影响广州地区Rh阴性红细胞采供情况的原因

目的 通过回顾分析广州地区Rh阴性血的采供情况,了解Rh阴性红细胞的供应特点,更好地实施Rh阴性血液库存管理,保障输血安全。方法 统计2011—2017年广州血液中心Rh阴性血液采集量与供应量,血型及医院供血量的分布情况。结果 2011—2017年广州血液中心Rh阴性全血采集量为11 283 U,占总采血量的0.39%(11 283/2 902 485),供应总量为11 693 U,占红细胞类成分血总供应量的0.43%(11 693/2 762 229),其中悬浮红细胞供应比例逐年增加,冰冻解冻去甘油红细胞供应比例逐年下降,并从2014年起Rh阴性悬浮红细胞供应增长率明显高于血液采集增长率;4种血型供应量总体比较,差异有统计学意义(F=22.217,P<0.05),A、B型Rh阴性悬浮红细胞供应增长最快,增长率分别为137.3%和134.1%,医院分布以三级以上及综合医院为主。结论 应根据广州地区Rh阴性红细胞采供特点,来持续改进Rh阴性血源招募和采供策略。通过建立一支Rh阴性定期献血者队伍,加强Rh阴性血液统筹管理和临床科室-输血科-采供血机构的沟通,促进Rh阴性血液的供需平衡,保障Rh阴性患者输血安全。
Objective To understand the characteristics of Rh-negative red blood cells (RBC) supply, improve the management of storage and guarantee the safety of blood transfusion. The collection and supply of Rh-negative RBC in Guangzhou city was retrospectively analyzed. Methods The amount of Rh-negative RBC collection and supply, the distribution of ABO blood type and the consumption of blood by different hospitals in Guangzhou Blood Center from 2011 to 2017 were statistically analyzed. Results During 2011-2017, 11 283 U Rh-negative RBC was collected in Guangzhou Blood Center, accounting for 0.39% of total RBC collection (11 283/2 902 485); 1 693 U Rh-negative RBC was supplied, accounting for 0.43% of the total blood supply (11 693/2 762 229). The proportion of suspended RBC supply increased year by year, in contrast with the decease of proportion of frozen defrosteddeglyceal RBC supply. In addition, the growth rate of Rh-negative suspended RBC supply was higher than that of RBC collection since 2014 (F=217, P<0.05). A-and B-Rh negative suspended RBC supply increased, with growth rates of 137.3% and 134.1%, respectively. The Rh negative RBC was predominantly supplied to Grade III and general hospitals. Conclusion By establishing a team of Rh-negative regular blood donors, the strategies for recruitment, collection and supply of Rh-negative RBC should be continuously improved according to the characteristics of Rh-negative RBC collection and supply in Guangzhou. The coordinating management of Rh-negative, as well as the communication of clinical departments, departments of transfusion and blood banks should be improved. In addition, the balance between supply and demand of Rh-negative blood should be promoted, and the safety of blood transfusion for Rh-negative patients can be improved.

个性化专属纪念品招募方式效果分析和研究

目的 分析广州血液中心以“生日献血纪念章”(下称生日纪念章)个性化专属纪念品对献血者的招募保留效果进行分析和研究。方法 2017年—2019年期间,广州血液中心生日当天献血者共11 790名,每半年生日当天献血的献血者人数,绘制折线图,分析2018年7月起“生日纪念章”活动前后,生日当天献血的献血者人数变化;卡方检验对比2组献血人群特征差异,分析该活动敏感人群。结果 2017年—2019年每半年生日当天献血人数总体持续增长,其中2018年7—12月较2018年1—6月,生日当天献血人数增长了406%,增长比例最大。活动前后2组人群性别、年龄、职业及既往献血次数分布差异均有统计学意义(P<0.05)。活动后男性、35岁及以上献血者、工人/职员及其他职业,固定献血者比例增加。结论 广州血液中心的“生日纪念章”活动对生日当天献血有明显的促进效果,其中,男性,年龄35岁及以上,工人/职业/其他职业及固定献血者对该活动更敏感,更可能选择生日当天献血。今后针对女性、25岁以下、学生及初次献血者,注意增加其他招募保留措施。
Objective To analyze the effect of the “Birthday Blood Donation Medal” (hereinafter referred to as the Birthday Medal) personalized souvenir to the blood donor recruitment and retention effect of blood donors in Guangzhou Blood Center. Methods From 2017 to 2019, there were a total of 11 790 blood donors donated blood on their birthday in Guangzhou Blood Center. Aline chart was drawn of the number of blood donors who donated blood on birthday to observe the trends every half a year from 2017 to 2019; Chi-square test was used to compare the characteristics of blood donors in two groups (before and after the “Birthday Blood Donation Medal” activity) and we analyzed the sensitive population of the activity. Results The number of blood donors on the birthday every half year from 2017 to 2019 continued to increase. Among them, the number of blood donors donated on birthday on July-December 2018 increased by 406% compared to the first six months of 2018, which was the largest increase. The differences in gender, age, occupation, and previous blood donation times between the two groups were statistically significant (P<0.05). After the activity, the percentage of male, 35-year-old and above blood donors, workers/clerks and other occupations, and regular blood donors increased. Conclusion The “Birthday Blood Donation Medal” activity in Guangzhou Blood Center has a significant promotion effect on blood donation on birthday. Among them, males, aged 35 and above, workers/occupations/other occupations and regular blood donors are more sensitive to this activity and are more likely to donate blood on birthday. In the future, other recruitment and retention measures should be taken to pay attention to women, under 25 years old, students and first-time blood donors.

