围术期康复指导在脊柱外科中的应用效果

目的 探讨脊柱外科围术期康复指导方案的临床疗效。方法 选取毕节市第三人民医院脊柱外科184例行择期手术治疗的患者,对照组继续脊柱外科原康复方案行术前及术后管理,观察组使用新的康复行为规范方案,即系统行术前预康复指导和术后规范管理。对患者的术后起床活动、在院时间、住院费用、疼痛恢复情况、满意调查情况进行对比。结果 术前,患者的性别、年龄、病种分布对比差异无统计学意义(P>0.05)。术后,观察组的术后下床活动时间(3.09±1.02)d、住院时间(10.73±3.96)d、住院费用(17 388±5 217)元、术后2天VAS评分(3.04±1.19)分、出院时VAS评分(2.36±1.25)分、住院患者满意度(89.80±8.20)分,均优于对照组的术后起床活动时间(4.44±1.58)d、住院时间(13.38±2.73)d、住院费用(23 242±7 971)元、术后2天VAS评分(4.01±1.44)分、出院时VAS评分(3.39±1.38)分、住院满意度(80.27±11.45)分。新的康复指导方案在脊柱外科患者中较对照组减轻术后疼痛、减少患者卧床时间及缩短住院时间(P<0.05)。结论 围手术期加快患者术后康复及提高术后恢复优良率,提高患者就医满意度,使医患关系更加融洽。
Objective To investigate the clinical effect of the rehabilitation guidance protocol of spinal surgery in perioperative period.Methods A total of 184 patients undergoing selective surgery in the spine surgery department of the Third People's Hospital of Bijie City were selected.The conventional rehabilitation group was the control group,which preoperative and postoperative management was continued with the original rehabilitation program of spine surgery;the rehabilitation guidance group was the observation group:the new rehabilitation behavior code program was used to provide systematic preoperative pre-rehabilitation guidance and postoperative standardized management.The patients' activities of getting up after surgery,days in hospital,economic use,pain recovery and satisfaction survey were compared.Results Before surgical treatment,the gender,age and disease distribution of patients were compared(P>0.05).After surgical treatment,observation group:Postoperative time of getting out of bed(3.09±1.02)d,length of hospitalization(10.73±3.96)d,hospitalization expenses(17 388±5 217)yuan,VAS score of 2 days after surgery(3.04±1.19),VAS score of discharge(2.36±1.25),inpatient satisfaction(89.80±8.12),were significantly better than the control group:Postoperative activity time(4.44±1.58)d,length of hospitalization(13.38±2.73)d,hospitalization expenses(23 242±7 971)yuan,VAS score(4.01±1.44),VAS score at discharge(3.39±1.38),hospitalization satisfaction(80.27±11.45).Compared with the control group,the new rehabilitation guidelines significantly reduced postoperative pain,bed time and hospital days in spinal surgery patients(P<0.05).Conclusions Speeding up the postoperative rehabilitation of patients and improving the rate of postoperative recovery,improving patients' sense of access to medical treatment,can make the doctor-patient relationship more harmonious.

