目的 评估使用炎症、免疫及营养指标对于胃癌根治术后发生腹膜转移的预测作用,并建立适用于术前风险评估的预测模型。方法 回顾性收集2022—2024年苏州市立医院85例晚期胃癌患者的临床资料,按术后是否发生腹膜转移分为转移组44例与非转移组41例。通过分析比较两组间的营养参数[血清白蛋白、纤维蛋白原及预后营养指数(PNI)]、炎症指标[中性粒细胞计数、C反应蛋白、中性粒细胞与淋巴细胞比值(NLR)及全身免疫炎症指数(SII)]、肿瘤标志物以及免疫功能指标(淋巴细胞计数、IL-2、IL-6、TNF-α和IFN-γ)等。使用单因素及多因素Logistic回归分析确定独立预测因子,并构建预测模型,采用Bootstrap法内部验证,计算校正后的曲线下面积及收缩因子,以优化模型性能。结果 多因素分析结果显示,IL-6(OR=1.116,95%CI:1.020~1.224)与CA125(OR=1.016,95%CI:1.006~1.026)为术后腹膜转移的独立预测因素,构建的预测模型区分能力良好,初始AUC为0.801(95%CI:0.702~0.900),经Bootstrap校正后AUC为0.797,模型校准度理想,具有良好的临床稳健性。结论 基于术前IL-6和CA125水平的预测模型,可用于评估胃癌患者根治术后发生腹膜转移的风险。该模型可在术前阶段有效识别高风险病例,为临床制定个体化辅助治疗方案及随访策略提供关键参考,并具有改善患者预后的潜在价值。
Objective To evaluate the predictive value of inflammatory,immune,and nutritional indicators for peritoneal metastasis after radical gastrectomy in gastric cancer and to establish a predictive model applicable for preoperative risk assessment.Methods Clinical data from 85 patients with advanced gastric cancer treated at Suzhou Municipal Hospital between 2022 and 2024 were retrospectively collected.Patients were divided into two groups based on postoperative peritoneal metastasis occurrence:a metastasis group(n=44)and a non-metastasis group(n=41).Indicators analyzed and compared between groups included nutritional parameters(serum albumin,fibrinogen,PNI),inflammatory markers(neutrophil count,CRP,NLR,SII),tumor markers and immune indicators(lymphocyte count,IL-2,IL-6,TNF-α,IFN-γ).Univariate and multivariate logistic regression analyses identified independent predictors for constructing the prediction model.Internal validation was performed using 1 000 bootstrap resamples to calculate the adjusted area under the curve(AUC)and shrinkage factor for model optimization.Results Multivariate analysis identified IL-6(OR=1.116,95%CI:1.020-1.224)and CA125(OR=1.016,95%CI:1.006-1.026) as independent predictors of postoperative peritoneal metastasis.The constructed predictive model demonstrated good discriminatory ability,with an initial AUC of 0.801(95%CI:0.702–0.900)and a bootstrap-corrected AUC of 0.797.The model exhibited ideal calibration and good clinical robustness.Conclusions This study established a predictive model based on preoperative IL-6 and CA125 levels to assess the risk of peritoneal metastasis after radical surgery in gastric cancer patients.The model effectively identifies high-risk cases preoperatively,providing critical guidance for developing individualized adjuvant treatment and follow-up strategies,with potential to improve patient prognosis.
