE-MRI,the ADC value of breast cancer,the short diameter of ALN(≥5 mm),and the edge of the tumor(blur)are the risk factors for the occurrence of ALNM in breast cancer.
乳腺癌是女性最常见的原发恶性肿瘤之一。目前,通过采用综合治疗手段,包括手术、新辅助治疗、辅助放化疗等多种手段,乳腺癌已成为疗效最佳的实体肿瘤之一。其中,新辅助治疗(NAT)包括新辅助化疗、靶向治疗和内分泌治疗,目的是使肿瘤降期、保乳、保腋窝,并可观察药物敏感性,是当前乳腺癌综合治疗中非常重要的组成部分,其治疗疗效对患者手术方式的选择和预后至关重要。尽管病理学评价在评估NAT疗效方面被公认为金标准,但其局限性在于只能通过有创手段在治疗后进行,无法在治疗前对患者做出准确预测。磁共振成像(MRI)作为一项广泛使用的乳腺成像技术,在评估NAT疗效中扮演着关键角色。近年来,人工智能技术,尤其是影像组学(Radiomics)和深度学习(Deep Learning),在医学影像分析领域取得了显著进展。这些技术能够从医学图像中提取大量肉眼难以识别的特征,揭示病变内部的微观结构和生物学行为,全面反映肿瘤的异质性,这不仅有助于临床医生更准确地区分良、恶性肿瘤,还能对恶性肿瘤的预后进行更为精确的评估。本文系统综述了近年来基于MRI的人工智能技术在乳腺癌新辅助治疗疗效评估中的应用研究,旨在促进人工智能在NAT临床实践中的应用和发展,为乳腺癌NAT治疗策略的优化和个性化医疗的实现提供科学依据。
Breast cancer is one of the most common primary malignant tumors in women.Currently,breast cancer has become one of the most effective solid tumors by using comprehensive treatment methods,including surgery,neoadjuvant therapy,adjuvant radiotherapy and chemotherapy.Among them,neoadjuvant therapy(NAT),including neoadjuvant chemotherapy,targeted therapy and endocrine therapy,is a very important part of the current comprehensive treatment of breast cancer.It aims to reduce the tumor stage,preserve the breast,preserve the armpit,and observe the drug sensitivity.Its therapeutic effect is crucial to the choice of surgical methods and prognosis of patients.Although pathological evaluation is recognized as the gold standard in evaluating the response to NAT,its limitation is that it can only be performed after treatment by invasive means,and cannot accurately predict response before treatment.As a widely used breast imaging technology,magnetic resonance imaging(MRI)plays a key role in evaluating the response to NAT.However,traditional MRI evaluation methods are limited by the individual differences of interobserver and the low repeatability of evaluation results,which affects the accuracy of efficacy evaluation to a certain extent.In recent years,artificial intelligence technology,especially radiomics and deep learning,has made significant progress in the field of medical image analysis.These techniques can extract a large number of features that are difficult to be recognized by the naked eye from medical images,reveal the internal microstructure and biological behavior of the lesion,and fully reflect the heterogeneity of the tumor.This not only helps clinicians to distinguish benign and malignant tumors more accurately,but also makes a more accurate assessment of the prognosis of malignant tumors.This article reviews the application and progress of MRI-based artificial intelligence technology in evaluating the response to neoadjuvant therapy for breast cancer in the past five years,aiming to promote the application and development of artificial intelligence in NAT clinical practice,and provide a scientific basis for the optimization of NAT treatment strategy and the realization of personalized medicine for breast cancer.
目的 探讨MRI液体衰减反转恢复序列(FLAIR)高信号血管征(HVS)对大脑中动脉(MCA)供血区域的诊断价值。方法 选择2021年1月—2022年10月期间我院收治的66例MCA急性脑梗死患者,观察HVS检出情况、HVS检出率与血管狭窄之间的关系及HVS对MCA急性脑梗死患者病情程度及预后的诊断价值。结果 66例MCA急性脑梗死患者中,MRI FLAIR序列检查示有HVS(HVS阳性)40例,无HVS(HVS阴性)26例。轻度狭窄、重度狭窄和闭塞分别为26、24、16例,HVS阳性者的狭窄程度较HVS阴性者严重(P<0.05)。HVS对MCA急性脑梗死患者病情程度的诊断价值:HVS阳性者梗死面积为(86.02±10.26)cm2,大于HVS阴性者的(72.36±9.01)cm2。HVS阳性者在入院时的美国国立卫生研究院卒中量表(NIHSS)评分为(12.87±2.36)分,高于HVS阴性者的(10.28±2.07)分(P<0.05)。结论 MRI FLAIR序列示HVS对判断MCA供血区域梗死有一定价值,有助于MCA供血区急性脑梗死的诊断,对临床预后的评估也具有一定意义。
目的 探讨磁共振成像(MRI)技术在膝关节隐匿性骨折临床诊断中的应用价值。方法 从我院2020年7月—2022年7月期间接诊的膝关节隐匿性骨折病例中随机抽取78例作为研究对象,所有病例均开展MRI、CT检查,分析并比较MRI和CT检查对该病变的检出率。结果 在78例膝关节隐匿性骨折病例中,MRI检查确诊76例,检出率97.44%,CT检查确诊61例,检出率78.21%,MRI检查对该疾病的检出率相比CT检查更高,差异有统计学意义(P<0.05)。MRI对半月板损伤、关节腔少量积液、韧带损伤、骨折塌陷及水平错位检出率相比CT更高,差异有统计学意义(P<0.05),两种检查方式在严重骨折检出率差异较小,差异无统计学意义(P>0.05)。结论 在膝关节隐匿性骨折的临床诊断中应用MRI对疾病检出率较高,且操作简便,在临床中具有较高的推广价值。
Objective To investigate the application value of magnetic resonance imaging(MRI)in the clinical diagnosis of occult knee fractures. Methods A total of 78 cases of occult knee fracture received by our hospital from July 2020 to July 2022 were randomly selected as research subjects,and MRI and CT examinations were performed on all cases,and the detection rates of MRI and CT examination were analyzed and compared. Results Among the 78 cases of occult knee fracture,76 cases were confirmed by MRI examination,with a detection rate of 97. 44%,and 61 cases by CT examination,with a detection rate of 78. 21%,and the detection rate of MRI examination for this disease was higher than that of CT examination,and the difference was statistically significant(P<0. 05). MRI had higher detection rate of meniscal injury,small effusion in the joint cavity,ligament injury,fracture collapse and horizontal misalignment than CT(P<0. 05),and the difference in the detection rate of severe fractures was small and not statistically significant(P>0. 05). Conclusions The application of MRI in the clinical diagnosis of occult knee fracture has a high disease detection rate and simple operation,and has high promotion value in clinical practice.
