无创正压通气老年患者鼻面部压力性损伤预防的研究进展

Research progress in the prevention of nasal and facial stress injuries in elderly patients with noninvasive positive pressure ventilation

       无创正压通气的使用是患者发生鼻面部压力性损伤的重要原因之一,随着我国防控政策的放开,新增老年新冠感染患者急剧增加。文章对使用无创正压通气发生相关压力性损伤现状进行综述,介绍老年患者使用无创正压通气发生鼻面部压力性损伤的主要影响因素、评估方法以及预防措施现状,旨在为临床医务人员预防其相关压力性损伤提供参考。
       The use of non-invasive positive pressure ventilation is one of the important reasons for nasal and facial pressure injuries in patients.With the release of China's epidemic prevention policy,the number of new elderly patients with COVID-19 has increased dramatically.This paper summarizes the current situation of stress injuries caused by noninvasive positive pressure ventilation,introduces the main influencing factors,evaluation methods and prevention measures of nasal and facial stress injuries in elderly patients with noninvasive positive pressure ventilation,aiming to provide a reference for clinical medical personnel to prevent their related stress injuries.
       器械相关压力性损伤(medical  device  related pressure injury,MDRPI)是指医疗设备和患者皮肤之间因热、湿和压力等造成的皮肤、黏膜等的相关损伤,近期报道的发病率约为28%[1],有医疗设备的患者压力性损伤(pressure injury,PI)发生率约为无医疗设备患者的2.4倍[2]
       2020年爆发的新型冠状病毒感染(新冠),其主要影响呼吸系统,随着时间的推移,病毒经历了多次变异,从最初的原始株到阿尔法、德尔塔等变种,直至当前广泛传播的JN.1系列变异株和XDV系列变异株,两者均为奥密克戎变异株。奥密克戎株不仅传播力强,传播速度快,还具有较高的隐蔽性。这些特性使得老年人和有基础健康问题的人群在感染后发展为重症的风险显著增[3]。Duca等[4]在2020年进行的一项研究显示,严重急性呼吸衰竭的入院患者占所有入院患者的6.1%~41%。随着我国人口老龄化加快、新冠防控政策的放开,医院收治的重症感染患者中尤其是老年人,发生呼吸衰竭的数量急剧增加。无创正压通气(noninvasive  positive  pressure ventilation,NIPPV)是危重患者呼吸衰竭管理的初始策略,与有创通气相比,使用NIPPV可以降低医院感染和常见气管插管相关并发症的风险,并在总体上缩短住院时间和降低相关的医疗费用[5-7]NIPPV通过减轻呼吸系统的负荷,降低呼吸肌肉的氧气消耗,提高肺泡的通气效率和气体交换,从而促进氧气的吸收和二氧化碳的排出,这种治疗方法能够显著改善患者的呼吸功能和整体健康状况[8]。然而,老年人代谢率降低,皮肤脆弱性增加;鼻梁和脸颊具有弯曲的轮廓,压力不易分配均匀,且鼻面部皮肤薄弱、抗压力小,是持续细胞和组织变形的较难耐受部位[9]。NIPPV面罩已被确定为MDRPI的来源,面罩的不匹配以及患者皮肤问题会降低NIPPV有效性及患者的依从[10]。据统计,NIPPV过程中鼻面部PI发生率为7%~27%。一项关于NIPPV使用的国际调查显示,由于患者对界面的依从性差和相关并发症,约42%的患者NIPPV失败[11]。本文通过回顾文献,分析使用NIPPV患者鼻面部PI预防的研究进展,旨在为临床医护人员预防NIPPV患者使用过程中发生鼻面部MDRPI提供参考。

1  国内外研究进展

1.1  界面材料的创新

       新型界面材料的研发是预防NIPPV鼻面部PI的关键[12]。当前研究集中在使用生物相容性更好、透气性更佳的材料,如硅胶和聚氨酯,这些材料能提供柔软的贴合感,减少长时间佩戴引起的不适。此外,材料的抗菌特性也在研究之中,以减少因细菌感染而加剧的皮肤损伤。智能材料如能够根据面部温度和湿度变化自动调节的凝胶,也被探索用于减轻压力点和摩擦[13]

1.2  个性化面罩设计

       通过3D扫描技术获取患者的面部几何数据,结合3D打印技术,可以制作出完美贴合患者面部轮廓的个性化NIPPV面罩[14]。这种定制化方法不仅提高了患者的舒适度,还有效分散面部压力,降低PI的风险。个性化面罩的设计还考虑了患者的皮肤敏感性和呼吸需求,以实现最佳的治疗效果和患者体验。

