目的 探究经鼻间歇正压通气(NIPPV)和经鼻持续正压通气(NCPAP)作为早产儿呼吸窘迫综合征气管插管机械通气拔管后无创呼吸支持过渡的临床效果的差异。方法 纳入2021年1月—2023年6月在广东省吴川市妇幼保健计划生育服务中心治疗的新生儿呼吸窘迫综合征早产儿66例,用随机数字表法将患儿分为NIPPV组(33例)和NCPAP组(33例)。NIPPV组患儿予经鼻间歇正压通气作为过渡期无创呼吸支持,NCPAP组患儿则予以经鼻持续正压通气作为过渡期无创呼吸支持。对比两组患儿治疗前后血气分析结果、脱机失败率、无创呼吸支持时间、无创后吸氧时间、总给氧时间等指标。结果 接受无创呼吸支持12 h后,NIPPV组患儿的PaO2升至(76.46±1.10)mmHg,高于NCPAP组患儿的(75.51±2.15)mmHg(t=2.249,P=0.028)。此外,NIPPV组患儿的SaO2升至(96.36±0.52)%,也高于NCPAP组患儿的(96.07±0.59)%(t=2.138,P=0.034)。而NIPPV组患儿的PaCO2则降至(41.39±0.74)mmHg,较NCPAP组患儿的(41.87±0.95 )mmHg低(t=-2.230,P=0.025)。NIPPV组无创呼吸支持时间为(3.09±0.52)d,短于NCPAP组的(3.45±0.62)d,且该差异有统计学意义(t=2.584,P=0.012)。同样的,NIPPV组总给氧时间(9.52±0.76)天较NCPAP组的(10.00±0.79)天短,且该差异有统计学意义(t=-2.548,P=0.013)。而两组在脱机失败率、无创呼吸支持后吸氧时间、不良反应发生率等方面比较差异无统计学意义(均P>0.05)。结论 与NCPAP模式相比,早产儿拔管后应用NIPPV模式进行无创呼吸支持取得的临床效果更优。
Objective To explore the difference of clinical effect of nasal intermittent positive pressure ventilation(NIPPV)and nasal continuous positive pressure ventilation(NCPAP)as non-invasive respiratory support mode after extubation in preterm infants with endotracheal intubation.Methods Sixty-six cases of preterm infants with neonatal respiratory distress syndrome treated in our hospital from January 2021 to June 2023 were included.All the subjects were randomly divided into NIPPV group(n=33)and NCPAP group(n=33).The blood gas analysis results,weaning failure rate,non-invasive respiratory support time,oxygen inhalation time after noninvasive ventilation,total oxygen administration time.were compared between the two groups.Results After 12 hours of noninvasive respiratory support,PaO2 in the NIPPV group increased to(76.46±1.10)mmHg,which was significantly higher than that(75.51±2.15)mmHg in the NCPAP group(t=2.249,P=0.028).In addition,SaO2 in the NIPPV group increased to (96.36±0.52)%,which was also significantly higher than that(96.07±0.59)% in the NCPAP group(t=2.138,P=0.034).The PaCO2 in the NIPPV group decreased to (41.39±0.74)mmHg,which was lower than that(41.87±0.95)mmHg in the NCPAP group(t=-2.230,P=0.025).The duration of non-invasive respiratory support in the NIPPV group(3.09±0.52) days was shorter than that(3.45±0.62)days in the NCPAP group,and the difference was statistically significant(t=2.584,P=0.012).Similarly,the total duration of oxygen administration in the NIPPV group(9.52±0.76)days was shorter than that(10.00±0.79)days in the NCPAP group,and the difference was statistically significant(t=-2.548,P=0.013).There were no significant differences in weaning failure rate,oxygen inhalation time after noninvasive respiratory support,and incidence of adverse reactions between the two groups(P>0.05).Conclusions Compared with NCPAP mode,NIPPV mode for non-invasive respiratory support in preterm infants after extubating has better clinical effect,and it is worthy of clinical application.

