目的 对我院静脉用药调配中心2017年1月—2019年12月期间的不合理医嘱情况进行汇总及分析,以提高合理用药水平。方法 收集2017年1月—2019年12月广州市第一人民医院静脉用药调配中心的不合理医嘱资料进行回顾性分析,归纳不合理医嘱类型、汇总分析不合理医嘱表现以及改正不合理情况。结果 主要不合理医嘱类型包括溶媒选择错误、溶媒量过多或过少、药物剂量不合理、药物配伍禁忌、其他(用药频次错误、药物选择错误、重复用药等)。结论 根据不合理医嘱情况的汇总分析,及时与临床科室沟通改正,保证合理用药。
Objective To summarize and analyze their rational prescriptions in pharmacy intravenous admixture services(PIVAS)of our hospital from January 2017 to December 2019,so as to improve the level of rational drug use. Methods The data of irrational prescriptions from January 2017 to December 2019 in the pharmacy intravenous admixture services of Guangzhou First People's Hospital were collected for retrospective analysis, and the types of irrational prescriptions were summarized, the performance of irrational prescriptions was analyzed and the irrational situation was corrected. Results The main types of irrational prescriptions include incorrect selection of solvent, improper consumption of solvent, improper drug dosage, incompatibility and others(improper frequency of administration, incorrect selection of drug, repeated administration, etc.). Conclusion According to the summary and analysis of irrational prescriptions, we timely communicate with clinical departments and ensure rational drug use.
目的 分析河源市学龄前儿童发生小细胞低色素性贫血的病因。方法 对我院进行健康体检小细胞低色素性贫血儿童287例进行血常规、血清铁蛋白及地中海贫血基因检测。结果 在所研究的287 例小细胞低色素性贫血儿童病例中,分别检出 α地中海贫血 127例,β地中海贫血 48例,α/β复合地中海贫血2例;铁缺乏 83例 (合并地中海贫血20例, 缺铁性贫血45例),不明原因贫血47例。地中海贫血检出率为61.67%,铁缺乏检出率为21.95% 。结论 地中海贫血是河源市学龄前儿童发生小细胞低色素性贫血最主要的原因,其次是铁缺乏,各年龄段儿童以轻度贫血为主,6月~1岁,1~3岁为铁缺乏高发年龄。α地中海贫血基因型以--SEA/αα最常见,β地中海贫血以βIVS-II-654/βN最常见,小细胞低色素症在静止型最常见。
Objective To analyze the causes of microcytic hypochromic anemia in preschool children in Heyuan City. Methods A total of 287 cases with microcytic hypochromic anemia were selected in our hospital. The indexes of hematology, serum ferritin were detected and genetic testing for thalassemia was performed. Results Through genetic analysis, 127 of 287 cases of microcytic hypochromic anemia were confirmed with α-thalassaemia,48 cases with β-thalassaemia,2 cases with compound α/β-thalassaemia and 83 cases with iron deficiency (20 thalassemia cases and 45 iron deficiency anemia cases). Thalassaemia detection rate was 61.67%, iron deficiency detection rate was 21.95%. Conclusion Thalassaemia was the main reason of microcytic hypochromic anemia in preschool children in Heyuan City, followed by iron deficiency. The mild anemia was the main type among all age groups, children aged 0.5-3 had higher iron deficiency rate. The main type of α-thalassaemia was --SEA/αα, the main type of β-thalassaemia was βIVS-II-654/βN and the main type of microcytic hypochromic was static type.
目的 探讨“分-全带教”在提高门诊实习生处方审核能力的应用效果。方法 对2016届实习生进行“分-全带教”,在实习初期、后期组织实习生对100张门诊处方进行处方审核,登记审核结果。结果 实施“分-全带教”后,处方成功审核例数实习初期的163例(27.17%)提高到后期的487例(81.17%)。结论 应用“分-全带教”模式,有利于门诊实习生掌握处方审核技巧,在提高实习生处方审核能力中有良好应用前景。
Objective To make a discussion of the application result of point-all teaching model in improvement of prescription review ability of interns at outpatient service process. Methods To implement point-all teaching model on interns enrolled in 2016. To ask them to review 100 outpatient prescription at the beginning and ending of the internship respectively and record the results of their review. Results After the implementation of point-all teaching model, the correct review of the prescription has been improved from 163 (27.17%) at the beginning to 487 (81.17%) at the ending. Conclusion The point-all teaching model is conducive for interns at outpatient department to master the technique for review, so it is promising in the improving the prescription review ability of interns.