1.3  智能监测系统

       集成到NIPPV系统中的智能监测设备能够实时跟踪面罩内的压力分布、湿度和温度,为医护人员提供关键数据,以便及时调整治疗方案。这些系统可能包括压力传感器、湿度计和温度探头,它们能够预测潜在的P I并提醒医护人员进行干预,从而提高预防措施的及时性和有效性[15]

1.4  敷料技术的进步

       敷料技术的进步为需要NIPPV患者提供了更多的选择[1]。新型敷料如水凝胶和泡沫敷料,不仅能够提供额外的缓冲,减少压力点的形成,还能够管理皮肤湿度,防止过度湿润导致的损伤[10-11]这些敷料的高吸收性和粘附性使其能够稳定地固定在皮肤上,同时保持皮肤的自然呼吸。

1.5  风险评估工具的开发

       专门针对NIPPV患者的PI风险评估工具的开发,有助于医护人员快速识别高风险患者[16]。这些工具通常分析患者的生理、心理和环境因素,通过算法预测PI的可能性。准确的风险评估工具能够指导临床决策,优化预防资源的分配。

2  NIPPV 患者鼻面部发生 MDRPI 的危险因素

2.1  患者因素

       高龄、意识不清、营养不良、循环障碍等是患者发生MDRPI危险的重要因素[12];老年人基础疾病多、机体功能及抗外界打击能力差等特点,是诱发PI的常见高危因素;高血糖、低血压、低蛋白等也是诱发PI的重要原因[13] 。祁进芳等[12]研究指出,水肿的患者由于营养物质交换差、组织压力的加剧,其面部发生MDRPI的风险是无水肿患者的3.4倍。此外,局部皮肤的温度也是患者发生MDRPI的重要影响因素,过低的皮温会导致血液循环差,长时间受压后易引起组织的坏死;皮温过高则血流循环加快,组织耗氧量增加,进而增加MDRPI的风险[13]。血管活性药物的长期使用与发生MDRPI风险相关,可能原因是重要脏器的血液供应通过血管收缩来维持,从而使外周皮肤组织灌注减少导致缺血、缺氧[12]。此外,镇静药物、脑病、神经疾病/损伤、表达障碍、昏迷等因素会减弱患者对组织缺血反应的压力和运动的意识,使患者活动能力下降、感觉知觉下降、灌注减少等增加了患者发生MDRPI的风险[14]

2.2  器械因素

       使用NIPPV设备本身会对患者的鼻面部产生压力,由于僵硬的口罩含有传统坚硬聚合物材料的通用设计,通常与面部组织的形状和生物力学特性不匹配,NIPPV面罩的长期使用,使之在皮肤狭窄的接触部位施加压缩力和剪切力,进而转化为复合负载状态(压缩、拉伸和剪切),加之固定带使面部软组织收紧,导致局部变形诱导的细胞死亡而对面部组织的活力产生风险[10]。此外,设备引起的面部潮湿使摩擦系数增加,面部PI风险增加了5倍[12]。研究表明,大约1/3的医院获得性PI与此相关[10,15]

2.3  使用时长

       有研究显示,NIPPV连续使用2 ~ 4 h,其发生MDRPI范围在5% ~ 50%[17]。不间断使用48 h后,由面罩引起MDRPI的可能性增加至100%[8]。受NIPPV压力效应影响,患者鼻面部出现PI率至少为17%[18]

2.4  医疗相关因素

       老人往往共病共存,病情复杂多变,医疗器械的使用大大增加了发生MDRPI的风险[19]。我国学者对重症监护室(Intensive Care Unit,ICU)护士对预防MDRPI的知信行现状进行调查,结果显示护士对MDRPI的态度积极,但知识以及行为水平有待提高[20]。随着防疫政策的开放,ICU床位缺乏,迫使普通病房的NIPPV使用增加。Cabrini[11]等进行了非ICU病房急性呼吸衰竭患者NIPPV使用的国际调查,接受调查的157个国家及地区中,66%的医院在普通病房应用了NIPPV,且使用率在不断增加,但普通病房缺乏相关培训以及使用方案,对NIPPV使用技术和组织方面存在显著的异质性、界面耐受不良和无法控制的鼻面部PI是NIPPV失败最常见的原因。