目的 评价现有关于干预组仅涉及不同针具针刺或联合常规针刺治疗腰椎间盘突出症系统评价的方法学质量,以比较不同针具针刺治疗腰椎间盘突出症的治疗效果。方法 通过CNKI(中国知网)、万方数据库、VIP(维普)、PubMed、Web of Science等数据库以及其他相关电子资源,对诊断、治疗腰椎间盘突出症的多种针灸技术进行全面的研究。使用AMSTAR2软件,对所有参与的系统性评估结果进行地分析,并根据不同的检测结果,确定文献筛选标准。通过Revman5.4和Cochrance风险偏倚工具,以及Stata16.0的网格Meta分析,挑选了3 381个满足纳入排除要求的随机对照试验。结果 以Stata 16.0统计软件制作排序图可知,杵针+电针的临床有效率SUCRA=81.6%排名第一;目测类比评分显示,银质针SUCRA(用于评估疗效的指标)=95%,排名第一。结论 根据现有的研究证据,杵针+电针以及银质针优于其他9种干预措施,但确切的结果仍需要大量的随机对照试验来证明。
Objective To evaluate the methodological quality of the existing systematic evaluation of the intervention group involving only different needle sets of acupuncture or combined with conventional acupuncture in the treatment of lumbar disc herniation,in order to compare the therapeutic effect of different needle sets of acupuncture in the treatment of lumbar disc herniation.Methods Through CNKI,Wanfang database,VIP,PubMed,web of science and other databases as well as other relevant resources,a variety of acupuncture techniques for the diagnosis and treatment of lumbar disc herniation were comprehensively studied.Through the use of AMSTAR2 software,all participating systematic evaluation results were effectively analyzed,and according to different test results,which literature meets the requirements were determined.Through revman5.4,Cochrane risk bias tool,and grid meta-analysis of stata16.0,3 381 RCTs meeting the inclusion and exclusion requirements were selected.Results According to the ranking chart made by stata16.0 statistical software,the clinical effective rate of pestle needle + electroacupuncture ranked first with sucra=81.6%.According to the visual analogy score,the silver needle sucra=95%,ranking first.Conclusions According to the existing research evidence,pestle needle + electroacupuncture and silver needle are better than the other nine interventions,but the exact results still need a large number of randomized controlled trials to prove.

目的 探讨NXT629改善肝胆结石形成的相关机制。方法 对C57BL/6J小鼠分别采用常规饮食或成石饮食(LD)喂养,并在LD组小鼠注射PPAR-α拮抗剂NXT629。通过苏木精-伊红染色法染色分析肝脂肪病变,油红O染色检测肝脏脂质的积累,分光光度法检测胆汁或血清中总胆固醇、甘油三酯、磷脂、总胆汁酸、胆固醇饱和指数、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇指标;qPCR法检测小鼠肝组织中ABCG5/8、CYP7A1、CYP7B1、PPAR-α和ABCB11 mRNA的表达情况。结果 NXT629通过靶向PPAR-α降低LD组小鼠肝脏中的ABCG5、ABCG8、ABCB11 mRNA水平以及增加CYP7A1、CYP7B1 mRNA水平,进而减少LD诱导的肝胆结石形成并改善脂质代谢紊乱。结论 NXT629可能通过影响脂代谢相关基因表达改善肝胆结石。
Objective To explore the mechanism on NXT629 improves hepatolithiasis formation.Methods C57BL/6J mice were fed either a regular diet or a lithogenic diet(LD),with the LD group receiving injections of PPAR-α inhibitor NXT629.Liver steatosis was analyzed via HE staining,hepatic lipid accumulation was detected by Oil Red O staining,and total cholesterol,triglycerides,phospholipids,total bile acids,cholesterol saturation index,low density lipoprotein cholesterol,and high density lipoprotein cholesterol levels in bile or serum were measured using assay kits.RT-qPCR was employed to determine the mRNA expression of ABCG5/8,CYP7A1,CYP7B1,PPAR-α,and ABCB11 in mouse liver tissues.Results The Results showed that NXT629 target PPAR-α to down-regulate the mRNA levels of ABCG5,ABCG8,and ABCB11 in the livers of LD-fed mice,while increasing the mRNA levels of CYP7A1 and CYP7B1,thereby reducing LD-induced hepatolithiasis formation and improving lipid metabolism disorders.Conclusions NXT629 can improve cholesterol gallstones by affecting the expression of genes related to lipid metabolism.