tNGS技术在天津市发热呼吸道症候群病原特征分析的应用研究

目的 利用靶向二代测序技术探究天津市发热呼吸道症候群病原特征,分析病原流行特征,总结规律,为发热伴呼吸道症候群患者治疗策略提供指导。方法 收集2023年1月—2023年12月天津地区某四家医院初诊为发热伴呼吸道症候群的4 039例病例,通过tNGS技术检测,分析鉴定病原微生物,研究不同标本类型病原微生物分布、年龄分布、性别分布、不同时间季节感染分布、微生物类型分布趋势等。结果 4 039例经tNGS技术检测,共检测出90种潜在病原体,总阳性率为94.80%(3 829/4 039)。包括36种细菌、29种病毒、5种非典型病原体和20种真菌。不同种类病原体检出频次排序:革兰阴性细菌(G-细菌)>RNA病毒>DNA病毒>革兰阳性细菌(G+细菌)>真菌>非典型病原体。男女性别阳性检出率比较差异无统计学意义(χ2=0.249,P=0.618);不同年龄段人群病原体谱有异质性,且阳性患者多分布在0~10岁和>60岁,阳性也集中在该两类人群,阳性率分别为47.90%(1 834/3 829)和31.24%(1 196/3 829)。在单一感染中,肺炎支原体占比最高,为4.91%,其次为新型冠状病毒,占1.59%,再次为呼吸道合胞病毒,占1.23%。不同季节不同感染类型具有差异,整体呈现夏冬季混合感染率高的特点,即夏季新冠病毒合并细菌(鲍曼不动杆菌、肺炎链球菌),冬季肺炎支原体合并甲型、乙型流感病毒多见。肺炎支原体对大环内酯类药物耐药率随月份呈现增加趋势。结论 tNGS 技术在天津市发热呼吸道症候群样本中获得了较高的病原体检出率,病原体覆盖面广,通过提供更多的病原体信息,可以有效辅助鉴别混合感染和耐药信息。
Objective Using targeted next-generation sequencing technology to explore the composition of the pathogenic spectrum of febrile respiratory syndrome ,analyze the epidemic characteristics of the pathogen,summarize the rules,and provide accurate guidance for the treatment strategy of patients with febrile respiratory syndrome.Methods By tNGS detection of 4039 cases with fever with respiratory diseases in four hospitals in Tianjin in 2023,pathogenic microorganisms were analyzed and identified,and the distribution of pathogenic microorganisms,age distribution,gender distribution,infection distribution in different times and seasons,and distribution trend of microbial types in different specimen types were studied.Results In 4039 cases,90 potential pathogens were detected,the total positive rate was 94.80%(3 829/4 039),including 36 kinds of bacteria,29 kinds of viruses,5 kinds of atypical pathogen and 6 kinds of fungi.The order of detection frequency of different types of pathogens:G-bacteria > RNA virus > DNA virus > G+ bacteria > fungi > atypical pathogens.There was no significant difference in the positive rate of male and female between the two groups(χ2=0.249,P=0.618).The pathogen spectrum of different age groups was heterogeneous. The positive patients were mostly distributed in 0-10 years old [47.90%(1 834/3 829)] and >60 years old [31.23%(1 196/3 829)].Among the single infection,Mycoplasma pneumoniae accounted for the highest proportion,4.91%(188/3829),followed by 2019-nCoV,1.59%(61/3 829),and respiratory syncytial virus,1.23%(47/3 829).There were differences in different infection types in different seasons,and the overall characteristics of mixed infection rate in summer and winter were high. In summer,the new coronavirus was combined with bacteria(Acinetobacter baumannii,Streptococcus pneumoniae),and in winter,Mycoplasma pneumoniae was combined with influenza A and B viruses.The resistance rate of Mycoplasma pneumoniae to macrolides showed an increasing trend with the time.Conclusions The tNGS technology had obtained a high pathogen detection rate in the samples of febrile respiratory syndrome in Tianjin.By providing more pathogen information,it can be effectively applied to the etiological diagnosis of respiratory infectious diseases and assist in the identification of mixed infections and drug resistance information.

