目的 分析流行性感冒(流感)患儿的流行病学特征及病原学分布。方法 纳入2020年3月—2023年2月在开封市祥符区第一人民医院确诊为流感的394例1~10岁患儿的咽拭子标本,通过实时荧光定量聚合酶链式反应完成流感病毒检测,分析此394例流感患儿感染的病毒类型及年龄、季节、性别、年份等分布情况。结果 甲型H1N1流感病毒为本次研究检出占比最高的流感病毒,占36.04%;男女患儿之间的甲型H3、甲型H1N1、乙型BV、乙型BY流感病毒占比比较差异无统计学意义(P>0.05);春季及冬季流感患儿占比最多,分别为41.12%、35.53%,且不同季节之间的甲型H3、甲型H1N1、乙型BV、乙型BY流感病毒占比比较差异有统计学意义(P<0.05);7~10岁流感患儿占比最高,为42.13%,不同年龄段患儿之间的甲型H3、甲型H1N1、乙型BV、乙型BY流感病毒占比比较差异无统计学意义(P>0.05);2020年流感患儿占比最高,为43.40%,后续依次为2022年的29.70%,2021年的26.90%;不同年份的冬季流感患儿占比比较差异有统计学意义(P<0.05);不同年份的各年龄段流感患儿占比比较差异无统计学意义(P>0.05);各年甲型H3、甲型H1N1、乙型BV、乙型BY流感病毒占比比较差异无统计学意义(P>0.05)。结论 年龄7~10岁的患儿为流感高危群体,且甲型流感为患儿主要感染类型,而甲型流感病毒多见于春、冬季,新型冠状病毒肺炎疫情期间防控政策变化对流感流行有影响,应加强春、冬季7~10岁年龄段患儿的甲流预防工作。
Objective To analyze the epidemiological characteristics and etiological distribution of children with influenza(flu).Methods The throat swab specimens of 394 children aged 1 to 10 years old who were diagnosed with influenza at the First People's Hospital of Xiangfu District,Kaifeng City from March 2020 to February 2023 were included.The samples were analyzed by real-time fluorescence quantitative polymerase chain reaction to complete influenza virus detection and analyze the virus types and distribution of age,season,gender,year,etc.among these 394 children with influenza were analyzed.Results Influenza A H1N1 virus was the influenza virus with the highest proportion detected in this study,accounting for 36.04%;there were differences in the proportions of influenza A H3,A H1N1,BV,and B BY influenza viruses between male and female children,with no statistical significance(P>0.05);the proportion of children with influenza is the largest in spring and winter,41.12% and 35.53% respectively,and proportions of influenza A H3,A H1N1,B BV,and B BY influenza in different seasons were significantly different(P<0.05).Children aged 7 to 10 accounted for the highest proportion of influenza patients,42.13%.Among children of different ages,the proportion of influenza A H3,A H1N1,B BV,B BY had no statistically significant difference(P>0.05).The proportion of children with influenza in 2020 was the highest at 43.40%,followed by 29.70% in 2022 and 26.90% in 21;in different years there is a statistically significant difference in the proportion of children with influenza in winter(P<0.05).There is no statistically significant difference in the proportion of children with influenza of all ages in different years(P>0.05).The preportions of influenza A H3 and A H1N1,B BV and B BY viruses in each year,had no statistically significant difference(P>0.05).Conclusions Children aged 7 to 10 years old are a high-risk group for influenza,and influenza A is the main type of infection in children.Influenza A viruses are more common in spring and winter.Changes in prevention and control policies during the new coronavirus pneumonia epidemic have an impact on the epidemic of influenza.Therefore,prevention of influenza A for children aged 7 to 10 years old in spring and winter should be strengthened.
