先天性巨结肠是遗传背景复杂的先天性肠神经系统发育异常性疾病,也是小儿外科临床工作中的常见疾病,以消化道末端肠神经节缺如为典型病理特点,肠梗阻、肠穿孔、严重的巨结肠相关性小肠结肠炎是疾病的主要并发症,尤其在低龄婴幼儿起病的病例中,仍是围术期死亡的主要原因。近些年来,越来越多的学者在解析其发病机制、遗传特征、治疗手段等方面不断探索创新,但在低龄婴幼儿,尤其是新生儿期寻找创伤小、特异性高、普适性强的早诊方法仍面临巨大挑战,早发现、早干预可以有效改善患儿的生活质量及疾病预后。本文对先天性巨结肠相关的早期诊断方法进行了系统综述,以期在传统经典的诊断方法上,不断创新求索,综合不同方法的利弊,提高临床应用中的诊断效能,为疾病早期诊断方面的研究提供理论基础。

吞咽障碍是慢性阻塞性肺疾病(COPD)的常见并发症之一,并且是COPD急性加重的危险因素之一,对患者预后造成不良影响。对COPD患者吞咽障碍的发生及严重程度进行准确的评估,是进行科学管理的首要前提。本文对COPD吞咽障碍评估工具的基本特点、应用现状及发展现状进行综述,为临床医务人员选择评估工具提供借鉴。
Dysphagia is one of the common complications of chronic obstructive pulmonary disease(COPD),and is one of the risk factors of acute exacerbation of COPD,which has adverse effects on the prognosis of patients.Accurate assessment of the occurrence and severity of dysphagia in COPD patients is the first step for scientific management.In this paper,the basic characteristics,application status and development status of assessment tools for COPD patients with dysphagia are reviewed,which can provide reference for the selection of assessment tools for clinical medical staff.

目的 探讨非小细胞肺癌(NSCLC)组织中转化生长因子β激活激酶1(TAK-1)与T细胞因子-4(TCF-4)在 mRNA以及蛋白水平的表达情况及其相关性,并分析两者与NSCLC患者临床病理因素的关系。方法 收集NSCLC手术标本51例,每例均包含肺癌组织及配对癌旁组织,所有患者的术后诊断均经病理结果证实,通过RT-PCR以及Western blot法检测TAK1、TCF-4在癌组织及配对癌旁组织中的表达情况,并通过SPSS进一步分析两者的相关性及其与临床病理因素的关系。结果 TAK1与TCF-4 mRNA以及蛋白水平在NSCLC患者癌组织中均高表达,其中TAK1蛋白的表达与NSCLC的TNM分期(P=0.022)、淋巴结转移(P=0.014)相关,TCF-4蛋白的表达与NSCLC的TNM分期相关(P=0.045)。TAK1在NSCLC组织中的表达与TCF-4呈正相关(r=0.427,P=0.002)。结论 TAK1 mRNA及蛋白水平在NSCLC组织中均高表达,并与TCF-4呈正相关,TAK1有可能成为NSCLC诊断及预后的一个潜在靶标,并且TAK1与TCF-4的联合应用有可能成为一种更为理想的NSCLC辅助诊断及临床治疗方法。
Objective To investigate the mRNA and protein expressions of transforming growth factor β-activated kinase 1(TAK1)and T cell factor-4(TCF-4)in non-small cell lung cancer(NSCLC)tissues and their correlation,and to analyze the relationship between TAK1/TCF-4 and clinicopathological factors in NSCLC patients.Methods Cancer tissues and matched adjacent tissues of 51 NSCLC patients in our hospital were collected.The postoperative diagnosis of all patients was confirmed by pathological results.The expression of TAK1 and TCF-4 in cancer tissues and paired adjacent tissues were detected by RT-PCR and Western blot,then SPSS was used to further analyze the correlation between TAK1 and TCF-4 and clinicopathological factors.Results TAK1 and TCF-4 mRNA and protein were highly expressed in NSCLC tissues,and TAK1 protein expression was significantly correlated with TNM stage of NSCLC(P=0.022)and lymph node metastasis(P=0.014);TCF-4 protein expression was significantly correlated with TNM stage of NSCLC(P=0.045).TAK1 expression in NSCLC tissues was positively correlated with TCF-4(r=0.427,P=0.002).Conclusions TAK1 mRNA and protein were highly expressed in NSCLC tissues and positively correlated with TCF-4.TAK1 may become a potential target for the diagnosis and prognosis of NSCLC,and the combined application of TAK1 and TCF-4 may become a more ideal method for the auxiliary diagnosis and clinical treatment of NSCLC.