目的 探讨重楼皂苷Ⅰ(PPI)对慢性髓系白血病细胞(K562)细胞的抑制作用及可能的作用机制。方法 采用CCK-8法筛选药物最适浓度,将培养时间为24 h的药物最适浓度作为后续实验的干预浓度。分组如下:(1)空白组;(2)PPI组;(3)抑制剂组;(4)PPI+抑制剂组。采用CCK-8法检测细胞增殖率;AO/EB染色观察细胞形态;流式细胞术检测凋亡率;ROS检测试剂盒检测活性氧(ROS)含量、还原型谷胱甘肽含量检测试剂盒检测谷胱甘肽(GSH)含量、细胞亚铁比色法测试盒检测细胞亚铁(Fe2+)含量;qRT-PCR法和蛋白免疫印迹法检测各组肿瘤蛋白53(p53)、钠氯离子依赖性氨基酸转运蛋白11(SLC7A11)、谷胱甘肽过氧化物酶4(GPX4)mRNA及蛋白表达量。结果 PPI抑制K562细胞的生长,且呈一定的剂量及时间依赖性(与同时间段的对照组0μmol/L比较,均P<0.01)。与空白组相比,PPI抑制K562细胞的增殖,提高了凋亡率,而铁死亡抑制剂(Ferrostian-1)的使用逆转了PPI对K562凋亡的促进作用(P<0.01)。与空白组相比,PPI组ROS、Fe2+含量升高,GSH含量下降,而铁死亡抑制剂的使用可下调ROS、Fe2+,上调GSH的含量(P<0.01)。PPI组较空白组p53 mRNA和蛋白表达水平升高,而SLC7A11、GPX4 mRNA和蛋白表达水平下降(P<0.05);PPI+抑制剂组细胞较重楼皂苷组p53 mRNA和蛋白表达水平下降,而SLC7A11、GPX4 mRNA和蛋白表达水平升高(P<0.05)。结论 PPI能够有效抑制K562细胞增殖,促进K562细胞铁死亡,其分子机制可能与p53信号通路的调控有关。
Objective To investigate the inhibitory effect of polyphyllin I(PPI)on chronic myeloid leukemia cells(K562)and its possible mechanism.Methods K562 cell line was cultured in suitable environment,and the optimal concentration of the drug was screened by CCK-8 method.The optimal concentration of the drug cultured for 24 hours was used as the intervention concentration of the follow-up experiment.Cells were divided into the following groups:(1)blank group,(2)saponins group,(3)inhibitor group and(4)saponins + inhibitor group.The cell proliferation rate was detected by CCK-8 method.The cell morphology was observed by AO/EB staining.The apoptosis rate was detected by flow cytometry.The contents of reactive oxygen species(ROS),glutathione(GSH)and ferrous(Fe2+)in different groups were detected,and the expression of mRNA and protein in different groups were detected by qRT- PCR and Western blot respectively.Results PPI significantly inhibited the growth of K562 cells in a dose-and time-dependent manner.Compared with the blank group,PPI significantly inhibited the proliferation of K562 cells and increased the apoptosis rate of K562 cells,while the use of ferroptosis inhibitor(Ferrostian-1)reversed the promoting effect of PPI on apoptosis of K562 cells.Compared with the blank group,the contents of reactive oxygen species(ROS)and ferrous iron(Fe2+)increased and the content of glutathione(GSH)decreased in the saponins group.The use of Ferrostian-1 could down-regulate the contents of ROS and Fe2+ and increase the content of GSH in the cells treated with the drug.Compared with the blank group,the expression of p53 mRNA and protein in the saponins group increased,while the expression of SLC7A11,GPX4 mRNA and protein decreased.The expression of p53 mRNA and protein in the saponins + inhibitor group was lower than that in the saponins group,while the expression levels of SLC7A11,GPX4 mRNA and protein increased.Conclusions PPI can effectively inhibit the proliferation of K562 cells and promote ferroptosis in K562 cells.The molecular mechanism can be related to the regulation of p53 signal pathway.