目的 探讨脑卒中患者多重耐药菌(MDROs)医院感染风险因素,并进行病原学特点分析。方法 选择2020年1月—2022年12月福建中医药大学附属福鼎医院神经内科病房收治的160例脑卒中患者为研究对象,评估患者的MDROs医院感染发生状况,调查患者的一般资料并进行多因素分析。结果 在160例患者中,发生医院感染20例,分离到病原体26株,其中8例样本为MDROs(研究组,其他归为对照组),来源于8例患者,占比5.00%,包括耐甲氧西林金黄色葡萄球菌(MRSA)3株,耐碳青霉烯大肠埃希菌(E.coli)2株、耐碳青霉烯肺炎克雷伯菌(KP)1株、全耐药KP1株、耐碳青霉烯PA1株。研究组的美国国立卫生院神经功能缺损(NIHSS)评分、急性生理学和慢性健康状况评价Ⅱ(APACHEⅡ)评分、糖尿病、低蛋白血症、置管留置时间、住院时间等与对照组对比差异有统计学意义(P<0.05)。二分类Logistic回归分析显示,上述指标均为导致MDROs医院感染发生的重要因素(P<0.05)。结论 脑卒中患者MDROs医院感染的发生率依然比较高,病原菌多为耐甲氧西林MRSA、耐碳青霉烯E.coli,患者的NIHSS评分、APACHEⅡ评分、糖尿病、低蛋白血症、置管留置时间、住院时间为主要的MDROs感染风险因素。
目的 对儿童重症肺炎支气管肺泡灌洗液(BALF)进行病原学分析及疾病预后的分析。方法 本研究选取2019年3月—2020年12月在我院儿科住院并进行肺泡灌洗治疗的40例重症肺炎患儿作为研究对象。通过对这些患儿在感染急性期肺泡灌洗液中的细菌、真菌、肺炎支原体等进行病原学检查以及T 淋巴细胞亚群的检测,了解台山地区儿童重症肺炎病原体情况及耐药性、T淋巴细胞亚群与疾病严重程度、预后评估的关系。结果 BALF病原学检测结果分析中,肺炎支原体27例,肺炎支原体+肺炎链球菌5例,肺炎支原体+中间葡萄球菌2例,肺炎支原体+铅黄肠球菌1例,肺炎支原体+嗜麦芽假单胞菌2例,病原菌阴性3例;本组病例血清T细胞亚群检测结果显示:大部分病例CD3+、CD4+、CD8+及CD4+/CD8+水平有不同程度的下降。其中CD3+水平下降的有6例,CD4+水平下降的有16例,CD8+水平下降的有17例,CD4+、CD8+水平同时下降的有14例,CD3+、CD4+、CD8+水平同时下降的有4例;BALF细胞总数(3673.1±377.9)×106 /L,巨噬细胞比例(23.6±17.6)%,淋巴细胞(22.1±16.2)%,中性粒细胞(46.5±24.8)%。结论 病原学分析儿童重症肺炎BALF的主要病原菌为肺炎支原体,血清T细胞亚群检测大多表现为CD4+、CD8+水平下降。
Objective To analyze the etiology of bronchoalveolar lavage fluid and prognosis of children with severe pneumonia. Methods In this study, 40 children with severe pneumonia who were hospitalized in the pediatrics department of our hospital and underwent alveolar lavage treatment from March 2019 to December 2020 were selected as the research objects. Through the detection of pathogens such as bacteria, fungi, Mycoplasma pneumoniae and T lymphocyte subsets of these children in the acute phase of infection, we can understand the pathogens and drug resistance of children with severe pneumonia in Taishan area and the relationship among drug resistance, T lymphocyte subsets and disease severity and prognosis assessment. Results In the analysis of the BALF pathogenic test results, there were 27 cases with Mycoplasma pneumoniae, 5 cases with Mycoplasma pneumoniae+Streptococcus pneumoniae, 2 cases with Mycoplasma pneumoniae+Staphylococcus intermedius, 1 case with Mycoplasma pneumoniae+Enterococcus casseliflavus, 2 cases with Mycoplasma pneumoniae+Pseudomonas maltophilia and 3 cases were pathogenic bacteria negative. The test results of serum T cell subsets of these cases showed that most of the cases had different degrees of decline in the levels of CD3+, CD4+, CD8+ and CD4+/CD8+. Among them, CD3+ levels decreased in 6 cases, CD4+ levels decreased in 16 cases, CD8+ levels decreased in 17 cases, CD4+ and CD8+ levels decreased in 14 cases, and CD3+, CD4+, and CD8+ levels decreased in 4 cases; total cell number of BALF was (3 673.1±377.9)×106/L, the proportion of macrophages was (23.6±17.6)%, lymphocytes had (22.1±16.2)%, and neutrophils had (46.5±24.8)%. Conclusions Pathogenic analysis showed that the main pathogen of BALF in children with severe pneumonia is Mycoplasma pneumoniae, and the detection of serum T cell subsets mostly showed a decrease in CD4+ and CD8+ levels.
