       主动脉外科已走过70年的辉煌岁月,现有诊疗体系已基本解决“挽救患者生命”的重要时代议题。如今,新一轮科技革命悄然来临,我国主动脉外科广大同仁应主动转变以“量”取胜的传统观念,寻找学科内容不足,明确诊疗环节短板,提炼问题底层逻辑,以“质”换空间,扩展学科边界。学科发展更应秉持开放包容之心态,坚持守正创新之原则,借助不同学科技术优势,努力解决目前诊疗活动各环节中仍存在的问题及争议点,为我国广大人民群众提供精细化的诊疗服务。

       For the past 70 years of aortic surgery,the current diagnosis and treatment strategies have generally solved the important issue of “saving lives”.Nowadays,the new scientific and technological revolution is coming.The practitioners in aortic surgery should change the traditional perception and look for the insufficiency and the shortcomings of diagnosis and treatment,which finally expand the boundaries of the discipline.Moreover,the discipline development shall be more open-minded and persist in innovation.We should take the advantages of different discipline technologies,and strive to solve the current issues and controversies remaining in diagnosis and treatment toward providing refined services for the public.

五种检测技术在尘肺合并肺结核诊断中的应用价值

The application value of five testing techniques in the diagnosis of pneumoconiosis combined with pulmonary tuberculosis

目的 探讨痰涂片找抗酸杆菌、痰利福平耐药实时荧光定量核酸扩增检测技术(Xpert MTB/RIF)、分枝杆菌菌种鉴定、痰抗酸杆菌培养和血γ-干扰素释放检测技术(TB-IGRA)5种检测技术在尘肺合并肺结核临床诊断中的应用价值。方法 纳入2016年7月—2021年5月在厦门大学附属第一医院住院治疗的尘肺合并肺结核患者,按照尘肺患者是否合并肺结核,将患者分为尘肺组(156例)和尘肺合并肺结核组(111例);比较两组患者的性别、年龄、接尘时间等一般资料,以及患者痰涂片、Xpert MTB/RIF、分枝杆菌菌种鉴定、痰培养和TB-IGRA的检测结果,分析尘肺合并肺结核患者5种检测技术阳性检出率的差异及其诊断价值。结果 在尘肺合并肺结核组中,TB-IGRA检测方法的阳性检出率最高(81.1%),高于其他4种检测方法(P<0.01);两两联合检测,以Xpert MTB/RIF+TB-IGRA组合的检测方式检出率最高(96.4%),高于其他9种组合(P<0.01)。结论 TB-IGRA检测方法对尘肺合并肺结核患者的阳性检出率较高,联合Xpert MTB/RIF检测后可进一步提高诊断效率,对早期诊断尘肺是否合并结核具有重要的临床诊断价值。
Objective To investigate the value of five testing techniques in the clinical diagnosis of pneumoconiosis combined pulmonary tuberculosis,including sputum smear,Xpert Mycobacterium tuberculosis/rifampicin(Xpert MTB/RIF),identification of Mycobacterium species,sputum acid-fast bacilli culture and tuberculosis-interferon-gamma release assays(TB-IGRA).Methods Patients with pneumoconiosis combined with tuberculosis who were hospitalized in the First Hospital of Xiamen University from July 2016 to May 2021 were included in the study.The patients were divided into the pneumoconiosis group(156 cases)and pneumoconiosis combined with tuberculosis group(111 cases)according to whether the pneumoconiosis patients were combined with tuberculosis or not.The general data of the patients in the two groups were compared with respect to gender,age,and dust exposure time,and the results of sputum smears,Xpert MTB/RIF,identification of Mycobacterium species,sputum acid-fast bacilli culture and TB-IGRA were collected to analyze the differences and the diagnostic value in the five testing techniques of the positivity rates for patients who have pneumoconiosis combined with pulmonary tuberculosis.Results In the group with pneumoconiosis combined with tuberculosis,the positive detection rate of TB-IGRA was the highest(81.1%),which was higher than other 4 testing methods(P<0.01).Combined testing in pairs suggested that the testing method of Xpert MTB/RIF and TB-IGRA combination was the highest(96.4%),significantly higher than the other 9 combinations(P<0.01).Conclusions TB-IGRA has higher positive detection rate for patients with pneumoconiosis combined with tuberculosis,and diagnostic efficiency can be further improved by combining Xpert MTB/RIF assay testing,which is of great clinical diagnostic value for the early diagnosis of pneumoconiosis combined with tuberculosis.