目的 研究数字化口内扫描印模技术(DIOS)对重度牙周炎(SP)患者口腔种植修复效果及龈沟液炎症因子水平的影响并进行分析。方法 选取医院2022年1月—2025年1月收治的100例SP患者进行研究。以密封信封抽签法分为数字化组(n=50)及常规组(n=50)。两组均开展口腔种植修复,常规组采用常规硅橡胶印模技术,数字化组则采用DIOS。对比两组印模制取时间、印模过程中疼痛程度及种植成功率、修复体满意度、龈沟液炎症因子水平、并发症发生情况。结果 数字化组印模制取时间短于常规组,VAS评分低于常规组,而种植成功率高于常规组(均P<0.05)。数字化组边缘密合度、咬合关系、邻接关系满意度均高于常规组(均P<0.05)。修复3个月后两组龈沟液各项炎症因子水平均低于修复前,且数字化组低于常规组(均P<0.05)。数字化组并发症总发生率低于常规组(P<0.05)。结论 DIOS可显著提高SP患者口腔种植修复效果,降低龈沟液炎症因子水平,减少并发症的发生。
Objective To investigate the effects of digital intraoral scanner(DIOS)on the oral implant restoration effect and the levels of inflammatory factors in gingival crevicular fluid in patients with severe periodontitis(SP).Methods A total of 100 SP patients admitted to the hospital from January 2022 to January 2025 were selected for the study.They were divided into the digital group(n=50)and the conventional group(n=50)by the sealed envelope lottery method.Both groups underwent oral implant restoration.The conventional group used the conventional silicone rubber impression technology,while the digital group underwent DIOS.The time of impression preparation,the degree of pain during the impression process,the success rate of implantation,the satisfaction of restorations,the levels of inflammatory factors in gingival crevicular fluid,and the occurrence of complications were compared between the two groups.Results The impression acquisition time of the digital group was shorter than that of the conventional group,the VAS score was lower than that of the conventional group,while the implantation success rate was higher than that of the conventional group(all P<0.05).The marginal fit,occlusal relationship and adjacent relationship in the digital group were all higher than those of the conventional group(all P<0.05).Three months after repair,the levels of various inflammatory factors in gingival crevicular fluid in both groups were lower than those at admission,and the digital group was lower than the conventional group(all P<0.05).The total incidence of complications in the digital group was lower than that in the conventional group(P<0.05).Conclusions DIOS can significantly improve the oral implant restoration effect of SP patients,reduce the levels of inflammatory factors in gingival crevicular fluid,and reduce the occurrence of complications.
目的 探索Nrf2/HO-1通路是否参与远志皂苷元(远志)在脑出血体外细胞模型中对BV2小胶质细胞炎症反应的调控作用。方法 采用红细胞与BV2小鼠小胶质细胞共培养构建体外脑出血模型。分别给予远志单独处理、远志联合Nrf2抑制剂ML385处理,检测炎症因子水平。收集细胞上清制备条件培养基,用于培养SH-SY5Y人神经母细胞瘤细胞,并检测其铁死亡水平。结果 模型组BV2细胞炎症因子(IL-1β,IL-6,TNF-α)水平较对照组显著升高(P<0.001);高浓度远志(20 μM)可降低炎症因子水平,并上调Nrf2/HO-1通路蛋白表达,而ML385能逆转该作用。模型组条件培养基可诱导SH-SY5Y细胞铁死亡,而经远志处理的BV2细胞条件培养基则减轻该现象;ML385预处理可削弱远志对SH-SY5Y细胞的保护作用,远志组铁离子、丙二醛、转铁蛋白受体1水平低于远志联合ML385组(P<0.05),铁转运蛋白、谷胱甘肽和谷胱甘肽过氧化物酶水平高于远志联合ML385组(P<0.05)。结论 远志皂苷元通过激活Nrf2/HO-1通路抑制BV2小胶质细胞炎症反应,并在脑出血体外模型中发挥神经保护作用。
Objective To investigate whether the Nrf2/HO1 signaling pathway is involved in the regulatory effect of tenuigenin(TEN)on the inflammatory response of BV2 microglial cells in an in vitro intracerebral hemorrhage(ICH)model.Methods An in vitro ICH model was established by coculturing BV2 murine microglial cells with red blood cells.Cells were treated with TEN alone or in combination with the Nrf2 inhibitor ML385,and the levels of inflammatory factors were measured.The conditioned medium collected from the cell supernatant was used to culture SHSY5Y human neuroblastoma cells,and ferroptosis levels were assessed.Results The levels of inflammatory factors(IL-1β,IL-6,TNF-α)in BV2 cells of the model group were significantly higher than those in the control group(P<0.001).High concentration of TEN(20 μM)could reduce the levels of inflammatory factors and up-regulate the expression of Nrf2/HO-1 pathway proteins,while ML385 could reverse this effect.The conditioned medium of the model group could induce ferroptosis in SH-SY5Y cells,while the conditioned medium of BV2 cells treated with TEN could alleviate this phenomenon.Pretreatment with ML385 could weaken the protective effect of TEN on SH-SY5Y cells.The levels of iron ions,malondialdehyde and transferrin receptor protein 1 in the TEN group were lower than those in the TEN combined with ML385 group(P<0.05),while the levels of ferroportin glutathione and glutathione peroxidase 4 were higher than those in the TEN combined with ML385 group(P<0.05).Conclusions TEN inhibits the inflammatory response of BV2 microglial cells by activating the Nrf2/HO1 pathway and exerts a neuroprotective effect in the in vitro ICH model.