目的 调查我院处方错误的数量和分类,保障门诊患者用药的安全性、有效性和经济性。方法 采用回顾性分析方法,对我院2016年1月—6月的门诊电子处方统计,对不合理处方及时电话沟通和改正,并将错误处方登记入册分析。结果 其中登记入册的错误处方占总处方数的0.24%,主要包括药物用法用量不合理、给药途径不当、药物相互作用不合理、禁忌证用药、重复用药、电脑输入剂量或单位错误等。结论 我院门诊电子处方仍存在一定的不合理现象,临床药师通过处方审核进行干预,并与医院管理和医师有机结合,努力开展药学监护,提高我院处方质量,促进合理用药,共同保证患者用药正确,有效,安全,经济,合理,降低医疗纠纷的发生。
目的 通过高通量测序法对多重耐药大肠埃希菌HX43进行耐药分子机制的研究。方法 用Illumina Miseq平台对HX43进行高通量测序,用Edena、RAST、ResFinder、MLST和BLAST等生物信息学工具或数据库进行数据分析,获得耐药基因相关序列数据。结果 HX43对多种临床常用抗生素均不敏感,仅对碳氢霉烯类药物敏感。对高通量测序数据的分析研究发现,该菌存在多种耐药基因,包括β-内酰胺类耐药基因3个(blaCMY-42、blaCTX-M-14和blaOXA-30),氨基糖苷类耐药基因5个(aac(3)-IIa、aadA5、 strA、 strB和aac(6′)-Ib-cr),喹诺酮类耐药基因1个(aac(6′)-Ib-cr),磺胺及甲氧苄啶类耐药基因3个(sul1、sul2和dfrA17),四环素耐药基因1个(tet(B)),氯霉素耐药基因2个(catB3和cmlA1),大环内酯类耐药基因2个(erm(B)和mph(A))。对包含blaCMY-42的contigs进行分析,发现该基因与ISEcp1插入序列、blc和sugE等基因相关联。质粒分型发现HX43携带5种不相容群的质粒。多位点序列分型(MLST)分析发现HX43属于ST3835,为国内外较少见的序列型。结论 高通量测序技术可准确获得临床菌株抗生素耐药的相关基因信息,为临床抗菌治疗提供重要的实验室数据支持。
Objective To investigate the molecular resistance mechanism of Escherichia coli HX43 by high-throughput sequencing. Methods HX43 was sequenced by the Illumina Miseq platform, and sequencing data were analyzed by the Edena, RAST, ResFinder, MLST and BLAST softwares and databases. Results HX43 was resistant to most common clinical antibiotics except carbapenems. Analysis of data revealed resistance genes to β-lactams (blaCMY-42, blaCTX-M-14 and blaOXA-30), aminoglycosides (aac(3)-IIa, aadA5, strA, strB and aac(6′)-Ib-cr), quinolones (aac(6′)-Ib-cr), trimethoprim/sulfonamides(sul1, sul2 and dfrA17), tetracyclines (tet(B)), chloramphenicol (catB3 and cmlA1), macrolides(erm(B) and mph(A)). Sequence analysis of the contig containing blaCMY-42 identified correlations of the gene with ISEcp1 insertion sequences, blc and sugE genes. Plasmid typing identified 5 plasmid incompatibility groups in HX43. MLST analysis found that HX43 belonged to ST3835, a relatively rare sequence type in the world. Conclusion Information of resistance genes can be obtained by high-throughput sequencing, which provides important experimental data for clinical antimicrobial treatment.
