目的 系统比较小针刀、局部浸润麻醉及两者联合疗法治疗腰椎间盘突出症(LDH)的临床疗效。方法 采用前瞻性随机对照设计,选取2023年1月—2025年1月收治的120例保守治疗无效的LDH患者,随机分为小针刀组(A组,n=40)、局部浸润麻醉组(B组,n=40)和联合治疗组(C组,n=40)。A组实施标准化小针刀松解术,采用“三点九刀”布点法松解粘连组织;B组采用超声引导下椎间孔外口及关节突关节囊周围利多卡因联合曲安奈德浸润麻醉;C组实施“麻醉-松解序贯疗法”(即先B组方案,后A组方案)。比较三组治疗前后视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)、腰椎活动度(ROM)、表面肌电图(sEMG)、疼痛压力阈值(PPT)及改良Macnab优良率。结果 治疗后,三组VAS、ODI评分均较治疗前下降(P<0.001)。组间比较显示,C组VAS(2.08±0.90分)、ODI(14.65±3.70分)改善显著优于A组(3.80±1.20分,21.00±4.25分)和B组(3.93±1.15分,22.10±4.65分)(均P<0.05),A、B两组间无统计学差异(P>0.05)。C组在腰椎前屈ROM(67.78±9.18°)、竖脊肌静息sEMG比值(0.19±0.05)、腰椎旁PPT(4.60±0.91 kg/cm2)上亦显著优于A、B两组(均P<0.05)。改良Macnab评价显示C组优良率达85.00%(34/40),高于A组的52.50%(21/40)和B组的55.00%(22/40)(P<0.05)。结论 小针刀与局部浸润麻醉单一疗法对LDH均有效,但联合应用可产生多模式协同效应,通过阻断疼痛传导、调节炎性微环境和改善生物力学失衡,在缓解疼痛、促进功能恢复及改善客观生理指标方面均展现出显著优势,为中西医结合治疗LDH提供了更优化的创新性方案,具有重要临床推广价值。
Objective To systematically evaluate and compare the clinical effectiveness and mechanistic actions of acupotomy,local infiltration anesthesia,and their combination in treating lumbar disc herniation(LDH).Methods This prospective,randomized controlled trial included 120 LDH patients unresponsive to conservative management,admitted between January 2023 and January 2025.Participants were randomly allocated to three groups:the Acupotomy Group(Group A,n=40)receiving standardized acupotomy release with a “three-point nine-needle” technique to address adhesions;the Local Infiltration Anesthesia Group(Group B,n=40)receiving ultrasound-guided infiltration of lidocaine combined with triamcinolone acetonide around the extraforaminal outlet and facet joint capsule;and the Combined Therapy Group(Group C,n=40)undergoing a sequential “anesthesia-release” protocol(first applied Group B therapy then Group A therapy).Assessments conducted before and after treatment included the Visual Analog Scale(VAS),Oswestry Disability Index(ODI),lumbar range of motion(ROM),surface electromyography(sEMG),pain pressure threshold(PPT),and the modified Macnab criteria.Results Following treatment,significant reductions from baseline were observed in VAS and ODI scores across all three groups(P<0.001).Between-group comparisons indicated that Group C achieved significantly greater improvement in VAS(2.08±0.90)and ODI scores(14.65±3.70)compared to Group A(3.80±1.20;21.00±4.25)and Group B(3.93±1.15;22.10±4.65)(all P<0.05),with no significant difference between Groups A and B(P>0.05).Group C also showed superior outcomes in lumbar flexion ROM(67.78±9.18°),the resting sEMG amplitude ratio of the erector spinae muscles(0.19±0.05),and paravertebral PPT(4.60±0.91 kg/cm2)compared to the other two groups(all P<0.05).Based on the modified Macnab criteria,the excellent-good rate was 85.00%(34/40)in Group C,significantly higher than the rates of 52.50%(21/40)in Group A and 55.00%(22/40)in Group B(P<0.05).Conclusions While both acupotomy and local infiltration anesthesia as standalone treatments are effective for LDH,their combined application produces a multimodal synergistic effect.This integrated strategy,which concurrently interrupts pain signaling,modulates the local inflammatory milieu,and ameliorates biomechanical dysfunction,demonstrates significant advantages in pain relief,functional recovery,and normalization of objective physiological measures.The findings present an optimized and novel integrative approach for LDH management,highlighting its considerable potential for broader clinical adoption.