Inflammatory bowel disease(IBD)is a chronic and frequently recurring disease, and low level of hope in patients may exacerbate disease activity, reduce treatment adherence,and lower their quality of life.This article reviews the current situation of hope level, assessment tools, influencing factors and intervention methods, to analyze the existing challenges and proposes future research directions, aiming to raise the attention of hope level management among healthcare professionals, and to provide reference for the construction of a scientific, reasonable and standardized management plan for hope level of patients with inflammatory bowel disease.
目的 免疫球蛋白A肾病(IgAN)与炎症性肠病(IBD)的相互作用机制尚未阐明。本研究旨在解析IBD与IgAN共病的关键特征基因及核心信号通路,以揭示肠-肾轴的分子调控网络。方法 于GEO数据库获取IBD(GSE75214)和IgAN(GSE93798)基因表达谱,筛选差异表达基因(DEGs)。通过蛋白互作网络(PPI)和拓扑算法(MCC、MNC、Degree、EPC等)识别核心特征基因,并结合公共数据库(CTD、DISEASES和GeneCards)和单细胞转录组测序(GSE171314)进行验证。通过Nephroseq数据库验证基因表达与临床表型的相关性。结果 共筛选出17个IBD-IgAN共病DEGs,PPI网络分析等确定以FOS、EGR1、CXCL2和JUNB为核心特征基因。功能富集分析显示白细胞介素-17(IL-17)信号通路显著激活。单细胞测序验证FOS、EGR1、CXCL2和JUNB基因在IgAN特异性高表达,并通过Nephroseq数据库验证其与尿蛋白和估算的肾小球滤过率下降(eGFR)显著相关。结论 本研究揭示IBD与IgAN共享IL-17通路异常激活及FOS、EGR1、CXCL2和JUNB的基因网络,为开发基于肠-肾轴调控的靶向治疗策略提供理论依据。
Objective The complex interplay between immunoglobulin A nephropathy(IgAN)and inflammatory bowel disease(IBD)remains poorly understood.This study aimed to identify key cross-talk genes and pivotal signaling pathways shared between IBD and IgAN,thereby elucidating the molecular regulatory network underlying the gut-kidney axis.Methods Transcriptomic datasets for IBD(GSE75214)and IgAN(GSE93798)were retrieved from the GEO database.Differentially expressed genes(DEGs)were screened,and shared DEGs were intersected.Protein-protein interaction(PPI)networks were constructed using STRING and Cytoscape,with topological algorithms applied to identify hub genes.Gene expression profiles were validated through(CTD,DISEASES and GeneCards)and single-cell RNA sequencing(GSE171314)and the Nephroseq database,focusing on clinical correlations with proteinuria and estimated glomerular filtration rate(eGFR).Results Seventeen shared DEGs were identified between IBD and IgAN.PPI network analysis revealed FOS,EGR1,CXCL2 and JUNB as core hub genes.Functional enrichment analysis demonstrated significant activation of the interleukin-17(IL-17)signaling pathway.Single-cell sequencing confirmed the specific upregulation of these genes in renal tubular epithelial cells of IgAN patients,which was further validated to correlate with proteinuria and eGFR decline.Conclusions IBD and IgAN share aberrant activation of the IL-17 pathway and a co-regulatory gene network involving FOS,EGR1,CXCL2 and JUNB,providing a theoretical foundation for developing therapeutic strategies centered on the gut-kidney axis.