乳腺癌是女性最常见的原发恶性肿瘤之一。目前,通过采用综合治疗手段,包括手术、新辅助治疗、辅助放化疗等多种手段,乳腺癌已成为疗效最佳的实体肿瘤之一。其中,新辅助治疗(NAT)包括新辅助化疗、靶向治疗和内分泌治疗,目的是使肿瘤降期、保乳、保腋窝,并可观察药物敏感性,是当前乳腺癌综合治疗中非常重要的组成部分,其治疗疗效对患者手术方式的选择和预后至关重要。尽管病理学评价在评估NAT疗效方面被公认为金标准,但其局限性在于只能通过有创手段在治疗后进行,无法在治疗前对患者做出准确预测。磁共振成像(MRI)作为一项广泛使用的乳腺成像技术,在评估NAT疗效中扮演着关键角色。近年来,人工智能技术,尤其是影像组学(Radiomics)和深度学习(Deep Learning),在医学影像分析领域取得了显著进展。这些技术能够从医学图像中提取大量肉眼难以识别的特征,揭示病变内部的微观结构和生物学行为,全面反映肿瘤的异质性,这不仅有助于临床医生更准确地区分良、恶性肿瘤,还能对恶性肿瘤的预后进行更为精确的评估。本文系统综述了近年来基于MRI的人工智能技术在乳腺癌新辅助治疗疗效评估中的应用研究,旨在促进人工智能在NAT临床实践中的应用和发展,为乳腺癌NAT治疗策略的优化和个性化医疗的实现提供科学依据。
Breast cancer is one of the most common primary malignant tumors in women.Currently,breast cancer has become one of the most effective solid tumors by using comprehensive treatment methods,including surgery,neoadjuvant therapy,adjuvant radiotherapy and chemotherapy.Among them,neoadjuvant therapy(NAT),including neoadjuvant chemotherapy,targeted therapy and endocrine therapy,is a very important part of the current comprehensive treatment of breast cancer.It aims to reduce the tumor stage,preserve the breast,preserve the armpit,and observe the drug sensitivity.Its therapeutic effect is crucial to the choice of surgical methods and prognosis of patients.Although pathological evaluation is recognized as the gold standard in evaluating the response to NAT,its limitation is that it can only be performed after treatment by invasive means,and cannot accurately predict response before treatment.As a widely used breast imaging technology,magnetic resonance imaging(MRI)plays a key role in evaluating the response to NAT.However,traditional MRI evaluation methods are limited by the individual differences of interobserver and the low repeatability of evaluation results,which affects the accuracy of efficacy evaluation to a certain extent.In recent years,artificial intelligence technology,especially radiomics and deep learning,has made significant progress in the field of medical image analysis.These techniques can extract a large number of features that are difficult to be recognized by the naked eye from medical images,reveal the internal microstructure and biological behavior of the lesion,and fully reflect the heterogeneity of the tumor.This not only helps clinicians to distinguish benign and malignant tumors more accurately,but also makes a more accurate assessment of the prognosis of malignant tumors.This article reviews the application and progress of MRI-based artificial intelligence technology in evaluating the response to neoadjuvant therapy for breast cancer in the past five years,aiming to promote the application and development of artificial intelligence in NAT clinical practice,and provide a scientific basis for the optimization of NAT treatment strategy and the realization of personalized medicine for breast cancer.

骨劈开术在水平骨量不足缺牙区种植中的临床应用进展

缺牙区水平骨量不足一直是口腔种植的重要难题。骨劈开术是常用于解决此难题的手术方式之一,目前临床常用术式是采用超声骨刀沿牙槽嵴顶矢状劈开后,用骨凿将唇(颊)侧骨板向唇(颊)侧移位后增加牙槽骨水平宽度,达到同期种植的目的。骨劈开并同期植入种植体降低了困难病例的手术难度和风险,减少了患者就诊次数,降低患者成本。但若劈开后种植体周围的间隙大于1 mm或唇侧骨板分离,则需要同期进行植骨。但骨劈开术的使用也具有一定的适应症,要求缺牙区垂直骨高度足够,水平骨宽度不足(3~5 mm),骨质为Ⅲ ~ Ⅳ类骨,患者无种植手术的局部和全身禁忌证。近年来的研究表明骨劈开术具有创伤小、手术安全有效、不用开辟第二创口即可获得较好的骨增量效果等优点,一定程度上扩大了种植手术适应证。现本文对骨劈开术的翻瓣术式、非翻瓣术式及术后效果的进展进行综述。
The insufficient horizontal bone mass in the missing tooth area has always been an important problem of dental implant. Bone splitting is one of the surgical methods commonly used to solve this problem. At present, the commonly used surgical method is to use ultrasonic bone knife to sagittal splitting along the crest of the alveolar ridge, and then use bone chisel to shift the lateral bone plate of the lip (buccal) to the lip (buccal) to increase the horizontal width of the alveolar bone, so as to achieve the purpose of simultaneous implantation. Bone splitting and implantation of implants in the same period reduced the surgical difficulty and risk of difficult cases, reduced the number of patient visits, and reduced the cost of patients. However, if the gap around the implant was more than 1 mm or the labial bone plate was separated after splitting, bone grafting should be performed simultaneously. But the use of bone splitting technique also has a certain indications, demanding sufficient vertical bone height in the missing area, inadequate width of horizontal bone (3 mm to 5 mm), Ⅲ~Ⅳ class bone, without local and systemic contraindicated of planting surgery. In recent years, the research shows that bone splitting surgery has the advantages of small trauma, safe and effective operation, and better bone increment effect obtained without opening the second wound. This article reviewed the progress of flap operation, flapless operation and postoperative effect of bone splitting.