本文综述近10年中医传统疗法中通过穴位刺激预防与治疗老年患者骨折术后谵妄的研究概况。术后谵妄是老年患者骨折术后常见的并发症,不仅延长患者住院时间、增加经济负担,同时与骨折术后死亡并发症的发生密切相关。但是本病机制尚未明确,在临床治疗上难以达成共识。针灸等穴位刺激是中医药治疗脑病的独具特色的疗法,近年来有关穴位刺激防治老年骨折术后谵妄的报道越来越多,本文主要探讨不同的穴位刺激方法干预对骨折术后谵妄发生率的影响,包括传统刺激方式如毫针、电针、穴位注射、穴位敷贴和新针疗法如耳针、头针等,认为穴位刺激能够较好预防骨折术后谵妄发生,在治疗上也有较好疗效。以期为进一步临床研究与应用提供参考。
This article summarized the research of acupoint stimulation in preventing delirium after fracture surgery in elderly patients in recent 10 years.Postoperative delirium is a common complication in elderly patients after fracture surgery, which affects life extremely and increases economic burden.However, the mechanism of this disease haven't been revealed, and it is difficult to reach consensus on clinical treatment.Acupoint stimulation, like acupuncture, is a unique treatment of encephalopathy with traditional Chinese medicine.In recent years, there are more and more reports on acupoint stimulation therapy to prevent and treat delirium after fracture surgery in the elderly.This article mainly study the influence of different acupoint stimulation therapy on treatment of delirium after fracture surgery, including electroacupuncture, auriculotherapy, scalp acupuncture, acupoint injection and acupoint application, etc.It is believed that acupoint stimulation can prevent delirium after fracture surgery, so as to provide reference for further clinical research and application.

目的 探究3D打印数字化塑形聚醚醚酮(PEEK)材料在颅骨修补个体化重建中的应用效果。方法 收集我院90例行颅骨修补个体化重建手术患者(2019年1月—2021年1月),按术中应用的颅骨修补材料不同分成PEEK组(n=30)、钛网(TM)组(n=60)。2组均行颅骨修补个体化重建术。对比2组围术期指标、手术前后简易智力状态量表(MMSE)、格拉斯哥预后评分(GOS)、颅骨缺损塑形满意度、并发症发生率、材料生物相容性及患者主观感觉情况。结果 与TM组相比,PEEK组住院时长更短、治疗费用更高(P<0.05);与术前相比,2组术后、6个月、12个月、18个月MMSE、GOS评分均升高,其中PEEK组升高幅度比TM组更为多(P<0.05);PEEK组总满意度为93.33%,与TM组的80.00%比较差异无统计学意义(P>0.05);PEEK组并发症总发生率为6.67%,与TM组的15.00%比较差异无统计学意义(P>0.05);PEEK组材料生物相容性及患者主观感觉情况与TM组比较差异无统计学意义(P>0.05)。结论 3D打印数字化塑形在颅骨修补个体化重建中,应用PEEK材料治疗费用更高,但可有效减少并发症,缩短住院时长,改善认知功能及预后,提升颅骨缺损塑形满意度,且材料生物相容性高,震动感和冷热感觉体验少。