肥胖是一种以慢性低度炎症为特征的进展性疾病,与多种代谢性疾病的发生、发展密切相关。脂肪组织作为一种内分泌和免疫器官,可分泌多种生物活性物质及炎症因子,参与肥胖患者体内的代谢过程。减重手术是治疗病态性肥胖及相关代谢性疾病的有效方法之一,能够调节机体内的炎症反应、有效改善代谢状态。但减重手术对于炎症因子的作用如何,目前国内外的文献证据仍有争议。本文将系统阐述肥胖与代谢性炎症的关系以及减重手术对炎症因子的影响,旨在为肥胖代谢外科的诊疗过程提供参考。
Obesity is a progressive disease characterized by chronic low-grade inflammation,which is closely related to the occurrence and development of a variety of metabolic diseases.As an endocrine and immune organ,adipose tissue can secrete a variety of bioactive substances and inflammatory factors,which participate in the metabolic process of obese patients.Bariatric surgery is one of the effective methods for the treatment of morbid obesity and related metabolic diseases,which can regulate the inflammatory response in the machine and effectively improve the metabolic state.However,the effect of bariatric surgery on inflammatory factors is still controversial at home and abroad.This article will systematically explain the relationship between obesity and metabolic inflammation and the effect of bariatric surgery on inflammatory factors,aiming to provide a reference for the diagnosis and treatment process of bariatric surgery.
目的 研究甲泼尼龙琥珀酸钠对重症支原体肺炎(SMPP)患儿的治疗效果及安全性。方法 采用随机数表法将南华大学附属长沙中心医院2021年1月—2022年12月收治的108例SMPP患儿分为两组,对照组(54例)采用常规治疗,研究组(54例)采用常规治疗联合甲泼尼龙琥珀酸钠治疗,比较2组临床疗效。结果 治疗后研究组炎症因子水平低于对照组,肺功能指标、健康状况评分均高于对照组,发热、咳嗽、肺啰音及肺阴影消失时间均短于对照组(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论 应用甲泼尼龙琥珀酸钠治疗可加快患儿症状缓解,改善机体炎症反应及肺功能,且安全性较高。
Objective To study the therapeutic effect and safety of methylprednisolone sodium succinate on children with severe Mycoplasma pneumoniae pneumonia(SMPP).Methods A total of 108 children with SMPP admitted to Changsha Central Hospital from January 2021 to December 2022 were divided into two groups by random number table method.The control group(54 cases)was treated with conventional therapy,and the study group(54 cases)was treated with conventional therapy combined with methylprednisolone sodium succinate.The clinical effects of the two groups were compared.Results After treatment,the levels of inflammatory factors in the study group were lower than those in the control group,and lung function indicators and health status scores were higher than those in the control group.The disappearance time of fever,cough,lung rales and lung shadows was shorter than that in the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions The application of methylprednisolone sodium succinate treatment can accelerate the relief of symptoms in children,improve the body's inflammatory response and lung function,and has high safety.
