目的 探讨计划-实施-检查-处理(PDCA)循环法及根本原因分析法在持续正压通气治疗重症肺炎患儿中的应用效果。方法 选取2021年1月—2023年1月医院收治并接受鼻塞式持续正压通气治疗的重症肺炎患儿80例,基于随机数字表法分为两组,每组各40例。对照组接受常规护理,观察组采用PDCA循环法联合根本原因分析法护理干预。比较两组临床症状改善时间、血气指标[动脉血氧分压(PaO2)、动脉血氧饱和度(SaO2)、动脉血二氧化碳分压(PaCO2)]、肺功能指标[第1秒用力呼气容积(FEV1)、潮气量(VT)]、炎症免疫指标[单核细胞趋化蛋白-1(MCP-1)、可溶性髓系细胞触发受体-1(sTREM-1)、可溶性细胞间黏附分子-1(sICAM-1)]水平变化,比较两组患儿家属满意度情况。结果 观察组患儿咳嗽、气促、哮鸣音、心率恢复及紫绀等症状改善用时均低于对照组,比较差异有统计学意义(P<0.05)。干预前,两组患儿PaO2、SaO2、PaCO2、FEV1、VT、MCP-1、sTREM-1、sICAM-1水平比较差异均无统计学意义(P>0.05);干预后,两组患儿上述指标水平均有不同程度变化(P<0.05),观察组PaO2(97.18±7.90 mmHg vs 90.30±7.12 mmHg)、SaO2(93.58±3.82% vs 86.30±4.21%)、FEV1(2.66±0.46 L vs 1.97±0.34 L)、VT(11.92±1.89% vs 9.83±1.10%)水平均高于对照组,观察组PaCO2(36.70±3.97 mmHg vs 40.65±3.79 mmHg)、MCP-1(58.45±11.94 ng/L vs 74.46±16.69 ng/L)、sTREM-1(36.25±8.30 ng/L vs 51.57±9.51 ng/L)、sICAM-1(187.52±31.22 mg/L vs 243.73±46.79 mg/L)水平低于对照组,差异均有统计学意义(P<0.05)。观察组患儿家属满意度整体优于对照组(P<0.05);观察组总满意率(97.5% vs 77.5%)高于对照组,差异有统计学意义(P<0.05)。结论 PDCA循环法及根本原因分析法应用于接受持续正压通气治疗的重症肺炎患儿,能够有效促进患儿临床症状改善,有利于血气及肺功能恢复,且可降低炎症反应,患儿家属满意度较高。
Objective To explore the application effect of Plan-Do-Check-Action(PDCA)circulation method and root cause analysis method in the treatment of children with severe pneumonia with continuous positive pressure ventilation.Methods From January 2021 to January 2023,80 children with severe pneumonia who were admitted to hospital and received nasal plug continuous positive pressure ventilation treatment were enrolled in this study.Based on the random number table method,they were divided into two groups,with 40 cases in each group.The control group received routine nursing,while the observation group received PDCA circulation method combined with root cause analysis nursing intervention.The improvement time of clinical symptoms,changes in blood gas indicators[arterial partial oxygen pressure(PaO2),arterial oxygen saturation(SaO2),arterial partial pressure of carbon dioxide(PaCO2)],lung function indicators [(forced expiratory volume in 1 second,FEV1),tidal volume(VT)],and inflammatory immune indicators [monocyte chemotactic protein-1(MCP-1),soluble myeloid cell trigger receptor-1(sTREM-1),soluble intercellular adhesion molecule-1(sICAM-1)] levels between the two groups were compared,and the family members’ satisfaction of the two groups was also compared.Results The improvement time for symptoms such as cough,shortness of breath,wheezing,heart rate recovery and cyanosis in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Before intervention,there was no statistically significant difference in the levels of PaO2,SaO2,PaCO2,FEV1,VT,MCP-1,sTREM-1 and sICAM-1 between the two groups of children(P>0.05).After intervention,the levels of the above indicators in both groups of children showed varying degrees of change(P<0.05).The levels of PaO2(97.18±7.90 mmHg vs 90.30±7.12 mmHg),SaO2(93.58±3.82% vs 86.30±4.21%),FEV1(2.66±0.46 L vs 1.97±0.34 L),VT(11.92±1.89% vs 9.83±1.10%)in the observation group were higher than those in the control group.The levels of PaCO2(36.70±3.97 mmHg vs 40.65±3.79 mmHg),MCP-1(58.45±11.94 ng/L vs 74.46±16.69 ng/L),sTREM-1(36.25±8.30 ng/L vs 51.57±9.51 ng/L)and sICAM-1(187.52±31.22 mg/L vs 243.73±46.79 mg/L)in the observation group were lower than those of the control group,with statistically significant differences(P<0.05).The overall satisfaction of the observation group was better than that of the control group(P<0.05),the total family members’ satisfaction rate of the observation group was higher than that of the control group(97.5% vs 77.5%),with a statistically significant difference(P<0.05).Conclusions PDCA circulation method and root cause analysis method applied to children with severe pneumonia who receive continuous positive pressure ventilation treatment,can effectively promote the improvement of clinical symptoms,be conducive to the recovery of blood gas and lung function,and reduce inflammatory reaction,with high family members’ satisfaction.