3  NIPPV 患者鼻面部 MDRPI 的评估与管理

3.1  风险评估

       3.1.1  风险评估工具   目前国内外尚无专门针对MDRPI发生风险的评估工具,也未检索到运用于NIPPV患者鼻面部的PI风险评估工具。目前,评估MDRPI使用的量表有Braden量表、Norton量表和Waterlow量表[21]。国外有研究显示[14],Braden量表在评估骨隆突处P I和MDRPI的关键风险因素一致。我国也有报道指出[22],Braden量表对NIPPV患者鼻面部PI发生具有预测性,其评分每增加1分,MDRPI发生率降低19.6%。国内外研究数据表明,Braden评分可以为COVID-19危重患者发生MDRPI提供有用的风险分层[14,22]。此外,目前尚无MDRPI的风险评估工具,未来还需开发MDRPI相关风险评估工具,构建使用NIPPV老年患者发生鼻面部PI的风险预测模型,提高临床医务人员识别风险的准确度、提高服务质量。
       3.1.2  评估时间   在入院后应尽快进行PI风险评估,个体重新定位后应立即进行结构化的皮肤和组织的评估[23]。压疮预防和治疗指南建议每天至少进行两次评估[17],但Bambi等[24]的研究指出,此建议是基于低水平的证据,在持续负荷下,PI可在4 ~ 6 h内形成,皮肤评估应每2~ 4 h进行一次。Manzano等[25]的研究发现,每2 h重新定位在预防PI方面并不比每4 h重新定位更有效,相反,增加了设备相关的不良事件和护理工作负担。因此,推荐应优先考虑依从性,确保每4 ~ 6 h进行一次有效的皮肤评估,并相应地修改预防措[25-26]

3.2  界面接口选择

       目前常用的界面接口主要有鼻罩、口鼻面罩、全脸面罩和头盔,尽管NIPPV可用的界面接口类型和设计越来越多,仍有10%~31%的患者发生PI[5]。良好的界面接口应具备无泄漏、稳定性好、无创伤、质量轻等特点,同时界面应具有灵活、柔软等特点,以减少面部创伤[17]。国外有研究对不同的设备进行了比较,并评估了它们在使用NIPPV时的有效性和耐受性。由于大量的空气流失,经鼻罩使用时的NIPPV的耐受性较差[8];但在某些情况下,由于幽闭恐怖症或频繁咳嗽,鼻面罩可能比口鼻面罩更耐受。Schallom等[18]的研究对比口鼻面罩和全脸面罩使用效果,全脸面罩由于其增大压力分布的特性,其使用降低了PI的发生率、口鼻口罩产生PI的平均时间为28.4 h,全脸面罩为61.37 h;且患者使用感更舒适,推荐作为传统口鼻面罩的合理替代方案。与面罩相比,随着时间的推移,头盔已被证实具有更大的耐受性和更低的NIPPV中断率,头盔的使用可减少漏气以及降低皮肤损伤[8]。Grigatti[9]等的一项研究也指出,在不同的NIPPV接口中,鼻罩和口鼻罩的耐受性最差,而头盔的耐受性似乎更好,可使NIPPV使用时间更长,失败率更低。此外,新推出的改良式无臂撑的头盔克服了引起腋窝损伤的问题,在呼吸机触发和加压率方面显示出更好的呼吸辅助性能,可发挥更好的患者-呼吸机相互作[27]

3.3  压力再分配

       通过重新放置界面接口、不同类型的界面接口进行轮换,可达到进行鼻面部压力再分配的作用,从而促进组织血液流动来减少缺血,常规使用4 h后重新放置界面接口,对能耐受患者进行重新放置后休息10 min[17]。建议连续使用NIPPV最长时间为11 h。在此之后,建议移除接口,让皮肤休息至少4 h[18]。对不能忍受NIPPV治疗中停顿的患者,使用“V”型鼻枕或头盔防护系统可以帮助预防面部皮肤损伤[18,23]。此外,施加在患者皮肤上的压力尽可能小,根据Schettino等[28]的研究,为防止泄漏,界面对皮肤的压力应该比吸气压力大2~3 cm/H2O,由于大多数环境缺乏客观压力测量技术,除以患者反馈以及经验指导外,可在背带和面部皮肤之间留出2根手指的距离,悬挂呼吸机电路以防止接口移动。为了缓解患者面部的压力再分配,建议使用覆盖脸颊和前额更大面积的全脸口罩,并定期旋转和交换口罩界面,以减少PI的发生率[18]

