64层螺旋CT三维重建技术在主动脉夹层介入术前评估中的价值

目的 探讨64层螺旋CT三维重建技术在主动脉夹层介入术前评估中的价值。方法 选择2023年7月—2024年5月在潜江市中心医院诊治的主动脉夹层疑似患者96例为研究对象,所有患者均进行64层螺旋CT三维重建技术检查, 记录成像参数。所有患者在64层螺旋CT三维重建后进行介入治疗, 以介入检查结果为金标准, 判断64层螺旋CT三维重建技术在主动脉夹层介入术前评估中的价值。结果 在96例患者中, 64层螺旋CT三维重建联合判断为主动脉夹层60例, 其中Ⅰ型32例、Ⅱ型20例、Ⅲ型8例,其中再现(VR)、最大密度投影(MIP)及多平面重建(MPR)分别判断为主动脉夹层56例、54例、56例。在96例患者中, 64层螺旋CT三维重建检查的破口层面主动脉平均直径(3.08±0.25)cm、破口至左锁骨下平均距离(4.05±0.26)cm, 与介入治疗检查(3.05±0.36)cm、(4.06±0.14)cm, 对比差异无统计学意义(t=0.671, P=0.503; t=0.332, P=0.740)。64层螺旋CT三维重建联合、VR、MIP、MPR对主动脉夹层的诊断灵敏度分别为98.33%、93.22%、89.83%、94.92%, 特异度分别为97.30%、97.30%、97.30%、100.00%, 准确率分别为98.96%、94.79%、92.71%、96.88%。结论 64层螺旋CT三维重建技术在主动脉夹层介入术前评估中具有很高的灵敏度、特异度与准确率。
Objective To explore and analyze the value of 64-slice spiral CT three-dimensional reconstruction technology in the preoperative assessment of aortic dissection(AD)intervention.Methods From July 2023 to May 2024, 96 cases of patients suspected of aortic dissection treated in our hospital were selected as the research subjects.All patients underwent 64-slice spiral CT three-dimensional reconstruction technology examination, and imaging parameters were recorded.After the 64-slice spiral CT three-dimensional reconstruction, all patients were given interventional treatment, and the results of the interventional examination were used as the “gold standard” to evaluate the value of 64-slice spiral CT three-dimensional reconstruction technology in the preoperative assessment of aortic dissection intervention.Results Among the 96 patients,64-slice spiral CT three-dimensional reconstruction combined diagnosis identified 60 cases of aortic dissection,including 32 type I, 20 type II, and 8 type III cases.volume rendering(VR), maximum intensity projection(MIP), and muhiplanar reconstrudion(MPR) identified 56, 54, and 56 cases of aortic dissection,respectively.In 96 patients, the mean aortic diameter at the lacerated level and the mean distance from the lacerated to the left subclavicular level were(3.08±0.25)cm and(4.05±0.26)cm respectively in 64-slice spiral CT three-dimensional reconstruction examination, compared with(3.05±0.36)cm and(4.06±0.14)cm in interventional examination.There was no difference in comparison(t=0.671, P=0.503; t=0.332, P=0.740).The sensitivity, specificity, and accuracy of the 64-slice spiral CT three-dimensional reconstruction combined, VR, MIP, and MPR for the diagnosis of aortic dissection were 98.33%, 93.22%, 89.83%, 94.92%, specificity were 97.30%, 97.30%, 97.30%, 100%, accuracy were 98.96%, 94.79%, 92.71%, 96.88%.Conclusions Technology of 64-slice spiral CT three-dimensional reconstruction has high sensitivity, specificity, and accuracy in the preoperative assessment of aortic dissection intervention and is worth promoting and applying clinically.

