目的 探讨中医药治疗小儿腺样体肥大(AH)的研究现状、研究热点及趋势,为本领域研究者提供借鉴。方法 检索中国知网(CNKI)数据库从2005年1月1日至2024年1月31日中医药治疗小儿腺样体肥大的相关文献。运用Excel 2019分析其发文量,运用CiteSpace 6.2.R6软件分析其作者、机构、关键词。结果 共纳入文献395篇,年发文量整体呈现波动上升的趋势;发文量最多的作者为姜之炎;发文量最多的研究机构为山东中医药大学。初步形成了以姜之炎、俞景茂、阎兆君为核心的研究团队;研究机构以中医类院校及其附属医院为主。高频关键词提示当前研究热点前三位为中医药治疗方法、临床疗效。结论 AH领域研究内容主要以内治法、外治法、作用机制为主;研究热点逐渐从临床研究转向作用机制等实验研究;应用“数据挖掘”“网络药理学”等计算机技术研究AH会成为趋势。
Objective To explore the research status,research hotspots,and trends of traditional Chinese medicine in the treatment of adenoid hypertrophy in children,to provide a reference for researchers in this field.Methods The relevant literature on the treatment of adenoid hypertrophy in children with traditional Chinese medicine was searched in the CNKI database from January 1,2005,to January 31,2024.Excel 2019 was applied to analyze the number of published papers,and CiteSpace 6.2.R6 software was applied to analyze its authors,institutions,and keywords.Results A total of 395 papers were included,and the annual number of papers showed a fluctuating upward trend.The author with the largest number of papers was JIANG Zhiyan.Shandong University of Traditional Chinese Medicine was the research institution with the largest number of papers.The core research teams including JIANG Zhiyan,YU Jingmao,and YAN Zhaojun were initially formed.The research institutions were mainly Chinese medicine colleges and their affiliated hospitals.High-frequency keywords suggest that the current top three research hotspots were traditional Chinese medicine treatment methods,clinical efficacy.Conclusions The research content in this field is mainly based on internal treatment,external treatment,and mechanism.The focus has gradually shifted from clinical research to experimental research such as mechanism.The application of computer technologies such as “data mining” and “network pharmacology” for this disease will become a trend.
目的 探究血清C反应蛋白与白蛋白比值(CRP/ALB)、中性粒细胞与淋巴细胞比值(NLR)、血糖不稳定指数(GLI)水平对重症肺炎患者预后的预测价值。方法 将我院2020年1月—2021年5月收治的126例重症肺炎患者作为研究对象,根据入院治疗28 d的预后情况,分为存活组(93例)和死亡组(33例)。对比2组患者的一般资料,采用多因素Logistic分析重症肺炎患者高危因素,应用ROC曲线评估血清CRP/ALB、NLR、GLI水平对重症肺炎患者预后的预测价值。结果 2组患者一般资料对比,NEU、CRP、APACHEⅡ、CRP/ALB、NLR、GLI水平差异有统计学意义(P<0.05);多因素Logistic回归分析显示APACHEⅡ评分、血清CRP/ALB、NLR、GLI水平是重症肺炎患者死亡的危险因素;ROC曲线分析结果显示,CRP/ALB、NLR、GLI曲线下面积分别为0.837,0.826,0.837。结论 CRP/ALB、NLR、GLI水平对重症肺炎患者预后均具有较高的预测价值,其中以CRP/ALB的预测价值最佳。
Objective To explore the prognostic predictive value of serum CRP/ALB, NLR and GLI in patients with severe pneumonia. Methods A total of 126 patients with severe pneumonia treated in our hospital from January 2020 to May 2021 were divided into survival group (93 cases) and death group (33 cases) according to the prognosis of 28 days of hospitalization. The general data of the two groups were compared, the high-risk factors of patients with severe pneumonia were analyzed by multivariate logistic, and the predictive value of serum CRP/ALB, NLR and GLI levels on the prognosis of patients with severe pneumonia was evaluated by ROC curve. Results There were significant differences in the levels of NEU, CRP, APACHE Ⅱ, CRP/ALB, NLR and GLI between the two groups (P<0.05); multivariate logistic regression analysis showed that APACHE Ⅱ score, serum CRP/ALB, NLR and GLI were the risk factors of death in patients with severe pneumonia; ROC curve analysis showed that the areas under CRP/ALB, NLR and GLI curves were 0.837, 0.826 and 0.837 respectively. Conclusions The levels of CRP/ALB, NLR and GLI had high predictive value in the prognosis of patients with severe pneumonia, among which CRP/ALB had the best predictive value.