3.4  局部皮肤管理

       老年患者皮肤松弛、皮下组织薄弱,骨骼突出部位易发生MDRPI[29]。危重患者外周循环处于低灌注状态,常出现代谢紊乱,从而削弱了皮肤保护的能力;加上因患者的汗液、湿气等原因,潮湿状态的皮肤屏障功能受损,其发生PI风险是干燥皮肤的5倍[30]。国外一项研究指出[29],卧床的老年患者皮肤上有更多的肠道相关菌群、更少的共生菌、更高的皮肤pH酸碱度和更低的水分流失, 通过保持皮肤干燥和清洁以及适当的滋润来保护皮肤是防止浸渍和减少摩擦的必要条件。指南建议使用pH酸碱度为4.0 ~ 7.0(微酸性到中性)的洗面乳,可以减少潜在的干燥、红斑和刺[31]。Avsar等[32]的研究也证实,使用pH酸碱度平衡的清洁湿巾清洁皮肤,可有效降低PI发生率。此外,保持皮肤的滋润可有效降低设备与皮肤的摩擦系数,常使用的凡士林软膏、乳霜或是可形成薄膜的皮肤保护剂,但支持何种类型的润肤霜的证据不一致[33]。一项随机对照试验[26]果显示,在与口鼻面罩接触的面部皮肤上使用高氧脂肪酸,在对防NIPPV相关的PI表现出最高的功效,并建议在NIPPV期间每4 ~ 6 h在界面接触区域使用高氧脂肪酸和(或)精油。

3.5  敷料选择

       减轻局部、持续的组织负荷和皮肤微环境的管理是用于预防PI材料的最关键性能标准[1,30]使用NIPPV界面可通过在口罩-面部接触区域提供额外缓冲来降低与使用NIPPV界面相关的PI发生的具体风险。Peko等[10]的一项研究显示,根据患者所使用的界面大小轮廓进行敷料剪裁的应用,在生物力学上有效地减少了面部组织暴露于升高的机械负荷。特别是在面部的骨突出区域(如鼻梁、下巴),可以有效地保存组织厚度,减少软组织暴露于升高的变形和应力,从而预防患者鼻面部MDRPI的发生[10,15]
       3.5.1  水胶体敷料   水胶体是预防MDRPI最常用的材料之一,有研究显示,早期预防性使用减压水胶体敷料可降低患者使用NIPPV发生PI风险[34]。但Gefen[1]等计算材料刚度和敷料的导热系数与皮肤之间的匹配度,结果显示用于预防MDRPI的水胶体与皮肤匹配的硬度及生物力学保护效果较差。此外,由于水胶体的强黏性,它可能会加重已经存在的压力损伤[35]
       3.5.2  水凝胶敷料   Grigatti等[9]对多种材料进行测试,结果显示水凝胶与皮肤的匹配程度最佳。水凝胶的敷料在所有测试的面部部位都显示出了保护效果,但在鼻梁和脸颊上效果最好,分别保持至少65%和89%的保护能力,而这两个部位损伤风险最大。有研究显示[36],凝胶敷料、泡沫敷料在降低NIPPV患者鼻面部PI发生率方面最具有优势。
       3.5.3  泡沫敷料  Peko等[37]通过生物工程建模,证明了柔软的多层硅泡沫敷料在使用NIPPV患者鼻面部具有良好的耐受性,能在患者俯卧位时最大限度地减少对皮肤的剪切,同时吸走水分,使汗液蒸发而不残留在皮肤上,有助于减少作用在皮肤上的摩擦力。该研究指出用于新冠病毒易感患者的敷料应根据患者头部尺寸进行剪裁,使用时间参照说明书,建议每天拉开敷料,检查敷料下的皮肤,敷料出现浸渍、变形等情况下及时更换。最后,这些敷料应与其他全面的预防PI策略结合使用,仔细定位和每2 h向头部和下巴进行小幅度移动以减轻压力[37]。我国学者也证实对泡沫敷料进行反向粘贴可有效降低使用NIPPV患者PI的发生率,提高患者的舒适度[38]

3.6  改进措施

       3.6.1  加强培训   进行系统化的培训,加强对医务人员NIPPV相关PI知识的培训,掌握医疗器械的用途和功能,并严格按照说明书使用,调节正确的压力水平,以确保不漏气的情况下避免对底层组织的过度压力[26]。提高护理人员对预防鼻面部PI的意识,开展包括评估、界面选择、防护措施等在内的教育,并制定有效的PI预防护理策略[39]王玉梅等[40]基于SKIN(皮肤评估Skin;卸载Keep Pressure Off;失禁管理Incontinence;潮湿管理Moisture)的概念框架,对NIPPV患者进行鼻面部PI预防实践,有效地提高了护理人员预防使用NIPPV患者预防PI的技能,降低了患者鼻面部PI的发生率。
       3.6.2  加强多学科的合作   国内外研究显示,医护人员、营养师、呼吸治疗师、伤口造口专科护士等构建的多学科PI管理团队,可快速识别患者发生PI风险,并迅速采取全面的护理措施,在降低PI发生率上取得了积极的效果[21]。同时,还需合理配备人力资源,建议护理人员参与产品的选择和质量改进措施,提供及时、有效的改进策略[41]