目的 探讨暴露喉返神经的甲状腺手术后患者发生声音嘶哑的原因。方法 选取于2019年1月—2020年12月间在我院接受甲状腺手术且在术中暴露喉返神经的患者,对出现术后声音嘶哑的19例患者进行为期12个月的临床随访,观察研究对象术后声音嘶哑的发生特点、持续时间并进行直接喉镜及颈部超声检查。结果 发生声音嘶哑的19例患者中,5例存在术中喉返神经损伤情况,其余14例患者术中喉返神经暴露及保护良好。直接喉镜检查示,该14例患者中,4例存在声带充血水肿现象,1例发生勺状软骨半脱位。术后1周左右的超声检查显示,该14例患者中有11例存在不同程度的创腔内积液。结论 虽常规暴露喉返神经减少了术后声嘶的发生率,但术中喉返神经损伤仍然是造成患者术后声嘶的原因之一。此外,术后创腔积液、麻醉插管导致的声带损伤及其他插管相关并发症等非直接手术因素也是造成这些患者术后声嘶的重要原因,应引起临床重视。
Objective To explore the cause of hoarseness of voice in patients after thyroidectomy with recurrent laryngeal nerve exposure. Methods The patients from January 2019 to December 2020 underwent thyroidectomy with exposure of recurrent laryngeal nerve during operation were selected. There were 19 cases of hoarseness of voice after operation followed up for 12 months. Postoperative observations included the characteristics of the hoarseness of voice, duration, and direct laryngoscope neck ultrasonography. Results A total of 19 patients had voice hoarseness, only 5 of them had recurrent laryngeal nerve injury during operation, the other 14 patients had good exposure and protection of recurrent laryngeal nerve. Direct laryngoscope showed that 4 of 14 patients had vocal cord edema and 1 had subluxation of arytenoid cartilage. About 1 week after operation, ultrasound examination showed that 11 of 14 patients had varying degrees hydrops of wound cavity. Conclusions Although the routine exposure of recurrent laryngeal nerve reduces the incidence of postoperative hoarseness of voice, the injury of recurrent laryngeal nerve is still a cause of postoperative hoarseness of voice. In addition, non-operative direct factors, such as fluid accumulation in the operative field, vocal cord injury caused by anesthetic intubation and other intubation related complications, are also important reasons for postoperative hoarseness of voice in these patients, which we should pay more attention to.

目的 分析银川地区单采献血者流失原因,制定回召策略。方法 对银川地区末次献血时间在2019年1月1日—2020年12月31日的1 498名单采献血者逐个致电回访,使用SPSS 26.0软件进行统计学分析,不同组间变量比较用χ2检验,P<0.05为差异有统计学意义。结果 成功召回355人(23.7%),和未召回组比较,性别无统计学差异(P>0.05),各年龄段、民族、职业、文化程度、血型之间均有统计学差异(P<0.05)。18~24岁、汉族、国家企事业单位、研究生及以上学历人群回召率最高;随着年龄增长、文化程度降低,回召成功率下降。未召回原因中工作繁忙占比最大,其次为在外地、身体原因、亲友献血者、失联、学生毕业、对献血误解、停车不便。结论 不断优化、细化单采献血服务,让工作更加人性化,加大献血正面形象的宣传,不断创新,从而保留固定献血者。
Objective To analyze the causes of apheresis blood donors loss in Yinchuan area, and to work out the call-back strategy. Methods A total of 1 498 blood donors whose last blood donation time was between January 1, 2019 and December 31, 2020 in Yinchuan area were called one by one. SPSS 26.0 software was used for statistical analysis. Variables between different groups were compared usingχ2 test, and P<0.05 was considered statistically significant. Results Three hundred and fifty-five people (23.7%) were successfully called back. Compared with the failed call-back group, there was no significant difference in gender (P>0.05), but there were significant differences in age, nationality, occupation, education level and blood type (P<0.05). People aged 18-24, with Han Nationality, working in national enterprises or institutions, or with graduate degree or above had the highest call-back rate; with the increase of age and the decrease of education level, the success rate of call-back decreased. Among the reasons for call-back failure, no free time accounted for the largest proportion, followed by being in other places, physical reasons, donors of relatives and friends, losing contact, graduated from school, misunderstanding of blood donation, and inconvenient parking. Conclusion We will continue to optimize and refine the apheresis blood donation service, make the work more humanized, expand the publicity of the positive image of blood donation, and innovate constantly, so as to retain the blood donors.