镇肝熄风汤加减在VBI性眩晕患者辅助治疗中的应用

目的 观察镇肝熄风汤加减在椎-基底动脉供血不足(VBI)性眩晕患者辅助治疗中的应用。方法 选取2019年5月—2021年2月我院收治的124例VBI性眩晕患者,按照随机数字表法1:1配对原则分为观察组(n=62)、对照组(n=62),其中对照组给予长春西汀+天麻素,观察组在对照组基础上辅以镇肝熄风汤加减,对比2组疗效、经颅多普勒检查结果[基底动脉、椎动脉平均血流速度(MV)与血管搏动指数(PI)]、眩晕评估量表(DARS)、眩晕障碍调查表(DHI)评分及血液流变学指标(纤维蛋白原、红细胞积压、全血粘度)。结果 观察组总有效率95.16%(59/62)高于对照组82.26%(51/62)(P<0.05);治疗14 d后观察组椎动脉、基底动脉PI低于对照组,MV高于对照组(P<0.05);治疗14 d后观察组DHI评分、DARS评分低于对照组(P<0.05);治疗14 d后观察组纤维蛋白原、红细胞积压、全血粘度低于对照组(P<0.05)。结论 镇肝熄风汤加减辅助治疗VBI性眩晕患者疗效显著,可有效减轻眩晕症状,调节血液粘度,改善椎-基底动脉血流,促进脑部供血恢复。

NT超声联合血清AFP水平检测在接受产前筛查胎儿神经管畸形诊断中的应用

目的 探讨胎儿颈部软组织(NT)超声检查联合血清甲胎蛋白(AFP)水平检测对产前胎儿神经管畸形诊断中的应用价值。方法 选取684例我院2018年4月—2021年4月接受产前检查的孕妇,所有孕妇分别采用NT超声检查、血清AFP水平检查,以引产结果为“金标准”,比较NT超声、血清AFP水平及联合检查诊断结果、诊断效能及对不同类型神经管畸形诊断符合率的影响。结果 经引产结果显示共48例神经管畸形胎儿;经NT超声检查共36例神经管畸形胎儿;经血清AFP水平检查共34例神经管畸形胎儿;经联合检查共47例神经管畸形胎儿;与NT超声、血清AFP水平单独检查相比,联合检查灵敏度89.58%、准确率98.68%、阴性预测值99.22%较高,漏诊率10.42%较低(P<0.05);与NT超声、血清AFP水平单独检查相比,联合检查对于脑膨出、隐形脊柱裂胎儿检出率较高(P<0.05)。结论 NT超声检查、血清AFP联合诊断准确率显著高于单独检查,可有效提高诊断效能,为临床筛查胎儿神经管畸形提供有效手段。