目的 探讨手动杠杆式胸外按压心肺复苏(CPR)装置对院内心肺复苏按压质量和复苏疲劳的影响。方法 将12名医学生志愿者分为手动杠杆式胸外按压CPR装置组(LDCPR)(n=6)和徒手标准CPR组(STCPR)(n=6)模拟院内心肺复苏场景进行CPR试验。LDCPR组采用手动杠杆式胸外按压CPR装置对模拟人进行连续6min连续胸外按压,STCPR组徒手对模拟人进行6min连续胸外按压。监测复苏期间按压的深度、频率以及按压者的心率和疲劳程度等变化。结果 随着连续胸外按压的进行,按压者的心率和疲劳程度都在不断增加,有效按压深度超过5 cm次数逐渐减少,但是在连续胸外按压的第4~6min,LDCPR组按压者的心率较STCPR组慢(P<0.05),疲劳程度较轻(P<0.05),有效按压的比例更高(P<0.05)。按压频率两组比较无明显差异。结论 在本模拟人试验中,手动杠杆式胸外按压CPR装置可减轻按压者疲劳,有助于提高院内CPR时长时间胸外按压的质量。
Objective We aimed to investigate simulated in-hospital cardiopulmonary resuscitation (CPR) quality and rescuer fatigue of external chest compression-only CPR by a manual leverage device(LDCPR). Methods All 12 volunteers (medical staffs: male 6 and female 6) were randomized to standard CPR by hand(STCPR)group (n=6)or LDCPR group(n=6). At STCPR group, continuous external chest compression-only was performed for 6 minutes to simulate in hospital cardiopulmonary resuscitation on a manikin; at LDCPR group, compression-only was performed for 6 minutes by a leverage device on a manikin. We measured blood pressure of the volunteers before and after performing each CPR technique, volunteers self-report fatigue scale and continuously monitored heart rate (HR) of the volunteers during each CPR technique by smart ring. CPR quality measures included chest compression rate and depth. Results During continuous external chest compression-only CPR, compressor's heart rate and fatigue scale were rising continuously, and percent of compress depth >5 cm were declining, too.Compressor's heart rate and fatigue scale were higher in the STCPR group than in the LDCPR group (P<0.05) during the fourth to sixth minutes CPR (P<0.05). Percent of compress depth > 5 cm was higher in the LDCPR group than in the STCPR group (P<0.05) during the fourth to sixth minutes of CPR (P<0.05).Compress rate had not differed between the 2 groups during CPR. Conclusion In the manikin study, manual leverage external chest compress device may reduce fatigue scale and improve long term compression quality during in-hospital cardiopulmonary resuscitation.
目的 对儿童化学发光免疫检验中标本量过少时的稀释方法分析。方法 选取我院2013年5月—2015年5月儿童常规筛查项目50例血清三碘甲状腺原氨酸(TT3)检测为例,其稀释介质分别采用TT3定制液S0、医用蒸馏水以及0.9%氯化钠,对其实施手工2倍(1∶1)和4倍(1∶3)稀释后,对比分析稀释后结合和原始数据差异。结果 结果和原始数据对比,2倍、4倍稀释S0组以及2倍医用蒸馏水稀释结果差异不大,对比没有统计学意义(P>0.05),其与各组均差异显著(P<0.05)。结论 在儿童血清三碘甲状腺原氨酸检测中对其实施2倍、4倍稀释S0以及2倍医用蒸馏水稀释能够满足医学检验需求,值得推广应用。
目的 采用二维斑点追踪技术(2D-STI)检测冠状动脉左前降支供血区心肌纵向应变,探讨其评估缺血心肌局部收缩功能的价值。方法 使用2D-STI检测不同狭窄程度(<50%、50%~75%、>75%)及未见明确狭窄的左前降支供血区心肌纵向应变参数:心内膜下、心外膜下及区域心肌收缩期纵向应变峰值-LS-endo、LS-epi、LS-Territorial,计算收缩期心肌跨壁纵向应变(LS-mural)。观察PCI术后供血区心肌纵向应变的变化。结果 各组内LS-endo均高于对应的LS-epi(均P<0.01)。随着左前降支狭窄程度增加,LS-endo、LS-epi、LS-Territorial、LS-mural逐渐减低(均P<0.01)。术后LS-endo、LS-epi、LS-Territorial逐渐增高,LS-mural先增高后缓慢减低再增高。结论 2D-STI可定量检测不同层次的心肌纵向应变,反映心肌局部收缩功能,为早期识别缺血心肌、评价PCI术后疗效提供参考依据。
Objective To detect myocardial longitudinal strain in the area supplied by left anterior descending coronary artery (LAD) via two dimensional speckle tracking image (2D-STI), and to investigate its estimated value of myocardial systolic function of ischemic myocardium. Methods Detection of myocardial longitudinal strain parameters in the area supplied by the LAD with different stenosis degree(<50%、50%~75%、>75%) and no definite stenosis via 2D-STI:subendocardial longitudianl strain (LS-endo), subepicardial longitudianl strain (LS-epi) and territorial longitudianl strain (LS-Territorial). Calculate transmural longitudinal strain (LS-mural) in the systole.Then observe the changes of myocardial longitudinal strain in the blood supply area after percutaneous coronary interventional therapy (PCI). Results LS-endo, LS-epi, LS-Territorial and LS-mural gradually decrease with the increase of LAD stenosis (all P<0.01). After operation, LS-endo, LS-epi and LS-Territorial increase progressively (all P<0.01). LS-mural increases first and then decreases and finally increases slowly. Conclusion Layer-specific quantification of myocardial longitudinal strain by 2D-STI may reveal the regional myocardial systolic function. It is useful to identify the ischemic myocardium in the early stage and evaluate the treatment effect of PCI.