目的 探讨无偿献血者ABO血型正反定型初筛结果不一致的原因分析。方法 选取2019年6月1日—2020年11月30日本血站的无偿献血者样本,采用Metis150全自动血型分析仪进行ABO正反定型初筛。初筛结果正反定型不一致先进行试管法复核,结果仍不一致时送血型参比实验室做进一步确认,并对造成ABO血型正反定型不一致的原因进行分析。结果 44 808例无偿献血者样本中初筛结果ABO血型正反定型不一致134例,发生率为0.30%。导致ABO正反定型不一致原因为:因方法学不同可经盐水介质试管法复检确认65例(48.51%);吸取红细胞或血浆异常加样量不准确25例(18.66%);仪器判读错误4例(2.98%);血型参比实验室通过延长反应时间或改变反应温度确定因抗体缺失或减少22例(16.42%)、冷凝集素2例(1.49%);增加抗A1、抗AB、抗H血清和吸收放散试验确定亚型6例(4.48%)、抗原减少2例(1.49%);洗涤自身红细胞吸收放散后定型、吸收后血清反定型确定不规则抗体8例(5.97%)。40例血型参比实验ABO血型血清学检测结果为:O型21例(52.50%)、A型8例(20%)、B型7例(17.5%)、AB型4例(10.00%)。结论 ABO血型正反定型不一致的原因很复杂。严格规范操作,减少不规则抗体对结果的影响,防止弱亚型的漏检等有效措施确保临床输血的安全。

目的 探讨Sysmex-2100全自动血液分析仪检测β-地中海贫血(β-地贫)患儿血小板参数显示不全的原因。方法 收集2017年4月—2020年4月期间本院确诊β-地贫患儿301人,其中重型病例204例,非重型97例。对301例患儿均采用希森美康Sysmex-2100血细胞分析仪进行血细胞分析,标本制作血涂片充分干燥后经瑞氏-吉姆萨染色,由主管技师及以上检验人员进行人工镜检。结果 301例β-地贫患儿血细胞分析结果出现血小板参数不显示者102例,占33.89%;其中有小红细胞、红细胞碎片、血小板聚集97例(95.10%);血小板直方图异常93例(91.18%)。重型β-地贫患儿血小板参数显示不全者75例,非重型β-地贫患儿血小板参数显示不全者27例,两组间血小板参数显示不全比例差异无统计学意义(P= 0.126)。血小板参数全显示病例的平均红细胞体积高于血小板参数显示不全病例,差异有统计学意义(P< 0.0001)。结论 小细胞低色素性红细胞及红细胞碎片化是β-地贫患儿血小板参数显示不全的主要原因,通过血小板直方图正常与否可大致判断血小板参数的可信度,采用PLT-O通道检测及涂片镜检查找原因,可提高结果的准确性。

目的 分析2016—2018年度广州市单采血小板报废的主要原因,并探讨如何降低单采血小板报废率。方法 统计2016—2018年度广州市单采血小板的采集量、报废数量和报废率等相关数据,分析报废原因,提出相应改进措施。结果 2016—2018年广州血液中心单采血小板总报废率为1.13%,三年的报废率逐年下降。报废的最主要原因为血液感染因子筛查不合格或ALT异常,占总报废率64.44%;其次分别为血量不足(占比14.4%)、采血耗材/血袋/热合口漏(占比11.01%)等。结论 我中心单采血小板报废率虽逐年下降,但仍需加强献血前宣传,动员更多的健康适龄公民参加无偿献血,同时重视采前血液感染因子的筛查工作、采血前及采血过程的护理干预、设备维护保养和产品处理能力的培训,进一步有效降低血液报废率。
Objective To analyze the main causes of apheresis platelets discarding in Guangzhou from 2016 to 2018, and discuss how to reduce the discarding rate. Methods Collected the data of apheresis platelets collection, the discarding number and the discarding rate in Guangzhou from 2016 to 2018,to analysis the reasons of apheresis platelets discarding and introducing improvement measures. Results From 2016 to 2018, the total discarding rate of apheresis platelets collected from Guangzhou Blood Center was 1.13%. In the three years,the discarding rate declined year by year.The main reasons for apheresis platelets discarding were the result of blood infection factor test or ALT was not qualified, accounting for 64.44% of the total scrapping rate. Then followed by insufficient blood volume (14.4%) and Blood consumables/blood bags/heat closure leakage (11.01%). Conclusion Although the rate of apheresis platelet discarding in Guangzhou Blood Center is decreasing year by year, we still need to enhance blood donation propaganda, mobilize more healthy citizens to participate in blood donation, and pay attention to the screening of blood infection factors before blood collection, nursing blood donor before and during blood collection, and training of ability in equipment maintenance and product handling, so as to further effectively reduce the rate of blood discarding.