目的 探究血脂异常对左心室结构的影响及其与微循环损伤之间的关联。方法 将20只新西兰大白兔随机划分为血脂异常组和正常对照组。饲养8周后获取所有实验兔的离体心肌组织,测量左心室心肌结构评估指标(包括心肌质量、心腔内径大小以及室壁厚度)和病理学指标(心肌微血管密度和胶原体积百分比);另外,通过对心肌组织氧化应激标记物的测定,评估实验兔心肌组织中一氧化氮(NO)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)以及丙二醛(MDA)等生化指标。结果 对于左心室结构指标,血脂异常组的左心室心肌质量及室间隔厚度较正常对照组显著增加(P均<0.001);对于组织病理学指标,血脂异常组的心肌微血管密度显著降低,而胶原体积百分比则显著升高(P均<0.001)。对于生化指标,血脂异常组NO、SOD以及GSH-Px水平均显著降低,MDA水平显著升高(P均<0.001)。室间隔厚度与心肌组织胶原体积百分比、MDA水平呈正相关,与心肌组织微血管密度、NO、SOD、以及GSH-Px水平呈负相关(P均<0.001)。与正常对照组对比,血脂异常组心肌组织出现明显脂肪变性、炎细胞浸润以及心肌纤维组织增生、排列紊乱。结论 血脂异常可导致新西兰大白兔左心室不良重塑,且与心肌微循环损伤之间存在密切关联,这提示血脂异常早期防控可能有助于改善心血管事件不良预后。
Objective To explore the impact of dyslipidemia on left ventricular(LV) structure and its association with myocardial microcirculatory injury.Methods Twenty New Zealand white rabbits were randomly divided into the dyslipidemia group and the normal control group.After 8 weeks of feeding,the isolated myocardial tissues of all experimental rabbits were obtained.The LV myocardial structure assessment indicators(including myocardial mass,heart cavity diameter and ventricular wall thickness) and pathological indicators(myocardial microvascular density and collagen volume fraction[CVF]) were measured.Additionally,the biochemical indicators such as nitric oxide(NO),superoxide dismutase(SOD),glutathione peroxidase(GSH-Px) and malondialdehyde(MDA) in the myocardial tissues of the experimental rabbits were evaluated by measuring the oxidative stress markers in the myocardial tissues.Results For the LV structural indicators,the myocardial mass and interventricular septal thickness in the dyslipidemia group were significantly increased compared with the normal control group(P<0.001 for both).For the histopathological indicators,the myocardial microvascular density in the dyslipidemia group was significantly decreased,while the CVF was significantly increased(P<0.001 for both).For the biochemical indicators,the levels of NO,SOD,and GSH-Px in the dyslipidemia group were significantly decreased,while the MDA level was significantly increased(P<0.001 for all).The interventricular septal thickness was positively correlated with the CVF and MDA level in myocardial tissue,and negatively correlated with the myocardial microvascular density,NO,SOD,and GSH-Px levels(P<0.001 for all).Compared with the normal control group,the myocardial tissue in the dyslipidemia group showed obvious fatty degeneration and inflammatory cell infiltration,as well as myocardial fibrous tissue hyperplasia and disordered arrangement.Conclusions Dyslipidemia can lead to adverse LV remodeling in New Zealand white rabbits and is closely related to myocardial microcirculatory injury.This suggests that early prevention and control of dyslipidemia may help improve the poor prognosis of cardiovascular events.