目的 研究喘憋性肺炎合并脓毒症患儿血清降钙素原(PCT)、C反应蛋白(CRP)水平的临床意义。方法 回顾性分析本院80例喘憋性肺炎患儿临床资料,根据是否合并脓毒症将80例患儿分为观察组(合并脓毒症,38例)和对照组(未合并脓毒症,42例)。比较两组患儿血清PCT、CRP水平,根据脓毒症严重程度将观察组患儿分为脓毒症组(14例)、严重脓毒症组(13例)及脓毒症休克组(11例),比较三组患儿血清PCT、CRP水平,记录患儿2周死亡率。分析血清PCT和CRP诊断喘憋性肺炎合并脓毒症患儿的准确性,探讨其判断预后的价值。结果 观察组患儿血清CRP和PCT水平高于对照组,差异均有统计学意义(P<0.05)。脓毒症休克组患儿血清CRP、PCT水平高于脓毒症组和严重脓毒症组,差异有有统计学意义(P<0.05)。严重脓毒症组患儿血清CRP和PCT均高于脓毒症组,差异有统计学意义(P<0.05)。80例患儿中死亡7例,死亡率8.75%,血清CRP与PCT早期诊断喘憋性肺炎患儿合并脓毒症的AUC分别为0.729和0.743(95%CL=0.617~0.841,0.637~0.849,P均<0.001),联合诊断的AUC分别为0.876(95%CL=0.792~0.960,P<0.001)。血清CRP和PCT判断患儿预后的AUC分别为0.794和0.813(95%CL =0.584~1.000,0.000~1.000,P =0.012,0.007),联合检测判断患儿预后的AUC为0.832(95%CL=0.638~1.000,P<0.001)。结论 喘憋性肺炎合并脓毒症患儿血清CRP和PCT均异常升高,监测血清CRP和PCT有助于脓毒症的早期诊断和患儿预后判断。
目的 探讨生物电抗无创心排监测(bioreactance noninvasive cardiac output monitoring,NICOM)心指数(cardiac index,CI)和总外周阻力指数(total peripheral resistance index TPRI)对呼吸困难病因诊断的临床价值。方法 采用前瞻性观察性研究的方法 ,纳入急性呼吸困难或慢性呼吸困难急性加重的患者共113例,临床医师根据2010年中华医学会心血管病学分会编委会组织编写的《急性心力衰竭诊断和治疗指南》为金标准分为心力衰竭组(n=55)和非心力衰竭组(n=58),通过NICOM监测CI、TPRI,构建受试者工作特征曲线(receiver operating characteristic curve,ROC曲线),分析CI和TPRI对心力衰竭的诊断价值。结果 心力衰竭组患者的CI低于非心力衰竭组(P<0.001);心力衰竭组患者TPRI高于非心力衰竭组(P<0.001);利用ROC曲线进行分析,CI曲线下面积(area under the curve,AUC)为0.792(95%CI:0.708~0.875,P=0.000),当CI截断值取2.65L/(min·m2)时,诊断心力衰竭的敏感度为63.6%,特异度为87.9%;TPRI的AUC为0.733(95%CI:0.641~0.825,P=0.000),当TPRI截断值取2 353dynes.sec/(cm5·m2)时,诊断心力衰竭的敏感度为72.7%,特异度为67.2%;CI联合TPRI诊断心力衰竭的敏感度为80%,特异度为65.5%。结论 NICOM监测CI对心力衰竭所致的呼吸困难特异度高,联合TPRI监测可提高敏感度。
Objective To investigate the clinical values of etiological diagnosis by testing cardiac index CI and total peripheral resistance index TPRI using bioreactance noninvasive cardiac output monitoring NICOM in patients with dyspnea. Methods Prospective,observational study was taken in 113 adult patients admitted with dyspnea or acute exacerbation of dyspnea in stable disease. The patients were divided into two groups namely heart failure group (n=55)and non-heart failure group(n=58) according to the guidelines on the diagnosis and treatment of acute heart failure(2010) as the standard criterion. All patients underwent CI and TPRI test by using NICOM. Receiver operating characteristic curve(ROC curve) was plotted to evaluate the diagnostic value of CI and TPRI. Results Compared with non-heart failure group,the CI was worse(P<0.001),and the TPRI was elevated(P<0.001). ROC curve showed that the area under the curve(AUC) of CI was 0.792(95%CI:0.708~0.875,P=0.000). The cut-off of CI was 2.65L/min/m2,the sensitivity was 63.6%,and specificity was 87.9%. The AUC of TPRI was 0.733(95%CI:0.641~0.825,P=0.000). The cut-off of CI was 2 353dynes.sec/cm5/m2,the sensitivity was 72.7%,and specificity was 67.2%. The sensitivity of CI combined TPRI was 80%,and specificity was 65.5%. Conclusion NICOM monitoring CI have high specificity,and combined TPRI monitored may improve sensitivity for dyspnea caused by heart failure.
目的 研究分析牙髓血管再生术治疗年轻恒牙的临床应用疗效。方法 选取本院2012年1月—2013年12月间年轻恒牙牙髓血管再生治疗的60例患者为研究对象,按随机数方式分成两组,每组30例。对照组患者应用根尖诱导成形手术治疗,观察组患者应用牙髓血管再生术进行治疗,统计比较两组患者术后VAS评分及临床疗效。结果 观察组患者治疗总有效率为93.3%,相较于对照组患者的90.0%差异无统计学意义(P>0.05);治疗前,两组患者VAS疼痛等级评分差异无统计学意义(P>0.05);治疗后,患者VAS评分降低明显,且观察组显著低于对照组,差异有统计学意义(P<0.05)。结论 临床中应用内封氢氧化钙糊剂血管再生术在年轻恒牙治疗中具有较好的应用效果,提高有效率,减轻疼痛,值得临床中应用与推广。