益气活血汤剂联合热敏灸在脊髓损伤后尿潴留患者中的应用

目的 探究益气活血汤剂联合热敏灸在脊髓损伤(SCI)后尿潴留患者中的应用效果。方法 选取2019年7月—2021年7月我院SCI后尿潴留患者80例,根据治疗方案不同分为观察组、对照组,各40例。对照组给予益气活血汤剂,观察组在此基础上进行热敏灸治疗。观察2组疗效、治疗前后排尿情况、膀胱功能、尿动力学指标[膀胱容量(VH2O)、膀胱顺应(BC)、逼尿肌压力(Pdet)、膀胱压力(Pves)、平均尿流速(Qave)]及不良事件发生率。结果 观察组总有效率92.50%高于对照组70.73%(P<0.05);治疗后观察组膀胱功能积分、平均排尿次数、平均漏尿次数、平均尿量、残余尿量均低于对照组(P<0.05);治疗后与对照组相比,观察组Pves较低,VH2O、Pdet、Qave、BC较高(P<0.05);观察组不良事件率2.50%与对照组5.00%相比,无差异(P>0.05)。结论 热敏灸联合益气活血汤剂治疗SCI尿潴留患者效果确切,可改善患者排尿异常症状,促进膀胱功能恢复,且安全可靠。
Objective To explore the effect of Yiqi Huoxue Decoction combined with heat sensitive moxibustion in patients with urinary retention after spinal cord injury (SCI). Methods From July 2019 to July 2021, 80 patients with urinary retention after SCI in our hospital were selected and divided into observation group and control group according to different treatment plans, 40 cases in each group. The control group was given Yiqi Huoxue decoction, and the observation group was treated with heat sensitive moxibustion additionally. Efficacy, urination before and after treatment, bladder function, urodynamic indexes [bladder volume (VH2O), bladder compliance (BC), pressure of detrusor (Pdet), pressure of vesical (Pves), average flow rate (Qave)] and incidence of adverse events in 2 groups were observed. Results The total effective rate of observation group was 92.50%, higher than that of control group (70.73%, P<0.05). After treatment, bladder function score, average times of urination, average times of urine leakage, average urine volume and residual urine volume in the observation group were lower than those in the control group (P<0.05). After treatment, compared with the control group, the observation group had lower Pves, higher VH2O, Pdet, Qave and BC (P<0.05). The adverse event rate of the observation group (2.50%) was not significantly different from control group (5.00%, P>0.05). Conclusions Heat sensitive moxibustion combined with Yiqi Huoxue decoction is effective in the treatment of SCI patients with urinary retention, which can improve the symptoms of abnormal urination and promote the recovery of bladder function, and is safe and reliable.

坐式DR轮椅在精神发育迟滞患儿胸部DR体检中的应用

目的 探讨分析利用坐式DR轮椅辅助精神发育迟滞患儿胸部DR体检的图像质量。方法 2020年7月—2021年2月,共80例精神发育迟滞患儿纳入本研究。根据协助拍片的方式不同,分为坐式DR轮椅协助组(实验组)和医护人员抓扶协助组(对照组)。采用SPSS 20.0秩和检验分析2组图像质量的差异。结果 2组摄片图像差异有统计学意义(P<0.001)。坐式DR轮椅协助精神发育迟滞患者的胸部DR体检图片质量较高,废片率低。结论 坐式DR轮椅协助精神发育迟滞患儿胸部DR摄片图片的质量高,避免重复拍片的几率,为精神发育迟滞患儿及家属减少不必要的辐射风险提供了可靠方案。
Objective To explore and analyze the image quality of chest DR examination of children with mental retardation using DR wheelchair. Methods From July 2020 to February 2021, a total of 80 children with mental retardation were included in this study.According to different ways of assistance in examination, they were divided into seated DR wheelchair assisting group (experimental group) and medical staff assisting group (control group).The SPSS 20.0 rank sum test was used to analyze the difference in image quality between the two groups. Results The difference of radiographic images between the two groups was statistically significant (P<0.001).Seated DR wheelchairs assisted patients with mental retardation were with high quality and low rejection rate in their chest DR examination pictures. Conclusions DR wheelchair could help children with mental retardation to take chest DR pictures with high quality, avoid the probability of repeated examination, and provide a reliable method for children with mental retardation and their families to reduce the risk of unnecessary radiation.