目的 探讨导致口服华法林患者国际标准化比值异常增高(INR>4.5)的原因。方法 选择148例做过心瓣膜置换术,出院后口服华法林抗凝,复查时国际标准化比值(INR>4.5)异常增高的患者。通过对其用药量以及服用其它药物(包括西药和中药)、营养品、食物等情况进行调查,分析导致口服华法林患者INR异常增高的有关因素,同时对PT检测过程中检验前、检验中的质量控制进行调查,排除检测过程中的影响因素,最后综合分析导致心瓣膜置换术后口服华法林患者INR异常增高的原因。结果 导致口服华法林患者INR异常增高的原因主要是药量过多,占全部调查的47.29%,其中医嘱剂量过多的为39.18%,遗忘重复服药或不理解医嘱的比例为8.10%;其次是受药物相互影响的比例为43.25%,其中受西药影响的比例为33.11%,受中药物影响的占10.14%;药物外影响的因素比例为9.47%,其中受饮食影响的比例为4.06%;受物理治疗影响的比例为5.41%。结论 导致口服华法林患者INR异常增高最主要的原因是用药量过多以及受其他药物的相互影响,同时受饮食及物理治疗的影响。
Objective To investigate the cause of abnormality increase of international normalized ratio (INR) (INR>4.5) in the patients who oral application Warfarin. Methods 148 patients were taken who had cardiac valve replacement operation and oral application warfarin post-discharge as subjects. Their INR abnormality increased when taking reexamination. Through the research of the drugs, the dose (including both Chinese crude drug and western medicine), the nourishments and the foods that the patients took, we find out factors that resulted in INR abnormality increase, while to investigate the quality control of laboratory department and analyze the process before and after PT test, to find out factors that affect the test. Finally, to analyze the causes of INR abnormality increase in patients took warfarin after cardiac valve replacement operation. Results The main reason for the abnormal increase in INR in patients with oral warfarin was the excessive dose, accounting for 47.29% of all surveys, of which 39.18% were excessively prescribed, and the proportion of patients who forgot to repeat or did not understand the doctor's order was 8.10%; The proportion of drug interaction was 43.25%, of which 33.11% were affected by western medicine, 10.14% were affected by Chinese medicine, 9.47% were affected by external medicine, and 4.06% were affected by diet; The proportion of treatment effects was 5.41%. Conclusion The main reason for the abnormal increase in INR in patients with oral warfarin is the excessive use of drugs and the interaction of other drugs, as well as the effects of diet and physical therapy.

目的 探讨机采血小板献血者流失原因,为建立一支稳固的机采血小板献血者队伍提供参考。方法 选择2009年1月1日—2012年12月31日河源市中心血站首次机采血小板献血者共382例为研究对象,根据其首次机采血小板献血后3年内是否再次参加机采血小板献血分为实验组和对照组,分别对研究对象进行电话回访调查。结果 3年内未再次参加血小板献血流失者118例,总流失率为30.89%;不同献血动机组流失率差异有统计学意义 (χ2= 18.552,P=0.000),其中动机不明确组流失率(52.49%)最高,寻求社会荣誉组流失率(15.79%)最低。发生过献血反应组(63.64%)流失率显著高于未发生献血反应组流失率(26.63%),两组间差异有统计学意义(χ2=8.501,P=0.004);对献血过程不满意组流失率(39.77%)显著高于满意组流失率(28.23%),两组间差异有统计学意义(χ2=16.413,P=0.000);认为机采时间过长组机采血小板献血者流失率(49.40%)显著高于不认为机采时间过长组流失率为(25.75%),两组间差异有统计学意义(χ2=17.014,P=0.000)。健康状况(体检结果)不合格组流失率(43.84%)显著高于合格组流失率(27.83%),两组间差异有统计学意义(χ2=9.022,P=0.030)。媒体(或网络)负面的宣传与献血者是否方便对机采血小板献血者流失率的影响不显著(χ2=1.579,P=0.209;χ2=2.130,P=0.144)。结论 机采血小板献血者流失主要与献血动机、献血过程感受以及自身健康等因素有关。