目的 探讨宣清导浊汤加减方联合血液透析治疗对老年慢性肾衰竭(CRF)患者肾功能、微炎症状态和毒素清除率的影响。方法 回顾性选取2019年10月—2022年10月我院收治的82例老年CRF患者,将其根据治疗方法分为联合组与单一组,每组41例。单一组仅采取血液透析治疗,联合组联合宣清导浊汤加减方治疗,对比两组患者治疗前后中医证候积分、毒素清除率、炎症因子变化。结果 治疗后,两组患者口淡不渴、肢体麻木、腰膝酸软、倦怠乏力、食少纳呆以及面色晦暗相关中医证候积分均降低,且联合组低于单一组(P<0.05);治疗后两组患者血肌酐、尿素氮、血β2-微球蛋白均降低,联合组低于单一组(P<0.05);治疗后两组患者白细胞介素-6、C-反应蛋白、肿瘤坏死因子-α炎性因子水平明显降低,且联合组更低(P<0.05)。两组在治疗期间均无不良反应。结论 宣清导浊汤加减方联合血液透析治疗老年CRF患者效果较佳,可通过降低机体炎症因子表达水平、提高毒素清除率来改善患者临床症状,且不会增加不良反应。
Objective To explore the effect of Xuanqing Daozhuo Decoction modified formula combined with hemodialysis treatment on renal function,micro inflammatory status,and toxin clearance rate in elderly patients with chronic renal failure(CRF).Methods Retrospectively,82 elderly CRF patients admitted to our hospital from October 2019 to October 2022 were selected and divided into a combination group and a single group according to the treatment methods,with 41 cases in each group. The single group was treated with hemodialysis only,and the combined group was treated with Xuanqing Daozhuo Decoction modified formula additionally.The changes of TCM syndrome scores,toxin clearance rate and inflammatory factors before and after treatment were compared between the two groups.Results After treatment,the scores of TCM syndroms including tasteless mouth and no thirst,numbness of limbs,soreness and weakness of waist and knee,fatigue,poor appetite and sluggish stomach were all decreased in 2 groups,and those in combined group were lower(P<0.05).After treatment,serum creatinine,blood urea nitrogen and β2-microglobulin were decreased in 2 groups,and those in combined group were lower(P<0.05).After treatment,the levels of interleukin-6,C-reactive protein and tumor necrosis factor-α inflammatory factors were significantly decreased in 2 groups,which were lower in combination group(P<0.05).The two groups had no adverse effects during the treatment period.Conclusions Xuanqing Daozhuo Decoction modified formula and hemodialysis can improve the clinical symptoms of elderly patients with CRF,reduce the expression level of inflammatory factors in patients,and increase the clearance rate of toxins in patients,and does not increase the adverse effects.
慢性炎症是心血管疾病的常见发病机制,它主要通过损伤内皮细胞、氧化应激和刺激血栓形成影响疾病发展。全身免疫炎症指数(SII)作为一种新型炎症指标,最早用于肿瘤患者的预后评估,现已在多学科领域广泛使用,它由中性粒细胞、血小板、淋巴细胞计算得到,更加全面地反映机体炎症状态。SII已经在多项研究中被证实具有良好的预测价值,该文对SII的优势和在心血管疾病的临床研究进展进行综述,为研究的进一步开展提供参考。
Chronic inflammation is a common pathogenesis of cardiovascular disease,which mainly affects disease progression by damaging endothelial cells,oxidative stress and stimulating thrombosis.As a new type of inflammatory index,the systemic immune inflammatory index(SII)was first used to evaluate the prognosis of cancer patients,and has been widely used in multidisciplinary fields.SII has been confirmed to have good predictive value in a number of studies,and this article reviews the advantages of SII and the progress of clinical research in cardiovascular diseases,so as to provide reference for further research development.
