目的 探讨多原发恶性肿瘤(MPMN)的临床特点,提高该类型肿瘤的认识,为临床诊断及治疗提供一定的经验。方法 分析2021年12月广州市第一人民医院呼吸与危重症学科二区收治的1例乳腺、甲状腺及肺三重多原发恶性肿瘤的临床特点及诊治经过,并结合相关文献进行回顾分析。结果 该例首患乳腺导管内癌并予手术切除,9年后再同时患肺癌及甲状腺癌,最终确诊为三重多原发恶性肿瘤。结论 通过对多原发恶性肿瘤的临床特点的分析研究,可一定程度提高临床医生对MPMN的认知以及早期临床鉴别的能力,亦为肿瘤患者早期诊断、早期治疗提供更好的时机,改善患者生活质量。
Objective To explore the clinical characteristics of multiple primary malignant neoplasms (MPMN), to improve the understanding of this type of tumors, and to provide some experience for clinical diagnosis and treatment. Methods The clinical characteristics, diagnosis and treatment of a case with triple MPMN of breast, thyroid and lung treated in the Respiratory and Critical Care Department (Area 2) of Guangzhou First People's Hospital in December 2021 were analyzed retrospectively. Results The breast intraductal carcinoma was the first tumor of that case and had been resected, and 9 years later, lung cancer and thyroid cancer occured at the same time, which were diagnosed as triple MPMN. Conclusions The analysis and study of the clinical characteristics of MPMN can improve the clinicians' cognition and the ability of early clinical differentiation, and provide a better opportunity for early diagnosis and early treatment of tumor patients, and improve their quality of life.

目的 报道1例中年女性胰腺实性假乳头状肿瘤病例,并通过文献回顾和复习,提高临床医生对此类罕见病的认识,减少该疾病的误诊、漏诊。方法 回顾性分析1例中年女性胰腺实性假乳头状肿瘤的诊治经过,结合以往的文献报道,总结该疾病的临床表现、诊治方法。结果 回顾文献显示:胰腺实性假乳头状肿瘤比较罕见,好发于年轻女性,属于低度恶性肿瘤。本例患者38岁女性,因腹痛入院。术后病理证实为胰腺实性假乳头状肿瘤。随访1年,未见明显复发和转移征象。结论 胰腺实性假乳头状肿瘤临床表现无特异性,诊断主要依靠影像学检查和组织病理学结果。手术切除是胰腺实性假乳头状肿瘤首选治疗手段。
Objective To report a case of solid pseudopapillary tumor of the pancreas and review literatures about this disease, so as to improve clinical understanding of this rare disease and reduce misdiagnosis and missed diagnosis. Methods The clinical manifestation,diagnosis and treatment of the patient with solid pseudopapillary tumor of the pancreas were analyzed retrospectively. The clinical manifestation and key points of diagnosis were summarized from literature review. Results Literature review showed that: solid pseudopapillary tumor of the pancreas is a rare low-grade malignancy tumors, which frequently occurs in young women. This case is a thirty-eight years old female patient who was admitted to the hospital due to abdominal pain. Postoperative pathology confirmed a solid pseudopapillary tumor of the pancreas. Follow-up of 1 year showed no obvious signs of recurrence and metastasis. Conclusion Solid pseudopapillary tumor of the pancreas has no specific clinical manifestations, the diagnosis mainly depends on imaging examination and histopathological results. Surgical resection is the preferred treatment for solid pseudopapillary tumors of the pancreas.

目的 探讨广州市第一人民医院2008—2019年所有就诊的肿瘤患者的具体情况,研究各类肿瘤的发病趋势和危害程度,为制定预防措施和工作重点提供依据。方法 根据广州市恶性肿瘤病例登记报告系统数据,对2008—2019年在广州市第一人民医院就诊的23 778例肿瘤病例从基本情况、肿瘤部位和随访情况等几方面进行回顾性统计分析。结果 2008—2019年位列前三位的肿瘤分别是结直肠癌、肺癌和肝癌,甲状腺癌占比呈明显上升趋势。30岁以下年龄组肿瘤种类以白血病为主,30~60岁年龄组肿瘤种类以结直肠癌为主,60岁以上年龄组肿瘤种类以肺癌为主。甲状腺癌的预后最好,胰腺癌的预后最差。纯手术组疗效最佳。结论 在广州地区合理优化资源进行重点肿瘤的筛查,才可以有限降低罹患肿瘤的风险,提高肿瘤患者生命质量。
Objective The aims of study were to explore the situation of all patients with cancer in Guangzhou First People's Hospital from 2008 to 2019, to find the incidence trends and hazards of different types of tumors, and to provide evidence for developing preventive measures. Methods According to the data of the Guangzhou tumor case report system, a retrospective statistical analysis was performed on the basic situation, tumor location and follow-up of 23 778 tumor cases in Guangzhou First People's Hospital from 2008 to 2019. Results The top three tumors in 2008—2019 were colorectal cancer, lung cancer and liver cancer. The tumor types under 30 years old were leukemia, the tumor types in the 30-60 age group were colorectal cancer, and the tumor types above 60 years old were mainly lung cancer. The prognosis for patients with thyroid cancer was the best, and the prognosis for patients with pancreatic cancer was the worst. The effect of the operation group was the best. Conclusion Rational optimization of resources for screening key tumors could reduce tumor incidence and improve the quality of life of cancer cases in Guangzhou area.