Traditional colonoscopy quality assessment methods have strong subjectivity and are time-consuming.In recent years,artificial intelligence technology has shown objectivity,timeliness,and comprehensiveness in colonoscopy quality control,with broad application prospects.This article comprehensively explores multiple application scenarios of AI in colonoscopy quality control,encompassing assessments of bowel preparation quality,recording of withdrawal times,polyp identification and classification,and prediction of early colorectal cancer invasion depth.Through specific research cases and data analysis,the effectiveness and advantages of AI technology are elucidated.AI technology is expected to play an increasingly significant role in enhancing the quality of colonoscopy and promoting early diagnosis and treatment of colorectal cancer.However,facing challenges from technology,ethics,regulations,and other aspects,continued efforts are needed in the future to continuously optimize algorithms,strengthen interdisciplinary collaboration,and promote the healthy and rapid development of AI technology in the medical field.

目的 分析贝伐珠单抗与化疗对晚期结直肠癌患者4种错配修复蛋白(MSH2、MLHI、MSH6、PMS2)水平影响。方法 选择2022年1月—2024年1月江苏省宿迁市中医院肿瘤科102例晚期结直肠癌患者,按抽签法分成两组,即化疗组及联合组,各51例。化疗组应用FOLFOX(亚叶酸钙+奥沙利铂+氟尿嘧啶)方案进行化疗,联合组采取贝伐珠单抗联合FOLFOX方案治疗。对比其肿瘤控制效果、免疫功能及MSH2、MLHI、MSH6和PMS2水平变化,并对比组间不良反应发生率。结果 联合组客观缓解率、疾病控制率高于化疗组(P<0.05);治疗后联合组患者CD4+、CD3+、CD8+、和CD4+/CD8+数值高于化疗组,且两组治疗后均低于治疗前(P<0.05);治疗后两组患者MSH2、MLHI、MSH6和PMS2缺失率对比无统计学意义(P>0.05),但其阳性、阴性表达情况对比差异有统计学意义,联合组MSH2、MLHI、MSH6和PMS2阳性率低于化疗组(P<0.05);两组不良反应发生率对比差异无统计学意义(P>0.05)。结论 对晚期结直肠癌在化疗基础上增加贝伐珠单抗抗肿瘤效果显著,改善其免疫功能,虽无法改善患者错配修复蛋白缺损,但可辅助降低其阳性率,且不影响安全性。
Objective To analyze the effects of bevacizumab and chemotherapy on the levels of four mismatch repair proteins(MSH2,MLHI,MSH6,PMS2)in patients with advanced colorectal cancer. Methods A total of 102 patients with advanced colorectal cancer from the Oncology Department of Suqian Traditional Chinese Medicine Hospital in Jiangsu Province from January 2022 to January 2024 were selected,and were divided into two groups according to the drawing method,namely the chemotherapy group and the combination group,with 51 patients in each group.The chemotherapy group received FOLFOX(calcium folinate+oxaliplatin+fluorouracil)regimen for chemotherapy,while the combination group received bevacizumab combined with FOLFOX regimen for treatment.Tumor control effect,immune function,and changes in MSH2,MLHI,MSH6,and PMS2 levels,and its incidence of adverse reactions were compared.Results The objective remission rate and disease control rate of the combined group were higher than those of the chemotherapy group(P<0.05).After treatment,the CD4+,CD3+,CD8+ and CD4+/CD8+ values in the combination group were higher than those in the chemotherapy group,and those after treatment were lower than those before treatment in both groups(P<0.05).After treatment,there were no significant differences in the deletion rates of MSH2,MLHI,MSH6,and PMS2 between the two groups of patients(P>0.05),but there was a significant difference in their positive and negative expression.The positive rates of MSH2,MLHI,MSH6,and PMS2 in the combination group were lower than those in the chemotherapy group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Adding bevacizumab on the basis of chemotherapy has a significant anti-tumor effect on advanced colorectal cancer,improving its immune function.Although it cannot improve the mismatch repair protein defect in patients,it can assist in reducing its positivity rate and has high safety.