华南地区鲍曼不动杆菌流行株的毒力变迁研究

The virulence of the prevalent clone ST457 of Acinetobacter baumannii in Southern China

目的 探讨我国南方地区鲍曼不动杆菌流行株ST457毒力进化方向。方法 收集南方地区5家大型三甲医院2012年—2015年血和/或痰标本来源的鲍曼不动杆菌菌株,运用不同毒力实验,比较ST457与其他CC92流行克隆株与非流行克隆株的毒力差异。结果 ST457感染患者早期死亡率高;ST457菌株平均产生物膜能力低于ST195和ST208;83.3%的鲍曼不动杆菌临床分离菌株在血清的存活率大于60%,其中43.3%的菌株可在血清内快速繁殖,不同ST型组间、菌血症组和非菌血症组间无差异;不同ST型菌株竞争后,ST457未展现出竞争优势。结论 与传统CC92流行株相比,ST457毒力表型与传统的CC92流行株类似,提示CC92克隆株共进化能力。部分流行株短时间内在血清中快速繁殖,提示其条件致病后潜在的血流感染的能力。
Objective To investigate the virulence of the prevalent clone ST457 of Acinetobacter baumannii in Southern China. Methods Isolates from sputum and/or blood samples from 2012 to 2015 in 5 large differences between clonal complex 92(CC92) and non-CC92 were estimated by biofilm assay, competition assay and serum assay. Results Patients with ST457 infection had high early mortality. The average biofilm production of ST457 was significantly lower than that of ST195 and ST208. In serum assay, 83.3% isolates survived in the serum with over 60% survival rate, of which 43.3% strains reproduced in the serum, with no difference between STs or between bacteremia and non-bacteremia group. After competing with different ST strains, ST457 shows no competitive advantage. Conclusions ST457 had a similar virulence phenotype with the traditional prevalent CC92 clone, suggesting the co-evolutionary ability for CC92 clone. Some clinical isolates were survived in human serum, suggesting the potential capacity of bloodstream infection.

血必净注射液对ANP大鼠肠道菌群及肠黏膜屏障功能的影响

Effect of Xuebijing injection on intestinal flora and intestinal mucosal barrier function in ANP rats

目的 探讨血必净注射液对ANP大鼠肠道菌群及肠黏膜屏障功能的影响。方法 40只SD大鼠随机分为空白组、假手术组、ANP组和血必净治疗组(每组10只),空白组不作任何处理,假手术组翻动十二指肠后关腹,ANP组和治疗组用4.5%牛磺胆酸钠溶液胆胰管逆行注射建模,治疗组在建模后经鼠尾静脉注射血必净注射液(3 mL/kg)。24 h后处死大鼠并采样,ELISA法测血AMS、CRP、LPS、TNF-α、IL-6、IL-1β、DAO和D-乳酸等指标,粪菌样本行16SrRNA高通量测序分析,实时定量PCR法检测5种细菌数量,病理检测胰腺和回肠组织,比较各组大鼠的指标。结果 ①ANP组大鼠血AMS升高,CRP、LPS、TNF-α、IL-6、IL-1β、DAO、D-乳酸水平以及胰腺、小肠病理评分均高于空白组和假手术组(P<0.001);②治疗组AMS低于ANP组,血必净可降低上述各种血清指标水平和胰腺、小肠病理评分(P<0.001);③肠道菌群微生态分析显示,血必净可改善ANP大鼠粪菌的丰富度和多样性,缩小与空白组、假手术菌种种类的差异,增加厚壁菌门菌量;治疗组乳酸杆菌、双歧杆菌和普拉梭菌的菌量高于ANP组,肠球菌和大肠埃希的菌量低于ANP组(P<0.001)。结论 血必净可增加ANP大鼠肠道菌群的丰富度和多样性,增加有益菌的含量,减少内毒素和促炎因子释放,改善肠黏膜屏障功能。
Objective To investigate the effect of Xuebijing injection on intestinal flora and intestinal mucosal barrier function in ANP rats. Methods 40 SD rats were randomly divided into blank group, sham operation group, ANP group and Xuebijing treatment group (10 in each group). The sham operation group closed the abdomen after turning the duodenum. The ANP model was established by retrograde injection of 4.5% sodium taurocholate solution into the biliopancreatic duct. Xuebijing injection (3mL/kg) was injected into the tail vein of the rats in the treatment group. 24 hours later, the rats were sacrificed and sampled. AMS, CRP, LPS, TNF-, il-6, il-1, DAO and d-lactic acid were measured by ELISA. The fecal bacteria samples were analyzed by 16SrRNA sequencing technique. Real-time quantitative PCR was used to detect the populations of 5 bacteria in fecal sample. The pathology of pancreas and ileum were examined, and the indexes of rats in each group were compared. Results ①In ANP group, AMS was increased, levels of CRP, LPS, TNF-, il-6, il-1, DAO, d-lactic acid, pancreatic and intestinal pathology scores were higher than those in the blank group and the sham group (P<0.001).②In treatment group,AMS was lower than ANP group, and Xuebijing could reduce the levels of the above factors and scores of pancreatic and intestinal pathology (P<0.001).③ The microecological results of intestinal flora showed that Xuebijing treatment could improve the richness and diversity of fecal bacteria, reduce the difference between Xuebijing group and blank group and sham operation group, and increase the quantity of firmicutes. The amount of Lactobacillus, Bifidobacteria and Clostridium prasei in the Xuebijing group was higher than that in ANP group, while the amount of enterococci and Escherichia coli was lower than that in the ANP group (P<0.001). Conclusion Xuebijing can increase the richness and diversity of intestinal flora, increase the content of beneficial bacteria, reduce the release of endotoxin and pro-inflammatory factors, and improve the intestinal mucosal barrier function in ANP rats.