目的 分析肺结核合并Ⅱ型呼吸衰竭患者经鼻高流量湿化氧疗期间应用多维度强化护理的效果。方法 纳入2022年12月—2025年1月在河北省胸科医院进行经鼻高流量湿化氧疗的肺结核合并Ⅱ型呼吸衰竭患者294例,随机分为强化护理组(147例,多维度强化护理)、对照组(147例,常规护理),测量比较两组患者在护理前后气道重塑指标、肺功能指标、血气分析指标,进行疗效评估。结果 护理后强化护理组气道壁面积、气道壁厚度低于对照组,气道腔面积高于对照组(P<0.05);护理后强化护理组第1秒用力呼气容积、用力肺活量高于对照组(P<0.05);护理后强化护理组动脉血氧分压高于对照组,动脉血二氧化碳分压低于对照组(P<0.05);强化护理组总有效率高于对照组(P<0.05)。结论 肺结核合并Ⅱ型呼吸衰竭患者经鼻高流量湿化氧疗期间接受多维度强化护理可改善其气道重塑及肺功能,有利于血气分析指标恢复,应用效果显著。
Objective To analyze the efficacy of multidimensional enhanced nursing during nasal high-flow humidified oxygen therapy in patients with pulmonary tuberculosis and type Ⅱ respiratory failure.Methods A total of 294 patients with pulmonary tuberculosis and type Ⅱ respiratory failure who received nasal high-flow humidified oxygen therapy at Hebei Chest Hospital between December 2022 and January 2025 were enrolled and randomly divided into an enhanced nursing group(147 patients,receiving multidimensional enhanced nursing)and a control group(147 patients,receiving conventional nursing).Airway remodeling,pulmonary function,and blood gas analysis were measured and compared between the two groups before and after nursing,and the efficacy was evaluated.Results After nursing,the enhanced nursing group had lower airway wall area and airway wall thickness,and higher airway lumen area than the control group(P<0.05).The forced expiratory volume in 1 second and forced vital capacity in the enhanced nursing group were higher than those in the control group(P<0.05).The blood oxygen partial pressure(PaO2)in the enhanced nursing group was higher than that in the control group,and the carbon dioxide partial pressure(PaCO2)was lower than that in the control group(P<0.05).The total effective rate in the enhanced nursing group was higher than that in the control group(P<0.05).Conclusions Multidimensional enhanced nursing during nasal high-flow humidified oxygen therapy can improve airway remodeling and lung function in patients with pulmonary tuberculosis and type Ⅱ respiratory failure,and is beneficial for the recovery of blood gas analysis indicators.The application effect is significant.
目的 探讨预后营养指数(PNI)对脓毒症合并心力衰竭患者发生谵妄的预测价值,并评估其在指导护理风险分层中的作用。方法 采用回顾性队列研究,纳入2022年2月—2024年9月广东省第二中医院收治的162例脓毒症合并心力衰竭患者。通过ROC曲线确定PNI预测谵妄的最佳截断值,并以此将患者分为高低PNI组。比较两组基线特征,采用Kaplan-Meier曲线分析谵妄累积发生率和28天院内死亡率,通过Cox比例风险模型评估PNI与谵妄及预后的独立关联,并利用ROC曲线评价其预测性能。最后利用亚组分析技术进一步评估PNI预测价值的普适性。结果 低PNI(≤35.72)是谵妄发生的独立危险因素。竞争风险模型显示,低PNI组10 d谵妄累积发生率达37.8%,高于高PNI组的10.0%。多因素分析表明,PNI每降低一个单位,谵妄风险增加约17.2%。限制性立方样条分析提示PNI与谵妄风险呈线性负相关。ROC曲线分析显示PNI预测谵妄的AUC为0.721,其预测效能优于白蛋白和SOFA评分。亚组分析进一步证实PNI的保护效应在不同临床特征患者中具有一致性。结论 PNI是脓毒症合并心力衰竭患者谵妄风险的独立预测指标,其预测效能优于传统单一指标。基于PNI的风险分层有助于早期识别谵妄高危患者,为实现精准护理干预提供重要依据。
Objective To explore the predictive value of the prognostic nutritional index(PNI)for delirium in patients with sepsis complicated by heart failure,and to evaluate its role in guiding nursing risk stratification.Methods A retrospective cohort study was conducted,including 162 patients with sepsis and heart failure admitted to Guangdong Provincial Second Hospital of Traditional Chinese Medicine between February 2022 and September 2024.The optimal PNI cut-off value for predicting delirium was determined using ROC curve analysis,and patients were divided into high and low PNI groups accordingly.Baseline characteristics of the two groups were compared.Kaplan-Meier curves were used to analyze the cumulative incidence of delirium and 28-day in-hospital mortality.Cox proportional hazards models were employed to assess the independent association between PNI and delirium as well as prognosis,and ROC curves were used to evaluate its predictive performance.Subgroup analysis was further utilized to assess the generalizability