目的 对比分析钝性分离扩皮法与常规扩皮法对乳腺癌术后患者经外周静脉置入中心静脉导管(PICC)置管应用效果。方法 选取2022年4月—2024年4月在天津肿瘤医院空港医院接受治疗的120例乳腺癌术后PICC置管患者,依据随机数字表法进行分组处理。对照组60例给予常规扩皮法,观察组60例给予钝性分离扩皮法,对比两组患者扩皮结果。结果 观察组患者满意度为96.67%,对照组患者满意度为86.67%(χ2=3.927,P=0.048);观察组患者的穿刺点血液浸湿面积分别为穿刺后即刻(0.87±0.14)cm2、1 d后(4.89±0.94)cm2以及3 d后(0.21±0.05)cm2,均低于对照组的(2.74±0.63)(9.89±2.04)(0.44±0.12)cm2,对比差异有统计学意义(t=22.444、17.243、13.704,P<0.05);观察组患者一次性送鞘成功率为98.33%,对照组患者一次性送鞘成功率为88.33%,观察组高于对照组(χ2=4.821,P=0.028);扩皮前两组患者的VAS评分无差异(P>0.05),扩皮后两组患者的VAS评分均降低,且观察组(1.75±0.54)分低于对照组(3.89±1.22)分,对比差异有统计学意义(t=12.425,P<0.001);观察组患者不良事件发生率为5.00%,对照组患者不良事件发生率为16.67%,观察组患者不良事件发生率低于对照组(χ2=4.227,P<0.05)。结论 钝性分离扩皮法能够降低穿刺点血液浸湿面积及不良事件发生率,提高一次性送鞘成功率,减轻患者疼痛感,提高患者满意度。
Objective To analyze the effect of blunt separating skin expansion and conventional skin expansion in PICC catheterization of patients after breast cancer surgery. Methods From April 2022 to April 2024,120 patients with postoperative PICC catheterization for breast cancer were selected and grouped according to the random number table method.Sixty patients in the control group received conventional skin expansion,and 60 patients in the observation group received blunt separation skin expansion,which the results of the two groups were compared.Results The patient satisfaction was 96.67% in the observation group,86.67% in the control group(χ2=3.927,P=0.048,P<0.05).In the observation group,the blood immersion area after catheterization,after 1 d and 3 d were(0.87±0.14),(4.89±0.94),(0.21±0.05)cm2,lower than those of the control group [(2.74±0.63)(9.89±2.04)(0.44±0.12)cm2],the comparative difference was statistically significant(t=22.444,17.243,13.704,P<0.05).The success rate of disposable sheath delivery in the observation group was 98.33%,which was higher than 88.33% in the control group(χ2=4.821,P=0.028<0.05).There was no difference in VAS scores between the two groups before the intervention(P>0.05),VAS scores decreased in both groups after the intervention,the score of the observation group(1.75±0.54)was lower than that of the control group(3.89±1.22),the difference was statistically significant(t=12.425,P<0.001).The incidence of adverse events in the observation group was 5.00%,and in the control group was 16.67%,which difference was significant(χ2=4.227,P<0.05).Conclusions Blunt separating skin expansion can reduce the area of blood immersion and the incidence of adverse events,improve the success rate of disposable sheath delivery,reduce patient pain,improve patient satisfaction,and have significant clinical application value.