目的 分析肝脓肿的临床基线及病原学特征,旨在提高临床对该病早期识别、病原体预判、并发症警惕及抗感染治疗能力。方法 收集广州番禺区中心医院2019年1月—2020年11月收治的肝脓肿病例相关临床及实验室资料。按病原体培养结果分成阳性组、阴性组。阳性组中,按不同致病菌分成肺炎克雷伯及非肺炎克雷伯亚组。比较不同组别间基线特征及实验室数据。结果 共收集肝脓肿71例,病原学培养阳性率83.1%(59例),其中肺炎克雷伯菌46例(80%)。最常见合并症包括糖尿病(54.9%)、胆道疾病(56.3%)、肺部感染(67.6%)等。病原体阳性组PCT和AST水平高于阴性组(P<0.05),但是两组之间基线特征无差异(P>0.05)。肺炎克雷伯亚组与其他病原体组相比,合并糖尿病的风险更高(P=0.007),PLT、PCT、ALT、AST和白蛋白的水平具有显著差异(P <0.05)。经过积极治疗,大多预后良好,未见死亡病例,其中7例患者发生严重侵袭性脓肿,遗留后遗症。结论 病原体阴性肝脓肿致病力方面与阳性组并无差别。肺炎克雷伯杆菌已成为肝脓肿中最常见菌群,其多见于合并糖尿病患者。其PLT、PCT、ALT、AST指标明显高于其他菌群组,伴白蛋白消耗明显。少数可伴有全身严重侵袭性脓肿。
Objective To investigate the clinical and etiological characteristics of pyogenic liver abscess (PLA) as a reference for its early identification,early prediction of pathogens, alertness to complications and rational antibacterial treatment. Methods A single center retrospective study was carried out in patients with PLA in Guangzhou Panyu Center Hospital from January 2019 to November 2020.The clinical and laboratory data of all liver abscess cases were concluded. Baseline characteristic and laboratory outcomes were grouped into Klebsiella pneumoniae group and non-Klebsiella pneumoniae subgroup according to different pathogenic bacteria. Results Among a total of 71 patients with liver abscess, 83.1%(n=59)had positive cultures. Klebsiella pneumoniae accounted for the most in 46 cases (80.0%). The most common complications included diabetes(50.7%), biliary tract disease(56.3%), pulmonary infection(67.6%) etc. The levels of PCT and AST in pathogen culture-positive group were higher than those in culture-negative group, with statistical significance (P<0.05),on the contrary there were no differences in baseline characteristic among the groups according to the results of pathogens. Compared with other pathogens, the group of Klebsiella pneumoniae liver abscess presented higher risk of diabetes.The levels of PLT, PCT, ALT, AST,and albumin in the two groups were different (P<0.05). Most patients had good outcomes, while 7 patients had residual problems represented with severe invasive abscess. Conclusion There were similar consequences between negative and positive liver abscesses. Klebsiella pneumoniae has become the most common bacteria in liver abscess. Among which, PLT、PCT、ALT、AST indexes are higher than that of other bacteria with obvious albumin depletion, even some of which may be accompanied by systemic severe invasive abscess.