目的 通过静脉输液批次决策的自动化、标准化以及个体化,实现静脉用药调配中心(PIVAS)工作同质化和高效化,进而促进临床安全合理用药。方法 根据药物使用的时辰药理学,调查分析人工决策输液批次存在的问题,建立批次时间划分表和制定批次决策规则,基于医院信息系统构建自动输液批次决策系统。由药师维护自动输液批次决策系统,针对该系统的应用效果评价分析。结果 应用自动批次决策软件后,PIVAS 批次不合格率下降,输液时间合理性提高。审方药师用于决策输液批次的时间减少,大幅度提高 PIVAS 和病区护士工作效率,同时降低 PIVAS输液配送出错频率。结论 自动输液批次决策系统构建完成之后,用药时间更合理,提高药物治疗效果;PIVAS工作效率明显提高,工作质量显著提升,输液批次不合格率降低,患者输液批次真正做到编排一致性,提高用药时间合理性,进而使临床用药更合理、更安全,实现个体化治疗。
ObjectiveThrough the automation, standardization and individualization of intravenous(IV)fluid batch decision-making, to realize the homogenization and high efficiency of Pharmacy Intravenous Admixture Services(PIVAS), to promot safe and rational clinical drug use.Methods According to the pharmacology of drug use, the problems existing in manual IV fluid batch decision were investigated and analyzed, and an automatic IV fluid batch decision system was constructed based on hospital information system.Batch time division table and batch decision rules were established, automatic IV fluid batch decision system was maintained by pharmacists, the application effect of the system were evaluated and analyzed.Results The unqualified rate of PIVAS batch decreased, and the rationality of IV transfusion time increased.The time for prescription checking pharmacists to make IV fluid batch decisions was shortened, which greatly improved the work efficiency of PIVAS and nurses, and reduced the error frequency of PIVAS IV fluid distribution.Conclusions After the completion of the automatic IV fluid batch decision system, the medication time is more reasonable and the drug treatment effect is improved.The work efficiency and quality of PIVAS are significantly improved, and the unqualified rate of IV fluid batches is reduced.The IV fluid batches of patients are really arranged in a consistent way, and the rationality of medication time is improved, so that clinical medication is more reasonable and safer, and individualized treatment is realized.

直肠癌是全球常见的消化道恶性肿瘤之一,准确的影像学评估对直肠癌的诊断和治疗决策至关重要。在各种影像学检查中,磁共振成像已逐渐成为直肠癌不可或缺的检查手段,其中酰胺质子转移加权(APTw)成像是当前直肠癌磁共振成像中的一种极具发展前景的成像技术,其是通过检测病灶中内源性可移动蛋白的酰胺质子与水质子之间的交换,可以更好地在分子水平评估直肠癌病灶的蛋白质浓度、pH值、温度和代谢状态等,在直肠癌中有极大的应用潜能,有助于实现精准医疗。本文主要综述APTw成像在直肠癌中的研究进展、发展前景及尚存在的问题。
Rectal cancer is one of the most common gastrointestinal malignancies worldwide, and accurate imaging evaluation is essential for the diagnosis and treatment of rectal cancer.In various imaging examinations, magnetic resonance imaging(MRI)has gradually become an indispensable means of rectal cancer, among which amide proton transfer-weighted(APTw)imaging is a promising imaging technology.APTw imaging is to detect the exchange between amide protons of endogenous mobile proteins in lesions and water quality particles, which can better assess the protein concentration, pH, temperature, and metabolic status of rectal cancer at the molecular level.It has great application potential in rectal cancer and contributes to precision medicine.This article mainly reviews the research progress, development prospect and remaining problems of APTw imaging in rectal cancer.