of PNI’s predictive value.Results A low PNI(≤35.72)is an independent risk factor for delirium.The competing risk model showed that the cumulative incidence of delirium in the low PNI group was 37.8% at 10 days,significantly higher than the 10.0% in the high PNI group.Multivariate analysis indicated that for each unit decrease in PNI,the risk of delirium increased by 17.2% approximately.Restricted cubic spline analysis suggested a linear negative correlation between PNI and delirium risk.ROC curve analysis demonstrated that the AUC of PNI for predicting delirium was 0.721,with its predictive performance surpassing that of albumin and SOFA score.Subgroup analysis further confirmed that the protective effect of PNI was consistent across patients with different clinical characteristics.Conclusions PNI is an independent predictor of delirium risk in patients with sepsis and heart failure,with superior predictive performance compared to traditional single indicators.Risk stratification based on PNI aids in the early identification of patients at high risk for delirium,providing an important basis for precise nursing interventions.
淋证是临床常见病证,病情易迁延、反复,现代医学以抗生素治疗为主,但面临耐药与易复发等困境,严重影响患者生活质量。中医认为,其核心病机在于“大气失转,枢机不利”,导致三焦水道壅滞,肾与膀胱气化功能失常,病位虽在下焦,实则与肺、肝、胆、心、脾、肾等多脏腑功能密切相关。治疗应以“疏利少阳,运转大气”为基本治则,以小柴胡汤为基础方和解枢机,并随证配伍清热利湿、温肾化气、健脾固本等法,旨在恢复三焦气化与水液代谢功能,以达到“大气复转,水道自通”之效。该治法体现了中医从整体出发、标本兼治的辨证思想,为淋证的临床诊治提供参考。
Strangury is a common clinical disease characterized by its persistent and recurrent course.Modern medicine primarily employs antibiotic therapy,yet challenges such as drug resistance and high recurrence rates remain,significantly affecting patients’ quality of life.In traditional Chinese medicine(TCM),its core pathogenesis involves “impaired movement of the Great Qi and obstruction of the pivotal mechanism”,which leads to stagnation of the Sanjiao water passages and dysfunction of qi transformation in the kidney and bladder.Although the disease is located in the lower Jiao,it is closely related to dysfunction of multiple organs,including the lung,liver,gallbladder,heart,spleen,and kidney.The treatment should follow the principle of “soothing and promoting Shaoyang,circulating Great Qi.” Xiaochaihu Decoction is used as the basic formula to harmonize the pivot mechanism,supplemented with therapies such as clearing heat and draining dampness,warming the kidney to promote qi transformation,and strengthening the spleen to consolidate the root,based on syndrome differentiation.This aims to restore the qi transformation function of the Sanjiao and water metabolism,thereby achieving the therapeutic goal of “restoring the movement of Great Qi and freeing the water passages.” This approach reflects TCM’s holistic concept and its dialectical thinking of addressing both root and branch,providing a reference for the clinical diagnosis and treatment of strangury.
脑卒中后认知障碍(PSCI)是脑卒中患者常见的并发症,严重影响患者的生活质量和社会功能。近年来,随着认知储备理论的发展和早期干预理念的普及,PSCI的早期干预模式逐渐成为研究热点。文章综述了PSCI早期干预模式的研究现状与进展,包括干预的理论基础、具体措施及未来发展方向,以期为临床实践和科研提供参考。
Post-stroke cognitive impairment(PSCI)is a common complication among stroke patients.It seriously affects the patients’ quality of life and social functions.In recent years,with the development of the cognitive reserve theory and the popularization of the concept of early intervention,the early intervention model for PSCI has gradually become a research hotspot.This article reviews the current research status and progress of the early intervention model for PSCI,including the theoretical basis of the intervention,specific measures,and future development directions,aiming to provide references for clinical practice and scientific research.