目的 探讨揭阳地区急性下呼吸道感染住院患儿病毒病原学特点。方法 对2 125例急性下呼吸道感染患儿应用直接免疫荧光法(DIF)进行A型流感病毒(甲型流感病毒IFA)、B型流感病毒(乙型流感病毒IFB)、呼吸道合胞病毒(RSV)、腺病毒(AdV)、副流感病毒1(PIVⅠ)、2(PIVⅡ)和3型(PIVⅢ)进行病毒学检测。结果 2 125例患儿鼻咽部分泌物标本中有538例检测出至少1种病毒,总阳性率25.3%,其中RSV 阳性率(19.7%)明显高于其他病毒,具有统计学意义。春、夏、冬季的RSV阳性率大致相当,明显高于秋季。婴儿期组RSV阳性率(27.2%)最高,幼儿期组(18.7%)次之,均显著高于学龄前期、学龄期,后2组阳性率无统计学差异,青春期组未检出RSV。结论 病毒是急性下呼吸道感染的重要病原体,而其中又以RSV为著,RSV感染具有显著的季节性和年龄特征性。
Objective To investigate the viral etiology feature in hospitalized children with acute lower respiratory infection in Jieyang area. Methods A total of 2 125 children with acute lower respiratory tract infection were screened by direct immune fluorescence assay (DIF) for influenza virus A (IFA), influenza virus B (IFB), respiratory syncytial virus (RSV), adenovirus (AdV), parainfluenza virus I (PIV Ⅰ), PIV Ⅱ and PIV Ⅲ. Results In 2 125 cases of nasopharyngeal secretory specimens, 538 cases were detected at least one kinds of viruses. The total positive rate was 25.3%, of which the positive rate of RSV (19.7%) was higher than that of other viruses. The positive rate of RSV was similar in spring, summer and winter, much higher than that in autumn.The positive rate of RSV in infancy group (27.2%) was the highest,then the second was the toddler's age(18.7%), both of which were higher than that in preschool age group and school age group. There was no significant difference in the positive rate between preschool age group and school age group. In addition, RSV was not detected in the adolescence group. Conclusion Virus is an important pathogen of acute lower respiratory tract infection. The most common virus is RSV, infection of which has seasonal and age characteristics.
目的 了解中山市儿童社区获得性肺炎病原学特点及其与年龄、抗生素使用的相关性,为疾病诊断和抗生素合理使用提供参考依据。方法 对中山市妇幼保健院儿科住院部2016年1月—2016年4月收治的小儿社区获得性肺炎临床资料、入院前使用抗生素和病原学检测结果进行回顾性分析。结果 共有155例研究对象纳入研究,细菌检出95株,其中革兰氏阴性菌61株,革兰氏阳性菌34株。流感嗜血杆菌,金黄色葡萄球菌,肺炎链球菌,大肠埃希氏菌,副流感嗜血杆菌的占构成比前五位。支原体和衣原体现症感染率约7.7%~8.4%。RSV检出率达25.8%。年龄在3 a以上、入院前使用过抗生素者,革兰阳性菌检出率较低(P<0.05)。入院前使用过抗生素者,革兰阴性菌检出率较高,且使用4 d及以上者高于使用3 d及以下者(P<0.05)。结论 住院儿童CAP的治疗,应综合考虑病人的年龄、入院前的抗生素使用情况,结合当地CAP 的可能优势病原选择合适的治疗方案。
Objective To explore the etiological characteristics and factors of community-acquired pneumonia(CAP) in children in Zhongshan for providing reference for the diagnosis and rational use of antibiotics for CAP. Methods Retrospective analysis were applied on clinical data and etiology results of CAP in children admitted to the pediatric inpatient department of Maternal and Child Health Hospital in Zhongshan from January to April, 2016. Results 155 children were enrolled. The number of bacteria detected was 95, including 61 Gram-negative bacteria, 34 Gram-positive bacteria. Haemophilus influenzae, Staphylococcus aureus, Streptococcus pneumoniae, Escherichia coli, Haemophilus parainfluenzae accounted the top five. The recent infection rate of Mycoplasma and Chlamydia were from 7.7% to 8.4%. RSV detection rate was 25.8%.The Children who were over 3 years old, pre-use of antibiotics, associated with lower detection rate of Gram-positive bacteria (P<0.05). The Children who were the pre-use of antibiotics, associated with higher detection rate of Gram-negative bacteria, and the use of four days and more higher than three days or less(P<0.05). Conclusion For the treatment of CAP in children, the patient's age and pre-use history of antibiotics should be considered, combined with local common type of disease pathogens, to select the appropriate treatment.