目的 总结以双下肢乏力为主要表现的、合并低叶酸血症的患者的临床特征。方法 选择自2017年1月—2020年12月在我院神经内科住院的患者,分为3组:双下肢乏力伴叶酸缺乏组,共23例;叶酸缺乏合并脑血管病组,共129例;叶酸缺乏的健康体检者,为来我院行健康体检、无意中发现叶酸水平降低者,共42例,比较3组患者特征。结果 双下肢乏力伴叶酸缺乏组患者的发病年龄在19~88岁之间,平均(63.82±20.24)岁,男女比为2.3∶1。起病时间(13.34±17.88)d。与叶酸缺乏合并脑血管病组以及与叶酸缺乏的健康体检者相比,双下肢乏力伴叶酸缺乏组患者脑叶缺血灶数量较少,差异有统计学意义,P分别=0.001和0.008;与叶酸缺乏的健康体检者相比,双下肢乏力伴叶酸缺乏组患者放射冠和侧脑室缺血灶数量无变化,差异无统计学意义,P>0.05;与叶酸缺乏合并脑血管病组以及与叶酸缺乏的健康体检者相比,双下肢乏力伴叶酸缺乏组患者整体脑组织缺血灶总数较少,差异有统计学意义,P分别<0.01和0.05。结论 临床上遇到双下肢乏力患者,尤其是发病年龄在63岁左右,男性,起病时间在13 d左右,颅内整体缺血灶、尤其脑叶缺血灶较少的患者,需要警惕低叶酸血症所致双下肢乏力的可能。
Objective To summarize the clinical characteristics of patients with lower extremity weakness as the main manifestation and hypofolicemia.Methods Patients admitted in our neurology inpatient center from January 2017 to December 2020 were selected and divided into 3 groups.Group A consisted of 23 cases of bilateral lower extremity weakness combined with folic acid deficiency.Group B consisted of 129 cases of folic acid deficiency combined with cerebrovascular disease.Group C consisted of 42 healthy people with folic acid deficiency who came to our hospital for health check-up and found that the level of folic acid was decreased accidentally.The clinical characteristics of the three groups of patients were compared.Results The age of onset in group A was between 19 and 88 years old,with an average of(63.82±20.24)years old,and the male to female ratio was 2.3∶1.The onset time was(13.34±17.88)days.Compared with the group B and group C,the number of cerebral lobe ischemic area in group A was lesser,and the difference was statistically significant,P=0.001 and 0.008,respectively.Compared with group C,the number of corona radiata and lateral ventricle ischemic lesions in group A did not change,and the difference was not statistically significant,P>0.05.Compared with group B and group C,the total number of ischemic lesions in the overall brain tissue of group A was lesser,and the difference was statistically significant,P<0.01 and P=0.05 respectively.Conclusions When we encounter patients with bilateral lower extremity weakness in clinical practice,especially the average age of onset is around 63 years old,male,the onset time is about 13 days,and the overall intracranial ischemic lesions,especially the lobar ischemic lesions are less,we need to think of the possibility of bilateral lower extremity weakness caused by hypofolate.

融合基因是指两个独立基因的编码区首尾相连所形成的且置于同一套调控序列控制的产物,研究表明许多癌症的发生与融合基因存在密切的联系。融合基因可作为癌症治疗的靶点,在癌症诊断及治疗领域中融合基因的研究具有重要意义。部分融合基因驱动癌症的机制已被初步揭示,但是有些真实存在的在肿瘤发生发展过程中有重要意义的融合基因由于工具和实验技术限制还未被发现。因此,对融合基因的分析预测研究方法逐渐成为关注的热点之一。本文探讨了目前常用的关于融合基因的分析工具及方法,为融合基因在癌症中的研究提供思路。
Fusion genes are the products of two independent genes whose coding regions are linked and controlled by the same set of regulatory sequences.Studies have shown that many cancers are closely linked to gene fusions.Fusion genes can be used as targets for cancer therapy,and the study of fusion genes is of great importance in cancer diagnosis and treatment.Some of the mechanisms of fusion genes driving cancer have been initially revealed,but there are more fusion genes which are important in the process of tumor development have not been discovered due to the limitation of tools and experimental techniques.Therefore,the analysis and prediction methods of fusion genes are becoming a hot topic of interest.In this paper,we discuss the commonly used analytical tools and methods on fusion genes to provide ideas for the study of fusion genes in cancer.

高压氧治疗是在高于当地压力的环境下,吸入高于当地大气氧浓度的氧来治疗疾病的方法,主要用于缺氧缺血性疾病或与缺氧缺血有关的疾病的治疗。炎症作为机体对刺激的一种防御机制,是感染、损伤、中毒等疾病共同的病理生理基础,在疾病的发生发展中扮演重要角色。高压氧与炎症的关系密切,围绕高压氧与炎症调节相关的研究进行综述,对深入认识高压氧的作用机制具有重要的意义。
Hyperbaric oxygen therapy is the treatment of diseases by inhaling oxygen at a concentration higher than the local atmospheric oxygen concentration in an environment that higher than the local pressure.It is mainly used in the treatment of hypoxic-ischemic diseases or diseases related to hypoxia-ischemia.As a defense mechanism against stimulation,inflammation is the common pathophysiological basis of infection,injury,poisoning and other diseases,which plays an important role in the occurrence and development of diseases.Hyperbaric oxygen is closely related to inflammation.The review on hyperbaric oxygen and inflammation regulation is of great significance to further understand the mechanism of hyperbaric oxygen.