目的 探讨免疫治疗联合化学治疗(化疗)对晚期非小细胞肺癌(NSCLC)患者淋巴免疫及生活质量的影响,为临床进一步治疗提供参考。方法 选择2021年6月—2023年6月天津市滨海新区大港医院收治的晚期NSCLC患者120例进行研究,按抽签法分为干预组及对照组,每组60例,对照组采取单纯化疗方案,干预组采取免疫联合化疗方案,对比两组临床疗效、药物不良反应,治疗前后免疫功能(CD3+、CD4+、CD8+)、糖类抗原199(CA199)、糖类抗原 125(CA125)、血清癌胚抗原(CEA)水平及健康状态调查表(QOL)评分。结果 干预组患者治疗总有效率高于对照组(68.33%>41.67%,P<0.05);治疗后干预组患者CD3+、CD4+比例高于治疗前及对照组治疗后,CD8+比例低于治疗前及对照组治疗后(P<0.05);治疗后干预组血清CA199、CA125、CEA水平均低于治疗前及对照组治疗后(P<0.05);干预组药物不良反应发生率为16.67%,对照组为36.67%,干预组低于对照组(P<0.05);治疗后干预组QOL各维度评分高于对照组及治疗前(P<0.05)。结论 与单纯化疗相比,免疫联合化疗治疗晚期NSCLC患者,能有效降低肿瘤标志物水平,改善患者免疫指标,减轻药物不良反应,提高患者疗效及生活质量。
Objective To explore the effect of immunotherapy combined with chemotherapy on lymphatic immunity and quality of life of patients with advanced non-small cell lung cancer(NSCLC),and to provide reference for further clinical treatment. Methods A total of 120 patients with NSCLC from June 2021 to June 2023 were selected and divided into observation group and control group evenly according to the method of drawing lots,control group was treated with chemotherapy,the observation group was treated with immunotherapy combined with chemotherapy,and the clinical efficacy and adverse drug reactions were compared between the two groups.Before and after treatment,immune function(CD3+,CD4+,CD8+),carbohydrate antigen 199(CA199),carbohydrate antigen 125(CA125),serum carcinoembryonic antigen(CEA)levels and health status questionnaire(QOL-RRB- scores)were measured.Results The total effective rate in the observation group was significantly higher than that in the control group(68.33%>41.67%,P<0.05).After treatment,the ratios of CD3+ and CD4+ in the observation group were significantly higher than those before treatment and control group after treatment,and the ratio of CD8+ was significantly lower than that before and after treatment in the control group(P<0.05).After treatment,the serum levels of CA199,CA125 and CEA in the observation group were lower than those before and after treatment in the control group(P<0.05).The incidence of adverse drug reactions was 16.67% in the observation group and 36.67% in the control group,which was significantly lower in the observation group than in the control group(P<0.05).After treatment,the QOL scores in the observation group were significantly higher than those in the control group and before treatment(P<0.05).Conclusions Compared with chemotherapy alone,immunotherapy combined with chemotherapy can effectively reduce the levels of tumor markers,improve the immune indexes of patients,reduce the adverse drug reactions,and improve the efficacy and quality of life of patients with advanced NSCLC.

目的 对入住重症监护病房(ICU)重症孕产妇患者的病种特征和住院费用进行分析。方法 从医院信息管理系统中搜索2009—2019年广州市某省级重症孕产妇救治中心ICU的重症孕产妇住院病例,对病例资料进行描述性流行病学分析。结果 2009—2019年该救治中心ICU共收治重症孕产妇1 616例,病种排名前五位依次为心血管疾病430例(26.61%)、血液及造血器官疾病310例(19.18%)、妊娠期特定并发疾病287例(17.76%)、消化系统疾病218例(13.49%)、呼吸系统疾病110例(6.81%)。患者经济负担中位数排名前五位依次为肌肉骨骼系统和结缔组织疾病(62 252.60元)、消化系统疾病(61 684.41元)、感染性疾病(42 945.70元)、血液及造血器官疾病(40 403.52元)、神经系统疾病(40 055.93元)。结论 入住ICU内的重症孕产妇以心血管疾病、血液及造血器官疾病为主,经济学分析表明肌肉骨骼系统和结缔组织疾病造成的经济损失较大。
Objective To analyze the disease characteristics and hospitalization expenses of critically ill maternal patients in intensive care unit(ICU).Methods Hospitalized cases of severe maternal disease in ICU of a provincial critical care center in Guangzhou from 2009 to 2019 were searched from the hospital information management system,and case data was analyzed by descriptive epidemiology.Results From 2009 to 2019,a total of 1616 critically ill maternal patients received intensive care treatment at this center.The predominant diseases observed were cardiovascular disorders(26.61%),blood and hematopoietic organ diseases(19.18%),specific pregnancy-related complications(17.76%),gastrointestinal ailments(13.49%),and respiratory disorders(6.81%).Among the top five patient groups,the median economic burden was the highest in musculoskeletal system and connective tissue diseases(62 252.600 yuan),followed by digestive system diseases(61 684.410 yuan),infectious diseases(42 945.700 yuan),blood and hematopoietic organ diseases(40 403.515 yuan),and nervous system disorders(40 055.930 yuan).Furthermore,a discernible correlation between hospitalization cost and length of stay was identified.Conclusions Cardiovascular diseases and disorders of blood and hematopoietic organs are the primary causes for maternal admissions to ICU.Economic analysis shows that musculoskeletal system and connective tissues diseases cause bigger economic loss .