基于力学仿真的下颌角截骨整形技术研究

Study on osteotomy technique of mandibular angle based on mechanical simulation

目的 对下颌角肥大钻凿法截骨整形手术进行生物力学研究,并探索该术式的优化改进。方法 采用有限元方法,建立下颌角钻凿法截骨整形手术及其改良术式的三维有限元模型并进行应力分析、比较。结果 模型-1(钻凿法下颌角截骨整形手术模型)下颌角断裂失效载荷在所有术式中最高,为2426N。其余的改良术式模型(模型-2~7)失效载荷均低于模型-1,分别为2018N、1854N、1447N、1183N、1028N、876N。同时,模型-7与其他模型对比,其应力分布范围最小,关节盘von Mises应力手术侧大于非手术侧,大部分区域为0.52~3.25 MPa之间,最大不超过9.86 MPa。结论 在非均匀钻孔加去下颌角颊侧皮质骨的同时施加最大咬合力和下颌支后缘约束的手术方法最优。
Objective To study the biomechanics of osteotomy surgery for mandibular angle hypertrophy with drilling method and to explore the optimization and improvement of this operation. Methods Using the finite element method, the three-dimensional finite element models of osteotomy surgery for mandibular angle hypertrophy with drilling method as well as its modified operations are established,meanwhile, the stress analysis and comparison are performed. Results Model-1 (mandibular angle osteotomy surgery model by drilling method) had the highest fracture failure load among all surgical methods, which was 2426N. The failure loads of the other modified surgical models (model-2 ~7) were all lower than that of model-1, which were 2018N, 1854N, 1447N, 1183N, 1028N and 876N, respectively. Meanwhile, compared with other models, the stress distribution region of model-7 was the smallest, with the von Mises stress in operating side of the articular disc was greater than that of the non-operating side, and most of the area was between 0.52~3.25MPa and the maximum of 9.86MPa. Conclusion The non-uniform drilling and removal of the buccal cortical bone of the mandibular angle are combined with the maximum occlusal force and the constraint of the posterior margin of the mandibular ramus is the best operation method.