连续捐献机采血小板献血者外周血细胞计数变化情况研究

目的 探索连续捐献机采血小板献血者血小板、白细胞和红细胞计数变化情况。方法 以2016年1月1日—2018年9月30日年期间首次献血且连续血小板捐献量在10 U及以上的849人为研究对象进行回顾性研究,采用同一群体的配对t检验来评估第一次与最后一次血小板、红细胞及白细胞计数的变化情况。将采用有序多分类Logistic回归分析调查期间的血小板捐献量对献血者外周血细胞计数的影响。结果 配对t检验表明,外周血PLt有增加趋势(t=-8.58,P<0.001);白细胞总体来说有减少趋势(t=5.348,P<0.001);红细胞无改变趋势(t=0.515,P=0.607);有序多分类Logistic回归分析结果显示:PLt的变化值与献血者年龄、性别以及第一次与最后一次献血的间隔期无关系,P>0.05;但是与血小板捐献量41 U及以上比起来,血小板捐献量在≤30 U的献血者,血小板计数增加的可能性相对较少(血小板捐献量为10~20 U,χ2=13.737,P<0.001;血小板捐献量为21~30 U,χ2=7.491,P=0.006);WBC的变化值与献血者年龄、性别及献血间隔期无关,P>0.05,但是与血小板捐献量41 U及以上比起来,血小板捐献量在10~20 U的献血者,白细胞计数增加的可能性相对较大,(OR=1.720,95%CI=1.136~2.605,P=0.010) RBC的变化值与献血者年龄、性别无关(P>0.05);第一次与最后一次献血间隔期越长,红细胞计数增加的可能性就越大,(OR=1.005,95%CI=1.000~1.009,P=0.030);但是与血小板捐献量并无关系。结论 血小板捐献间隔期不少于2周间隔期的连续血小板献血者,其外周血PLt和RBC在一定时间内变化情况会受到血小板捐献量的影响而发生增加和减少的变化,但均在正常范围内波动。
Objective To explore the changes of platelet, white blood cell and red blood cell counts of long-term platelet blood donors. Methods A retrospective study was conducted on 849 platelet blood donors who donated for the first time and continuously donated amounts to 10U plateletor or more from January 1, 2016 to September 30, 2018.The paired t test of the same group was used to evaluate the changes of platelet, red blood cell and white blood cell counts between the first time and the last time donation during the study period. Ordinal multinomial logistic regression was conducted to analyze the effects of platelet donation on the peripheral blood cell count of the donor during the survey. Results Paired t-test result showed that there was a increase in PLt (t=-8.58, P<0.000 1);a decrease in WBC(t=5.348, P<0.000 1); and no significant change in RBC (t=0.515, P=0.607).The results of ordinal multinomial logistic regression analysis showed that the change in PLt had no relationship with age, sex, and interval between the first and last blood donation, P>0.05. Compared with donors who donated 41U or above,the possibility of an increase in platelet count was relatively small for those who donated 30U or below(platelet donation amount 10~20U,χ2=13.737,P<0.000 1;platelet donation amount 21~30U,χ2=7.491,P=0.006). There was no relationship between age, gender, and blood donation interval and WBC changes, P>0.05. Compared with donors who donated 41U or above, WBC was more likely to increase for those who donated 10~20 U (OR=1.720, 95%CI=1.136~2.605, P=0.010).RBC changes had nothing to do with age, gender and platelet donation amount of the blood donor, P> 0.05; the longer the interval between the first and last blood donation took, the more likely the red blood cell count increased, (OR=1.005, 95%CI=1.000~1.009, P=0.030). Conclusion For continuous platelet donors with platelet donation intervals of no less than 2 weeks, platelet donation amount will affect the peripheral blood counts,and all the blood conuts are within the normal range.