目的 构建并验证主动脉夹层B型(TBAD)患者急性期预后的列线图预测模型,帮助临床医生在急性期内更准确地评估TBAD患者的死亡风险,并制定更合适的治疗策略。方法 回顾性分析从重症监护医学信息数据库v2.2 中提取的399例 TBAD患者的人口学资料和临床资料,结局为TBAD患者急性期(≤14 d)内死亡。先采用最小绝对收缩选择算法回归筛选特征变量,再采用多因素分析确定独立预后因素,并据此构建预测模型。通过受试者工作特征曲线、校准曲线、决策曲线分析(DCA)评价列线图预测模型的性能和临床适用性。结果 APS Ⅲ评分、二氧化碳总量、红细胞分布宽度为TBAD患者14 d内死亡的独立预测因素。列线图预测模型在内部验证中的受试者工作特征曲线下面积为0.776(95% CI:0.691 ~ 0.860),Hosmer-Lemeshow 检验P=0.604,校准曲线和标准曲线高度重合,表明该模型具有良好的区分度和校准度。同时,DCA曲线显示,预测模型在大部分的阈值概率范围内提供了显著的净收益。结论 本研究基于APS Ⅲ评分、二氧化碳总量、红细胞分布宽度构建的列线图预测模型可以较准确地预测TBAD患者14 d内的死亡风险,有助于临床医生制定更合适的个体化治疗策略。
Objective To develop and verify a nomogram for predicting acute phase outcomes in patients with type B aortic dissection(TBAD),enabling clinicians to more precisely evaluate mortality risk in TBAD patients during the acute stage and to devise better treatment plans.Methods This retrospective study analyzed demographic and clinical data of 399 TBAD patients from the Medical Information Mart for Intensive Care IV v2.2,focusing on mortality within 14 days of the acute phase in TBAD patients. Initially,the Least Absolute Shrinkage and Selection Operator regression was employed for feature variable selection,and then multivariate analysis was used to identify independent prognostic factors for constructing the predictive model.The nomogram predictive model's effectiveness and clinical applicability were assessed via the Receiver Operating Characteristic curve,calibration curve,and Decision Curve Analysis(DCA).Results Acute Physidogy Score Ⅲ score,total carbon dioxide,and red blood cell distribution width emerged as independent predictors of 14-day mortality in TBAD patients.The internal validation of the nomogram predictive model showed an area under the curve of 0.776(95%CI:0.691-0.860),with a Hosmer-Lemeshow test P-value of 0.604. The close alignment of the calibration and standard curves suggested the model's strong discriminative power and calibration. Furthermore,the DCA curve revealed that the predictive model offered substantial net benefits within a wide range of threshold probabilities.Conclusions This study's nomogram,developed using APS Ⅲ score,total carbon dioxide,and red blood cell distribution width,accurately predicts the 14-day mortality risk in TBAD patients,assisting clinicians in creating better personalized treatment plans.