促红细胞生成素对肝癌生长影响的临床研究

Clinical study on the effect of erythropoietin on the growth of liver cancer

目的 分析促红细胞生成素(EPO)及促红细胞生成素受体(EPOR)在肝细胞癌(NCC)以及正常组织中的表达规律,以及它们和肝细胞癌微血管密度(MVD)之间的关系。方法 选取我院手术切除的肝细胞肝癌的标本30例,取肿瘤边缘2.0 cm的肝组织作为对照,同时取正常肝脏组织10例做为阴性对照。利用酶联免疫吸附实验(ELISA)检测各组织中EPO及EPOR表达水平,利用免疫组织化学方法染色检测微血管密度(MVD)。对比癌组织和癌旁组织EPO、EPOR及MVD差异,分析NCC中EPO、EPOR、MVD与肿瘤病理特征的关系,分析EPO、EPOR表达水平与MVD之间的关系。结果 HCC组织中,EPR、EPOR、MVD均高于癌旁组织和正常组织,差异有统计学意义(P<0.001),EPR、EPOR、MVD在癌旁组织和正常组织中,差异无统计学意义(P>0.05)。肿瘤大小>5 cm、存在包膜侵犯、存在远处转移以及高中分化的HCC中,EPR、EPOR、MVD水平高于肿瘤大小≤5 cm、无包膜侵犯、无远处转移以及低分化的水平,差异有统计学意义(P<0.05)。Person相关分析结果显示,EPO表达水平与MVD的相关系数r=0.651(P<0.001),EPOR表达水平与MVD的相关系数r=0.620(P<0.001)。结论 EPO、EPOR、MVD在HCC中呈现高水平,且与肿瘤大小、局部侵犯、远处转移及分化程度有关,其机制可能与EPO、EPOR增加MVD有关。
Objective To analyze the expression of erythropoietin (EPO) and erythropoietin receptor (EPOR) in hepatocellular carcinoma (NCC) and normal tissues,and their relationship with hepatocyte microvessel density (MVD). Methods Thirty specimens of hepatocellular carcinoma hepatectomy were selected from our hospital. The liver tissue at the edge of the tumor was taken as a control,and 10 cases of normal liver tissue were used as a negative control. The expression levels of EPO and EPOR in each tissues were detected by enzyme-linked immunosorbent assay (ELISA),and microvessel density (MVD) was detected by immunohistochemistry. The differences of EPO,EPOR and MVD between each tissues were compared. The relationship between EPO,EPOR,MVD and tumor pathological features in NCC was analyzed. The relationship between EPO and EPOR expression levels and MVD was analyzed. Results In HCC tissues,EPR,EPOR and MVD were higher than those in adjacent tissues and normal tissues. The difference was statistical difference (P<0.001). EPR,EPOR and MVD were not statistically significant in adjacent tissues and normal tissues. P>0.05). The levels of EPR,EPOR,and MVD in tumors with tumor size >5 cm,invasion of the capsule,distant metastasis,and high-differentiation were higher than those of tumor size ≤ 5 cm,no capsule invasion,no distant metastasis,and poor differentiation. The difference was statistical difference (P < 0.05). Person correlation analysis showed that the correlation coefficient between EPO expression level and MVD was r=0.651 (P<0.001),and the correlation coefficient between EPOR expression level and MVD was r=0.620 (P<0.001). Conclusion EPO,EPOR and MVD are highly expressed in HCC,and are related to tumor size,local invasion,distant metastasis and differentiation. The mechanism may be related to EPO and EPOR increasing MVD.