软组织重建治疗儿童蟹钳指畸形的方法研究

目的 探讨软组织重建治疗儿童蟹钳指畸形的临床效果。方法 2019年1月—2022年12月对25例蟹钳指畸形或蟹钳指畸形术后出现“Z”形畸形患儿进行软组织重建治疗,观察术后1年Tada评分、并发症发生情况。结果 术后1年,25例患儿的Tada评分优16例(64%),良8例(32%),差1例(4%)。术后1年Peds QL评分(92.74±9.52)高于术前(79.25±7.43),比较差异有统计学意义(P<0.001)。指间关节伸直障碍、骨折愈合延迟各发生1例,并发症总发生率8%。结论 软组织重建治疗儿童蟹钳指畸形,患儿无需经历一期切除重复拇指,二期行截骨矫形手术或者切除多指的多次手术,只需一次手术就完成蟹钳指截骨矫形,具有治疗周期短、患儿痛苦少、家庭经济成本低,且能显著提升患儿生存质量,临床效果满意等优点。
Objective To investigate the clinical effect of soft tissue reconstruction in the treatment of crab-claw finger deformity in children.Methods From January 2019 to December 2022,soft tissue reconstruction was performed in 25 patients with crab-claw finger deformity or “Z-shaped” deformity after surgery,and Tada score and complications were observed one year after surgery.Results One year after surgery,16 of 25 children had excellent Tada scores(64%),8 of 25 had good scores(32%),and 1 had poor score(4%).The Peds QL score 1 year after operation(92.74±9.52)was significantly higher than that before operation(79.25±7.43),and the difference was statistically significant(P<0.001).Dysextension of interphalangeal joint and delayed fracture healing occurred in 1 case,the total complication rate was 8%.Conclusions In the treatment of crab-claw finger deformity and soft tissue reconstruction,children do not need to undergo one-stage resection of the repetitive thumb,second-stage osteotomy and orthopedic surgery or multiple operations to remove extra fingers,and just only one operation is needed to complete the osteotomy and orthopedic surgery of crab-claw finger,which has the advantages of short treatment cycle,less pain for children and low economic cost for families.It significantly improves the quality of life of children and has satisfactory clinical effects.

冷敷对前交叉韧带重建术后疼痛肿胀及关节活动度的影响

目的 探索冰袋冷敷对膝关节前交叉韧带重建术后关节疼痛肿胀和活动度的影响。方法 回顾性分析自2021年1月—2021年7月在我院因前交叉韧带损伤行前交叉韧带重建术的患者资料,将符合标准的71例行单侧前交叉韧带重建术的患者按住院号的单双号分成2组。其中,对照组38名患者,试验组33名患者。对照组38名患者术后不用冰袋冷敷;试验组33名患者术后采用冰袋冷敷。用视觉模拟评分表评估对照组和试验组患者术前、术后的疼痛,并测量患者的膝关节肿胀程度和活动度。结果 对照组和试验组患者术前视觉模拟评分比较,差异无统计学意义(P>0.05),试验组在术后6、24、48、72小时四个不同的时间点时疼痛视觉模拟评分均明显低于对照组,差异有统计学意义(P<0.001);对照组和试验组患者术前关节活动度比较,差异无统计学意义(P>0.05),试验组在术后4、5、6、7天四个不同的时间点时关节活动度优于对照组,差异有统计学意义(P<0.001);对照组和试验组患者术前膝关节肿胀程度比较,差异无统计学意义(P>0.05),试验组在术后1、2、3天三个不同的时间点时膝关节肿胀程度低于对照组,差异有统计学意义(P<0.001)。结论 冰袋冷敷能够有效减轻前交叉韧带重建术后膝关节疼痛肿胀并改善关节活动度。
Objective To explore the effect of ice pack cold compress on joint pain,swelling and motion after anterior cruciate ligament reconstruction. Methods The data of patients undergoing anterior cruciate ligament reconstruction due to anterior cruciate ligament injury in our hospital from January 2021 to July 2021 were retrospectively analyzed,and 71 patients who received unilateral anterior cruciate ligament reconstruction meeting the standards were divided into two groups according to the odd and even numbers of hospital admission. There were 38 patients in the control group and 33 patients in the experimental group. Thirty-eight patients in the control group did not use ice packs after surgery. Thirty-three patients in experimental group were treated with ice pack after operation. Visual analogue scale was used to evaluate the preoperative and postoperative pain of the control group and experimental group,and the degree of knee swelling and range of motion were measured. Results There was no significant difference in preoperative visual analogue scale between control group and experimental group (P>0.05). The scores of pain in the experimental group were significantly lower than that in the control group at 6,24,48 and 72 hours after surgery,with statistical significance (P<0.001). There was no significant difference in preoperative range of motion between control group and experimental group(P>0.05), the range of motion of the experimental group was significantly better than that of the control group at four different time points of 4,5,6 and 7 days after operation,with statistical significance(P<0.001). There was no significant difference in the degree of preoperative knee joint swelling between the control group and the experimental group(P>0.05),and the degree of knee joint swelling in the experimental group was lower than that in the control group on,1,2 and 3 days after surgery,three different time points, with statistically significant difference(P<0.001). Conclusions Ice pack cold compress can effectively reduce the pain and swelling of the knee joint after anterior cruciate ligament reconstruction and improve the range of motion of the joint.