目的 探讨急性卒中合并医院获得性肺炎患者的病原学特点及药物敏感性情况。方法 回顾性分析116例急性卒中合并医院获得性肺炎患者临床资料,记录其病原菌分布特点;分析检出率最高的三种病原菌耐药情况。结果 ①此次入组的116例受试患者呼吸道分泌物样本中共分离出160株病原菌,其中革兰阳性菌50株(31.3%),革兰阴性菌88株(55.0%),真菌22株(13.8%);肺炎克雷伯菌、绿铜假单胞菌及金黄色葡萄球菌为分布构成比最大的前三位病原菌,构成比分别为17.5%、15.6%和12.5%;②三种常见病原菌均对万古霉素及利奈唑胺药物敏感性较高,可达100.0%;左氧氟沙星对金黄色葡萄球菌敏感性较高,但肺炎克雷伯菌及绿铜假单胞菌等革兰阴性菌对其耐药性较强;头孢唑林对肺炎克雷伯菌及绿铜假单胞菌敏感性较高。结论 急性卒中合并医院获得性肺炎患者多以革兰阴性菌为常见病原菌,肺炎克雷伯杆菌、铜绿假单胞菌及金黄色葡萄球菌最多见;病原菌对常见抗菌药物存在不同程度耐药性,需引起临床重视,根据病原学分析结果科学合理使用抗菌药物以提升治疗效果。
目的 分析基层医院ICU VAP的危险因素及病原学情况。方法 回顾性分析2013年1月—2014年12月本院ICU收治的机械通气>48 h 的118例患者,分VAP组和非VAP组,分析VAP的危险因素及病原学情况。结果 VAP组与非VAP组在紧急气管插管,机械通气时间,抗生素种类,糖皮质激素,PPI及镇静药物使用>7天,返流,MODS,ICU停留时间的项目,两组比较差异有统计学意义。紧急插管:52.2% vs 31.9%,P=0.029;机械通气时间:(9.8±3.5)vs(7.3±2.8)天,P=0.038;抗生素种类>2种:52.2% vs 26.4%,P=0.005;使用糖皮质激素:56.5% vs 27.0%,P=0.001;PPI使用>7天:65.2% vs 40.3%,P=0.008;镇静药物使用>7天:58.7% vs 38.9%,P=0.035;返流:50% vs 29.2%,P=0.022;合并MODS:47.8% vs 22.2%,P=0.004;ICU停留时间:(13.6±6.6)vs(10.2±5.3)天,P=0.023。使用糖皮质激素、机械通气时间、ICU停留时间是VAP的独立危险因素(多因素Logistic分析的OR值:2.481、1.234、1.075)。基层医院ICU VAP主要以革兰氏阴性菌(82.3%)为主。结论 使用激素、机械通气时间、ICU停留时间是基层医院ICU VAP的独立危险因素;而VAP病原菌感染以G-菌为主,可经验使用G-菌敏感的抗生素。
Objective To analyze pathogens and risk factors of VAP in a general ICU of a primary hospital. Methods Totally 112 patients(from 2013-01 to 2014-12) under mechanical ventilation over 48 h were retrospectively studied. The patients were assigned into VAP group and non-VAP group. The independent risk factors and pathogens of VAP were analyzed. Results There was significant difference between VAP group and non-VAP group in terms of emergent tracheal intubation, MV time, types of antibiotics used, the use of hormones,the use of PPI and sedative drugs for more than 7 days, regurgitation, MODS, ICU stay time. Emergent tracheal intubation:52.2% vs 31.9%,P=0.029;MV time:9.8±3.5day vs 7.3±2.8day,P=0.038;types of antibiotics used > 2 kinds:52.2% vs 26.4%,P=0.005;the use of hormones:56.5% vs 27.0%,P=0.001;the use of PPI >7day:65.2% vs 40.3%,P=0.008;the use of sedative drugs >7day:58.7% vs 38.9%,P=0.035;regurgitation:50% vs 29.2%,P=0.022;MODS:47.8% vs 22.2%,P=0.004;ICU stay time:13.6±6.6day vs 10.2±5.3day,P=0.023. The use of hormones,MV time, ICU stay time were the independent risk factors of VAP[odds ratio(OR) of multivariate logistic regression:2.481、1.234、1.075]. The main pathogens of VAP were gram-negative bacteria (82.3%). Conclusion The study shows that the use of hormones,MV time, ICU stay time are the independent risk factors of VAP; gram-negative bacteria are the main pathogens of VAP. Once VAP occurs, they can be treated with anti-gram-negative bacteria antibiotics.