血必净注射液对SAP大鼠TLR4信号通路介导肠黏膜屏障功能障碍的影响

目的 探讨血必净注射液对SAP大鼠TLR4信号通路介导肠黏膜屏障功能障碍的影响。方法 24只SD大鼠随机分成空白组(n=8)、对照组(n=8)和治疗组(n=8)。对照组和治疗组用4.5%牛磺胆酸钠溶液胆胰管逆行注射制备SAP模型,空白组采用等量生理盐水逆行注射。治疗组在造模3 h后经鼠尾静脉注射血必净注射液(3 mL/kg)。三组大鼠造模后观察24 h,然后处死取胰腺和小肠组织送病理检查,采用荧光RT-PCR技术检测TLR4和NF-κB表达水平,采用ELSIA法检测血清TNF-α、IL-6、淀粉酶(AMS)及二胺氧化酶(DAO)水平,比较三组大鼠各项指标。结果 对照组和治疗组小肠组织TLR4和NF-κB表达以及血清TNF-α、IL-6、AMS及DAO水平均高于空白组(P>0.05),治疗组小肠组织TLR4和NF-κB表达以及血清TNF-α、IL-6、AMS及DAO水平低于对照组(P<0.05)。结论 血必净注射液通过干预SAP大鼠TLR4信号通路,降低小肠组织TLR4和NF-κB的表达,减轻小肠组织的炎症反应,对肠黏膜屏障具有一定的保护作用。
Objective To investigate the effect on intestinal mucosal barrier dysfunction (IBF) of Xuebijing injection mediated by Toll-like receptor 4 (TLR4) signal pathway in rats of severe acute pancreatitis (SAP). Methods 24 Sprague Dawley (SD) rats were randomly divided into Sham group (n=8), control group (n=8) and treatment group (n=8). The SAP model was established by retrograde injection of 4.5% sodium taurocholate into the biliopancreatic duct in control group and treatment group, while control group was injected with the same amount of saline. In treatment group, Xuebijing injection (3 mL/kg) was injected via tail vein after 3h of modeling. All rats were monitored and sacrificed after 24 hours of modeling. Samples of pancreas and intestine were collected for pathologic determination. A fluorescent RT-PCR was used to determine the expression of TLR4 and NF-κB of small intestine. The serum levels of TNF-α, IL-6, amylase (AMS) and diamine oxidase (DAO) were measured by using ELISA. All parameters of three groups were compared. Results The expression of TLR4 and NF-κB of small intestine in control group and treatment group were higher than it in control group (P<0.05), as well as the serum levels of TNF-α, IL-6, AMS and DAO (P<0.05). The expression of TLR4 and NF-κB of small intestine in treatment group were lower than it in control group (P<0.05), as well as the serum levels of TNF-α, IL-6, AMS and DAO (P<0.05). Conclusion Xuebijing injection may not only reduce the expression of TLR4 and NF-κB of small intestine, but also alleviate the inflammation reaction of small intestine by interfering with TLR4 signal pathway, which may have a protective effect on intestinal mucosal barrier in SAP rats.

急性呼吸窘迫综合征患者血清甲状腺素水平变化

目的 研究分析急性呼吸窘迫综合征(ARDS)患者血清甲状腺素变化与疾病的严重程度的相关性。方法 选取2016年1月—2018年1月在我院重症医学科收治的123例急性呼吸窘迫综合征及41例非急性呼吸窘迫综合征患者,根据2012柏林定义,ARDS又再划分为轻度组(26.67 kPa2/FiO2≤40.00 kPa,PEEP或CPAP≥0.49 kPa)、中度组(13.33 kPa2/FiO2≤26.67 kPa,PEEP≥0.49 kPa) 及重度缺氧组(PaO2/FiO2≤13.33 kPa,PEEP≥0.49 kPa),同时再选取同一时段内在我院进行常规体检的41例健康成人作为对照,对他们血清的甲状腺激素水平测定。结果 健康成人对照组与非ARDS组比较, 血清TSH、FT4、TT4、FTS、TT3水平测定差异没有统计学意义(P>0.05),ARDS组较另外两组以上指标均有降低(P<0.05),轻、中、重三组间比较,血清甲状腺素的差异存在统计学意义。结论 急性呼吸窘迫综合征患者血清甲状腺素水平与缺氧程度呈正相关,随着疾病严重程度加大,其水平差异的变化更加明显,动态监测甲状腺素水平对疾病严重程度的判断和对预后的评估具有重大意义。
Objective To analyze serum thyroid hormone level change of acute respiratory distress syndrome and the correlation between the severity of disease. Methods 123 cases (including mild 26.67 kPa2/FiO2≤40.00 kPa,PEEP or CPAP≥0.49 kPa,moderate 13.33 kPa 2/FiO2≤26.67 kPa,PEEP≥0.49 kPa and severe PaO2/FiO2≤13.33 kPa,PEEP≥0.49 kPa)of patients with acute respiratory distress syndrome and 41 cases of non-ARDS were involved. from January 2016 to January 2018 in our hospital and 41 cases of healthy people by routine check-up in our hospital during the same period were selected,then the serum thyroid hormone levels were determined. Results Compared with healthy control group,TSH、FT4、TT4、FTS、TT3 level in non-ARDS group has no statistical difference(P>0.05),while compared with the rest of the two groups,all indexes in ARDS group were lower (P<0.05). And TSH、FT4、TT4、FT3、TT3 levels in moderate ARDS group decreased compared with those in relatively mild ARDS (P<0.05). The level of each index in severe ARDS group decreased compared with the moderate ARDS group (P<0.05). There was statistical difference in detection index level in three groups. Conclusion The serum thyroid hormone level of ARDS patients is positively correlated with hypoxemia. As the pathological severity changes ,the level of hypoxemia changes are more apparent. Dynamic monitoring of serum thyroid hormone is of great significance in severity judgement and prognosis evaluation.