目的 汇总分析肝硬化患者消化道出血风险预测模型,为今后模型的建立和优化提供参考。方法 系统检索中国知网、维普、PubMed数据库在2025年4月22日前公开发表的所有肝硬化患者消化道出血风险预测模型,按纳入标准筛选文献,对最终纳入文章分析摘录并系统汇总,包括模型特征、危险因素及模型预测评估效果等信息。结果 共检索3 603篇预测模型相关研究论文,最终纳入30篇,其中中国27篇、韩国1篇、印度1篇、埃及1篇。22项研究收集了肝硬化病因,其中病毒性肝病最多(72.94%,2 922/4 006),药物性肝病及非酒精性脂肪性肝病最少(均为0.02%,1/4 006)。在研究类型上,有28篇单中心研究,2篇为多中心研究,其中有12个模型未进行验证,只有1个模型进行了外部验证,其余模型只进行了内部验证,曲线下面积(AUC)范围0.680~0.994。根据模型纳入因素特点,分为血常规指标、凝血指标、生化指标、影像学指标、复合指标、其他指标共6种,其中纳入因素最多为影像学指标,最少为凝血指标。在纳入危险因素中,第1位为门静脉直径,第2位为血小板计数,第3位为血红蛋白水平及脾脏硬度,所有因素中与脾脏相关的指标最多。结论 肝硬化患者消化道出血风险预测模型研究质量有待提升,影像学指标应用最广,脾脏相关指标重要性突出,门静脉直径、血小板计数、血红蛋白水平及脾脏硬度为最常用的危险预测因素。
Objective To summarize and analyze the prediction models for gastrointestinal bleeding risk in patients with cirrhosis,providing references for the establishment and optimization of future models.Methods A systematic search was conducted in CNKI,VIP,and PubMed for all published prediction models for gastrointestinal bleeding risk in patients with cirrhosis before April 22,2025.Articles were screened according to the inclusion criteria,and the finally included articles were analyzed and summarized,including model characteristics,risk factors,and model prediction evaluation effects.Results A total of 3 603 related research papers on prediction models were initially retrieved,and 30 were finally included,with 27 from China,one from South Korea,one from India,and one from Egypt.Among the 22 studies that collected the etiology of cirrhosis,viral hepatitis was the most common(72.94%,2 922/4 006),while drug-induced liver disease and non-alcoholic fatty liver disease were the least common(0.02%,1/4 006).In terms of study type,28 were single-center studies and two were multicenter studies.Among them,12 models were not validated,only one model was externally validated,and the rest were only internally validated,with an area under the curve range of 0.680-0.994.According to the characteristics of the factors included in the models,they were divided into six types of indicators:blood routine,coagulation,biochemistry,imaging,composite,and others,among which imaging indicators were the most common and coagulation indicators were the least.In the included risk factors,the first was portal vein diameter,the second was platelets count,and the third was hemoglobin level and spleen stiffness,with the most factors related to the spleen.Conclusions The quality of studies on prediction models for gastrointestinal bleeding risk in cirrhosis patients needs to be improved.Imaging indicators are the most widely used,and spleen-related indicators are of prominent importance,with portal vein diameter,platelets count,hemoglobin level,and spleen stiffness being the most commonly used risk prediction factors.