目的 探讨对于重症肺部感染患者采用药物+纤维支气管镜肺泡灌洗吸痰术治疗的效果。方法 选取2021年9月—2023年5月在郑州市第一人民医院ICU中94例肺部感染患者,分为观察组、对照组,对照组进行常规治疗,观察组采用药物+纤维支气管镜肺泡灌洗吸痰术治疗,对比两组患者的疗效、症状的缓解时间、炎性因子、实验室相关指标以及不良反应。结果 治疗后,观察组患者的治疗有效率91.49%高于对照组76.60%(χ2=3.887,P<0.05),观察组患者的发热、肺部湿啰音、咳嗽以及咳痰等症状的缓解时间分别为(3.09±1.25)(4.17±2.24)(3.95±1.53)(4.05±1.77)d,均低于对照组(5.14±2.43)(7.03±3.23)(6.40±2.62)(6.32±1.81)d(t=5.143、4.988、5.536、6.147,P<0.05),观察组的 C 反应性蛋白(C-reactive protein,CRP)、白细胞介素-6(Interleukin-6,IL-6)以及Toll样受体4(Toll-like receptor 4,TLR-4)水平分别为(3.64±0.87)mg/L、(54.59±10.65)ng/mL、(7.94±1.57)%,均低于对照组(7.51±1.43)mg/L、(87.66±11.17)ng/mL、(12.11±2.48)%(t=15.850、14.690、9.740,P<0.05),观察组的气道压力、动脉血二氧化碳分压(partial pressure of carbon dioxide in artery,PaCO2)水平分别为(6.92±3.60)cmH2O、(43.19±6.29)mmHg,低于对照组(8.68±2.98)cmH2O、(55.43±5.95)mmHg,观察组动脉氧分压(Partial arterial oxygen pressure,PaO2)(86.06±5.31)mmHg、血氧饱和度(oxyhemoglobin saturation,SpO2)(98.03±6.63)%高于对照组(68.04±5.19)mmHg、(90.22±5.51)%要高(t=2.582、9.692、16.638、6.221,P<0.05),观察组中的不良反应发生率为2.13%与对照组10.64%比较差异无统计学意义(χ2=2.849,P>0.05)。结论 对处于ICU中的肺部感染患者治疗时,使用药物+纤维支气管镜肺泡灌洗吸痰术能够改善患者症状,降低炎性因子水平。
Objective To investigate the efficacy of medication combined with bronchoscopic alveolar lavage and sputum aspiration in ICU patients with severe pulmonary infection. Methods Ninety-four patients with pulmonary infection in the ICU of Zhengzhou First People's Hospital from September 2021 to May 2023 were selected and divided into observation group and control group.The control group received routine treatment,and the observation group was treated with medication and bronchoscopic alveolar lavage and sputum aspiration.The efficacy,duration of symptom remission,inflammatory factors,laboratory-related indicators and adverse reactions were compared between the two groups. Results After treatment,the effective rate of 91.49% in observation group was significantly higher than 76.60% in control group(χ2=3.887,P<0.05).The relief time of fever,pulmonary rale,cough and sputum in observation group was(3.09±1.25)d,(4.17±2.24)d,(3.95±1.53)d,and(4.05±1.77)d,respectively,which were significantly lower than the control group[(5.14±2.43)d,(7.03±3.23)d,(6.40±2.62)d and(6.32±1.81)d](t=5.143,4.988,5.536,6.147,P<0.05).The levels of CRP,IL-6 and TLR-4 in the observation group were(3.64±0.87)mg/L,(54.59±10.65)ng/mL and(7.94±1.57)%,respectively.They were significantly lower than(7.51±1.43)mg/L,(87.66±11.17)ng/mL and(12.11±2.48)% in the control group(t=15.850,14.690,9.740,P<0.05).The airway pressure and PaCO2 levels of the observation group were(6.92±3.60)cmH2O and(43.19±6.29)mmHg,respectively,which were significantly lower than those of the control group[(8.68±2.98)cmH2O and(55.43±5.95)mmHg].The levels of PaO2[(86.06±5.31)mmHg] and SpO2[(98.03±6.63)%] in the observation group were higher than those in the control group[(68.04±5.19)mmHg and(90.22±5.51)%],and there were statistically significant differences(t=2.582,9.692,16.638,6.221,P<0.05).The adverse reactions of 2.13% in the observation group were lower than 10.64% in the control group,and there was no statistically significant difference(χ2=2.849,P>0.05). Conclusions In ICU patients with severe lung infection,the treatment of drugs combined with alveolar lavage and fiberoptic bronchoscopic aspiration is better,and can significantly improve the symptoms and inflammatory factor levels of patients.