数字乳腺三维断层融合摄影技术联合MR波谱在乳腺高危病灶诊断中的应用

目的 探究数字乳腺三维断层融合摄影技术(DBT)联合MR波谱在乳腺高危病灶诊断中的应用。方法 选取2020年6月—2021年6月来我院进行乳腺钼靶X线摄影并诊断为BI-RADS 4级及以上的100例乳腺高危病灶患者为研究对象,对入选患者行MR波谱及DBT检查,分析MR波谱征象及DBT乳腺病变摄影特征,以病理结果为标准,评价MR波谱与DBT及两者联用对乳腺高危病灶的诊断疗效。结果 DBT敏感度、特异度、阳性预测值、阴性预测值均高于MR波谱;DBT漏诊率、误诊率均低于MR波谱漏诊率、误诊率,2组诊断方法比较(P<0.05)。MR波谱及DBT对≥2 cm恶性病变的病理诊断结果符合率比较,差异均无统计学意义(P>0.05)。MR波谱及DBT对乳腺良性病变、恶性病变<2 cm诊断结果符合率比较,DBT对乳腺高危患者的病理诊断结果符合率高于MR波谱(P<0.05)。结论 乳腺高危病灶诊断中选择DBT可对乳腺高危病灶诊断中做出准确的分析和判断,对乳腺高危病灶诊断更具有应用价值,值得临床采纳。
Objective To explore the application of digital breast tomosynthesis (DBT) combined with MR wave in the diagnosis of high-risk breast lesions. Methods A total of 100 patients with breast high-risk lesions diagnosed as BI-RADS 4 or above by mammography in our hospital from June 2020 to June 2021 were selected as the research objects. The selected patients were examined by MR spectrum and DBT. The signs of MR spectrum and the photographic characteristics of DBT breast lesions were analyzed. Based on the pathological results, the diagnostic efficacy of MR spectrum, DBT and their combination in the diagnosis of breast high-risk lesions was evaluated. Results The sensitivity, specificity, positive predictive value and negative predictive value of DBT were higher than those of MR spectrum; the missed diagnosis rate and misdiagnosis rate of DBT were lower than those of MR spectrum (P<0.05). There was no significant difference in the coincidence rate of MR spectrum and DBT in the pathological diagnosis of malignant lesions ≥ 2 cm (P>0.05). The coincidence rate of MR spectrum and DBT in the diagnosis of benign and malignant breast lesions<2 cm was higher than that of MR spectrum (P<0.05). Conclusions Selecting DBT in the diagnosis of breast high-risk lesions can make accurate analysis and judgment in the diagnosis of breast high-risk lesions. It has higher application value in the diagnosis of breast high-risk lesions and is worthy of clinical adoption.

纳米材料在肾脏中的应用与清除机制

纳米材料作为一种新兴材料,在疾病诊断与治疗中拥有极大的应用潜力。为平衡纳米材料的生物活性和生物毒性之间的关系,清楚了解纳米材料在体内的清除机制和清除速率至关重要。其中,肾脏,作为人体重要的清除器官,直接影响纳米材料从血液清除到尿液的速率,进而影响其在生物体内的血液循环、肿瘤靶向效率和体内滞留等系列问题,最终影响其临床应用。基于近年来已报道的关于纳米材料与肾脏相互作用的诸多研究,本文系统介绍了肾脏结构、纳米材料在肾脏中的传输机制、理化性质对其肾脏清除效率的影响以及纳米材料在肾脏疾病诊疗中的应用。本文有望为精准纳米医学的发展以及纳米材料未来的临床转化起到促进作用。
Nanomaterials, as emerging materials, show great promise for the detection, diagnosis and treatment of diseases. Fully understanding their elimination pathway and clearance efficiency is important to better balance the bioactivity and biotoxicity of nanomaterials. Kidneys, as a major organ, play a key role in the clearance of nanomaterials from blood to urine. The clearance rate of nanomaterials in the kidneys directly affects their blood circulation, retention in the body and tumor targeting efficiency as well as final clinical applications. Based on current understanding of nano-bio interactions of nanomaterials in the kidneys, in this review, we introduced the kidney structure and elimination mechanism of nanomaterials in the kidneys. Then, how the physicochemical properties of nanomaterials affect their renal clearance efficiency are summarized. Finally, their applications in the kidney disease detection and treatment are presented. The basic understanding of nano-bio interactions of nanomaterials in the kidneys would promote the development of precise nanomedicine and their future clinical translation.