目的 探讨孕中晚期口腔二维轴向切面超声在产前诊断胎儿唇腭裂中的应用价值,评估其对唇裂、唇腭裂及继发腭裂的检出率和准确性。方法 本研究为前瞻性研究,共纳入2020年3月—2023年12月在我院超声科行产前超声诊断的3 666例孕中晚期胎儿,其中经超声检查诊断出111例唇腭裂,这些病例归为唇腭裂组,其余3 555例归入正常对照组。所有受试者均通过口腔二维轴向切面超声进行评估,诊断结果经产后随访确认。结果 在正常对照组中,上唇及上牙槽突的评估成功率为100%,软硬腭分界线显示率为99%,软腭显示率为85%。在唇腭裂组中,超声对唇裂、唇腭裂及继发腭裂的检出率分别为45%、48%和7%,且超声诊断的灵敏度、特异度和预测值均超过98%。结论 孕中晚期口腔二维轴向切面超声在产前诊断胎儿唇腭裂中具有较高的准确性和可靠性,为早期干预和临床管理提供了重要的影像学依据。
Objective To explore the application value of two-dimensional axial section ultrasound in prenatal diagnosis of fetal cleft lip and palate in the second and third trimesters of pregnancy,and to evaluate its detection rate and accuracy for cleft lip,cleft lip and palate,and secondary cleft palate.Methods This study is a prospective study that included 3 666 fetuses in the second and third trimesters of pregnancy.Among them,111 cases of cleft lip and palate were diagnosed by ultrasound examination.These cases were classified as the cleft lip and palate group,and the remaining 3 555 cases were classified as the normal control group.All subjects were evaluated by oral two-dimensional axial section ultrasound,and the diagnostic results were confirmed by postpartum follow-up.Results In the normal control group,the success rates of evaluating the upper lip and upper alveolar process were 100%.The present rate of the soft hard palate boundary was 99%,and the present rate of the soft palate was 85%.In the cleft lip and palate group,the detection rates of cleft lip,cleft lip and palate,and secondary cleft palate by ultrasound were 45%,48% and 7% respectively,and the sensitivity,specificity,and predictive value of ultrasound diagnosis exceeded 98%.Conclusions Two dimensional axial section ultrasound of the oral cavity in second and third trimesters of pregnancy has high accuracy and reliability in prenatal diagnosis of fetal cleft lip and palate,providing strong imaging evidence for early intervention and clinical management.
目的 通过机器学习方法构建脓毒症谵妄患者30 d死亡的预测模型,并识别关键预测因子。方法 采用基于医疗信息集成重症监护数据库(Medical Information Mart for Intensive Care IV)的回顾性队列研究方法,boruta筛选重要特征,并通过决策树,K近邻,LightGBM,随机森林,支持向量机,XGBoost构建模型进行分析,通过ROC曲线下面积进行评估,利用F1分数、召回率、精确率、特异度、灵敏度和阳性预测值比较模型表现。结果 XGBoost模型在训练集和验证集中的ROC曲线下面积分别为0.906和0.762,表明该模型具有良好的预测能力,入院年龄、红细胞分布宽度和白细胞计数是最重要的预测因子。结论 基于机器学习的脓毒症谵妄患者预后预测模型展现出良好的预测效能,为临床早期干预提供了重要参考依据。
Objective To construct a 30-day mortality prediction model for patients with sepsis-associated delirium using machine learning methods and identify key predictive factors.Methods A retrospective cohort study was conducted based on the Medical Information Mart for Intensive Care IV database.Important features were selected using the Boruta algorithm,and models including Decision Tree,K-Nearest Neighbors,LightGBM,Random Forest,Support Vector Machine,and XGBoost were constructed and analyzed.Model performance was evaluated using the area under the reciver operater characteristic(ROC)curve(AUC),along with F1 score,recall,precision,specificity,sensitivity,and positive predictive value.Results The XGBoost model demonstrated strong predictive performance,with AUC values of 0.906 in the training set and 0.762 in the test set.Key predictors identified included admission age,red blood cell distribution width,and white blood cell count.Conclusions The machine learning-based prediction model for sepsis-associated delirium prognosis exhibits robust predictive efficacy,providing a valuable tool for early clinical intervention.