关节镜下膝关节前交叉韧带损伤重建术治疗的临床研究

目的 探讨在关节镜下膝关节前交叉韧带损伤重建术治疗的临床效果。方法 选取2018年3月—2020年3月之间我院接受住院治疗的80名膝关节前交叉韧带损伤患者作为研究对象。根据手术方式的不同,常规手术进行交叉韧带损伤的重建术治疗为常规组,关节镜下膝关节前交叉韧带损伤重建术治疗为研究组,每组各40名患者,比对两组的治疗效果。结果 通过数据得出常规组的膝功能评分(71.8±12.6)及膝功能不稳评分(13.2±1.6)要低于研究组膝功能评分(83.9±12.9)及膝功能不稳评分(19.3±2.3),差异有统计学意义(P<0.05)。在术后疼痛评分上研究组(3.35±0.23)各项数据均优于常规组(4.35±1.69),差异有统计学意义(P<0.05)。研究组并发症发生率仅为5%,常规组并发症发生率有17.5%,研究组数据远要低于常规组,差异有统计学意义(χ<sup>2=5.178,P=0.023)。结论 在膝关节镜下前交叉韧带损伤重建术治疗可有效改善膝关节活动性、稳定性、有效减轻患者的疼痛感,减少并发症的发生,促进膝关节功能的康复。关节镜下进行膝关节前交叉韧带损伤重建术治疗具有临床推广和采纳的价值。
Objective To investigate the clinical effect of arthroscopic anterior cruciate ligament (ACL) reconstruction. Methods 80 patients with ACL injury in our hospital from March 2018 to March 2020 were selected as the research objects. According to the different surgical methods, patients underwent the traditional open surgery for cruciate ligament injury reconstruction treatment were selected as the control group,patients underwent arthroscopic anterior cruciate ligament injury reconstruction treatment were as the study group, 40 patients in each group, the treatment effect of the two groups were compared. Results The Lysholm score (71.8±12.6) and and Lysholm instability score (13.2±1.6) of the control group were lower than those of the study group (83.9±12.9 and 19.3±2.3), the differences were statistically significant (P <0.05). In the postoperative pain score, the study group (3.35±0.23) data was better than the control group (4.35±1.69), the difference was statistically significant (P <0.05). The incidence of complications in the study group was only 2.5%, and that in the control group was 17.5%, which was significantly lower than the control group, and the difference was statistically significant (χ2= 5.178, P= 0.023). Conclusion Arthroscopic anterior cruciate ligament reconstruction can effectively improve the activity and stability of knee joint, effectively reduce the pain of patients, reduce the occurrence of complications, and promote the rehabilitation of knee joint function. Arthroscopic anterior cruciate ligament reconstruction has the value of clinical promotion and adoption.

ADMIRE重建技术在肝脏CT扫描中的应用价值探讨

目的 探讨FORCE CT实时迭代ADMIRE重建技术在肝脏CT扫描中的应用价值。方法 临床疑肝病患者50例,行FORCE CT肝脏平扫后,分别采用FBP重建和ADMIRE-1~5级重建,比较6组图像的平均CT值、噪声、SNR、CNR及图像质量的主观评分及诊断效能等。结果 比较6组图像显示,肝实质平均CT值无统计学差异(P>0.05);噪声、SNR、CNR及主观评分间均有差异(P<0.05),ADMIRE-5重建噪声最小、SNR、CNR最大。图像主观评分ADMIRE-1和ADMIRE-5图像评分低于FBP,ADMIRE-2~4高于FBP,绝对评分最高为ADMIRE-3重建,但六种重建图像对病变诊断效能一致。结论 肝脏CT平扫结合FORCE CT 实时迭代ADMIRE重建技术能有效降低图像噪声及提高图像质量,具有潜在降低扫描剂量的作用。
Objective: To discuss the value of ADMIRE reconstruction technique in the liver plain CT scan by the comparison of displays to the liver separately by ADMIRE and traditional FBP reconstructions after DSCT scan. Methods: 48 patients who were clinically doubted to have hepatic lesions were scanned by DSCT in liver, then six groups of images for FBP and ADMIRE 1-5 on the average CT value, noise, signal to noise ratio ( SNR ), contrast to noise ratio (CNR) and subjective scoring on image quality were compared. Results: 6 groups of images were compared,and the average CT value of liver parenchyma has no statistical differences (P>0.05);The differences were statistically significant (P<0.05)on noise, SNR, CNR and subjective scoring,ADMIRE5 reconstruction has the least noise and the largest SNR and CNR.For the images of ADMIRE1 and ADMIRE5, the image subjective scoring is lower than that by FBP; for the images of ADMIRE2 and ADMIRE4, the image subjective scoring is higher than that by FBP; and ADMIRE-3 has the highest score.But for the rate of lesion detection, all the images were the same. Conclusion: ADMIRE reconstruction strength 3 makes the best image quality.ADMIRE reconstruction can decrease noise and improve SNR and CNR,as well as have a potential effect decreasing the scan dose.