4  讨 论

       在NIPPV鼻面部PI的预防领域,个性化面罩设计和智能监测系统的创新正逐渐改变临床实践。尽管3D打印技术为患者提供了定制化的面罩,但成本、耐用性及快速面部形状变化的适应性仍是挑战。同时,智能监测系统虽然能提供实时数据,但其集成度、数据解读及患者隐私保护也需进一步研究。此外,敷料技术的进步为患者带来了更舒适的体验,但敷料的适应性、成本效益和个体反应差异也不容忽视。
       风险评估工具的开发和多学科团队合作是提高预防效果的两大支柱。然而,评估工具的准确性、实用性和多学科团队的组织管理仍需深入探讨。患者教育在自我管理中扮演着关键角色,但教育策略的有效性和临床实践指南的更新实施同样面临挑战。远程医疗技术提供了新的监测和支持途径,但其可接受性、监管和服务质量保证也是我们必须考虑的问题。
       临床试验和随机对照试验是验证预防措施有效性的基础,但试验设计的复杂性、资源限制和伦理考量不容忽视。此外,如何确保研究成果能够转化为临床实践,提高患者护理质量,也是我们必须面对的挑战。通过不断探索和改进,我们可以更好地理解NIPPV鼻面部PI预防的复杂性,为患者提供更安全、更有效的治疗方案。
       本文通过文献检索总结了医疗设备的患者PI相关危险因素,如老年、营养不良和循环障碍,以及面罩设计和材料,阐明了MDRPI的多面性。我们应以患者为中心,通过分析患者年龄、营养不良、循环障碍等相关因素以及面罩设计和材料等仪器因素,尽早开展个性化评估避免医疗设备导致的患者PI的发生。此外我们还应关注在临床环境中实施这些预防措施所面临的实际挑战,如资源可用性、人员培训和合规性问题。医护人员应采用全面、多学科方法使用标准化风险评估工具、整合最佳实践来选择和管理皮肤界面,改善患者治疗体验,并最终提高治疗效果。
       尽管本研究为预防接受NIPPV治疗的患者鼻面部压力损伤的研究进展进行了总结与归纳,但必须认识到本结果的局限性:(1) 研究结果可能不适用于所有患者人群,特别是那些具有不同人口统计学特征或共病患者。由于患者样本可能无法充分代表需要NIPPV的个体多样性,因此,在将其应用于其他情境时需要谨慎解释结果。(2)数据收集方法可能存在一定局限性。
       患者教育对于提高自我管理能力至关重要,但需考虑文化、教育水平和个人偏好等因素,制定个性化的教育计划,并评估其效果。同时,临床实践指南的更新应基于最新证据,考虑资源、培训和组织文化的影响,确保及时应用到实践中,并评估对患者结果的影响。远程医疗技术虽具有巨大潜力,但需深入探讨其可接受性、监管问题和与现有医疗体系的整合,确保服务质量和安全性,评估对患者结果的影响。此外,临床试验设计面临科学性、伦理性和可行性的挑战,需平衡资源限制和研究需求,深入讨论以提高试验的有效性和实用性。

5  结 论

       本综述系统地分析了NIPPV在老年患者中使用时鼻面部PI的预防现状和挑战。通过文献回顾,我们认识到了个性化面罩设计、智能监测系统、先进敷料技术以及多学科团队合作在预防措施中的重要性。这些创新方法和技术的应用,虽然在提高患者舒适度和减少损伤风险方面显示出潜力,但仍需克服成本、标准化生产、数据解读和团队协调等多方面的挑战。
       进一步地,通过文献回顾我们发现风险评估工具在早期识别高风险患者中的关键作用,以及患者教育在提高自我管理能力中的重要性。然而,评估工具的准确性和实用性需要通过更多临床试验来验证,而教育策略的有效性则需要根据患者的个体差异进行定制。此外,临床实践指南的更新和实施应基于最新的研究成果,以确保医护人员能够提供基于证据的护理。
        综上所述,预防NIPPV应根据患者的个体情况,尽早进行个性化的评估,及时识别发生鼻面部PI的高危风险因素给予预防措施,正确选择及使用NIPPV界面接口及防护敷料。专业系统培训、多学科团队合作能有效预防使用NIPPV老年患者鼻面部PI,提高治疗成功率,提高医疗质量。