目的 探讨长链非编码核糖核酸肺腺癌转移相关转录本 1(LncMALAT1)通过竞争性结合微小RNA-506-3p(miR-506-3p)调控Zeste同源物增强子2(EZH2)影响膀胱癌增殖的机制。方法 收集2023年1月—2024年10月的92例外科手术切除的膀胱癌组织及对应的癌旁组织标本, 利用Western blot和定量实时逆转录聚合酶链式反应(qRT-PCR)方法检测LncMALAT1和EZH2的表达情况。根据患者预后分为不良组(n=34)和良好组(n=58), 收集患者的性别、年龄、肿瘤直径、血管侵袭情况、TNM分期、远处转移情况等临床指标, 结合临床病理指标分析LncMALAT1和EZH2与膀胱癌患者预后的关系。通过体外实验,包括qRT-PCR、Western blot、平板克隆和EdU实验,验证LncMALAT1对EZH2表达和膀胱癌细胞增殖的影响。利用生物信息学技术预测LncMALAT1与miR-506-3p的相互作用,并通过qRT-PCR验证在膀胱癌细胞中上调LncMALAT1表达后miR-506-3p的表达变化。结果 单因素结果显示, 血管侵袭情况、TNM分期、远处转移情况、LncMALAT1及EZH2表达水平均与膀胱癌患者预后不良有关, 差异有统计学意义(均P<0.05)。分析结果发现LncMALAT1与EZH2在膀胱癌组织中的表达呈正相关。体外实验结果显示, 上调LncMALAT1表达后, EZH2的表达显著上调, 且膀胱癌细胞的增殖能力显著提高(均P<0.05)。qRT-PCR验证表明,上调LncMALAT1表达后,miR-506-3p的表达显著下调(P<0.05), 提示LncMALAT1通过竞争性结合miR-506-3p调控EZH2,进而影响膀胱癌细胞的增殖进展。结论 LncMALAT1通过竞争性结合miR-506-3p调控EZH2促进膀胱癌增殖功能,进而加快膀胱癌细胞的增殖进展, 可为膀胱癌的治疗提供新的潜在靶点。
Objective To explore the mechanism of long non-coding ribonucleic acid metastasis - associated lung adenocarcinoma transcript 1(LncMALAT1)regulating enhancer of Zeste homolog 2(EZH2)through competitive combination with microRNA-506-3p(miR-506-3p)to affect the proliferation of bladder cancer.Methods A total of 92 pairs of bladder cancer tissues and corresponding adjacent normal tissues were collected from surgical resections between January 2023 and October 2024.The expression levels of LncMALAT1 and EZH2 were detected using Western blot and qRT-PCR.The patients were divided into poor group(n=34)and good group(n=58)according to their prognosis.Clinical data, such as gender, age, tumor diameter, vascular invasion, TNM stage, and distant metastasis were collected, and the relationship between LncMALAT1 and EZH2 and the prognosis of bladder cancer patients was analyzed with clinical pathological indicators.Through in vitro experiments, including qRT-PCR Western blot, plate cloning and EdU experiment were conducted to verify the effect of LncMALAT1 on EZH2 expression and bladder cancer cell proliferation.Bioinformatics technology was used to predict the interaction between LncMALAT1 and miR-506-3p, and qRT-PCR was used to verify the change of miR-506-3p expression after up regulating LncMALAT1 expression in bladder cancer cells.Results The univariate results showed that vascular invasion, TNM stage, distant metastasis, LncMALAT1 and EZH2 expression levels were related to the poor prognosis of bladder cancer patients, and the difference was statistically significant(all P<0.05).The results showed that the expression of LncMALAT1 and EZH2 in bladder cancer was positively correlated.In vitro experiment results showed that after up regulating LncMALAT1 expression, EZH2 expression was significantly up-regulated, and the proliferation ability of bladder cancer cells was significantly improved(all P<0.05).QRT-PCR validation showed that the expression of miR-506-3p was significantly down regulated after the expression of LncMALAT1 was up-regulated(P<0.05), suggesting that LncMALAT1 could regulate EZH2 through competitive combination with miR-506-3p, thereby affecting the proliferation and progression of bladder cancer cells.Conclusions LncMALAT1 can promote the proliferation of bladder cancer cells by competitively combining with miR-506-3p to regulate EZH2, and then accelerate the proliferation of bladder cancer cells, which can provide a new potential target for the treatment of bladder cancer.