目的 分析影响广州青少年自杀行为的因素。方法 选取100例自杀行为青少年为观察组,选100例健康青少年为对照组,对比两组青少年家庭结构、家庭教育方式、自身人格特征等指标。多因素Logistic回归分析其自杀行为危险因素。结果 观察组青少年抑郁量表、焦虑量表、攻击性人格及冲动性人格量表分值均高于对照组高;单亲家庭或父母常年在外、家庭教育方式不当、学校无健康心理教育课、抑郁或焦虑、冲动或攻击性人格、负面事件、消极应对是青少年自杀行为独立危险因素,P均<0.05。结论 家长及学校应正确引导青少年处事方式,积极面对生活,打消自杀意念。
Objective To analyze the factors influencing the suicide behavior of adolescents in Guangzhou. Methods The 100 cases of adolescent suicide behavior were selected as the observation group, 100 healthy adolescents were selected as control group, compared with two groups of adolescent family structure, family education, personality traits and other indicators. Multivariate logistic regression analysis of risk factors was taken for suicidal behavior. Results In the observation group adolescent depression scale, anxiety scale, attack impulse personality and personality amount scores were higher than those in the control group, single parent family or parents staying outside, improper family education, school health psychological education, depression or anxiety, impulsivity, or aggressive personality, negative events, negative coping were independent risk factors for adolescent suicide, P<0.05. Conclusion Parents and schools should have guide to adolescents, toface life positively, and give up thought of suicide.
目的 探讨加味骨洗方外治法用于治疗类风湿关节炎的疗效以及类风湿因子等临床指标的变化。方法 选取100例类风湿关节炎患者,随机分为治疗组和对照组各50例。治疗组给予加味骨洗方及TDP特定电磁疗灯导入的外治,对照组给予TDP特定电磁疗灯磁疗。结果 在缓解症状和恢复功能方面治疗组总有效率为84%,对照组总有效率为58%,治疗组各临床指标与对照组相比均有差异(P<0.05)。结论 加味骨洗方治疗类风湿关节炎疗效显著,能有效降低类风湿因子等炎性指标,值得临床推广。
Objective To discuss curative effects of rheumatoid arthritis treatment and clinical indicators transformation including rheumatoid factor by additional Guxi prescription external therapy. Methods We selected 100 patients who were suffered from rheumatoid arthritis, divided randomly into two groups. 50 patients for the treatment group and another 50 for the control group. The treatment group was treated by the additional Guxi prescription external therapy. While the control group was treated by the infrared magnetic therapy. Results The effective proportion of the treatment group was 84%, while the control group total was 58% in the aspect of relieve the symptoms and recovery of functions. Besides, all clinical indicators of the treatment group were inferior to the control group(P<0.05). Conclusion There are effects for treating rheumatoid arthritis by utilizing additional Guxi prescription external therapy. This therapy which can effectively reduce the inflammatory indexes of the rheumatoid factor is worth promoting clinically.