目的 分析重症烧伤患者血培养标本中检测出念珠菌的临床分布,探究重症烧伤患者血流念珠菌感染的病原学特征。方法 选取2012—2023年在广州市红十字会医院住院治疗的重症烧伤患者血培养标本1 148份,分析分离出念珠菌的非重复患者病死率、菌种分布、同时送检的其他类型标本念珠菌培养结果及患者念珠菌血流感染的检出时间与季节分布、抗菌药物使用情况及对常用抗真菌药物的耐药情况。结果 1 148份血培养标本中77份检出真菌,阳性率为6.71%。检出念珠菌感染的非重复患者27例,其中近平滑念珠菌13例,构成比为48.15%、白念珠菌8株,构成比为29.63%。血与静脉导管培养均检出念珠菌的有22例(81.48%);血与伤口分泌物培养圴检出念珠菌的有10例(37.04%);血、静脉导管、伤口分泌物培养圴检出念珠菌的有4例(14.81%)。静脉导管检出念珠菌高于其他类型标本。27例重症烧伤患者血流感染检出念珠菌的时间主要分布在入院后第2~3周、季节主要分布在春夏季。近平滑念珠菌、白念珠菌、热带念珠菌对氟康唑敏感率分别为83.33%、87.50%和75.00%。结论 重症烧伤患者血流近平滑念珠菌检出率最高,发生血流感染时间主要在春夏季及烧伤入院后第2~3周,静脉导管留置是增加重症烧伤患者念珠菌血流感染的因素。我院念珠菌对抗真菌药物具有较高敏感性。
Objective To retrospective analyze the clinical distribution of Candida species detected in blood cultures of patients with severe burns and to investigate the etiological characteristics of Candida bloodstream infections in these patients.Methods A total of 1 148 blood culture specimens were collected from patients with severe burns hospitalized at an institution between 2012 and 2023.Patients data with Candida bloodstream infections isolated from 1 148 blood culture specimens were analyzed,including mortality rates,species distribution,Candida culture results from other simultaneously collected specimen types,and the timing and seasonal distribution of Candida bloodstream infections,the use of antibiotics and resistance to commonly-used antifungal drugs.Results A total of 1 148 blood culture samples,77 fungi were separated,resulting in a positive rate of 6.71%.Among the 27 patients with Candida infections,13 cases(48.15%)were caused by Candida parapsilosis and 8 cases(29.63%)by Candida albicans.Candida was isolated from both blood and intravenous catheter cultures in 22 cases,with a positivity rate of 81.48%.Candida was isolated from both blood and wound secretion cultures in 10 cases(positivity rate of 37.04%),and 4 cases from blood,intravenous catheter and wound secretion cultures(positivity rate of 14.81%).The detection rate of Candidafrom intravenous catheters was higher than that from other specimen types.Candida bloodstream infections were most commonly observed during the 2nd and 3rd week after admission,with a seasonal peak in spring and summer.The susceptibility rates of Candida parapsilosis、Candida albicans and Candida tropicalis to fluconazole were 83.33%、87.50% and 75.00%,respectively.Conclusions The detection rate of Candida parapsilosis in bloodstream infections among patients with severe burns was the highest.These infections predominantly occur during the spring and summer and in the 2nd and 3rd week post-admission.The presence of intravenous catheters significantly contributes to Candida infections.The Candida in the hospital has high sensitivity to antifungal drugs.