多层螺旋三维CT重建引导下的改良Evans分型对老年脆骨性粗隆间骨折微创治疗的临床指导

目的 利用新的分型方法指导现有的微创技术下内固定的选择,提示预后,帮助制定术后康复计划。方法 将200例患者随机分成两组,一组为旧Evans分型组,另一组为改良Evans分型组,通过多层螺旋三维CT重建引导下改良Evans分型,对三个重点区域(股骨内距、头颈部及粗隆外侧入针点部位)的CT成像,利用多平面重组(MPR)、表面遮盖显示(SSD)及容积再现(VR)等多种后处理,分析骨折线走形、局部的微骨折、骨小梁分布、骨皮质厚度的骨折部内环境变化。简化整合到Evans分型中。来实现完善影像分型,有效地指导微创手术。对比两组患者的疗效。结果 新Evans分型组98例患者愈合良好,时间为9~17周,中位数为11.2周。其中伤口感染3例,髋关节内翻畸形3例,骨骨头坏死3例,按照髋关节治疗标准评分,优53例,良31例,可9例,差7例。优良率(包括优和良)为84%。旧Evans分型组80例患者愈合良好,时间为9~18周,中位数为11.8周。其中伤口感染10例,髋关节内翻畸形5例,股骨头坏死8例,按照髋关节治疗标准评分,优48例,良26例,可10例,差16例。优良率(包括优和良)为74%。两组对比,新Evans分型组优良率明显高于旧Evans分型组,差异有统计学意义。P<0.05。结论 多层螺旋三维CT重建引导下的改良Evans分型对老年脆骨性粗隆间骨折的微创治疗有重要的临床指导意义。

多层螺旋CT重建技术在儿童胸部复合型骨折诊断中的价值

The value of MSCT reconstruction in the diagnosis of children's chest complex fractures

目的 回顾性分析常规放射检查中儿童胸部复合型骨折漏诊的原因,并且探讨多层螺旋CT后处理技术在儿童胸部复合型骨折中的诊断价值。方法 搜集本院2012年1月—2015年8月间因外伤行胸部照片和CT扫描的外伤患儿共123例,男81例,女42例,年龄9个月~15岁,平均年龄4.3岁。所有病例在完成胸部照片后1~3 d,进行胸部容积CT扫描,并对数据进行三维重建后处理,其结果与常规DR检查结果相比较。结果 在123例患儿中,常规胸部DR片显示明确胸部骨折39例61处,其中胸部单纯性骨折24例,胸部复合型骨折15例。可疑骨折48例;36例未见明确骨折但存在胸部合并症。CT检查显示明确的胸部骨折76例,142处,其中胸部单纯性骨折42例,胸部复合型骨折34例。与胸部DR平片相比,CT显示胸部新增骨折有18处在单纯性骨折病例中,63处在复合型骨折病例中。CT重建技术对复合型骨折的检出率高于DR平片,差异有统计学意义(P<0.05)。结论 胸部CT容积扫描并综合运用三维重建技术处理,可以提高胸部复合型骨折的检出率,相对于常规放射照片更能提供详细和直观的信息,包括骨折部位、类型、程度以及邻近结构的损伤,并对治疗方案的选择及预后的评估有重要指导价值。
Objective To investigate the reasons for missed diagnoses of children's chest complex fractures in chest radiographs and to discuss the value of multi-slice spiral CT (MSCT) reconstruction in the diagnosis of children's chest complex fractures. Methods 123 children underwent chest digital radiography(DR) and MSCT scans after traumas were collected in our hospital between January 2012 and August 2015. This population consisted of 81 male and 42 female patients, with a mean age of 4.3 years(range from 9 months to 15 years). All participants underwent chest MSCT scans in 1-3 days after completing chest DR, and then we used 3D reconstruction to process the data and compared the results with those processed by DR. Results By chest DR tests we identified 39 cases (61 places) out of 123 participants with 24 simple fractures, 15 complex fractures. And 48 chest fractures were suspected. We also identified 36 cases had chest complications without fracture. By MSCT scans 76 cases (142 places) were identified, among which 42 were simple and 34 were complex. Compared with chest DR, MSCT scans can identified 18 more places of simple fractures and 63 more places of complex fractures. It was statistically significant higher detection rate of MSCT scan than DR. Conclusion Chest MSCT scan combined 3D reconstruction technology can increase the detection rate of chest complex fracture. Compared with DR, it provides more detailed and visualized information, including fracture position, type, severity and adjacent structure damage. It has important guiding value on the selection of therapy and prognosis evaluation.
